Amaç: Bu çalımada , bebeklerde ve okul öncesi yaş gruplarındaki çocuklarda mini perkütan nefrolitotomi (Mini-PCNL) uygulanan başarı oranını ve komplikasyonları, kompleks böbrek taşları için iki farklı boyutta traktlarla karşılaştırdık.Materyal ve Metot: Ocak 2002 ile Haziran 2018 tarihleri arasında mini PCNL uygulanan 7 yaşından küçük 260 pediatrik hastanın kayıtları geriye dönük olarak araştırıldı. Karmaşık böbrek taşı hastalığı olan yüz kırk dokuz okul öncesi çocuk ve bebek (66 kız ve 83 erkek) dahil edildi. Çalışmada toplam 158 mini-PCNL gerçekleştirildi ve iki farklı gruba ayrıldı (14Fr grubu, n = 104 [% 69.8]; 20Fr grubu, n = 45 [% 30.2]).Bulgular: Ortalama hasta yaşı 14Fr ve 20Fr gruplarında sırasıyla 3.5 ± 2.0 ve 5.2 ± 2.2 yıl idi (p = 0.158). Hemoglobin düşüşünde (0.73 gr / dL'ye karşı 1.0 gr / dL, p =, Aim: We compared success rate and complications in infants and preschool-aged children who had undergone mini-percutaneous nephrolithotomy (Mini-PCNL) with two different sizes of tracts for complex renal calculi.Materials and Methods: Two thousand sixty pediatric patients younger than 7 years old who had undergone mini-PCNL between January 2002 and June 2018 were evaluated retrospectively. One hundred forty-nine preschool-aged children and infants (66 girls and 83 boys) with complex kidney stone disease were included. A total of 158 mini-PCNL were performed in the study. These were classified into two groups (the 14Fr group, n=104 [69.8%]; the 20Fr group, n=45 [30.2%]). Historically, transurethral resection of the prostate has been the gold standard for the treatment of benign prostatic hyperplasia (BPH). Laser technology has been used to treat BPH for > 15 years. Over the past decade, it has gained wide acceptance by experienced urologists. This review provides an evidence-based update on laser surgery for BPH with a focus on photoselective laser vaporization and holmium laser enucleation of the prostate surgeries and assesses the safety, efficacy, and durability of these techniques.

Amaç:Kliniğimizde prostat kanseri şüphesiyle transrektal prostat iğne biyopsisi yapılan hastaların verilerinin retrospektif olarak değerlendirilmesi amaçlanmıştır.Gereç ve yöntem:Şubat 2003 ve Ekim 2007 tarihleri arasında kliniğimize başvuran, TRUS ve DRE' de anormal prostat bulguları ve/veya PSA yüksekliği nedeniyle prostat kanseri şüphesi taşıyan 1202 hastadan 230' u rebiyopsi olmak üzere toplam 1432 biyopsi alındı. İşlem; ilk biyopsilerde 10 çekirdek biyopsi kalıbı kullanılarak, periprostatik lokal anesteziyle, TRUS kılavuzluğunda yapıldı. Rebiyopsilerde ise transizyonel zonlar da biyopsiye dahil edilerek 12 odaktan toplam 22 biyopsi alındı. Ancak; anormal rektal inceleme bulgusu varsa, önceki biyopsi sonuçları, prostat boyut ve konfigürasyonu ve ultrasondaki şüpheli lezyonların durumuna göre fazladan biyopsiler de eklendi.Bulgular:Histopatolojik inceleme sonucu prostat kanseri tanısı alan hastalarda; yaş ortalaması 68.4±17, ortalama PSA değeri 172.4±1317.2 ng/ml, prostatit rapor edilen hasta grubunda; ortalama PSA değeri 12.5±24.8 ng/ml, yaş ortalaması 62.8±16.8, BPH rapor edilen hasta grubunda ise; ortalama PSA 10.0±17.8 ng/ml, yaş ortalaması 62.5±15.3 bulundu. Toplam 408 biyopside (% 28.5), hastaların % 33.9' unda prostat kanseri tespit edilmiştir. Biyopsi pozitiflik oranları; sadece PSA yüksekliğinde % 17.4, sadece DRE bulgusunda % 24.2, PSA yüksekliği + DRE bulgusunda % 60.7, TRUS' da hipoekoik nodül varlığında % 42.4, DRE bulgusu + TRUS' da hipoekoik nodül varlığında % 57.7, PSA yüksekliği + TRUS' da hipoekoik nodül varlığında % 43.3, PSA yüksekliği + DRE bulgusu + TRUS'da hipoekoik nodül varlığında ise % 66.7 olarak saptanmıştır. İlk biyopside prostat kanseri tespit oranı % 30, rebiyopsilerde % 20.4 olarak hesaplanmıştır. Rebiyopsilerde biyopsi pozitiflik oranı en fazla % 40' lık oranla ilk biyopsilerinde atipi izlenip, anormal DRE bulguları olan hasta grubunda izlenmiştir. Biyopsi pozitif grupta biyopsi negatif gruba göre; yaş, PSA, PSAD ve transizyonel zon PSAD anlamlı olarak yüksek, toplam prostat hacmi anlamlı olarak düşük bulunmuştur. Biyopsi Gleason skorları karşılaştırmasında RRP sonrasında hesaplanan Gleason skorları biyopside bildirilen değerlerden % 39.2 hastada yüksek, % 22.8 hastada düşük, % 30 hastada ise eşit tespit edilmiştir. Rebiyopsilerde VAS düzeylerinin arttığı görüldü. TRİB' ne bağlı komplikasyonlardan en sık % 42 oranla hematüri gözlendi ve alınan parça sayısının artmasıyla; hematüri, rektal kanama ve üretroraji görülme oranının belirgin şekilde artığı saptandı.Sonuç:Prostat kanseri kesin tanısında; DRE, serum total PSA ölçümü veya TRUS' da hipoekoik lezyonların tespiti gibi metodların hiç biri tek başına yeterli duyarlılık ve özgüllükte olmadığından TRİB, halen altın standarttır ve günümüzde güvenli kabul edilen, genellikle ayaktan hasta koşullarında uygulanabilen bir işlemdir. Serum total PSA değeri yüksek olan hastalarda kanser saptama oranlarımızın düşük olması bizi biyopsi yapmayı düşünmekten vazgeçirmemelidir. Bu hasta grubunda aksi ispatlanıncaya kadar prostat kanseri riski olduğu her zaman akılda tutulmalıdır. DRE muayenesi normal olan hastalarda prostat biyopsisi endikasyonu için PSA eşik değerinin 2.5 ng/ml' ye düşürülmesi gerekli gibi görünmektedir ancak bu konuda net mesajlar vermek için çok sayıda çalışmaya ihtiyaç olduğu da bir gerçektir. ABSTRACTPurpose:It is aimed in our clinic to analyzed that patients who had undergone transrectal prostate needle biopsy with the suspect of prostate cancer retrospectively.Materials and Methods:Between February 2003 and October 2007, transrectal prostate needle biopsy performed in 1202 patients with abnormal digital rectal examination (DRE) and transrectal ultrasonography of the prostate (TRUS) findings and/or serum total prostate specific antigen (PSA) level. 230 rebiopsy and total 1432 biopsy were taken with periprosthatic local anesthesia. Process; at the first biopsy it has been done by using 10 cores biopsy model at TRUS guide, at the rebiopsy 22 biopsy was taken from 12 cores by adding the transitional zones to the biopsy. But according to the suspicious lesions in TRUS and abnormal DRE findings, more biopsy was added.Results:At the and of the histopathological examination it was found that in the patients who had prostate cancer; the mean age was 68.4±17 and mean PSA value was 172.4±1317.2 ng/ml in the patients group who had prostatitis; the mean age was 62.8±16.8 and mean PSA value was 12.5±24.8 ng/ml in the patients group who had reported with benign prostatic hyperplasia (BPH) the mean age was 62.5±15.3 and mean PSA value was 10.0±17.8 ng/ml. 408 cases (28.5%), 33.9% of the patients, had prostate cancer. Positive biopsy rates; it has been determined that in the height of PSA 17.4%, in the DRE finding 24.2%, in PSA height + DRE finding 60.7%, in TRUS with the hypoechoic nodule 42.4%, in DRE finding + TRUS with hypoechoic nodule 57.7%, in PSA height + TRUS with the hypoechoic nodule 43.3%, in PSA height + DRE finding + TRUS with the hypoechoic nodule 66.7%. It has been accounted that determination prostate cancer rate in the first biopsy is 30%, in rebiposy 20.4%. In biopsy positive group PSA, PSAD and transitional zone PSAD has been found meaningfully higher than in negative group but total prostate volume lower than in negative group. With the comparision biopsy Gleason scores, after RRP the values which have been informed in biopsy, the accounted Gleason scores have been observed high in 39.2% patients, low in 22.8% patients and equal in 30% patients.Conclusions:In the certain diagnosis of prostate cancer; none of the methods such as DRE, total serum PSA measurements or TRUS has sufficient sensitive and specific. Because of this, TRİB is still golden standart and nowadays it is expacted safety and generally practice process. In the patients who have high serum total PSA levels, even the cancer diagnosis rate is low, it doesn?t have to make us give up our thoughts to make biopsy. In this patients group it has to be thought that always there is prostate cancer risk until the opposite is proved. In the who have normal DRE findings, it seems necessary to decrease the PSA threshold value 2.5 ng/ml, but it is true that lots of studies needed to give clear messages in this subject. 68
