Abstract

To evaluate the effects of rehabilitation (physical and cognitive) treatments on the diagnosis severity and Disability Rating Scale (DRS) scores, adjusted for a number of potential confounders measured at baseline, in a large cohort of patients with disorders of consciousness across time.An observational, longitudinal (2 evaluations), multicenter project was made in 90 Italian centers.Patients (N=364) with a diagnosis of disorders of consciousness.Primary outcome was the severity of diagnosis, expressed on an ordinal scale (OtherMCSVSdeath). In the Other group were included patients who emerged from an MCS and recovered consciousness. The secondary outcome was the DRS score (range of 0-30 with 30 being the worst value). The DRS is a tool used to define the level of residual disability, commonly used to classify the level of functional impairment in patients with acquired brain injury. Both outcomes were measured for each wave.A total of 364 subjects having a complete set of demographic, clinical, and pharmacologic data were included in the propensity score (PS) analysis. Results showed that the rehabilitation treatments (physical and cognitive) reduced the clinical worsening over time in both severity diagnosis and DRS (around 6.5 points) in patients with disorders of consciousness across different propensity score strategies (ie, PS matching, PS adjustment, and PS-weighted procedures). In addition, cognitive protocols seem to be limited to patients with a median value of DRS=23.Our propensity score analysis suggests that rehabilitation treatment protocols seem effective and should be applied to a broader spectrum of patients with disorders of consciousness.

Abstract

The Proton-Ion Medical Machine Study (PIMMS) group was formed following an agreement between the Med-AUSTRON (Austria) and the TERA Foundation (Italy) to combine their efforts in the design of a cancer therapy synchrotron capable of accelerating either light ions or protons. CERN agreed to support and host this study in its PS Division. A close collaboration was also set up with GSI (Germany). The study group was later joined by Onkologie-2000 (Czech Republic). Effort was first focused on the theoretical understanding of slow extraction and the techniques required to produce a smooth beam spill for the conformal treatment of complex-shaped tumours with a sub-millimetre accuracy by active scanning with proton and carbon ion beams. Considerations for passive beam spreading were also included for protons. The study has been written in two parts. The more general and theoretical aspects are recorded in Part I and the specific technical design considerations are presented in the present volume, Part II. An accompanying CD-ROM contains supporting publications made by the team and data files for calculations. The PIMMS team started its work in January 1996 in the PS Division and continued for a period of four years.

Abstract

The recently endorsed World Health Organization (WHO) International Classification of Functioning, Disability and Health (ICF) is proposed as a new tool to describe human functioning and health in a multiprofessional approach for individuals of all ages. Its application to paediatric neurorehabilitation may be of great help, especially in children with complex disabilities. However, experience with its application in this field is still limited. We tested the application of ICF in 40 children (26 males, 14 females; age range 3 to 18 years; mean age 11 years 1 month, SD 5 years 3 months) with various types and degrees of disability. We tested the applicability and reliability of the classification, and studied its correlation with well-established measures of functioning (Verbal IQ; gross motor function measure; functional independence measure). The ICF proved to be applicable and reliable, and strongly correlated with established scales. However, several of the Activity and Participation components do not fully capture the developmental nature of many abilities of children. Our study, although acknowledging the universal application of the ICF, and the ICF's value as a clinical tool, calls for its specific adaptation to accommodate better the peculiarities of child functioning and disability. Abstract

The interaction of human and bovine serum albumin with Cibacron Blue and Blue Dextran in aqueous solution was studied by means of difference spectroscopy. Both human and bovine albumin interact strongly with underivatized Cibacron Blue in three independent binding sites (K = 105). On the contrary, Blue Dextran interacts strongly only with human albumin, but does not bind appreciably to bovine albumin. These results suggest that the binding sites are exposed and easily accessible in human albumin, while in bovine albumin they are sterically hindered and therefore not accessible to the bulky Blue Dextran.

Abstract

When purified preparations of human chorionic somatomammotropin (HCS) were examined by polyacrylamide electrophoresis in the presence of mercaptoethanol, four new fast-moving bands were observed, in comparison with the pattern obtained in the absence of the reducing agent. We have found that the new bands correspond to fragments due to the presence of two broken bonds inside the main loop of the HCS chain, and one of these breakage points has been located at Leu 100. It has also been shown that the nicked form corresponds to the slow-moving band in the electrophoretic pattern, obtained in the absence of mercaptoethanol. The nicked form of HCS can be attributed to artefacts during the preparation or to metabolic events. Comparison with HCS prepared by affinity chromatography from retroplacental blood seems to indicate that the former hypothesis is correct.

Abstract

With the advent of recombinant DNA techniques and the identification of closely linked DNA markers, it is now possible to determine whether at-risk subjects have inherited the Huntington's disease (HD) gene. Since the presymptomatic testing is only applicable to those at-risk subjects with diagnosed genetic haplotype segregating with HD in their families, neurologists, neuropsychologists and neurophysiologists purpose to correlate clinical evidences and genetic data. At the present time using neuropsychological tests, nonspecific abnormalities have been observed in visuospatial abilities, memory and in functions associated with the frontal lobes; still the general reduction of visual evoked potential amplitude has to be considered a very interesting finding in the at-risk subjects with probability of having inherited the HD gene.
