3. Magnetic Design of a Compact GaToroid for Very High Energy Electron and Pre-Clinical Hadron Beams

Abstract

GaToroid is a novel configuration of a beam delivery system for charged particles therapy based on a steady-state, axis-symmetric field configuration. The basic idea is to use fixed toroidal magnets, producing an axis-symmetric (or periodical axis-symmetric) field configuration that can bend beams from several directions onto the patient location. In principle, neither magnets nor patient need to be moved. In addition, the field of the magnets of this toroidal gantry is static. Such system is perfectly adapted to FLASH therapy, a novel method of beam dose delivery that has shown benefits for electrons and potential for hadrons. In this paper we describe the magnetic design of a “compact GaToroid”, which could be built with iron-dominated magnets, suitable for 200 MeV electron beams, as well as protons at 70 MeV. The gantry profits from the use of toroidal quadrupoles, which greatly simplify the beam transmission. An additional novelty is the use of a resonant kicker as the vector magnet. Starting from the requirements for beam transmission, we derive first the magnet main characteristics (strength, aperture, size and mass) and report the detailed magnetic field calculations in support of the design.

We report heat pulse experiments at room temperature that cannot be described by Fourier's law. The experimental data is modelled properly by the Guyer--Krumhansl equation, in its over-diffusion regime. 01 juli 2023, Contains fulltext : 294874.pdf (Publisher’s version ) (Open Access), PURPOSE: Medical imaging has become increasingly important in diagnosing and treating oncological patients, particularly in radiotherapy. Recent advances in synthetic computed tomography (sCT) generation have increased interest in public challenges to provide data and evaluation metrics for comparing different approaches openly. This paper describes a dataset of brain and pelvis computed tomography (CT) images with rigidly registered cone-beam CT (CBCT) and magnetic resonance imaging (MRI) images to facilitate the development and evaluation of sCT generation for radiotherapy planning. ACQUISITION AND VALIDATION METHODS: The dataset consists of CT, CBCT, and MRI of 540 brains and 540 pelvic radiotherapy patients from three Dutch university medical centers. Subjects' ages ranged from 3 to 93 years, with a mean age of 60. Various scanner models and acquisition settings were used across patients from the three data-providing centers. Details are available in a comma separated value files provided with the datasets. DATA FORMAT AND USAGE NOTES: The data is available on Zenodo (https://doi.org/10.5281/zenodo.7260704, https://doi.org/10.5281/zenodo.7868168) under the SynthRAD2023 collection. The images for each subject are available in nifti format. POTENTIAL APPLICATIONS: This dataset will enable the evaluation and development of image synthesis algorithms for radiotherapy purposes on a realistic multi-center dataset with varying acquisition protocols. Synthetic CT generation has numerous applications in radiation therapy, including diagnosis, treatment planning, treatment monitoring, and surgical planning.

21. Sexual dysfunction in women with PCOS:a case control study

Abstract

STUDY QUESTION: What is the relationship of sex steroid levels with sexual function in women with and without polycystic ovary syndrome (PCOS)? SUMMARY ANSWER: Women with PCOS reported more sexual dysfunction and more sexual distress compared to those without PCOS, but only few and weak associations between androgen levels and sexual function were observed. WHAT IS KNOWN ALREADY: The literature shows that women with PCOS report lower levels of sexual function and sexual satisfaction and more sexual distress. Contributing factors seem to be obesity, alopecia, hirsutism, acne, infertility, anxiety, depression, and low self-esteem. In women with PCOS clinical and/or biochemical hyperandrogenism is common; its relationship with sexual function is, however, inconclusive. STUDY DESIGN, SIZE, DURATION: This observational prospective case control study with 135 women (68 PCOS, 67 control) was conducted from March 2017 until March 2020. PARTICIPANTS/MATERIALS, SETTING, METHODS: Heterosexual women with and without PCOS, aged 18–40 years, in a steady relationship and without any comorbidities, underwent an extensive medical and endocrine screening using liquid chromatography-tandem mass spectrometry and validated sexual function questionnaires. MAIN RESULTS AND THE ROLE OF CHANCE: Women with PCOS reported significantly lower sexual function (Female Sexual Function Index (FSFI) P < 0.001, partial g 2 ¼ 0.104), higher levels of sexual distress (Female Sexual Distress Scale-Revised P < 0.001, partial g 2 ¼ 0.090), and they more often complied with the definition of sexual dysfunction (41.2% vs 11.9%, P < 0.001, Phi V ¼ 0.331) and clinical sexual distress (51.5% vs 19.4%, P < 0.001, Phi V ¼ 0.335). Regression analysis adjusted for confounders showed only few and weak associations between androgen levels and sexual function, with each model explaining a maximum of 15% sexual function.
