Background and Objectives: Effective communication in oncology is crucial, but challenging due to the complex information and emotional burden associated with a cancer diagnosis. This cross-sectional study investigated the communication preferences of 155 Romanian adults diagnosed with ENT cancers and explored the relationship between these preferences, their levels of psychological distress, and sociodemographic factors. Materials and Methods: Participants completed the KOPRA questionnaire, assessing communication preferences, and the Hospital Anxiety and Depression Scale (HADS) to measure psychological distress. Results: The results revealed that patients strongly prioritized active involvement in their care (Patient Participation and Patient Orientation—PPO) and open communication with healthcare providers (Effective and Open Communication—EOC). While emotional support was valued, it was considered less critical than PPO and EOC. Notably, communication about personal matters was deemed the least important aspect of communication. A high prevalence of psychological distress was observed, particularly among widowed individuals and females. No direct correlation was found between communication preferences and distress. Conclusions: These findings underscore the importance of shared decision-making, clear information exchange, and a patient-centered approach in the context of ENT cancer care, while also highlighting the need for routine screening and appropriate support for psychological well-being in this patient population. Background: Patients aged 80 years and above are at increased risk for severe COVID-19 outcomes. This study aimed to evaluate the prognostic utility of the derived neutrophil-to-lymphocyte ratio (dNLR), aspartate-aminotransferase-to-lymphocyte ratio index (ALRI), aspartate-aminotransferase-to-platelet ratio index (APRI), and systemic immune inflammation index (SII) in predicting severe disease, intensive care unit (ICU) admission, and mortality among COVID-19 patients aged 80 years and older. Methods: In this retrospective cohort study, 138 elderly patients (≥80 years) and 215 younger controls (<65 years) with confirmed COVID-19 were included. Laboratory data at admission were collected, and the dNLR, ALRI, APRI, and SII scores were calculated. Receiver operating characteristic (ROC) curve analysis was performed to assess the predictive performance of these indices. Results: The SII had the highest area under the ROC curve (AUC) for predicting severe disease in elderly patients (AUC = 0.857, 95% CI: 0.795–0.919, p < 0.001), with an optimal cutoff value of 920 × 10⁹/L (sensitivity 86%, specificity 78%). Elevated SII was significantly associated with increased risk of ICU admission (hazard ratio (HR): 2.9, 95% CI: 1.8–4.6, p < 0.001) and mortality (HR: 3.2, 95% CI: 1.9–5.2, p < 0.001). Similarly, dNLR showed good predictive value (AUC = 0.792, 95% CI: 0.722–0.862, p < 0.001). Conclusions: SII and dNLR are valuable prognostic biomarkers for predicting severe outcomes in COVID-19 patients aged 80 years and above. Background: The prognostic value of baseline inflammatory markers in sepsis remains controversial, with conflicting evidence regarding their association with mortality. The dynamic changes in these markers over time might offer additional insights into disease progression and patient outcomes. Methods: This retrospective observational study included 138 patients with severe infections. The inflammatory biomarkers procalcitonin (PCT), C-reactive protein (CRP), and lactate (LAC) were measured at three time points: upon hospital admission (baseline), approximately 24–48 h after admission (second measurement; M2), and 48–72 h after admission (third measurement; M3). The primary outcome was 30-day mortality. A Mann–Whitney U test was used to compare the biomarker levels between the survivors and non-survivors. A Spearman's correlation was used to assess the relationships between the baseline parameters. A logistic regression and a receiver operating characteristic (ROC) curve analysis were employed to evaluate the prognostic value of the baseline markers and their dynamic changes. Results: The baseline LAC and SOFA score were significantly associated with 30-day mortality. The percentage decrease in PCT, CRP, and LAC from the baseline to M3 emerged as strong predictors of survival, with the ROC curve analysis demonstrating superior discriminatory ability compared to the baseline values. CRP_Delta exhibited the highest AUC (0.903), followed by PCT_Delta (0.843) and LAC_Delta (0.703). Conclusions: The dynamic changes in these inflammatory biomarkers, particularly PCT, CRP, and LAC, offer valuable prognostic information beyond their baseline levels in predicting 30-day mortality in severe infections. The long-term sequelae of SARS-CoV-2 infection are still under research, since extensive studies showed plenty of systemic effects of the viral infection, extending even after the acute phase of the infection. This study evaluated kidney function tests six months after SARS-CoV-2 infection in patients clinically diagnosed with Post-COVID Syndrome, hypothesizing persistent renal dysfunction evidenced by altered kidney function tests compared to baseline levels. Continuous eGFR decrease <30 at six months post-infection was considered the main study outcome. Conducted at the "Victor Babes" Hospital, this retrospective observational study involved adults with laboratory-confirmed SARS-CoV-2 infection and clinically-diagnosed Post-COVID Syndrome, excluding those with prior chronic kidney disease or significant renal impairment. Kidney function tests, including serum creatinine, blood urea nitrogen (BUN), estimated glomerular filtration rate (eGFR), alongside markers of kidney damage such as proteinuria and hematuria, were analyzed. Among 206 participants, significant differences were observed between the control (n = 114) and the Post-COVID group (n = 92). The Post-COVID group exhibited higher serum creatinine (109.7 μmol/L vs. 84.5 μmol/L, p < 0.001), lower eGFR (65.3mL/min/1.73 m2 vs. 91.2 mL/min/1.73 m2, p < 0.001), and elevated BUN levels (23.7 mg/dL vs. 15.2 mg/dL, p < 0.001) compared to the control group. Regression analysis highlighted significant predictors of continuous eGFR decrease <30 at six months post-infection. The development of acute kidney injury (AKI) during the initial COVID-19 illness emerged as a strong predictor of reduced eGFR (β = 3.47, p < 0.001). Additional factors, including a creatinine increase (23 μmol/L above the normal range) and an elevated Albumin to Creatinine Ratio (ACR) (>11 mg/g above the normal range), were significantly associated with eGFR reduction. Patients with Post-COVID Syndrome demonstrate significant renal impairment six months post-SARS-CoV-2 infection. This longitudinal study investigates the psychosocial effects of long-COVID Syndrome, a domain still not extensively researched. It specifically evaluates the quality of life, coping mechanisms, anxiety and depression levels in COVID-19 survivors, differentiating between those with and without long-COVID Syndrome. Conducted at the Victor Babes Hospital for Infectious Diseases and Pulmonology in Timisoara, Romania, the study utilized a cohort of patients diagnosed with mild to moderate COVID-19. The following standardized tools: WHOQOL-BREF for quality of life, COPE-60 for coping strategies, and the Hospital Anxiety and Depression Scale (HADS), were employed for the assessment. The sample consisted of 86 patients displaying persistent post-acute symptoms and 432 asymptomatic patients at the 6-month post-discharge mark. Patients with frequent post-acute symptoms reported significantly higher levels of fatigue (8.2 ± 1.4), cognitive difficulties (7.5 ± 1.6), and respiratory challenges (7.8 ± 1.3), along with a markedly lower overall quality of life (7.0 ± 1.5) compared to their asymptomatic counterparts. HADS scores revealed elevated depression (6.8 ± 1.9) and anxiety (7.1 ± 2.3) in the symptomatic group. Quality of life, as evaluated through the use of WHOQOL-BREF, showed lower scores in the symptomatic cohort across physical (58.8 ± 15.8), mental (56.3 ± 16.4), and social domains (50.2 ± 17.5). COPE-60 findings indicated a higher prevalence of disengagement (56.4%) and emotion-focused coping strategies (61.8%) in the symptomatic group, in contrast to 30.1% and 37.0%, respectively, in the asymptomatic group. The study highlights that long-COVID Syndrome significantly deteriorates the quality of life and is associated with increased depression and anxiety levels. The prevalent use of disengagement and emotion-focused coping strategies among patients with persistent symptoms suggests a need for enhanced psychosocial support tailored to this subgroup. Currently, approximately 20,000 buffaloes are raised in Romania, of which 11% are found in the Făgăraș area. The present study was carried out to estimate the capacity of milk synthesis by 205 buffaloes cows raised in this area, which achieved 1059 lactations, with the rank of lactations from 1 to 14, in an observation interval of 15 years, by determining the correlation between the indicators of milk production and reproduction (lactation duration, total amount of milk of a lactation, total amount of fat, total amount of protein, interval between calvings and service period). The variability analysis shows a lack of homogeneity of the milk production and reproduction characteristics, regardless of the lactation rank, their average being moderately representative, except for the interval from calving to the fertile mount where the average is not representative. Reducing the variability of milk production and reproduction indicators can be achieved over time through management. Increasing the potential of milk production, in buffaloes cows, can be done through a simultaneous selection that takes into account the duration of lactation, the milk production from the control days and the service period. Currently, approximately 20,000 buffaloes are raised in Romania, of which 11% are found in the Făgăraș area. The present study was conducted to observe the effect of season on the components of raw milk obtained from buffaloes cows reared in this region. 320 milk samples were collected during the morning milking, during the grazing and stalling seasons, from 80 buffaloes cows in different stages of lactation. The fat, protein, lactose, non-fat dry matter (NFS), density and pH content were determined from the collected samples. Microbiological determinations mainly considered somatic cell count (NSC) and total germ count (NTG). The individual analysis of milk samples from buffaloes revealed significant differences in terms of the variation of these parameters, the researches carried out highlighted differences determined by the breeding system of the animals involved in the study, the feeding regime, as well as the reference season. Therefore, the results of the present research indicated that the season and stage of lactation influence the physicochemical and microbiological parameters of milk and could be minimized by better management practices. Abstract Background Internal control genes with highly uniform expression throughout the experimental conditions are required for accurate gene expression analysis as no universal reference genes exists. In this study, the expression stability of 24 candidate genes from Triticum aestivum cv. Cubus flag leaves grown under organic and conventional farming systems was evaluated in two locations in order to select suitable genes that can be used for normalization of real-time quantitative reverse-transcription PCR (RT-qPCR) reactions. The genes were selected among the most common used reference genes as well as genes encoding proteins involved in several metabolic pathways. Findings Individual genes displayed different expression rates across all samples assayed. Applying geNorm, a set of three potential reference genes were suitable for normalization of RT-qPCR reactions in winter wheat flag leaves cv. Cubus: TaFNRII (ferredoxin-NADP(H) oxidoreductase; AJ457980.1), ACT2 (actin 2; TC234027), and rrn26 (a putative homologue to RNA 26S gene; AL827977.1). In addition of these three genes that were also top-ranked by NormFinder, two extra genes: CYP18-2 (Cyclophilin A, AY456122.1) and TaWIN1 (14-3-3 like protein, AB042193) were most consistently stably expressed. Furthermore, we showed that TaFNRII, ACT2, and CYP18-2 are suitable for gene expression normalization in other two winter wheat varieties (Tommi and Centenaire) grown under three treatments (organic, conventional and no nitrogen) and a different environment than the one tested with cv. Cubus. Conclusions This study provides a new set of reference genes which should improve the accuracy of gene expression analyses when using wheat flag leaves as those related to the improvement of nitrogen use efficiency for cereal production.

The metabolic profile test in animals is used to assess their nutritional status, to diagnose disease and assess fertility. The aim of this paper was to investigate the metabolic profile in buffalo cows. Forty buffaloes from the Research and Development Station for Buffaloes Secaia -Romania, were divided into two groups (20 heads/group). Estrus induction protocols were applied to the first group (G1), while the second group consisted of buffalo cows that express natural oestrus (G2). When the artificial insemination was performed, blood samples were taken and the following parameters were determined: hemoglobin, total serum protein, phosphorus, calcium, magnesium, alkaline phosphatase, cholesterol, glucose, lipase, urea. G2buffalocows had significantly higher concentrations of hemoglobin, serum protein, glucose and inorganic phosphorus, while the serum calcium levels were lower in the group. The concentration of magnesium, alkaline phosphatase, cholesterol, lipase and urea did not differ significantly between the buffalo groups (p>0.05). The Ca:P ratio for G2buffaloes was lower (1.94:1) compared to G1buffaloes (2.89:1). It can be concluded that the level of hemoglobin, serum protein, glucose, calcium and inorganic phosphorus plays a major role in the reproduction of buffaloes. [ABSTRACT FROM AUTHOR]

The development of reproductive biotechnologies, such as oestrus synchronization (OS) associated with artificial insemination (AI) in buffaloes, could have a significant impact on milk yields obtained at farm level. The aim of this paper was to study the effects of OS and fixed-time AI in buffaloes. Thirty buffalo cows with anoestrus were divided into three groups L1, L2 and L3 (10/group) at the Research and Development Station for Buffaloes Sercaia. The following protocols were applied: L1 - GnRH on day 1, prostaglandins on day 7, GnRH on day 9, followed by AI at 10 days (resulted in 50% pregnancy rate); L2 - GnRH was administered on day 1, prostaglandin was administered on day 7, GnRH was administered on day 9, AI was performed at 48 hours after the prostaglandin administration (resulted in 80% pregnancy rate); L3 - prostaglandin was administered on day 1, GnRH was administered on day 12, prostaglandin was administered on day 19, GnRH was administered on day 21, within 17-24 hours after last administration of GnRH AI was performed (resulted in 50% pregnancy rate). Large calving intervals in water buffalo cows is attributed to the existence of periods of the year when sexual activity is absent. It is considered that if fertilization did not occur before the buffalo cows entered in the winter season, when kept indoors, then it will take a significant longer time until they enter again in heat and the calving interval will be negatively influenced. The work was conducted over 4 years and consisted of the evaluation of ovarian response following administration of a prostaglandin hormone (PGF2α) during the winter period for a number of 61 buffalo cows, between 5 and 14 years of age. In 22 buffalo cows that showed oestrus within 5 days of PGF2α administration, two artificial inseminations were performed at 12 hours intervals. In 39 buffalo cows that did not show any signs of oestrus, a second dose of PGF2α was administrated 11 days after the first administration, after which they were artificially inseminated at 48 and 78 hours, respectively. Of the 22 buffalo cows that showed oestrus, 19 cows became pregnant, and out of the 39 buffalo cows with prolonged treatment, only 20 heads remained pregnant. The treatment of sexual inactivity in water buffalo cows during indoor hosing by hormonal methods with PGF2α leads to the shortening of the calving interval, the cost of hormonal treatments being balanced by the increase in the number of calves obtained and the milk production. Parasite infections are one of the major constraints for profitable animal industry worldwide. Despite of the importance of water buffalo (Bubalus bubalis) in the overall economy of many countries, little is known about the occurrence and prevalence of parasitic species and its potential impact on the animal health and productivity. Therefore, the present study aimed to determine the endo-parasitophauna in a water buffalo farm in Romania. For this, during of April 2018, a coprological study was conducted, in which a total number of 105 buffaloes, allocated in four age category (< 6 months, n = 21; 6 months-2 years, n = 20; heifers, n = 30; milking, > 3 years, n = 34) were included. Fecal samples, collected from individual animals, were subjected for analysis by standard qualitative copro-parasitological methods to detect parasitic stages (protozoan oocysts, helminth eggs and larvae). Overall, 55.2% of samples were positive for parasite infection, of which, 36.2% as single parasite species and 18.9% with mixed infections of two (13.8%) and three (5.2%), respectively, parasite species. The endoparasite community included protozoa (Eimeria spp., Giardia intestinalis), trematodes (Fasciola hepatica, Paramphistomum spp., Dicrocoelium dendriticum), tapeworms (Moniezia spp.), and nematodes (lung-worms and digestive strongyles), with different prevalence value, according to the age category. The aim of this study was to investigate the effect of GnRH and PGF2α administration on the manifestation of estrus and on the conception rate in Romanian buffalo dairy buffaloes. Twenty-eight dairy buffaloes from the Şercaia Buffalo Breeding Research and Development Station, aged between 5 and 12, in the postpartum period, were divided into four batches of seven heads each, as follows: Lot I (between 25 and 30 postpartum days), Lot II (40 to 45 days postpartum), Lot III (55 to 60 days postpartum) and Lot IV (75 days postpartum). Dairy buffaloes were given intramuscularly GnRH (2.5 ml Receptai) on day 0, followed by intramuscular injection of PGF2α (2.5 ml Alfabedyl) on day 7 after GnRH injection. Estrus was observed 24-48 hours after PGF2α injection. Artificial insemination was performed 12-18 hours after visible signs of estrus. The results of the study showed that there were no differences (p> 0.05) between estrus treatments (100%), estrus onset (35.8 to 36.4 hours), estrus duration (17 to 18 hours) and conception rate (100%). The knowledge of haematological and urine biomarkers is useful to diagnose various metabolic and pathological disorders, which have a negative impact on the overall performances of dairy species. The aim of this study was to investigate the haematological and urine parameters in lactating dairy cows comparative with lactating buffaloes. The study was carried out on sixty-eight Romanian Black and Spotted cattle and on fifty Romanian buffalo. The obtained values for hemoglobin concentration (HGB), hematocrit percentage (HCT), white blood cells count (WBC), lymphocytes percentage (LY), monocytes percentage (MO), and neutrophil percentage (NE), varied significantly (p<0.01) between cows and buffaloes. For other haematological indices, such as mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCH), and mean corpuscular hemoglobin concentration (MCHC), statistical differences were recorded. Urine examination of cows and buffaloes showed that all the parameters studied were within the normal physiological limits, with minor fluctuations for protein level (30 mg/dl). Metabolic profile is used to detect possible nutritional errors with negative effects on animal's production. The aim of this study was to establish some reference values of blood constituents for lactating buffaloes related to the season. Thus, 15 buffalo females, clinically healthy, from the S.C.D.C.B. Sercaia-Brasov were screened for hematological and biochemical blood profile during the summer and the winter period. Feeding system was differentiated by season (conserved forage and corn fodder in the winter season and green mass in the summer season). The results for hematological parameters revealed no significantly statistical differences (p>0.05) between seasons. The average values of the determined biochemical parameters (total cholesterol, serum levels of Mg) had values within the normal range, with no statistically differences between seasons. Total protein level varied significantly distinct (p<0.01) between the winter season 7.02±1.13 mg/dL and the summer season 8.20±0.92 mg/dL. The values obtained for the mineral profile (serum Ca and inorganic P) varied significantly (p<0.05) between seasons. The level of PAL and GOT enzymes registered values above normal physiological limits, the activity of these enzymes being considered as an indicator for acute liver necrosis. The work aims to study the economic status of some buffalo farms in Fagaras Area. These information are absolutely necessary for developing a program for active conservation of Indigenous Romanian Buffalo. Biological and economic efficiency is an objective of any farm to obtain expected benefits. Lower production costs are a goal of all producers in the field. In accordance with the purpose, they were followed two aspects: study the influence of farm size on parameters that influence production costs and economic efficiency of the unit; to establish to what extent the economic efficiency of the farm is influenced by how the production is harnessed. Whatever the size of the farm, milk and Telemea cheese are produced under conditions of total economic inefficiency. It appears advisable to increase the global production of milk at the farm level. Increasing the volume and quality of milk production and diversification of products offered for sale are efficient ways to minimize the cost per unit of product. Diversification of production at farm level should be a strategic objective of buffaloes exploitation in Romania in order to preserve the genetic resources and biodiversity. [ABSTRACT FROM AUTHOR]

Fertility in buffalo is low. That could be a question of species adaptation in new climate or a genetic trait. Scientists think buffaloes entered the South-est of Europe on two ways one with the Hune and Avar people coming trough the North of Black Sea up to the intra Charpathian Hils and a later one more prolonged with penetratio trough Egypt, Small Asia and Greece at the time when some people migrated up to the Danube River. Buffaloes which originated from Egypt were domesticated before the ones that came through the North of Black Sea. That means in Romania there are two different buffalo populations: the Carpathian Buffalo adapted to reproduce in a cold and wet climate, outside the area of its wild ancestors and a Danube Buffalo that underwent more influence domestication apart. These two populations reproduced separately by territorial state insulation. All buffalo breeds including the ones in the native domestication area show grouped calving. The present paper has to answer if grouped calving in buffalo is due to photoperiodic exchange along the year or to the poor feeding in some seasons. Aim of this study was to investigate the blood parameters in Carpathian Romanian Buffaloes in order to establish their physiological ranges. Eight buffaloes from Şercaia-Braşov area were screened for hematological and biochemical profile during the winter period. Animals were fed on hay and beet pulp. Hematological parameters (hemoglobin, red blood cells, white blood cells, hematocrit, lymphocytes, monocytes, neutrophils) and biochemical parameters (including total proteins, urea, cholesterol, alkaline phosphatase, aspartate transaminase, alanine transaminase, total calcium, inorganic phosphorus and magnesium) were assessed. Preliminary results obtained after hematological examination were included within the following limits: 12,6-16,9 g/dL for hemoglobin; 6,48-9,51 x106/µL for red blood cells; 35,23-45,65% for hematocrit; 4,04-9,57 x103/µL for white blood cells; 45,3-76,8% for lymphocytes; 0,7-9,2% for monocytes; 22,5-45,9% for neutrophils. Results obtained after biochemical examination were included within the following limits: 4,0-9,2 g/dL for total proteins; 12-30 mg/dL for urea; 47-86 mg/dL for cholesterol; 92-171 U/L for alkaline phosphatase; 23-48 U/L for aspartate transaminase; 22-46 U/L for alanine transaminase; 5,0-9,3 mg/dL for total calcium; 1,3-3,9 mg/dL for inorganic phosphorus; 1,27-3.17 mg/dL for magnesium. Management of cryptococcal infections among patients suffering from acquired immunodeficiency syndrome (AIDS) represents a medical challenge. This retrospective study aims to describe the disease management and outcomes among 24 AIDS patients who suffered from Cryptococcus neoformans meningitis. The parameters evaluated from our patients' database records include epidemiological data, clinical manifestations, biochemical and microbiological analysis of patients' cerebrospinal fluid (CSF), treatment profiles, and disease outcomes. All patients included in the study had a lymphocyte count of less than 200 CD4/mm3. Of the 24 patients included in this study, five had been diagnosed with HIV infection since childhood, after receiving HIV-infected blood transfusions. The most prominent symptom was fatigue in 62.5% of patients, followed by nausea/vomiting and headache. Seven patients had liver cirrhosis due to hepatitis B virus (HBV) or hepatitis C virus (HCV) infection, while Kaposi sarcoma and cerebral toxoplasmosis were found in two patients. Six out of 24 patients died due to bacterial sepsis and acute respiratory distress syndrome (ARDS). High intracranial pressure was the strongest predictive factor for mortality (OR = 2.9), followed by ARDS (OR = 1.8), seizures at disease onset (OR = 1.4), and diabetes mellitus (OR = 1.2). Interestingly, patients younger than 40 years old had a significantly lower survival rate than that of the older patients. Before developing Cryptococcal meningitis, all patients had low adherence to the early ART treatment scheme and skipped the follow-up visits. All patients received a combination of amphotericin B and flucytosine as induction therapy, adding fluconazole for maintenance. Simultaneously, AIDS HAART was initiated at diagnosis of the cryptococcal infection. There is considered that if buffalo cow fertilization has not occurred before entering the barn to be protected against the cold weather of the winter she will get in anestrous state and new heats will delay for a long time. The present paper refers to the results of a prostaglandin PGF2a derivate treatment of 44 cases of after calving anestrous in buffalo cows registered during the housing period in the last 4 years. The buffalo cows aged between 6 and 14 years. The time interval from the former last heat was over 90 days. The treatment consists of intra muscular injection 2 ml of one of cloprostenol derivates. After the first injection 15 buffalo cows out of 44 ones came in heat within 5 days. All of them were two times inseminated artificially at a 12 hours interval. Out of them 12 buffalo cows became pregnant. The other 29 buffalo cows have received a second doze of prostaglandins 11 days after the first treatment. All of them have received blind insemination after 48 and 72 hours from the second treatment. This time 9 buffalo cows became pregnant. Result of the treatment has to be considered as a good one since 24 cows out of 44 treated cases (54, 55%) became pregnant. Perhaps 14 buffalo cows didn't react to the treatment. Nevertheless the price of the 73 doses of prostaglandin used for the treatment is lower compared to the feeding cost saved by shortening the buffalo cows what became pregnant. [ABSTRACT FROM AUTHOR]
