1. With-In Child Protective Factors among Rural Head Start Children

Abstract

This analysis examined key protective factors among preschool children in low-income, rural areas. Teacher-reported Devereux Early Childhood Assessments for Preschoolers were completed for 182 Head Start children (54% female, 46% male) from seven rural, midwestern communities. The majority of children were in the typical range for each protective factor and for behavioral concerns. In comparison to a standardized sample, the rural sample had lower mean scores for all protective factors. Females had statistically significantly higher scores for initiative, self-regulation, attachment/relationships, and total protective factors, while having lower behavioral concerns than males. T-scores for initiative were statistically significantly higher for 4-year-olds than for 3-year-olds. Statistically significant negative correlations were also found between behavioral concerns and each protective factor, with the strongest negative correlation occurring with self-regulation. There was no statistically significant difference in T-scores between the fall (i.e., Time 1) and spring (i.e., Time 2). This analysis adds to existing literature by delineating the prevalence of with-in child protective factors among preschool children in rural, low-income communities and identifying areas in which these children are most in need of additional support.

Abstract

Background: Tuberculosis diagnosis might be delayed or missed in children with severe pneumonia because this diagnosis is usually only considered in cases of prolonged symptoms or antibiotic failure. Systematic tuberculosis detection at hospital admission could increase case detection and reduce mortality. Methods: We did a stepped-wedge cluster-randomised trial in 16 hospitals from six countries (Cambodia, Cameroon, Côte d'Ivoire, Mozambique, Uganda, and Zambia) with high incidence of tuberculosis. Children younger than 5 years with WHO-defined severe pneumonia received either the standard of care (control group) or standard of care plus Xpert MTB/RIF Ultra (Xpert Ultra; Cepheid, Sunnyvale, CA, USA) on nasopharyngeal aspirate and stool samples (intervention group). Clusters (hospitals) were progressively switched from control to intervention at 5-week intervals, using a computer-generated random sequence, stratified on incidence rate of tuberculosis at country level, and masked to teams until 5 weeks before switch. We assessed the effect of the intervention on primary (12-week all-cause mortality) and secondary (including tuberculosis diagnosis) outcomes, using generalised linear mixed models. The primary analysis was by intention to treat. We described outcomes in children with severe acute malnutrition in a post hoc analysis. This study is registered with ClinicalTrials.gov (NCT03831906) and the Pan African Clinical Trial Registry (PACTR202101615120643). Findings: From March 21, 2019, to March 30, 2021, we enrolled 1401 children in the control group and 1169 children in the intervention group. In the intervention group, 1140 (97·5%) children had nasopharyngeal aspirates and 942 (80·6%) had their stool collected; 24 (2·1%) had positive Xpert Ultra. At 12 weeks, 110 (7·9%) children in the control group and 91 (7·8%) children in the intervention group had died (adjusted odds ratio [OR] 0·986, 95% CI 0·597–1·630, p=0·957), and 74 (5·3%) children in the control group

Abstract

Perspectives of cancer survivors, caregivers, and social workers as key stakeholders on the clinical management of financial toxicity (FT) are critical to identify opportunities for better FT management. Semi-structured interviews (cancer survivors, caregivers) and a focus group (social workers) were undertaken using purposive sampling at a quaternary public hospital in Australia. People with any cancer diagnosis attending the hospital were eligible. Data were analysed using inductive-deductive content analysis techniques. Twenty-two stakeholders (n = 10 cancer survivors of mixed-cancer types, n = 5 caregivers, and n = 7 social workers) participated. Key findings included: (i) genuine concern for FT of cancer survivors and caregivers shown through practical support by health care and social workers; (ii) need for clarity of role and services; (iii) importance of timely information flow; and (iv) proactive navigation as a priority. While cancer survivors and caregivers received financial assistance and support from the hospital, the lack of synchronised, shared understanding of roles and services in relation to finance between cancer survivors, caregivers, and health professionals undermined the effectiveness and consistency of these services. A proactive approach to anticipate cancer survivors' and caregivers' needs is recommended. Future research may develop and evaluate initiatives to manage cancer survivors and families FT experiences and outcomes.

21. A Character-Level Approach to the Text Normalization Problem Based on a New Causal Encoder

Abstract

Text normalization is a ubiquitous process that appears as the first step of many Natural Language Processing problems. However, previous Deep Learning approaches have suffered from so-called silly errors, which are undetectable on unsupervised frameworks, making those models unsuitable for deployment. In this work, we make use of an attention-based encoder-decoder architecture that overcomes these undetectable errors by using a fine-grained character-level approach rather than a word-level one. Furthermore, our new general-purpose encoder based on causal convolutions, called Causal Feature Extractor (CFE), is introduced and compared to other common encoders. The experimental results show the feasibility of this encoder, which leverages the attention mechanisms the most and obtains better results in terms of accuracy, number of parameters and convergence time. While our method results in a slightly worse initial accuracy (92.74%), errors can be automatically detected and, thus, more readily solved, obtaining a more robust model for deployment. Abstract

During calibration, an eye-tracker fits a mapping function from features to a target gaze point. While there is research on which mapping function to use, little is known about how to best estimate the function's parameters. We investigate how different fitting methods impact accuracy under different noise factors, such as mobile eye-tracker imprecision or detection errors in feature extraction during calibration. For this purpose, a simulation of binocular gaze was developed for a) different calibration patterns and b) different noise characteristics. We found the commonly used polynomial regression via least-squares-error fit often lacks to find good mapping functions when compared to ridge regression. Especially as data becomes noisier, outlier-tolerant fitting methods are of importance. We demonstrate a reduction in mean MSE of 20% by simply using ridge over polynomial fit in a mobile eye-tracking experiment.

Abstract

Cardiac four-dimensional phase-contrast magnetic resonance imaging (4D PC-MRI) acquisitions have gained increasing clinical interest in recent years. They allow to non-invasively obtain extensive information about patient-specific hemodynamics, and thus have a great potential to improve the diagnosis, prognosis and therapy planning of cardiovascular diseases. A dataset contains time-resolved, three-dimensional blood flow directions and strengths, making comprehensive qualitative and quantitative data analysis possible. Quantitative measures, such as stroke volumes, help to assess the cardiac function and to monitor disease progression. Qualitative analysis allows to investigate abnormal flow characteristics, such as vortices, which are correlated to different pathologies. Processing the data comprises complex image processing methods, as well as flow analysis and visualization. In this work, we mainly focus on the aorta. We provide an overview of data measurement and pre-processing, as well as current visualization and quantification methods. This allows other researchers to quickly catch up with the topic and take on new challenges to further investigate the potential of 4D PC-MRI data.

Contains fulltext : 107854.pdf (Publisher’s version ) (Open Access), This paper describes a framework for establishing a reference airway tree segmentation, which was used to quantitatively evaluate fifteen different airway tree extraction algorithms in a standardized manner. Because of the sheer difficulty involved in manually constructing a complete reference standard from scratch, we propose to construct the reference using results from all algorithms that are to be evaluated. We start by subdividing each segmented airway tree into its individual branch segments. Each branch segment is then visually scored by trained observers to determine whether or not it is a correctly segmented part of the airway tree. Finally, the reference airway trees are constructed by taking the union of all correctly extracted branch segments. Fifteen airway tree extraction algorithms from different research groups are evaluated on a diverse set of twenty chest computed tomography ({CT}) scans of subjects ranging from healthy volunteers to patients with severe pathologies, scanned at different sites, with different {CT} scanner brands, models, and scanning protocols. Three performance measures covering different aspects of segmentation quality were computed for all participating algorithms. Results from the evaluation showed that no single algorithm could extract more than an average of 74\% of the total length of all branches in the reference standard, indicating substantial differences between the algorithms. A fusion scheme that obtained superior results is presented, demonstrating that there is complementary information provided by the different algorithms and there is still room for further improvements in airway segmentation algorithms.
