Abstract

The article describes an institutional clinical trial involving heart transplantation in three infants who underwent orthotopic transplantation from three infant donors who died from cardio-circulatory causes. The recipients' six-month survival rate was 100%, with no late deaths, that is, within 3.5 years, and they reported good functional and immunologic outcomes.

Abstract

Stenting of the ductus arteriosus may be an effective method for prolonging the lives of infants with hypoplastic left-heart syndrome. Hypoplastic left-heart syndrome is a birth defect characterized by malformation of the heart. The survival of infants with this syndrome has improved with the introduction of heart transplants for newborns in 1985. The ductus arteriosus is a fetal blood vessel that usually closes before birth. It should remain open in infants with hypoplastic left-heart syndrome until a suitable donor heart is found. Stenting of the ductus arteriosus is a surgical procedure to maintain this opening. Five infants with hypoplastic left-heart syndrome underwent stenting of the ductus arteriosus. Four infants later had a successful heart transplant. One infant died from cardiac arrest 24 hours after surgery.

Abstract

Life-threatening congenital heart defects that cannot be treated or surgically corrected occur in some newborn infants. These babies need heart transplants, however, there are risks to such procedures. The infants will undergo rapid growth during childhood, and the transplanted organ must function well under such conditions. Transplants usually necessitate drug therapy to suppress the immune system and prevent rejection of the transplant, yet infants must develop immunity against a variety of infectious agents. Finally donor availability is a problem; infant deaths are particularly distressing, and organ donation may not seem appropriate. Twenty-five infants have undergone heart transplants at Loma Linda University School of Medicine, and their cases, as well as the cardiovascular characteristics of the infant donors, are reviewed. Of the 25 children, 21 are alive, and have survived for a range of 4 to 40 months. The four deaths occurred within 12 days of transplantation. Survivors have been maintained on cyclosporin immunosuppressive therapy; azathioprine was also used immediately following surgery. The steroid drug prednisone has been used only during rejection episodes. Early detection and treatment of rejection without recourse to traumatic invasive procedures is an important achievement of the program. In this pioneering project, survival rate of infants reached that of the best adult programs. Donor health did not impede transplant success; even hearts obtained following prolonged cardiac arrest and metabolic acidosis (acid produced due to lack of oxygen, which could damage heart tissue) did not appear to be problematic. The availability of donor organs and commitment by the institution to development of infant cardiac transplantation without guarantee of financial repayment by insurance companies (who frequently fund adult but not pediatric heart transplants) are the factors which limit these programs. When successful, the programs can provide nearly normal heart function to infants who otherwise are not expected to survive past one year of age.
