2. Is Night Surgery a Nightmare for Lung Transplantation?

Abstract

Night work is frequently associated with sleep deprivation and is associated with greater surgical and medical complications. Lung transplantation (LT) is carried out both at night and during the day and involves many medical healthcare workers. The goal of the study was to compare morbidity and mortality between LT recipients according to LT operative time. We performed a retrospective, observational, single-center study. When the procedure started between 6 AM and 6 PM, the patient was allocated to the Daytime group. If the procedure started between 6 PM and 6 AM, the patient was allocated to the Nighttime group. Between January 2015 and December 2020, 253 patients were included. A total of 168 (66%) patients were classified into the Day group, and 85 (34%) patients were classified into the Night group. Lung Donors’ general characteristics were similar between the groups. The 90-day and one-year mortality rates were similar between the groups (90-days: n = 13 (15%) vs. n = 26 (15%), p = 0.970; 1 year: n = 18 (21%) vs. n = 42 (25%), p = 0.499). Daytime LT was associated with more one-year airway dehiscence (n = 36 (21%) vs. n = 6 (7.1%), p = 0.004). In conclusion, among patients who underwent LT, there was no significant association between operative time and survival.

Abstract

Introduction Lung transplantation (LTx) aims at improving survival and quality of life for patients with end-stage lung diseases. Venoarterial extracorporeal membrane oxygenation (VA-ECMO) is used as intraoperative support for LTx, despite no precise guidelines for its initiation. We aim to evaluate two strategies of VA-ECMO initiation in the perioperative period in patients with obstructive or restrictive lung disease requiring bilateral LTx. In the control ‘on-demand’ arm, high haemodynamic and respiratory needs will dictate VA-ECMO initiation; in the experimental ‘systematic’ arm, VA-ECMO will be pre-emptively initiated. We hypothesise a ‘systematic’ strategy will increase the number of ventilatory-free days at day 28.Methods and analysis We designed a multicentre randomised controlled trial in parallel groups. Adult patients with obstructive or restrictive lung disease requiring bilateral LTx, without a formal indication for pre-emptive VA-ECMO before LTx, will be included. Patients with preoperative pulmonary hypertension with haemodynamic collapse, ECMO as a bridge to transplantation, severe hypoxaemia or hypercarbia will be secondarily excluded. In the systematic group, VA-ECMO will be systematically implanted before the first pulmonary artery cross-clamp. In the on-demand group, VA-ECMO will be implanted intraoperatively if haemodynamic or respiratory indices meet preplanned criteria. Non-inclusion, secondary exclusion and VA-ECMO initiation criteria were validated by a Delphi process among investigators. Postoperative weaning of ECMO and mechanical ventilation will be managed according to best practice guidelines. The number of ventilator-free days at 28 days (primary endpoint) will be compared between the two groups in the intention-to-treat population. Secondary endpoints encompass organ failure occurrence, day 28, day 90 and year 1 vital status, and adverse events.Ethics and dissemination The sponsor is the Assistance Publique–Hôpitaux de Paris. The ECMOToP protocol version 2.1 was approved by Comité de Protection des Personnes Ile de France VIII. Abstract

The integration of distributed renewable energy sources and the multi-domain behaviours inside the cyber-physical energy system (smart grids) draws up major challenges. Their validation and roll out requires careful assessment, in term of modelling, simulation and testing. The traditional approach focusing on a particular object, actual hardware or a detailed model, while drastically simplifying the remainder of the system under test, is no longer sufficient. Real-time simulation and Hardware-in-the-Loop (HIL) techniques emerge as indispensable tools for validating the behaviour of renewable sources as well as their impact/interaction to with the cyber-physical energy system. This paper aims to provide an overview of the present status-quo of real-time and HIL approaches used for smart grids and their readiness for cyber-physical experiments. We investigate the current limitations of HIL techniques and point out necessary future developments. Abstract

This paper presents a novel digital control strategy successfully implemented for a soft magnetic material characterization bench (Epstein frame type). The main objective is to control the magnetic induction waveform whatever the applied excitation and the material under study. Given the nonlinear nature of the magnetization curves of magnetic materials, an original model-free based control technique is considered. Special mention should be made of the interesting dynamic properties in closed-loop against the changes of the operating point related basically to the hysteresis form. Abstract

Abstract Background Some guidelines advocate for managing patients with penetrating thoracic wounds in trauma centres with cardiothoracic surgery. This systematic approach is questionable. Only 15% of these patients require surgery. It is known that clinical examination fails to detect hemopneumothorax in penetrating trauma. However, no studies have evaluated the combined diagnostic performance of vital signs and the clinical evaluation of wounds. The clinical characteristics of wounds have not been investigated. We aimed to evaluate the ability of combinations of pre-hospital signs to rule out invasive chest stab trauma. Methods This was a prospective observational study. All consecutive adult patients hospitalized in the perioperative acute care unit of a tertiary university hospital were included. Injury diagnoses were provided by exploratory surgery and imaging tests. Patients with a final diagnosis of invasive wounds (IWs) and patients with only superficial wounds were compared. Data regarding management and outcome were analysed. Results A total of 153 patients were included. After imaging or surgery, 58 (38%) patients were diagnosed with only superficial wounds, and 95 (62%) were diagnosed with thoracic or abdominal IWs. The false-negative rate of pre-hospital evaluations in the diagnosis of IWs was 42% [31–51]IQR25–75. In stable patients, pre-hospital data could not rule out IWs, with a negative predictive value of 58% and a positive predictive value of 70%. Twenty-nine (19%) patients required early emergent cardiothoracic surgery. Among these patients, 8 (28%) had no evidence of IWs in the pre-hospital period. Among the 59 patients without pre-hospital signs of IWs, 19 (33%) underwent at least one emergent procedure. Conclusions The combination of pre-hospital vital signs, visual evaluation of wounds, and physical examination failed to rule out IWs in patients with chest stab wounds. This implies that caution is needed in triage decision-making.

Abstract

The rural electrification of Sub-Saharan Africa and South-East Asia is crucial to end the energy poverty in which around 1 billion people are trapped. Swarm electrification, i.e., the progressive building of decentralized and decarbonized electric infrastructure in a bottom-up manner, tackles rural electrification challenges by quickly providing modern and reliable electricity services to unelectrified communities while fostering local socio-economic development. This paper follows the technological approach of this electrification model and presents the development of a DC microgrid with decentralized production and storage suitable for rural electrification. This DC microgrid aims at interconnecting nanogrids, small collective autonomous power units composed of a solar panel and a lead–acid battery for 4 to 6 households, to increase the electrical services brought to the community and enhance the economic sustainability of this rural electrification model. The design of the proposed microgrid as well as its control algorithm are thoroughly addressed and tested from software simulations and experimental testing to field deployment in Madagascar. Extensive software, experimental and field-tests results are illustrated, and the microgrid design feedback is given. This paper overall validates the proper operation of the proposed microgrid, confirming the technical feasibility of the swarm electrification approach.

Abstract

The identification of a permanent establishment is the principle used by the Member States of the European Union to determine whether a company is subject to the tax jurisdiction of a State. In the digital context, the notion of permanent establishment is often questioned by States, as companies can provide services remotely without having any physical presence in the territory of a State. This thesis analyses the principle of permanent establishment for traditional enterprises and the possibility of applying this principle to digital companies. To do this, the author will compare the solutions provided by France, the OECD, and the European Commission, in order to determine the tax place of digital companies.

Abstract

The lungs are a frequent target of metastatic breast cancer cells, but the underlying molecular mechanisms are unclear. All existing data were obtained either using statistical association between gene expression measurements found in primary tumors and clinical outcome, or using experimentally derived signatures from mouse tumor models. Here, we describe a distinct approach that consists of using tissue surgically resected from lung metastatic lesions and comparing their gene expression profiles with those from nonpulmonary sites, all coming from breast cancer patients. We show that the gene expression profiles of organ-specific metastatic lesions can be used to predict lung metastasis in breast cancer. We identified a set of 21 lung metastasis–associated genes. Using a cohort of 72 lymph node–negative breast cancer patients, we developed a 6-gene prognostic classifier that discriminated breast primary cancers with a significantly higher risk of lung metastasis. We then validated the predictive ability of the 6-gene signature in 3 independent cohorts of breast cancers consisting of a total of 721 patients. Finally, we show that the signature improves risk stratification independently of known standard clinical variables and a previously established lung metastasis signature based on an experimental breast cancer metastasis model. Abstract

The rural electrification of Sub-Saharan Africa and South-East Asia is crucial to end the energy poverty in which around 1 billion people are trapped. Swarm electrification, i.e., the progressive building of decentralized and decarbonized electric infrastructure in a bottom-up manner, tackles rural electrification challenges by quickly providing modern and reliable electricity services to unelectrified communities while fostering local socio-economic development. This paper follows the technological approach of this electrification model and presents the development of a DC microgrid with decentralized production and storage suitable for rural electrification. This DC microgrid aims at interconnecting nanogrids, small collective autonomous power units composed of a solar panel and a lead–acid battery for 4 to 6 households, to increase the electrical services brought to the community and enhance the economic sustainability of this rural electrification model. The design of the proposed microgrid as well as its control algorithm are thoroughly addressed and tested from software simulations and experimental testing to field deployment in Madagascar. Extensive software, experimental and field-tests results are illustrated, and the microgrid design feedback is given. This paper overall validates the proper operation of the proposed microgrid, confirming the technical feasibility of the swarm electrification approach.

Abstract

Variable Speed Hydro-Electric Plant (VS-HEP) equipped with power electronics has been increasingly introduced into the hydraulic context. This paper is targeting a VS-HEP Power Hardware-In-the-Loop (PHIL) real-time simulation system, which is dedicated to different hydraulic operation schemes tests and control laws validation. Then, a proper hydraulic model will be the key factor for building an efficient PHIL real-time simulation system. This work introduces a practical and generalised modelling hydraulic modelling approach, which is based on ‘Hill Charts’ measurements provided by industrial manufacturers. The hydraulic static model is analytically obtained by using mathematical optimization routines. In addition, the nonlinear dynamic model of the guide vane actuator is introduced in order to evaluate the effects of the induced dynamics on the electric control performances. Moreover, the reduced-scale models adapted to different laboratory conditions can be established by applying scaling laws. The suggested modelling approach enables the features of decent accuracy, light computational complexity, high flexibility and wide applications for their implementations on PHIL real-time simulations. Finally, a grid-connected energy conversion chain of bulb hydraulic turbine associated with a permanent magnet synchronous generator is chosen as an example for PHIL design and performance assessment.

Abstract

The prevalence, risk factors and outcomes associated with culture-positive preservation fluid (PF) after lung transplantation (LT) are unknown. From January 2015 to December 2020, the microbiologic analyses of PF used to store the cold ischaemia-placed lung graft(s) of 271 lung transplant patients were retrospectively studied. Culture-positive PF was defined as the growth of any microorganism. Eighty-three (30.6%) patients were transplanted with lung grafts stored in a culture-positive PF. One-third of culture-positive PF were polymicrobial. Staphylococcus aureus and Escherichia coli were the most frequently isolated microorganisms. No risk factors for culture-positive PF based on donor characteristics were identified. Forty (40/83; 48.2%) patients had postoperative pneumonia on Day 0 and 2 (2/83; 2.4%) patients had pleural empyema with at least one identical bacteria isolated in culture-positive PF. The 30-day survival rate was lower for patients with culture-positive PF compared with patients with culture-negative PF (85.5% vs. 94.7%, p = 0.01). Culture-positive PF has a high prevalence and may decrease lung transplant recipient survival. Further studies are required to confirm these results and improve understanding of the pathogenesis of culture-positive PF and their management.

Or, les passages du milieu protege en milieu ordinaire relevent d’exceptions. L’article explore ce paradoxe a partir de methodes qualitatives et quantitatives. L’etude montre que la majorite des personnes interrogees ne souhaite pas travailler en milieu ordinaire, percu comme trop violent. Cette vision du milieu ordinaire est renforcee par le discours des salarie·e·s des ESAT, qui faconnent les perspectives des personnes handicapees et effectuent un tri parmi les personnes souhaitant aller vers le milieu ordinaire.

Abstract

Bronchiolitis obliterans syndrome (BOS), the main manifestation of chronic lung allograft dysfunction, leads to poor long-term survival after lung transplantation. Identifying predictors of BOS is essential to prevent the progression of dysfunction before irreversible damage occurs. By using a large set of 107 samples from lung recipients, we performed microarray gene expression profiling of whole blood to identify early biomarkers of BOS, including samples from 49 patients with stable function for at least 3 years, 32 samples collected at least 6 months before BOS diagnosis (prediction group), and 26 samples at or after BOS diagnosis (diagnosis group). An independent set from 25 lung recipients was used for validation by quantitative PCR (13 stables, 11 in the prediction group, and 8 in the diagnosis group). We identified 50 transcripts differentially expressed between stable and BOS recipients. Three genes, namely POU class 2 associating factor 1 (POU2AF1), T-cell leukemia/lymphoma protein 1A (TCL1A), and B cell lymphocyte kinase, were validated as predictive biomarkers of BOS more than 6 months before diagnosis, with areas under the curve of 0.83, 0.77, and 0.78 respectively.
