The adipokines play an important to control insulin secretion and glucose. This study aimed to investigate the association between maternal circulating adipokine levels and ADIPOQ gene polymorphism among pregnant women subjects with GDM and normal glucose tolerance (NGT).Methods: Participants including 229 normal pregnant women and 197 GDM pregnant women were enrolled from 2015 to 2018 at Siriraj hospital. Serum adipokine levels including adiponectin, adipsin/factor D, NGAL/Lipocalin-2, total PAI-1, and resistin were measured by immunoassay. ADIPOQ variations were investigated including − 11377C/G (rs266729), +45T/G (rs2241766), and +276G/T (rs1501299).Results: Serum adiponectin concentration was also significantly decreased among the GDM who had aged less than 35 years old whereas adipsin levels were significantly lower among the GDM who had aged more than 35 years old. Also, adiponectin and total PAI-1 levels were significantly lower among the GDM who had a BMI of less than 30 kg/m2. The G allele frequency of ADIPOQ +45T/G was significantly different between GDM and controls (p = 0.03). ADIPOQ +45T/G was associated with an increased risk of GDM (odds ratio [OR]: 1.554; 95% confidence interval [CI]: 1.010– 2.390; p=0.045). The − 11377C/G was affected by the level of adiponectin (p = 0.04). The C allele of − 11377C/G SNP declined serum adiponectin levels and may be a risk factor for GDM.Conclusion: This study revealed that genetics play important roles in circulating adipokines among pregnant women. ADIPOQ polymorphisms had significant associations with adiponectin levels in GDM patients.Keywords: adiponectin, genetics, adipokine, gestational diabetes mellitus

Objective: The aim of the present study was to evaluate the obstetric complications associated with isolated fetal congenital heart disease (CHD) by comparing pregnancies with and without this condition., Methods: In this retrospective matched comparative study at Siriraj Hospital, Thailand, we included 233 postnatally confirmed fetal CHD cases and 466 unaffected fetuses. Controls were selected at a 2:1 ratio, ensuring that they matched the cases in terms of maternal age, parity, and history of preterm deliveries., Results: Fetal CHD was significantly associated with an increased risk of spontaneous preterm labor (30% vs 9.7%; adjusted odds ratio [aOR] 2.42; 95% confidence interval [CI]: 1.35-4.36; P = 0.003), delivery before 34 gestational weeks (11.6% vs 0.6%; aOR 12.33; 95% CI: 3.32-45.78; P < 0.001), and pre-eclampsia (11.6% vs 2.8%; aOR 2.19; 95% CI: 1.01-4.76; P = 0.047). Newborns with CHD were significantly more likely to be small for gestational age (10.7% vs 5.2%; aOR 2.09; 95% CI: 1.11-3.94; P = 0.022). Intriguingly, a prenatal diagnosis of CHD was associated with a reduced risk of preterm delivery in affected pregnancies (P = 0.002)., Conclusion: Pregnancies affected by isolated fetal CHD demonstrated a higher propensity for several adverse outcomes. These findings underscore the importance of prenatal CHD detection and tailored perinatal care to potentially improve both pregnancy outcomes and neonatal health., (© 2024 The Author(s). Objective: To determine the incidence of excessive gestational weight gain (GWG) among overweight and obese pregnant women, its associated factors, and pregnancy outcomes., Methods: A total of 355 overweight or obese singleton pregnant women who were included. Obstetric characteristics, weight gain, and pregnancy outcomes, were extracted from medical records. GWG was categorized according to the Institute of Medicine recommendation. Comparisons were made between individuals with inadequate, normal, and excessive GWG. Logistic regression analysis was performed to determine independent associated factors for excessive GWG., Results: Majority of the women were overweight (68.7%), 38.9% were nulliparous, and mean pre-pregnancy body mass index was 28.9 kg/m2. Excessive GWG was observed in 53% of the women. Women with excessive GWG had significantly higher weight gain in every trimester. Risk of excessive GWG increased in women ≤30 years, while gestational diabetes (GDM) significantly decreased the risk. Women with excessive GWG had a significantly higher primary cesarean section rate. Both women with normal and excessive GWG showed higher rate of having large for gestational age (LGA) infants (P=0.003). Maternal age of ≤30 years significantly increased the risk of excessive GWG (adjusted odds ratio [aOR], 1.91; 95% confidence interval [CI], 1.11-3.27) and GDM significantly decreased this risk (aOR, 0.40; 95% CI, 0.24-0.67)., Conclusion: The incidence of excessive GWG among overweight and obese women was 53%. Maternal age of ≤30 years significantly increased this risk while women with GDM were significantly decreased risk. Primary cesarean section and fetal LGA significantly increased in women with excessive GWG.

Objectives: This study aims to evaluate the prevalence of pregnant women whose children are at higher risk for childhood allergies and to assess knowledge of risk assessment and prevention strategies., Methods: A cross-sectional study was conducted on 310 pregnant women in an antenatal care clinic at a tertiary care hospital in Thailand. In addition to baseline demographic and obstetric characteristics, all participating pregnant women were asked to complete a questionnaire regarding risk evaluation and knowledge of childhood allergies on various topics. A childhood allergy risk assessment was evaluated based on the history of allergy disease in immediate family members. The questionnaire on knowledge was derived from a guideline issued by the Allergy, Asthma, and Immunology Association of Thailand, with possible scores of 0-30., Results: The mean maternal age was 30.6 years, and 139 (44.8%) were nulliparous. Overall, 86 couples (27.7%) were at high risk for childhood allergies. The mean total knowledge score was 15.2 out of 30, and only 24 women (7.7%) had an overall score of >20, and 40 women (12.9%) had an overall score of ≤10. The mean knowledge score for almost every subtopic was less than half of the possible points, except for the risk reduction strategies during pregnancy. Comparisons between those with higher and lower scores (≥16 vs. ≤15 points) showed that women with higher knowledge scores were significantly more likely to have had a previous child with an allergy (p=0.010)., Conclusion: The prevalence of pregnant women whose children were at higher risk for childhood allergies was 27.7% (86 of 310 couples). The women had limited knowledge of childhood allergies with regard to risk assessment, risk reduction strategies, and various interventions. Objectives: To determine the incidence of overt diabetes in pregnancy (ODIP) among women with 50-g GCT results ≥ 200 mg/dL and compare characteristics and pregnancy outcomes between women with and without gestational diabetes (GDM)., Methods: A retrospective cohort study was conducted in 212 pregnant women whose 50-g GCT results ≥ 200 mg/dL. ODIP was diagnosed from 75-g OGTT if fasting plasma glucose ≥ 126 and/or 2-h plasma glucose ≥ 200 mg/dL. Various characteristics and pregnancy outcomes were compared between ODIP and those with and without GDM., Results: Incidence of ODIP was 1.9% of all pregnant women and 23.6% of women with 50-g GCT ≥ 200 mg/dL. Women with ODIP and GDM were more likely to be overweight or obese than those without GDM (52%, 39.6%, and 18.2%, p < 0.001). Women with ODIP had significantly higher 50-g GCT results, lower gestational weight gain, and were less likely to deliver vaginally. Insulin therapy was significantly more common in women with ODIP compared to GDM (70.2% vs. 15.4%, p < 0.001). Rates of LGA, macrosomia, and other neonatal outcomes were comparable. BMI ≥ 25 kg/m 2 and 50-g GCT ≥ 240 mg/dL independently increased the risk of any abnormal glucose tolerance [adjusted OR 3.22 (95% CI 1.55-6.70) and 2.28 (95% CI 1.14-4.58)] and ODIP [adjusted OR 9.43 (95% CI 2.15-41.38) and 6.36 (95% CI 2.85-14.18)], respectively., Conclusion: Incidence of ODIP was 23.6% of women with 50-g GCT ≥ 200 mg/dL. BMI ≥ 25 kg/m 2 and 50-g GCT ≥ 240 mg/dL independently increased the risk of GDM and ODIP. Objectives The objective of this study was to determine the incidence of preeclampsia and associated cesarean section (CS) rate according to the Robson classification. Methods A retrospective cross-sectional study was conducted on a total of 670 women who delivered at a tertiary care hospital in Thailand during January to March 2023. All women were classified into 10 groups according to the Robson classification, and preeclampsia was identified. Overall and group-specific incidence of preeclampsia and CS rate were estimated. Comparison of CS rate was made between those with and without preeclampsia using the Chi-squared test. Relative risks (RR) and corresponding 95% confidence intervals were estimated. Results The majority of women were in group 1 (34%) and group 3 (30.7%). Overall CS rate was 40.6% with highest contribution from group 1, 5, and 10. Incidence of preeclampsia was 9.1%, and the majority were in groups 10 (29.5%) and 1 (23%). Preeclampsia significantly increased the rate of overall CS (RR 1.8, p<0.001). The risk of CS significantly increased in group 1 (RR 1.8, p=0.043), group 3 (RR 3.5, p=0.025), and group 10 (RR 1.9, p=0.006). Preeclampsia accounted for 15.4% of all CS, with the highest contribution in group 2 (37.5%), group 10 (31.1%), group 3 (16.7%), and group 1 (10.8%). Without preeclampsia, the overall CS rate was relatively reduced by 6.9%, with the largest relative reduction in group 10 (14.3%), group 3 (11.5%), group 2 (6.3%), and group 1 (5.2%). Conclusion The incidence of preeclampsia was 9.1%, and preeclampsia significantly increased the rate of overall CS. Objective: To determine the risk factors associated with late-onset GDM (diagnosed between 24 and 28 weeks of gestation) after normal early screening., Methods: A case-control study was conducted in 600 singleton pregnant women who started antenatal care before 20 weeks with normal early GDM screening. Repeat screening was performed at 24-28 weeks. Cases were 120 women with late-onset GDM and 480 controls were those without GDM. Risk factors for late-onset GDM were evaluated and pregnancy outcomes were compared., Results: Cases were significantly older, and more likely to be overweight or obese. 50-g GCT of ≥ 160 mg/dL and abnormal 1 value of 100-g OGTT significantly increased the risk of late-onset GDM (p = 0.004 and < 0.001 respectively). Independent risk factors were abnormal 1 value of 100-g OGTT from first screening (adjusted OR 5.49, 95% CI 2.70-11.17, p < 0.001), age ≥ 30 years (adjusted OR 2.71, 95% CI 1.66-4.43, p < 0.001), DM in family (adjusted OR 1.76, 95% CI 1.07-2.88, p = 0.025), and BMI ≥ 25 kg/m 2 (adjusted OR 1.86, 95% CI 1.17-2.97, p = 0.009). Background: Prepregnancy underweight, and gestational weight gain has been associated with increased risk of adverse pregnancy outcomes, including preterm birth, low birthweight (LBW) and small for gestational age (SGA), but with conflicting results. The objectives were to compare the incidence of SGA, LBW, and other pregnancy outcomes between prepregnancy underweight and normal weight women and to evaluate possible associated risk factors., Methods: A retrospective cohort study was conducted in 220 underweight women (prepregnancy BMI of <18.5 kg/m 2 ) and 440 normal weight women (prepregnancy BMI 18.5-24.9 kg/m 2 ). Data were extracted from medical records and compared between the 2 groups, including baseline and obstetric characteristics, labor and delivery data, pregnancy, and neonatal outcomes., Results: Underweight women were significantly younger and more likely to be nulliparous. They were significantly more likely to have weight gain below recommendation (33.6% vs. 23.2%, P<0.001). SGA and LBW were significantly more common in underweight compared to normal weight women (10.9% vs. 7%, P=0.034 and 13.2 vs. 7.3%, P=0.013, respectively). Other adverse neonatal outcomes were comparable. Logistic regression analysis showed that inadequate weight gain was the independent risk for both SGA and LBW (adjusted OR 2.20, 95%CI 1.19-4.09, P=0.012) and adjusted OR 2.31, 95%CI 1.28-4.159, P=0.005, respectively)., Conclusions: Risk of both SGA and LBW were significantly increased in underweight compared to normal weight women. Inadequate weight gain was independently associated with increased risk of both SGA and LBW.

Background:  Body mass index (BMI) has commonly been used to evaluate the risk of gestational diabetes mellitus (GDM), but BMI does not always represent body fat mass distribution. Body fat index (BFI), which includes the measurement of subcutaneous adipose tissue (SAT) and visceral adipose tissue (VAT), has been suggested to be a better predictor for GDM than BMI., Objective: The objective of this study is to compare the risk of GDM among pregnant females with BFI of >0.5 and ≤0.5., Methods: Maternal abdominal subcutaneous adipose tissue (SAT) and visceral adipose tissue (VAT) thickness were measured by ultrasonography before 14 weeks of gestation, and BFI was calculated (VAT×SAT/height). The study group was 160 females with BFI of >0.5, and the comparison group was 80 females with BFI of ≤0.5. All females received GDM screening during the first antenatal visit and at 24-28 weeks of gestation. The rate of GDM was compared between the two groups. The correlation between BFI and BMI and their diagnostic ability for GDM were evaluated. Logistic regression analysis was performed to determine the independent associated factors for GDM., Results: Females with BFI of >0.5 were significantly older (p=0.033) and had higher body mass index (BMI) (p<0.001) and were more likely to be overweight or obese (p<0.001). BFI correlated well with BMI (correlation coefficient of 0.736, p<0.001). GDM was significantly more common in females with BFI of >0.5 (24.4% versus 11.3%, p=0.017). The diagnostic ability for GDM between BFI and BMI was similar (areas under receiver operating characteristic {ROC} curves of 0.641 and 0.646, respectively). Significant independent risk factors for GDM were a BFI of >0.5 and a BMI of ≥25 kg/m 2 (adjusted odds ratio {OR}, 3.8; 95% confidence interval {CI}, 1.5-9.2), age of ≥30 years (adjusted OR, 2.8; 95% CI, 1.2-6.4), and family history of diabetes mellitus (DM) (adjusted OR, 4.0; 95% CI, 1.9-8.3)., Conclusion: Females with BFI of >0.5 were significantly more likely to have GDM. The diagnostic ability of BFI and BMI for GDM was comparable. Background: An inappropriate gestational weight gain (GWG) among pregnant women with overweight/obesity is a crucial health problem. Its prevalence remains high worldwide, particularly in urban areas. The prevalence and predicting factors in Thailand are lack of evidence. This study aimed to investigate prevalence rates, antenatal care (ANC) service arrangement, predictive factors, and impacts of inappropriate GWG among pregnant women with overweight/obesity in Bangkok and its surrounding metropolitan area., Methods: This cross-sectional, retrospective study used four sets of questionnaires investigating 685 pregnant women with overweight/obesity and 51 nurse-midwives (NMs) from July to December 2019 in ten tertiary hospitals. Multinomial logistic regression identified predictive factors with a 95% confidence interval (CI)., Result: The prevalence rates of excessive and inadequate GWG were 62.34% and 12.99%. Weight management for pregnant women with overweight/obesity are unavailable in tertiary cares. Over three-fourths of NMs have never received weight management training for this particular group. ANC service factors, i.e., GWG counseling by ANC providers, quality of general ANC service at an excellent and good level, NMs' positive attitudes toward GWG control, significantly decreased the adjusted odds ratio (AOR) of inadequate GWG by 0.03, 0.01, 0.02, 0.20, times, respectively. While maternal factors, sufficient income, and easy access to low-fat foods reduce AOR of inadequate GWG by 0.49, and 0.31 times. In contrast, adequate maternal GWG knowledge statistically increased the AOR of inadequate GWG 1.81 times. Meanwhile, easy access to low-fat foods and internal weight locus of control (WLOC) decreased the AOR of excessive GWG by 0.29 and 0.57 times. Finally, excessive GWG significantly increased the risk of primary C/S, fetal LGA, and macrosomia 1.65, 1.60, and 5.84 times, respectively, while inadequate GWG was not associated with adverse outcomes., Conclusion: Prevalence rates of inappropriate GWG, especially excessive GWG remained high and affected adverse outcomes. The quality of ANC service provision and appropriate GWG counseling from ANC providers are significant health service factors. Introduction: Aberrant immune and inflammatory response is thought to be involved in the pathogenesis of gestational diabetes mellitus (GDM)., Objective: To investigate the genetic polymorphisms and levels of adipokines/adipocytokines that influence the risk of developing GDM in Thai women., Research Design & Methods: This case-control recruited 400 pregnant Thai women. A total of 12 gene polymorphisms at ADIPOQ, adipsin, lipocalin-2, PAI-1, resistin, IL-1β, IL-4, IL-17A, TGF-β, IL-10, IL-6, and TNF-α were analyzed by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) assay and RNase H2 enzyme-based amplification (rhAmp) SNP assay. Serum levels of adipokines/adipocytokines were evaluated using Luminex assays., Results: Mean age, weight before and during pregnancy, body mass index before and during pregnancy, blood pressure, gestational age at blood collection, and median 50 g glucose challenge test were significantly higher in GDM women than control. Significantly lower adiponectin and higher IL-4 levels were found in GDM compared to controls (p = 0.001 and p = 0.03, respectively). The genotype frequencies of IL-17A (rs3819025) were significantly different between GDM and controls (p = 0.01). Using additive models, IL-17A (rs3819025) and. TNF-α (rs1800629) were found to be independently associated with increased risk of GDM (odds ratio [OR]: 2.867; 95 % confidence interval [CI]: 1.171-7.017; p = 0.021; and OR: 12.163; 95 %CI: 1.368-108.153; p = 0.025, respectively). Objective The aim of this study is to compare the rate of spontaneous preterm delivery between gestational diabetes mellitus (GDM) and normal pregnancy. Pregnancy outcomes and associated risk factors for spontaneous preterm delivery were evaluated. Methods A retrospective cohort study was conducted on 120 GDM and 480 normal pregnant women. All women received GDM screening with 50-g glucose challenge test and 100-g oral glucose tolerance test at the first visit and repeated at 24-28 weeks. Data were retrieved from medical records and included baseline and obstetric characteristics, preterm risks, GDM risks, and pregnancy outcomes. Spontaneous preterm birth was defined as delivery before 37 completed weeks of gestation that had been preceded by spontaneous labor. Results GDM women were more likely to be ≥ 30 years (p=0.032) and have previous GDM (p=0.013). Incidence of overall preterm delivery was significantly higher in GDM women (17.5% vs. 8.5%, p=0.004), as well as the incidence of spontaneous preterm delivery (15.8% vs. 7.1%, p=0.004). GDM women had less gestational weight gain (p<0.001) and were less likely to have excessive weight gain (p=0.002). GDM women were more likely to deliver large for gestational age (LGA) (p=0.02) and macrosomic infants (p=0.027). Neonatal hypoglycemia was significantly more common among GDM women (p=0.013). Multivariate analysis showed that previous preterm birth and GDM independently increased the risk of spontaneous preterm delivery (adjusted OR: 2.56, 95% CI: 1.13-5.79, p=0.024, and adjusted OR: 2.15, 95% CI: 1.2-3.84, p = 0.010, respectively). Conclusion GDM and previous preterm birth significantly increased the risk of spontaneous preterm delivery. A total of 1016 pregnant women attending antenatal clinic before 20 weeks of gestation during September 2018 to February 2019 were included in a cohort study with repeated cross-sectional assessments. The study was aimed to determine prevalence and characteristics of gestational diabetes mellitus (GDM) and pregnancy outcomes by early universal screening approach. GDM screening was performed during first visit and repeated during 24-28 weeks of gestation, as necessary, using a 50-g glucose challenge test followed by a 100-g oral glucose tolerance test for GDM diagnosis. Overall prevalence of GDM was 18.6%. A significantly higher prevalence of GDM was observed among high-risk than low-risk women (21.3% vs. 13.1%, p  = 0.002). GDM among low-risk women contributed to 23.3% of all GDM cases. The majority of GDM (76.2%) were diagnosed before 20 weeks of gestation, with 74.5% occurring in high-risk women and 81.8% occurring in low-risk women. When initial screening tests were normal, risk of GDM diagnosed during 24-28 weeks was 6.0% (7.5% among high-risk women and 3.1% among low-risk women). Compared to those without GDM, women with GDM significantly had lower gestational weight gain ( p  < 0.001), higher prevalence of preeclampsia ( p  = 0.001), large for gestational age (LGA) ( p  = 0.034) and macrosomia ( p  = 0.004). These outcomes were more pronounced among high-risk women with GDM. Impact Statement What is already known on this subject? Universal GDM screening is recommended during 24-28 weeks of gestation, either by 1- or 2-step approach. Some also recommend early GDM screening among high-risk women. Prevalence of early-onset GDM varies between studies and benefits of early diagnosis and treatment are still controversial. What do the results of this study add? Early universal GDM screening identified more women with GDM and majority could be diagnosed before 20 weeks of gestation. GDM among low-risk women contributed to 23.3% of all cases. Adverse pregnancy outcomes were more common among high-risk women with GDM. This approach could be useful and can be implemented in other settings, especially those that serve high-risk population or with high GDM prevalence. What are the implications of these findings for clinical practice and/or further research? Early universal GDM screening should be considered in settings with high prevalence of GDM and high-risk women. However, benefits of early detection and treatment of GDM should be determined in more details in the future, especially in terms of cost-effectiveness and improvement in pregnancy outcomes.

The achievement of recommended decision-to-delivery interval (DDI) of ≤30 minutes in emergency caesarean section (CS) is relatively low in developing countries. This study was aimed to compare DDI in emergency CS before and after the implementation of a specific care process improvement protocol, called 'code blue'. A total of 300 women underwent emergency CS were included; 150 consecutive cases before (during 2015-2016) and the other 150 consecutive cases after (during 2017-2018) 'code blue' implementation. Timing of decision-to-delivery process was compared. The results showed that median DDI was significantly shorter after 'code blue' implementation (22 vs. 52.5 minutes, p <.001). DDI of ≤30 minutes was achieved in 80% of the women under 'code blue' compared to 8% before implementation ( p <.001). Significant improvements were observed regardless of decision time. Pregnancy and neonatal outcomes were comparable between the two periods. The implementation of 'code blue' protocol for emergency CS results in significantly shorter DDI and other time intervals.Impact Statement What is already known on this subject? Achievement of recommended decision-to-delivery interval (DDI) of ≤30 minutes in emergency caesarean section is relatively low in developing countries. Various setting-specific care improvement processes have been reported to shorten DDI. What do the results of this study add? A multidisciplinary care improvement process ('code blue') that developed according to specific evidence and based on a hospital's context can significantly shorten DDI as well as other time intervals in women requiring emergency CS. What are the implications of these findings for clinical practice and/or further research? The 'code blue' protocol could be used as a model for other hospitals and health care settings to develop their own specific quality improvement process in order to shorten DDI for emergency CS. Collaboration and communication between all staff members could help in better identification of significant barriers as well as development of appropriate solutions. Further studies are also needed to determine whether the shortened DDI could improve neonatal outcomes.

Gestational Diabetes Mellitus (GDM) is the most common metabolic complication during pregnancy and is associated with serious maternal and fetal complications such as pre-eclampsia and stillbirth. Further, women with GDM have approximately 10 times higher risk of diabetes later in life. Children born to mothers with GDM also face a higher risk of childhood obesity and diabetes later in life. Early prediction/diagnosis of GDM leads to early interventions such as diet and lifestyle, which could mitigate the maternal and fetal complications associated with GDM. However, no biomarkers identified to date have been proven to be effective in the prediction/diagnosis of GDM. Proteomic approaches based on mass spectrometry have been applied in various fields of biomedical research to identify novel biomarkers. Although a number of proteomic studies in GDM now exist, a lack of a comprehensive and up-to-date meta-analysis makes it difficult for researchers to interpret the data in the existing literature. Thus, we undertook a systematic review and meta-analysis on proteomic studies and GDM. We searched MEDLINE, EMBASE, Web of Science and Scopus from inception to January 2022. We searched Medline, Embase, CINHAL and the Cochrane Library, which were searched from inception to February 2021. We included cohort, case-control and observational studies reporting original data investigating the development of GDM compared to a control group. Two independent reviewers selected eligible studies for meta-analysis. Data collection and analyses were performed by two independent reviewers. The PROSPERO registration number is CRD42020185951. Of 120 articles retrieved, 24 studies met the eligibility criteria, comparing a total of 1779 pregnant women (904 GDM and 875 controls). A total of 262 GDM candidate biomarkers (CBs) were identified, with 49 CBs reported in at least two studies. We found 22 highly replicable CBs that were significantly different (nine CBs were upregulated and 12 CBs downregulated) between women with GDM and controls across various proteomic platforms, sample types, blood fractions and time of blood collection and continents. We performed further analyses on blood (plasma/serum) CBs in early pregnancy (first and/or early second trimester) and included studies with more than nine samples (nine studies in total). We found that 11 CBs were significantly upregulated, and 13 CBs significantly downregulated in women with GDM compared to controls. Subsequent pathway analysis using Database for Annotation, Visualization and Integrated Discovery (DAVID) bioinformatics resources found that these CBs were most strongly linked to pathways related to complement and coagulation cascades. Our findings provide important insights and form a strong foundation for future validation studies to establish reliable biomarkers for GDM.

Background: Periodontitis (P) has emerged as a risk factor for gestational diabetes mellitus (GDM) through immune cell function alterations, elevated proinflammatory mediators, and increased reactive oxygen species (ROS). The main objective of present study was to determine associations between pregnancy with and without GDM and P. The secondary objective was to compare ROS production in peripheral blood cells (PBCs) of pregnant women with and without GDM., Methods: This cross-sectional case-control study included 128 pregnant women: 64 with and 64 without GDM. All participants were examined for clinical parameters of GDM and periodontal conditions. Associations between GDM-related periodontal data and GDM risk were evaluated by multiple logistic regression. PBCs were isolated and cultured. ROS productions in each PBCs types was investigated by flow cytometry with ROS antibodies., Results: P was significantly more prevalent in pregnant women with GDM than in those without GDM (57.8% versus 37.5%), with an odds ratio (OR) of 2.28, and a 95% confidence interval (CI) of 1.12 to 4.64 (P = 0.022). The OR (95% CI) was 2.59 (1.19 to 5.65) (P = 0.017) after adjusting for potential confounding factors, including diabetes mellitus (DM) family history, age ≥30 years, body mass index, and maternal age. ROS levels in all PBCs types were significantly higher in the GDM than in the non-GDM group (P < 0.05)., Conclusion: This study supported the association between P and GDM and indicated that P may be a risk factor for GDM. Background/purpose: MicroRNA-223 (miR-223) is involved in several inflammatory diseases, including gestational diabetes mellitus (GDM) and periodontitis. We first described a procedure for purifying miR-223 from gingival crevicular blood (GCB) of pregnant women with or without GDM and periodontitis. This study aimed to determine whether GDM and/or periodontitis modifies miR-223 expression in pregnant women and to analyze miR-223-targeted messenger RNA (mRNA) expression levels in GCB compared to peripheral blood (PB)., Materials and Methods: Pregnant women were allocated to 4 groups: 10 women with GDM and periodontitis (GDM/P), 10 women with GDM without periodontitis (GDM/NP), 9 women with periodontitis and without GDM (NGDM/P) and 10 women without either condition (NGDM/NP). Clinical parameters of GDM and periodontal status were examined. GCB and PB were collected to assess miR-223, ICAM-1, IL-1β and β1-integrin gene expression by quantitative real-time polymerase chain reaction., Results: The GDM/P group demonstrated the highest miR-223 expression levels among the 4 groups in GCB. A significant difference was found between GDM/P and GDM/NP group ( P  = 0.04). In contrast, the GDM/P showed the lowest miR-223 expression level in PB among the 4 groups. Purpose: To assess the accuracy of capillary blood glucose (CBG) compared to conventional venous plasma glucose (VPG) testing for 50-g glucose challenge test (GCT) in gestational diabetes (GDM) screening., Methods: A total of 300 women were enrolled and 50-g GCT for GDM screening was offered. At 1 h after glucose loading, CBG was evaluated by CONTOUR® PLUS glucose meter by well-trained nurses immediately after venipuncture for VPG. Results of CBG were compared with those from VPG to evaluate its accuracy. Women with venous plasma glucose > 140 mg/dL were offered 100-g OGTT for GDM diagnosis., Results: The mean age was 30.2 years and the mean gestational age at testing was 21.8 weeks. GDM was diagnosed in 34 women (11.3%). The mean VPG was 142.1 ± 32.9 mg/dL and the mean CBG was 129.3 ± 33.5 mg/dL. Mean difference was -12.3 ± 12.5 mg/dL, corresponding to -8.8 ± 11.4%. CBG significantly correlated with VPG with correlation coefficient of 0.929, p  < 0.001. In the detection of abnormal 50-g GCT results (VPG ≥ 140 mg/dL), at 126 mg/dL cutoff, CBG had sensitivity of 92.5%, specificity of 81.8%, and positive and negative predictive values of 82.8%and 92%. This retrospective cohort study aimed to determine prevalence of GDM diagnosed before 24 weeks of gestation (early-onset GDM) and evaluate associated risk factors and compare pregnancy outcomes between different GDM status. A total of 1200 pregnant women attending antenatal clinic before 24 weeks of gestation were included. GDM screening was offered during first visit and repeat during 24-28 weeks of gestation, using 50-g GCT and 100-g OGTT. GDM was diagnosed in 110 women (13.9%) and early-onset GDM was found in 57 women (9.2%), which accounted for 65.9% of all GDM. Early-onset GDM had significant lower gestational weight gain and higher rates of preeclampsia, LGA infants, and NICU admission. Independent associated factors for early-onset GDM were age ≥ 30 years (aOR 4.89, 95%CI: 2.08-11.50, p  < .001), and previous GDM (aOR 12.26, 95%CI: 3.86-38.93, p  < .001) while DM in family was the only independent factor for late-onset GDM (aOR 2.53, 95%CI: 1.42-4.51, p  = .002).IMPACT STATEMENT What is already known on this subject? Reported prevalence of early-onset GDM varies between studies, depending on the screening strategy and criteria used. Despite treatment, early-onset GDM has been associated with increased adverse maternal and neonatal outcomes in many previous studies. The risks associated with early-onset GDM and the evidence for benefit of early treatment are still unclear. What do the results of this study add? The results showed that early-onset GDM accounted for majority (65.9%) of all GDM. Despite treatment, early-onset GDM increased risk of preeclampsia, LGA infants, and NICU admission. Independent associated factors for early-onset GDM were age ≥ 30 years, and previous GDM while DM in family was the only independent factor for late-onset GDM. What are the implications of these findings for clinical practice and/or further research? Early GDM screening and intensive management, especially in high-risk women, should be implemented to minimise the risks of adverse outcomes. Further studies are needed to determine appropriate criteria to define early-onset GDM and to identify women at higher risk in different population and settings with different screening strategies. Effective management and approaches for this subgroup of GDM should also be further investigated.

Objective: To investigate the association between the ABO blood group and gestational diabetes mellitus (GDM)., Methods: A case-control study was conducted in 600 pregnant women who received GDM screening with 50-g GCT and diagnosis by 100-g OGTT according to institutional guidelines. The cases included 200 women with GDM and another 400 normal pregnant women were randomly selected as controls. Various characteristics and ABO blood group were extracted from medical records and compared between cases and controls to determine their association with GDM. Logistic regression analysis was performed to determine independent associated factors for GDM adjusting for potential confounders., Results: Univariate analysis showed that significant factors associated with GDM were age ≥30 years, family history of DM, overweight, and obesity. Only blood group O significantly increased risk of GDM (OR 1.51, 95% CI 1.06-2.13, p  = .020). Logistic regression analysis showed that blood group O independently increased the risk of GDM (adjusted OR 1.99, 95% CI 1.32-3.0, p  = .001). The risk of GDM was enhanced in women with blood group O with family history of DM (adjusted OR 3.5, 95% CI 1.57-7.81, p  = .002) while it was attenuated among those without (adjusted OR 1.6, 95% CI 0.97-2.64, p  = .064)., Conclusion: Blood group O independently increased the risk of GDM.

Objective: This study was conducted to compare the pregnancy rates of ultrasound-guided intrauterine insemination (UG-IUI) and classical intrauterine insemination (C-IUI) cycles., Study Design: A total of 320 infertile women were enrolled and randomized into an UG-IUI group, and a C-IUI group. All participants received an oral medication for ovarian stimulation. With both groups, the IUIs were scheduled and performed by doctors in their residency and fellowship training, under supervision. The duration and difficulty of the procedures were assessed. A pregnancy test was offered 3 weeks later if the participants did not have menstruation., Results: The demographic and other baseline characteristics of the groups (baseline hormone levels, cervical length, uterine position, endometrial thickness, and expertise of the providers) were comparable. The pregnancy rates were similar, with 6.9 % and 6.3 % for the UG-IUI and C-IUI groups, respectively. In the UG-IUI group, the pregnancy rate of the multigravida women was three times higher than that of the nulligravida women (15.4 % vs. 5.0 %; p = 0.13). Although the duration of the procedure was shorter for the UG-IUI group (p < 0.05), the level of difficulty was similar for the two groups., Conclusions: For oral-medication stimulated cycles, UG-IUI did not increase the pregnancy rate more than with C-IUI. This cohort study aimed to determine the association between false-positive 50-g GCT and incidence of LGA and to evaluate predictive roles of third-trimester ultrasonographic examination. A total of 200 women with false-positive 50-g GCT and 188 women without GDM risks were enrolled. Third-trimester ultrasonographic examinations were offered. Rate of LGA during third trimester and at birth were compared between groups. Factors associated with LGA and diagnostic properties of third-trimester ultrasonography were evaluated. Incidence of LGA by third-trimester ultrasound and at birth were significantly higher in women with false-positive GCT (19.0% vs. 10.6%, p  = .03 and 22% vs. 13.8%; p  = .04). Factors associated with LGA included multiparity (adjusted OR 2.32, p  = .01), excessive weight gain (adjusted OR 2.57, p  = .01) and LGA by ultrasound (adjusted OR 9.79, p  < .001). Third-trimester ultrasonography had 47.1% sensitivity, 92.1% specificity and LR + and LR- of 5.96 and 0.57 in identifying LGA infants.Impact statement What is already known on this subject? Women with abnormal GCT but normal OGTT (false positive GCT) might have some degree of glucose intolerance so that GDM-related outcomes could develop, including LGA, macrosomia, shoulder dystocia, and caesarean delivery. Roles of ultrasonography in the prediction of LGA and macrosomia has been reported with mixed results. What do the results of this study add? The results showed that the incidence of LGA, by third-trimester ultrasound and at birth, were significantly increased in women with false-positive GCT. Multiparity, excessive weight gain and LGA by third-trimester ultrasound significantly increased the risk of LGA. Third-trimester ultrasonography had 47.1% sensitivity, 92.1% specificity and LR + and LR- of 5.96 and 0.57 in identifying LGA infants. What are the implications of these findings for clinical practice and/or further research? More intensive behavioural and dietary interventions, together with weight gain control and monitoring, may be needed in women with false-positive GCT to minimise the risk of LGA. Third trimester ultrasonographic examination might be helpful to detect and predict LGA at birth and should be included into routine clinical practice. Further studies that are more widely generalisable are needed to elucidate the relationship between false-positive GCT and adverse pregnancy outcomes and to investigate the benefits of ultrasonographic examination in the prediction of LGA and macrosomia.

Objectives: To compare fetal anterior abdominal wall thickness (AAWT) between women with and without GDM during third trimester and to determine accuracy of AAWT to predict large for gestational age (LGA) infants., Materials and Methods: A total of 250 pregnant women, including 125 women with GDM and 125 women without GDM, were enrolled. Tansabdominal ultrasonographic examinations were performed at 28-30, 32-34 and 36-38 weeks. In addition to standard fetal biometries, AAWT was measured. Patient characteristics and ultrasonographic measurements were compared between groups. Sensitivity and specificity of AAWT for identifying LGA were evaluated., Results: While standard fetal biometries were comparable, mean fetal AAWT in GDM women were significantly higher than those without GDM at 28-30 weeks (2.8 ± 0.8 vs. 2.6 ± 0.6, p = 0.006) and 32-34 weeks (4.0 ± 0.9 vs. 3.5 ± 0.8, p = 0.042). LGA infants had significantly higher fetal AAWT at each time point only in GDM women. Using cut off values of AAWT of ≥2.0, 3.0, and 4.0 mm at 28-30, 32-34, and 36-38 weeks, sensitivity for LGA diagnosis in GDM women were 94.4%, 93.9%, and 89.3%, respectively. The use of abdominal circumference (AC) at >90th percentile showed lower sensitivity but higher specificity, regardless of GDM status. Combination of both measurements increased sensitivity to approximately 90% or higher in every time point, especially among GDM women., Conclusion: Significant increase in fetal AAWT was observed in GDM women at 28-30 and 32-34 weeks. Fetal AAWT significantly increased among LGA infants and had higher sensitivity than AC in identifying LGA during third trimester. Objective: To investigate the relationship between ABO blood group and preeclampsia. Methods: A case-control study was conducted, including 230 and 460 women with and without preeclampsia. ABO blood groups were compared and associated factors for preeclampsia were determined. Results: Blood group O was significantly more common in early-onset and less common in late-onset preeclampsia. Regression analysis showed that blood group O decreased the risk of late-onset preeclampsia (aOR 0.63, 95%CI 0.42-0.93) but increased the risk of early-onset preeclampsia (aOR 1.97 95%CI 1.05-3.69). Conclusion: Blood group O decreased the risk of late-onset preeclampsia while it increased the risk of early-onset preeclampsia.

Introduction: There is little information on the effect of maternal characteristics and on-admission laboratory parameters to the therapeutic serum magnesium sulfate (MgSO 4 ) levels in women with preeclampsia (PE). We sought to identify factors that may predict timely attainment of therapeutic serum magnesium levels after intravenous administration for seizure prophylaxis. Materials and methods: On-admission factors of 360 women with PE who received intravenous MgSO 4 (4-g loading and 2-g/h maintenance) for seizure prophylaxis were retrospectively reviewed. Parameters of those who attained therapeutic serum concentrations (4.8-8.4 mg/dL) within 2 h (Group A) and those who did not (Group B) were compared. Results: There was no seizure or magnesium toxicity in this cohort. Median (min-max) level of serum magnesium was 4.3 (2.5-8.4) mg/dL. Women in Group A ( n  = 105) had lower gestational age, body mass index (BMI), and platelets count, higher blood urea nitrogen (BUN), serum creatinine, uric acid, direct bilirubin, aspartate aminotransferase, alanine aminotransferase, lactate dehydrogenase, prothrombin, and partial thromboplastin times than those in Group B ( n  = 255) ( p  < .05). Women with mild PE were less likely to attain therapeutic serum magnesium levels compared with those with severe phenotypes (adjusted OR 23.57, 95% CI 8.20-67.76 versus adjusted OR 14.72, 95% CI 3.56-60.89, respectively; p  < .05), which may be explained by their significantly lower serum BUN and uric acid ( p  < .05). Conclusions: On-admission factors, especially BMI and renal clearance indices, of women with PE may affect timely attainment of therapeutic serum magnesium levels. Validation of its clinical impact requires further study focusing on women with severe PE.

The aims of this study were to determine prevalence of childbirth fear among uncomplicated pregnant women in Siriraj Hospital and possible associated factors. A total of 305 uncomplicated, singleton pregnant women were enrolled during early third trimester. All participants were interviewed regarding baseline demographic, social, economic, family, and obstetric characteristics. Fear of childbirth (FOC) was evaluated by Thai version of Wijma Delivery Expectancy/Experience Questionnaire Version A (W-DEQ). W-DEQ scores and level of FOC were compared between various characteristics to determine possible associated factors. Mean W-DEQ score was 51.9. Prevalence of low, moderate, high, and severe FOC were 18.4%, 64.9%, 16.1%, and 0.7%, respectively. Mean FOC score was significantly higher in women with unplanned than planned pregnancy ( p  = .033). Women with high to severe FOC were significantly less likely to have adequate income ( p  = .03), have family support ( p  = .02), have been told about delivery ( p  = .03), and have witnessed delivery ( p  = .01). IMPACT STATEMENT What is already known on this subject? Prevalence of fear of childbirth (FOC) in Western countries varies from 8 to 27%. FOC have been related to several unwanted conditions, including obstetrics complications, operative vaginal delivery, increased analgesic use in labour, elective caesarean section, postpartum depression, and impaired maternal-infant relation. Several factors related to FOC have been reported, including maternal age, parity, gestational age, history of a vacuum or forceps extraction, previous caesarean section, previous adverse perinatal outcome, low education, low socio-economic level, psychiatric problems, personality, lack of social support, and low self-esteem. What do the results of this study add? Prevalence of low, moderate, high, and severe FOC in Siriraj Hospital were 18.4%, 64.9%, 16.1%, and 0.7%, respectively. This was relatively lower than those reported from Western countries. Mean FOC score was significantly higher in women with unplanned than planned pregnancy. High to severe FOC was significantly related to low financial and family support and less understandings on delivery process. Differences in the results might be partly due to the differences in study population characteristics of Thais, including ethnics, religions, beliefs, perceptions, social structures, and social norms. What are the implications of these findings for clinical practice and/or further research? Future researches are suggested to explore and understand more about social and cultural factors associated with FOC. Identification of women with high or severe degree of FOC could help in preparing the women at risk before or during pregnancy to lessen FOC in order to improve their childbirth experiences. In addition, effective interventions to reduce FOC should be developed, evaluated, and implemented in the future.

The study aimed to compare the incidence of large for gestational age (LGA) infants between women with a false positive and normal glucose challenge test (GCT), and to evaluate the factors associated with LGA. A total of 480 pregnant women at risk for gestational diabetes mellitus (GDM); 160 with a false positive GCT and 320 with normal GCT results were included. The incidence of LGA and other pregnancy outcomes were compared between the two groups. Possible associated factors for LGA were also evaluated. Women with a false positive GCT were significantly older and more likely to be multiparous. The incidence of LGA was comparable between the false positive and normal GCT groups (15.6% vs. 13.1%, p = .456). Other pregnancy outcomes were also comparable. Logistic regression analysis showed that pre-pregnancy underweight significantly reduced the risk of LGA (adjusted OR 0.25, 95% CI 0.07-0.87, p = .029) while a second trimester weight gain >7 kg significantly increased the risk of LGA (adjusted OR 3.13, 95% CI 1.67-5.89, p < .001). Women with a false-positive GCT (abnormal GCT but normal OGTT) can be considered as having an early form of glucose intolerance which similar adverse outcomes to GDM could develop. Previous studies have reported that a mild maternal hyperglycaemia in the absence of GDM is associated with LGA, macrosomia, shoulder dystocia and a caesarean delivery. There is no current recommendation for any intervention or treatment among women with a false positive GCT. What the results of this study add? The results of this study showed that an incidence of LGA was not significantly increased in the false positive GCT groups and that other pregnancy outcomes were comparable. A pre-pregnancy underweight significantly reduced the risk of LGA while a second trimester weight gain >7 kg significantly increased the risk of LGA. What the implications are of these findings for clinical practice and/or further research? As a gestational weight gain is modifiable, behavioural and a dietary intervention as well as a close monitoring of the weight gain could help in lowering the risk of LGA, even in the absence of GDM. Further studies which are more widely generalisable are needed to elucidate the relationship between 50 g GCT and the adverse outcomes and to investigate the benefits of a specific intervention among this specific group of women.

Objectives: To describe the characteristics of abnormal oral glucose tolerance test (OGTT) values at gestational diabetes mellitus (GDM) diagnosis and their associations with clinical characteristics, and to evaluate the effect on GDM diagnosis if any OGTT value was omitted., Materials and Methods: A cross-sectional study was conducted in 415 women diagnosed with GDM. The OGTT results were recorded and analyzed., Results: Of the 415 included women, mean gestational age at GDM diagnosis was 19.2 weeks and 57.6% were diagnosed before 20 weeks. The highest proportions of abnormal values were found at the 1st and 2nd hour (85.3% and 96.6%, respectively). If the 3rd hour OGTT value was omitted, 16.7% of GDM cases would be missed. Number of abnormal OGTT values and abnormal FPG were significantly associated with obesity. Only pre-pregnancy overweight and obesity independently associated with insulin requirement (adjusted OR: 2.28, 95%CI: 1.02-5.06; p = .044; and adjusted OR: 6.29, 95%CI: 2.67-14.85; p < .001, respectively)., Conclusions: Over half of the GDM women had three or four abnormal OGTT values. Omission of the 3rd hour OGTT value would result in 16.7% of patients not being diagnosed with GDM. Number of abnormal OGTT values and abnormal FPG were associated with obesity, and insulin requirement was associated with pre-pregnancy overweight and obesity.

Objective: To determine the decision-to-delivery interval (DDI) in emergency cesarean delivery and associated factors., Methods: A total of 431 pregnant women who were indicated for emergency cesarean delivery were included. Clinical information and timing of process after decision until delivery and pregnancy outcomes were evaluated., Results: Mean age was 30 years, and 59.4% were nulliparous. Mean gestational age at delivery was 37.9 weeks. The decision was made during normal office hour in 33.2%. Median decision-to-operating room interval, decision-to-incision interval, and DDIs were 45, 70, and 82 minutes, respectively. Only 3.5% of patients had DDI ≤30 minutes, while 52.0% had DDI >75 minutes. During after office hours, every time interval was significantly shorter and 4.9% had DDI ≤30 minutes compared to 0.7% in normal office hours ( P =0.001). Compared to other indications, time intervals were significantly shorter in those with non-reassuring fetal heart rate (FHR), and DDI ≤30 minutes was achieved in 18.8% vs. 0.8% ( P <0.001). Shortest DDI was observed among those with non-reassuring FHR during after office hours. Neonatal outcomes were comparable between different DDIs., Conclusion: Only 3.5% of emergency cesarean delivery had a DDI ≤30 minutes (median 82 minutes). Objective: To determine the incidence and associated factors of superimposed preeclampsia among pregnant women with chronic hypertension., Methods: A total of 300 pregnant women diagnosed with chronic hypertension were reviewed. Data were retrieved from medical records, including obstetric data, characteristics of hypertension, and pregnancy outcomes. Incidence of superimposed preeclampsia was estimated. Various characteristics were compared to determine associated risk factors., Results: Mean age of the cohort was 34.3 years, 47% were nulliparous, 50% had hypertension before pregnancy, and the others presented with hypertension before 20 weeks. Incidence of superimposed preeclampsia was 43.3% (95% confidence interval (CI) 37.8-48.9). Women with superimposed preeclampsia were significantly more likely to have mean arterial pressure (MAP) ≥105 mmHg at 18-20 and 24-28 weeks. Adverse neonatal outcomes were significantly more common among women with superimposed preeclampsia, including small for gestational age, low birth weight, asphyxia, and neonatal intensive care unit admission. Logistic regression analysis demonstrated that only MAP ≥105 mmHg at 24-28 weeks was independently associated with the increased risk of superimposed preeclampsia by 1.8-fold (adjusted OR 1.8, 95% CI 1.1-3.1, p = 0.031)., Conclusion: Incidence of superimposed preeclampsia was 43.3% among pregnant women with chronic hypertension, with increased adverse neonatal outcomes. High MAP ≥105 mmHg during late second trimester might be an important predictor of the condition.

Aims: The objective of this study was to determine the relationship between second trimester weight gain and the risk of gestational diabetes (GDM) after normal first trimester screening., Methods: A total of 195 women at risk for GDM who had normal first trimester screening results were enrolled. The study group consisted of 65 women who had second trimester weight gain > 7 kg, while another 130 women with weight gain ≤ 7 kg served as the comparison group, matched 1:2 by pre-pregnancy body mass index. GDM screening was repeated during 24-28 weeks of gestation and the incidence of GDM was compared between the groups. Other possible associated factors were evaluated., Results: Mean age, pre-pregnancy body mass index, parity, GDM risks, first trimester weight gain, timing of GDM screening and initial test results were comparable between the groups. The incidence of GDM was significantly higher in the study than in the comparison group (24.6% vs 10.8%, P = 0.012). Logistic regression analysis showed that second trimester weight gain >7 kg and an abnormal 1 oral glucose tolerance test value on first trimester screening independently increased the risk of GDM (adjusted odds ratio 2.6, 95% confidence interval 1.13-6.0, P = 0.025 and 8.36, 95% confidence interval 2.62-26.66, P < 0.001, respectively)., Conclusion: Second trimester weight gain > 7 kg and an abnormal 1 oral glucose tolerance test value at first screening increased the risk of GDM in at-risk women., (© 2016 Japan Society of Obstetrics and Gynecology.)

Rationale, Aims and Objectives: It has been proposed that delivery should be accomplished within 30 minutes after diagnosis of fetal distress. The objective of this study was to determine the decision-to-delivery interval (DDI) in emergency caesarean delivery for non-reassuring fetal heart rate (FHR)., Methods: A total of 272 term, singleton pregnant women who underwent an emergency caesarean section for non-reassuring FHR were included. Patient characteristics and clinical data were reviewed. The timing of the decision-to-delivery process was reviewed., Results: The mean age was 28.7 years; the mean gestational age at delivery was 38.4 weeks; and 93.7% were in FHR category 2. The decision for emergency caesarean delivery was made during normal office hours in 31.6%. Median time for decision-to-operating room, decision-to-incision and decision-to-delivery was 42.3, 48.5 and 56 minutes, respectively. Only 6.6% of women had a DDI of <30 minutes, whereas 30.5% had a DDI of >75 minutes. Significantly shorter intervals were observed for every endpoint among patients in FHR category 3, and they were significantly more likely to deliver within 30 minutes than were those in FHR category 2 (41.2% vs. 4.3%, P < 0.001). Similar results were observed for cases that occurred during normal and after hours. Neonatal outcomes were comparable among different DDI categories., Conclusion: Only 6.6% of women with non-reassuring FHR achieved the 30-minute goal for caesarean delivery (median 56 minutes). Aims: To compare the incidence of large-for-gestational-age (LGA) infants in women diagnosed with gestational diabetes mellitus (GDM) early and late in pregnancy, and evaluate associated factors., Methods: A total of 284 women with GDM who commenced antenatal care before 20 weeks of gestation were enrolled: 142 were diagnosed before 20 weeks (early GDM) and 142 were diagnosed after 20 weeks of gestation after normal initial screening tests (late GDM). Incidence of LGA infants were compared. Factors associated with LGA and pregnancy outcomes were evaluated., Results: Both groups had comparable baseline characteristics. The late GDM group were more likely to gain weight greater than recommended (P = 0.009) and less likely to have optimal glycemic control (P = 0.035). Incidences of maternal and neonatal complications, including LGA, were not significantly different between the groups. Logistic regression analysis demonstrated that the timing of GDM diagnosis was not significantly associated with LGA. Less gestational weight gain than recommended decreased the risk of LGA by 76% (adjusted odds ratio [OR] 0.24, 95% confidence interval [CI] 0.09-0.67, P = 0.007), while gestational weight gain greater than recommended doubled the risk of LGA (adjusted OR 1.99, 95% CI 1.03-3.87, P = 0.041). Good glycemic control also reduced the risk of LGA by 66% (adjusted OR 0.34, 95% CI 0.16-0.71, P = 0.886)., Conclusion: Gestational weight gain and glycemic control, but not timing of diagnosis, were independently associated with LGA in women with GDM. Objective: Gestational diabetes mellitus (GDM) has been related to various maternal and neonatal complications. The degree to which GDM is related to an increased rate of cesarean section is less certain. This study was aimed at comparing the incidence of emergency cesarean delivery between pregnant women with GDM and normal pregnant women., Materials and Methods: The study group consisted of 237 term, singleton pregnant women with GDM. Another 237 uncomplicated, normal pregnant women were randomly selected and served as the comparison group. Those who were scheduled for elective cesarean delivery and overt DM were excluded. Data were retrieved from medical records, including demographic data, antenatal and intrapartum care data, route of delivery, indications for cesarean delivery, and neonatal outcomes., Results: The study group had a significantly higher mean age and body mass index, and the participants were more likely to be overweight/obese. The rate of emergency cesarean delivery was significantly higher in the study group than in the comparison group (31.6% vs. 19.4%, p = 0.002). The study group was more likely to have Cephalo-pelvic disproportion (CPD) (20.3% vs. 13.1%, p = 0.036) as an indication for cesarean delivery. Birth weight was significantly higher (by 200 g) in the study group. When stratified by parity, significant differences in cesarean delivery rates were observed only among nulliparous women. Logistic regression analysis showed that GDM significantly increased the risk of emergency cesarean delivery (adjusted odds ratio 1.9, 95% confidence interval 1.03-3.5, p = 0.039) only among nulliparous women, adjusted for age, body mass index, and gestational weight gain., Conclusion: The incidence of emergency cesarean delivery increased significantly among nulliparous GDM pregnant women, compared with that in normal pregnant women., (Copyright © 2016. Background: Anogenital warts (AGWs) are common results of sexually transmitted infection (STI). Human papillomavirus (HPV) types 6 and 11, which are non-oncogenic types, account for 90% of the clinical manifestations. Although the quadrivalent HPV vaccine has been launched, AGW remains prevalent in some countries and shows association with abnormal cervical cytology., Objectives: To study the prevalence of abnormal cervical cytology (low grade squamous intraepithelial lesions or worse; LSIL+) in immunocompetent Thai women newly presenting with external AGWs., Materials and Methods: Medical charts of all women attending Siriraj STI clinic during 2007-2011 were reviewed. Only women presenting with external AGWs who were not immunocompromised (pregnant, human immunodeficiency virus positive or being on immunosuppressant drugs) and had not been diagnosed with cervical cancer were included into the study. Multivariate analysis was used to determine the association between the characteristics of the patients and those of AGWs and LSIL+., Results: A total of 191 women were eligible, with a mean age of 27.0±8.9 years; and a mean body mass index of 20.6±8.9 kg/m2. Half of them finished university. The most common type of AGWs was exophytic (80.1%). The posterior fourchette appeared to be the most common affected site of the warts (31.9%), followed by labia minora (26.6%) and mons pubis (19.9%). The median number of lesions was 3 (range 1-20). Around 40% of them had recurrent warts within 6 months after completing the treatment. The prevalence of LSIL+ at the first visit was 16.3% (LSIL 12.6%, ASC-H 1.1%, HSIL 2.6%). After adjusting for age, parity and miscarriage, number of warts ≥ 5 was the only factor associated with LSIL+ (aOR 2.65, 95%CI 1.11-6.29, p 0.027)., Conclusions: LSIL+ is prevalent among immunocompetent Thai women presenting with external AGWs, especially those with multiple lesions.

Aim: The aim of this study was to determine the correlation between cord blood and maternal serum magnesium levels among pre-eclamptic pregnant women treated with magnesium sulfate., Materials and Methods: A total of 36 pregnant women, >28weeks' gestation, diagnosed with pre-eclampsia and treated with magnesium sulfate (MgSO(4) ), were enrolled. Maternal and umbilical cord blood samples were obtained immediately after delivery and sent for determination of magnesium and calcium levels., Results:   Mean maternal age was 27.4±6.4years and mean gestational age was 38.1±1.8weeks. Most were diagnosed with pre-eclampsia (94.5%), others had HELLP syndrome. Mean total MgSO(4) the women received was 14.4±6.0g and mean duration of treatment was 5.1±3.3h. Mean maternal serum and cord blood magnesium levels were 2.2±0.4 and 1.8±0.3mmol/L, respectively. Maternal magnesium and calcium levels showed significant positive correlation with their levels in cord blood (P<0.001). Significant positive correlation was found between total dose as well as duration of MgSO4 and maternal and cord blood magnesium levels (P<0.001) but was inversely correlated with maternal and cord blood calcium levels., Conclusions: Maternal serum magnesium and calcium levels were correlated with cord blood levels in pre-eclamptic pregnant women who received MgSO4 therapy., (© 2011 The Authors. This study aimed to evaluate the correlation and agreement between Siriraj liquid-based cervical cytology (Siriraj -LBC) and conventional cytology. A total of 479 women who attended the Department of Obstetric and Gynaecology Siriraj Hospital for cervical cancer screening were enrolled. For each woman collection of cervical cells was performed using VCE technique. After smearing cells on a glass slide for conventional cytology, both broken ends of wooden spatula and cotton swabs were then placed into a plastic vial containing a specific preservative solution for Siriraj-LBC. All specimens were prepared and interpreted by experienced cytotechnologists at the Gynecologic Cytology Unit. Interpretations of the results from one technique were made without knowledge of those from the other technique. The results from both techniques were compared for agreement and correlation. Colposcopy or histology was used as the gold standard. The overall detection rate of abnormal cervicovaginal cells was higher by Siriraj-LBC than by conventional cytology (11.1% vs. 1.67%, P <0.001). These two techniques had high diagnostic agreement of 89.77%, and minimal to fair correlation with a Kappa of 0.128 (P<0.001) and a Spearman rho correlation coefficient of 0.394 (P <0.001). There were 49 cases whose Siriraj-LBC revealed higher cytologic grading than did the conventional cytology; there were no cases of the opposite result. The gold standard was available in 45 cases with abnormal cytology by Siriraj-LBC, revealing a positive predictive value (PPV) of 71.1% for Siriraj-LBC and 97.8% for conventional cytology, and a negative predictive value (NPV) of 42.2% for the conventional cytology. In conclusion, The results from Siriraj-LBC and conventional cytology have high diagnostic agreement and minimal to fair correlation. The Siriraj-LBC increases detection rate of abnormal cervicovaginal cells with probable decrease in false negatives but increase in false positives from the baseline values by conventional cytology. Therefore the screening performance of Siriraj-LBC is not inferior to the conventional cytology and this approach may be used as an alternative screening method for cervical cancer.

Objective: Maternal pre-pregnancy weight is a risk of developing preeclampsia. Whether it is also associated with the disease severity is still elusive. This retrospective cohort was to determine the association between body mass index (BMI) at term and severity of preeclampsia., Material and Method: BMIs on the delivery date of 229 patients were analyzed with various indicators of the disease severity. The corrected BMI (cBMI), calculated by an exclusion of feto-placental unit, was additionally analyzed., Results: Neither maternal BMI nor cBMI correlated with the disease severity (p = 0.15 and 0.36). Patients who did and did not require MgSO4 do not have different BMI or cBMI (p = 0.12 and 0.23). Neonatal weight from severe disease arm does not differ from those with mild disease (p = 0.51). Counter-intuitively the correlations between birth weight and maternal BMI were stronger in the severe compared to the mild group (p = 0.0 and 0. 03)., Conclusion: Neither BMI nor cBMI at the time of delivery predict the severity of preeclampsia or the need for seizure prophylaxis. Birth weight of the baby born from preeclamptic mother might be affected by multiple factors.

Objective: To examine the rate of women with normal initial results to glucose tolerance tests who have abnormal results to subsequent testing, and estimate the risk of gestational diabetes mellitus (GDM) in these women., Methods: Baseline plasma glucose levels were classified as normal if they were less than 120 mg/dL (group 1) or between 120 and 139 mg/dL (group 2) by the 50-g glucose challenge test (GCT); as abnormal if they were found abnormal by the 50-g GCT but normal by the 100-g glucose tolerance test (OGTT) (group 3); and as abnormal if 1 of the four 100-g OGTT values was abnormal (group 4). A second testing session with the 50-g GCT and 100-g OGTT was performed between the 24th and 28th weeks of pregnancy for 900 women at risk whose initial test results were normal., Results: Of the 823 women with normal baseline results who completed the study, 41.4% had abnormal results to the second 50-g GCT, and gestational diabetes mellitus was diagnosed by the 100-g OGTT in 7.0% of these 823 women. Compared with group 1, the women in groups 2, 3, and 4 were at a significantly increased risk of having an abnormal result to the second 50-g GCT. They were also at a significantly increased risk for GDM. The adjusted odds ratios (ORs) were 3.0 for group 2 (95% confidence interval [CI], 1.2-7.2), 4.9 for group 3 (95% CI, 2.2-11.0), and 11.3 for group 4 (95% CI, 3.9-32.6)., Conclusion: The risk of developing GDM significantly increased with increasing baseline plasma glucose levels by the 50-g GCT.

Objective: To determine ultrasonographic appearances in pregnant women clinically diagnosed with threatened abortion., Design: Cross-sectional study., Material and Method: Seven hundred and seventy six pregnant women clinically diagnosed with threatened abortion and receiving ultrasonographic examination were enrolled Data on ultrasonographic characteristics were obtained from records at the Maternal-fetal Medicine unit. Pregnancy outcomes were reviewed from medical records., Results: The ultrasonographic findings demonstrated 328 (42.3%) viable pregnancy, 178 (22.9%) embryonic death, 176 (22.7%) anembryonic pregnancy, 25 (3.2%) incomplete abortion, 24 (3.1%) complete abortion, seven (0.9%) molar pregnancy, four (0.5%) ectopic pregnancy, and 34 (4.4%) inconclusive finding. Two hundred and sixty viable pregnancies were available for follow-up and revealed that 229 (88.1%) eventually delivered while 31 (11.9%) ended up with abortion. The two groups were not significantly different regarding age, parity, history of abortion, and gestational age at diagnosis., Conclusion: Ultrasonographic findings in patients clinically diagnosed with threatened abortion demonstrated viable pregnancy in nearly half of the cases. Transvaginal ultrasonography is useful in establishing definite diagnosis and appropriate treatment among these patients.

Objective: To identify risk factors associated with retained placenta after vaginal delivery., Design: Case-control study., Material and Method: Medical records of 234 pregnant women whose gestational age > or =28 weeks were reviewed. Cases comprised of 78 women with retained placenta after vaginal delivery and controls comprised of 156 women with spontaneous placental delivery. Associated risk factors were examined Chi-square test and logistic regression analysis were used for analysis of data., Results: Cases were significantly older than controls (29.3 +/- 6.4 vs. 27.0 +/- 6.4 years respectively, p = 0.01). Cases were likely more significant than controls to have a previous history of uterine curettage (20.5% vs. 6.4% respectively, p = 0.001) and premature rupture of membranes (35.9% vs. 22.4% respectively, p = 0.029). Between the two groups, there were no differences in gestational age, parity, previous abortion, induction of labor, oxytocin, and pethidine usage. Logistic regression analysis showed that independent risk factors for retained placenta were age (adjusted OR 1.06, 95% CI 1.01-1.11), previous uterine curettage (adjusted OR 4.2, 95% CI 1.7-9.9), and PROM (adjusted OR 2.2, 95% CI 1.2-4.1)., Conclusion: Maternal age, previous uterine curettage, and PROM were independently associated with increased risk of retained placenta. The condition should be aware of among pregnant women with such risk factors.

Objective: To evaluate the rate of abnormal results of repeated screening tests for gestational diabetes mellitus when initial tests were normal and related factors., Subjects: Six hundred women who had clinical risk factors for gestational diabetes mellitus (GDM) who attended the antenatal care clinic at Siriraj Hospital before 24 weeks of gestation, between January and June 2005 were recruited All had normal screening test., Material and Method: All subjects were followed throughout their pregnancies. All received repeated screening and confirmatory test at 28-32 weeks ofgestation. All data of screening and confirmatory test results were collected and analyzed., Results: Six hundred pregnant women who had normal screening test for GDM were enrolled Eighty-seven cases failed to take the second screening test. Of the remaining 513 cases, 154 (30.0%, 95% CI 28.2%-36.3%) had abnormal results in repeated screening tests. Among them 20 cases (3.9%) were diagnosed as GDM. Pregnant women who were > or =30 years old or had result of 50 g GCT > or =120 mg/dl had significant increased risk for abnormal repeated screening tests., Conclusions: Pregnant women with clinical risk of GDM should receive repeated screening tests when they are 28-32 weeks of gestation. Higher risk was observed among women > or =30 years old or those with a result of 50 g GCT > or =120 mg/dl.

Objective: To determine the rate of prophylactic antibiotics usage in uncomplicated gynecologic abdominal surgery in Siriraj Hospital, before and after guideline implementation., Material and Method: Two hundred and fifty-eight women who underwent elective gynecologic surgery for a benign condition were enrolled. The patients were divided into two groups, before and after guideline implementation (control and study group). Medical records of these women were reviewed to determine the rate of prophylactic antibiotics usage, rate of postoperative infection, and antibiotics cost., Results: The most common prophylactic antibiotics used were Cefazolin (41.9%), Cefoxitin (36.4%), and Augmentin (9.7%). Rate of single dose of cefazolin usage were not significantly different between the two groups (10.1% and 12.4% respectively, p = 0.482). However, the rate of postoperative oral antibiotic usage was significantly decreased (31.8% and 14.7% respectively, p = 0.001). The reduction of oral antibiotic usage was significant among staff only. The rate of postoperative infection between control and study groups, as well as between single and other antibiotic prophylaxis were similar. Had a single dose of cefazolin been administered to all patients, the antibiotics cost would have been reduced by 102,012 Baht or 91.8%., Conclusion: Rate of a single dose of cefazolin usage as antibiotic prophylaxis in gynecologic surgery was similar to before guideline implementation. However, postoperative oral antibiotics usage decreased significantly, especially among the staff.

Objective: To study the prevalence of bacterial vaginosis (BV) in pregnant women with preterm labor in Siriraj hospital., Material and Method: A cross-sectional study of 158 pregnant women with suspected preterm labor was performed between January and July 2005. The subjects enrolled in the present study were between 28+0 and 36+6 menstrual weeks. BV blue test was performed on the vaginal fluid collected from lower one- third of vagina., Results: The prevalence of BV in women in the preterm labor group was 25.8% compared to 14.1% in the preterm contraction group (p = 0.07)., Conclusion: Compared with preterm contractions a higher prevalence of BV was found in the pregnant women with preterm labor. Given that a quarter of pregnant women with preterm labor tested positive for BV, it might be appropriate to perform this test in the triage setting.

Objectives: To evaluate the relationship between maternal height of < 155 cm and the risk of caesarean delivery due to cephalo-pelvic disproportion (CPD) among nulliparous women., Design: Retrospective cohort study, Material and Method: A total of 660 term (GA > or = 37 weeks), uncomplicated singleton nulliparous pregnant women were enrolled on admission to labour room. The patients were divided into two groups based on maternal height, < or = 155 cm (440 cases) as control and < 155 cm (220 cases) as study group. The medical records of these women were reviewed. Various baseline clinical characteristics were collected. Intrapartum characteristics and maternal and neonatal outcomes were recorded., Results: The rate of caesarean section of all indications was only slightly higher among study group than control group (16.4% and 13.7% respectively, p = 0.514). No significant difference was observed in the rate of caesarean delivery due to CPD (7.3% and 10.5% in control and study group respectively, p = 0.376). The rate of caesarean delivery due to CPD was highest among those with height < 150 cm (p < 0.001). Mean birth weight was significantly lower among study group than control group (2,927.7 +/- 368.1 g and 3,068.4 +/- 358.5 g respectively, p < 0.001). Low birth weight (< 2,500 g) was significantly higher among study group than control group (10.9% and 3.2% respectively, p < 0.001)., Conclusion: Term singleton nulliparous pregnant women with maternal height < 155 cm were not associated with a greater likelihood of caesarean section for CPD. However, mean birth weight was significantly lower and low birth weight was significantly increased among mothers with short stature.
