BACKGROUND: The objective was to describe patients presenting themselves at emergency-care settings following a suicide attempt in eight culturally different sites [Campinas (Brazil), Chennai (India), Colombo (Sri Lanka), Durban (South Africa), Hanoi (Viet Nam), Karaj (Iran), Tallinn (Estonia), and Yuncheng, (China)]. METHOD: Subjects seen for suicide attempts, as identified by the medical staff in the emergency units of 18 collaborating hospitals were asked to participate in a 45-minute structured interview administered by trained health personnel after the patient was medically stable. RESULTS: Self-poisoning was the main method of attempting suicide in all eight sites. Self-poisoning by pesticides played a particularly important role in Yuncheng (71.6% females, 61.5% males), in Colombo (43.2% males, 19.6% females), and in Chennai (33.8% males, 23.8% females). The suicide attempt resulted in danger to life in the majority of patients in Yuncheng and in Chennai (over 65%). In four of the eight sites less than one-third of subjects received any type of referral for follow-up evaluation or care. CONCLUSIONS: Action for the prevention of suicide attempts can be started immediately in the sites investigated by addressing the one most important method of attempted suicide, namely self-poisoning. Regulations for the access to drugs, medicaments, pesticides, and other toxic substances need to be improved and revised regulations must be implemented by integrating the efforts of different sectors, such as health, agriculture, education, and justice. The care of patients who attempt suicide needs to include routine psychiatric and psychosocial assessment and systematic referral to professional services after discharge. [ABSTRACT FROM AUTHOR]

BACKGROUND: The objectives were to assess thoughts about suicide, plans to commit suicide and suicide attempts in the community, to investigate the use of health services following a suicide attempt, and to describe basic socio-cultural indices of the community. METHOD: The community survey was one component of the larger WHO multisite intervention study on suicidal behaviours (SUPRE-MISS). In each site, it aimed at randomly selecting and interviewing at least 500 subjects of the general population living in the catchment area of the emergency department where the intervention component of the study was conducted. Communities of eight SUPRE-MISS sites (in Brazil, China, Estonia, India, Iran, South Africa, Sri Lanka, and Viet Nam) participated plus two additional sites from Australia and Sweden conducting similar surveys. RESULTS: Suicide attempts (0.4-4.2%), plans (1.1-15.6%), and ideation (2.6-25.4%) varied by a factor of 10-14 across sites, but remained mostly within the ranges of previously published data. Depending on the site, the ratios between attempts, plans, and thoughts of suicide differed substantially. Medical attention following a suicide attempt varied between 22% and 88% of the attempts. CONCLUSIONS: The idea of the suicidal process as a continuous and smooth evolution from thoughts to plans and attempts of suicide needs to be further investigated as it seems to be dependent on the cultural setting. There are indications, that the burden of undetected attempted suicide is high in different cultures; an improved response from the health sector on how to identify and support these individuals is needed. OBJETIVO: A internação psiquiátrica, embora onerosa, continua sendo um recurso terapêutico indispensável para pacientes graves. A internação em unidades psiquiátricas de hospital geral (UPHG) tem sido recomendada como a melhor alternativa aos macro-hospitais psiquiátricos. Nesse sentido, realizou-se estudo com o objetivo de verificar variáveis socioeconômicas e clínicas associadas com o êxito ou o fracasso da internação. MÉTODOS: Foram revistos dados sociodemográficos e clínicos de 1.463 pacientes internados em uma UPHG de hospital geral no período 1986-1997, utilizando-se análise de regressão logística politômica univariada e multivariada. RESULTADOS: Por meio da análise multivariada, foram identificados três fatores associados a um pior desfecho da internação (condição de alta ruim e maior tempo de permanência): idade >60 anos, não exercício prévio de função social e diagnóstico de transtorno psicorgânico. CONCLUSÃO: Embora mais de 80% dos pacientes tratados nessa UPHG tenham apresentado significativa melhora dos sintomas, um subgrupo se beneficiou menos da internação, parecendo requerer intervenções de reabilitação psicossocial ao invés da hospitalização.

OBJETIVO: Traduzir e adaptar para o português o questionário de qualidade de vida denominado "King's Health Questionnaire" (KHQ) em mulheres com incontinência urinária. MÉTODOS: Cientes dos objetivos da pesquisa científica, dois tradutores brasileiros prepararam duas versões do KHQ para o português, as quais foram retro-traduzidas por outros dois tradutores ingleses. As diferenças foram harmonizadas e pré-testadas em um estudo piloto. As versões finais do KHQ e de outro questionário, o "Short-Form Health Survey" (SF-36), já vertido e publicado em português, foram simultaneamente administradas a 156 e 119 pacientes, respectivamente. Foram testadas as propriedades psicométricas do KHQ como confiabilidade (consistência interna e teste-reteste) e validade de constructo. O reteste foi realizado em um período de duas semanas, a partir da primeira entrevista. RESULTADOS: O processo de adaptação cultural não alterou a versão em português do KHQ comparado ao original, exceto no modo de administração para pacientes com baixo grau de alfabetização. Neste caso, o questionário mudou de auto-avaliação para ser lido para as pacientes durante entrevista com o pesquisador. Para as outras pacientes, o KHQ foi auto-administrado. O alfa de Cronbach padronizado do KHQ foi de 0,87 e avaliado por seus domínios variou de 0,49 a 0,92. A confiabilidade, medida pelo índice de correlação intraclasses (ICC) foi considerada de moderada a forte em todos os domínios e na escala de medidas de gravidade, variando de 0,53 a 0,81. O coeficiente de correlação de Pearson entre o KHQ e o SF-36 foi considerado de fraco a moderado na maioria dos domínios afins, variando de -0,27 a -0,53. CONCLUSÕES: A versão para o português do KHQ, traduzida e adaptada para seu uso em mulheres brasileiras com queixas de incontinência urinária. Representa um importante instrumento para a avaliação de mulheres incontinentes em pesquisa clínica.

OBJETIVO: Estudar prospectivamente a população internada em um hospital-dia (HD) em relação a fatores que poderiam influenciar na melhora e na duração da internação. MÉTODOS: Foram entrevistados, para obtenção de dados sociodemográficos e avaliação da evolução, 34 pacientes internados no Hospital-Dia da Faculdade de Medicina de Botucatu, Unesp, durante um ano. O diagnóstico psiquiátrico foi avaliado pela CIDI (Composite International Diagnostic Interview), a sintomatologia psiquiátrica pela BPRS (Brief Psychiatric Rating Scale) e a incapacitação psicossocial pela DAS (Psychiatric Disability Assessment Schedule). Todos os pacientes foram acompanhados, e seus familiares, entrevistados. RESULTADOS: Predominaram mulheres (76%), jovens (61,8%), sem vínculo conjugal (71%), sem trabalho (82,4%), com diagnóstico de transtornos afetivos (44,1%) e com internações psiquiátricas prévias (44%). Apenas quatro (12%) pacientes apresentavam uma "síndrome maior" segundo BPRS. Houve considerável incapacitação psicossocial dos pacientes em alguns papéis sociais. Maior renda per capita foi um fator associado à melhor evolução. As internações duraram em média 74 dias. Pacientes com internações prévias tenderam a permanecer menos tempo no HD. CONCLUSÕES: Portadores de transtornos afetivos e quadros "não-psicóticos" geralmente não necessitam de internação por período integral em hospital psiquiátrico. Contudo, os pacientes deste estudo tiveram um elevado número de internações psiquiátricas prévias, provavelmente por necessitarem de um nível de atendimento além das possibilidades dos ambulatórios. Entretanto, pacientes com maior número de internações -- em tese mais graves -- tenderam a permanecer menos tempo no HD, o que suscita dúvidas quanto à sua adesão a serviços abertos, bem como aos possíveis fatores facilitadores dessa adesão. Em um momento de crescimento expressivo no número de serviços de internação parcial no Brasil, como nos últimos anos, mais estudos são necessários a fim de esclarecer para quem e para quê são destinados esses serviços.

The objective of this study was to describe the information that women with tubal infertility recalled having received from physicians when they consulted for a reproductive tract infection (RTI) and the information physicians reported having provided to women consulting for RTIs. Interviews were conducted with 16 women presenting primary tubal infertility selected on the basis of purposeful sampling criteria. Also, 15 physicians working in the public health network in the city of Campinas, Brazil, were selected at random and interviewed. Analysis of the women's statements concerning previous RTI-related medical consultations showed that they did not recall having received any information on either prevention or the impact of RTI on their reproductive future. Analysis of interviews with physicians showed that the information they provided to women consulting for RTIs was incomplete and unclear. The information women recalled having received and that which physicians remembered having provided at the time of treating a patient with RTI was similar. In conclusion, these women lacked adequate or complete information and that it was probably not possible for them to adopt measures to avoid repetition of RTI and minimize risk of tubal infertility.

Para estimar a prevalência de transtornos do humor, foram utilizadas a entrevista estruturada, "Clinical Interview Schedule" (CIS-R), e a escala "Hospital Anxiety and Depression" (HAD) em 78 pacientes internados em uma enfermaria geral de adultos (43 homens e 35 mulheres, média de idade = 43,2 anos). Foi encontrada prevalência instantânea de 39% de transtornos do humor. Dezesseis (20,5%) pacientes preencheram critérios para ansiedade, a maioria dos casos sendo de gravidade leve. Vinte e seis (33%) casos de depressão foram detectados, 7 dos quais de gravidade moderada. Observou-se uma combinação de sintomas de preocupação, depressão, ansiedade e insônia. A HAD mostrou-se de fácil compreensão pelos pacientes. As subescalas de ansiedade e de depressão tiveram consistência interna de 0,68 e 0,77, respectivamente. A correlação dos itens com as respectivas subescalas sugere que essas possuem validades convergentes, não discriminantes. Com ponto de corte 8/9, a sensibilidade e a especificidade foram 93,7% e 72,6%, para ansiedade, e 84,6% e 90,3%, para depressão. Na prática clínica, a utilização da HAD poderia auxiliar na detecção de casos de transtornos do humor que necessitam de tratamento.

Objective: Suicide is one of the leading causes of mortality worldwide, especially among young subjects. Suicide is considered the outcome of a multidimensional and complex phenomenon, which is a result of the interaction between several factors. The association between psychopathology and suicide has been extensively investigated. Major depression plays an important role among the psychiatric diagnoses associated with suicide. This finding seems to be confirmed by different study designs, and in distinct populations. The present paper aims to briefly review the recent findings regarding the suicide-related clinical features of depression. Moreover, strategies for suicide prevention were also reviewed., Review: Recent references were identified and grouped in order to illustrate the main contributions about depression and suicide. Briefly, the literature review stresses the high prevalence of major depression among subjects presenting suicide behaviors. Psychopathological traits, such as aggression and impulsivity play a relevant role in triggering suicidal behaviors. Strategies for suicide prevention were also reviewed in Brazil and internationally. In general, detection and treatment are effective in reducing suicide rates., Conclusion: Studies regarding suicide behaviors have had a pragmatic approach, and generated a large body of evidence about correlates of suicide. However, these studies have not been able to provide a consistent theoretical explanation for this phenomenon. The recent adoption of modern strategies represents a possibility of enhancing the research capability of such studies. In order to be clinically useful, findings should make it possible to deepen the understanding over the experience of a suicidal person, as well as to design specific strategies for prevention and treatment in population subgroups.

Objective: Questionnaires for adequate assessment of the urinary function of neurological patients are not available in Portuguese. The Qualiveen, developed in France for evaluation of general and urinary-related impact on QoL, has been successfully used in neurological patients., Aim: To translate the Qualiveen, to adapt it crossculturally, and to validate it for Portuguese., Methods: Fifty-one patients with spinal cord injury (SCI), multiple sclerosis (MS), and myelomeningocele (MMC) completed both Qualiveen and the International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF) twice, at an mean interval of 15.5 days., Results: A reliability study revealed internal consistency (Cronbach's alpha coefficients 0.75-0.90 for all four domains). Test-retest reliability demonstrated moderate to strong stability (intraclass correlation coefficient range from 0.62 to 0.86). Construct validity was assessed using the Qualiveen's scores and ICIQ-SF final score. It was significant for the majority of Qualiveen's score domains compared with the ICIQ-SF final score., Conclusion: The Portuguese version of the Qualiveen was successfully adapted and validated. It can be used for evaluation of general and urinary QoL in neurological patients.

Introduction: The process of restructuring psychiatric care in South America began in the 80's and increased in the 90's. This process had its landmark in the Declaration of Caracas (1990), which reinforced the need for alternative services to the mental hospital., Objectives: to analyse the current situation of the psychiatric care in South America, with special emphasis on Psychiatric Units in General Hospitals (PUGHs)., Methods: Two mail surveys were carried out. Questionnaires were sent to all Ministries of Health, Psychiatric Societies and key informants., Results: All countries were represented in the returned questionnaires. In 7 countries there is a mental health plan. Material and human resources are inadequate in most countries. There has been a decrease in the total number of psychiatric beds over the last tem years. PUGHs were created in all the countries surveyed., Conclusions: All South America countries have a mental health program. A higher percentage of the programs is devoid of an evaluation system and almost all the countries lack budget estimates for mental health care. It was observed that the total number of psychiatric beds has been decreasing in most countries, and that the PUGHs and other decentralized centers were adopted as an alternative to the mental hospital.

Introduction: It is not clear as to the process by which the psychiatrist attracts referrals and the reasons and motivations that doctors have for referring or not. This study describes the modus operandi of referrals to the psychiatrist in the context of a university general hospital., Method: A semi-structured interview was carried out on 50 doctors (35 male, 15 female) from the teaching hospital of the Universidade Estadual de Campinas, Brazil. Data was analyzed qualitatively, by means of content analysis, according to the psychodynamic approach., Results: Some referrals need to be worked through in order for the doctor to overcome his resistance. Others are automatic: because something moved him deeply, the doctor directs his anguish and responsibility outside himself, without realizing the motives that lead him to do so., Conclusion: Referral is used in a variety of ways and different things are expected of the psychiatrist, all influenced by unconscious aspects of the patient-doctor-specialist triad.

A self-report instrument comprising twenty visual analogue scale statements about attitudes towards depression was sent to all 110 general practitioners working in primary health care centres in the city of Campinas, Brazil. The statements of the Depression Attitude Questionnaire (DAQ) cover three main areas: nature of depression, treatment preferences and professional reactions regarding depressed patients. Seventy-eight (71%) doctors returned the questionnaire. Forty-two percent of the doctors believe "it is difficult to differentiate whether patients are presenting with unhappiness or a clinical depressive disorder that needs treatment". Sixty-three percent believe that "antidepressants usually produce a satisfactory result in the treatment of depressed patients in general practice". However, most doctors would refer patients in need of anti-depressants (66%) or psychotherapy (60%) to a specialist. They also view the treatment of depression as "heavy going" (46%) and "unrewarding" (57%). These findings suggest a need for further education of general practitioners on the nature, diagnosis and management of depressive disorders.

The revised Clinical Interview Schedule (CIS-R) and the Hospital Anxiety and Depression (HAD) Scale were used to estimate the prevalence of mood disorders among 78 consecutive admissions to a general medical ward in a university general hospital in Brazil (43 males and 35 females; mean age = 43.2yr). Interviewers also completed a 5-point symptom severity scales for anxiety and depression. The definition of cases of anxiety [and depression] was based on two criteria: a. score > or = 2 on the CIS-R section of anxiety [> or = 4 on the CIS-R sections of depression and depressive ideas]; and b. score > or = 2 on the clinical severity scale for anxiety [score > or = 2 on the clinical severity scale for depression]. A 39% prevalence rate of affective disorders was found. Sixteen (20.5%) patients met criteria for anxiety, most of the disorders being of mild severity. Twenty-sic patients (33%) were depressed, 7 of them in a moderate degree. The HAD was easily understood by the patients. Anxiety and depression subscales had internal consistency of 0.68 and 0.77, respectively. At a cut-off point of 8/9 sensibility and specificity were 93.7% and 72.6% for anxiety, and 84.6% and 90.3% for depression. HAD items correlated positively with the respective subscales. To a lesser degree, they also correlated with the alternative subscale. Our findings confirm the high prevalence of mood disorders among medical in-patients. In clinical practice, the HAD may have a useful role in detecting those patients requiring further psychological care.

A mailing survey assessed consultation-liaison (C-L) service delivery and training among all 23 approved psychiatric residency training programs throughout the country. Twenty questionnaires were returned. Outpatient, inpatient, emergency, and consultation services were present together in 35% of the academic hospitals where residency training took place. Consultation service was provided by 18 (90%) responding programs. Eleven of those reported C-L training as part of their general psychiatry rotation. Nevertheless, educational objectives, weekly case conferences, and didactic seminars were provided by only five of them. The training time varied from 40 to 560 hours, 190 hours on average. The same five programs offered an elective third-year residency training, including liaison with specific units. The results illustrate the migration of academic psychiatric departments from the psychiatric hospital to the general hospital as well as the beginning of C-L training programs in Brazil.
