Abstract

Background The BOLERO-2 trial reported efficacy and safety of Everolimus (EVE) and Exemestane (EXE) combination in HR+ advanced breast cancer (ABC) patients. The BALLET trial further evaluated the safety of EVE-EXE in HR+ ABC patients, without reporting efficacy data. Aim of the EVA real-life study was to collect data of efficacy and safety of EVE-EXE combination in the clinical setting, as well as exploring efficacy according to EVE Dose-Intensity (DI) and to previous treatment with Fulvestrant. Patients and methods This study aimed to describe the outcome of ABC pts treated with EVE-EXE combination in terms of median duration of EVE treatment and ORR in a real-life setting. Results From July 2013 to December 2015, the EVA study enrolled 404 pts. Median age was 61 years (33–83). Main metastatic sites were: bone (69.1%), soft tissue (34.7%) and viscera (33.2%). Median number of previous treatments was 2 (1–7). 43.3% of the pts had received Fulvestrant. Median exposure to EVE was 31.0 weeks (15.4–58.3) in the whole population. No difference was observed in terms of EVE exposure duration according to DI (p for trend = 0.27) or type of previous treatments (p = 0.33). ORR and Disease Control Rate (DCR) were observed in 31.6% and 60.7% of the patients, respectively, with the lowest ORRs confined in CHT pre-treated patients or in those who received the lowest DI of EVE. Grade 3-4 adverse events (AEs) were reported in 37.9% of the patients. Main AEs were: stomatitis (11.2%), non-infectious pneumonitis - NIP (3.8%), anaemia (3.8%) and fatigue (3.2%). Conclusions The EVA study provided new insights in the use of EVE-EVE combination in HR+ ABC pts many years after the publication of the pivotal trial. The combination is safe and the best response could be obtained in patients receiving the full dose of EVE and/or after hormone-therapy as Fulvestrant in ABC.

Abstract

Metronomic chemotherapy (mCHT) refers to the minimum biologically effective dose of a chemotherapy agent given as a continuous dosing regimen, with no prolonged drug-free breaks, that leads to antitumor activity. Aim of the present study is to describe the use of mCHT in a retrospective cohort of metastatic breast cancer (MBC) patients in order to collect data regarding the different types and regimens of drugs employed, their efficacy and safety. Between January 2011 and December 2016, data of 584 metastatic breast cancer patients treated with mCHT were collected. The use of VRL-based regimens increased during the time of observation (2011: 16.8% - 2016: 29.8%), as well as CTX-based ones (2011: 17.1% - 2016: 25.6%), whereas CAPE-based and MTX-based regimens remained stable. In the 1st-line setting, the highest ORR and DCR were observed for VRL-based regimens (single agent: 44% and 88%; combination: 36.7% and 82.4%, respectively). Assuming VRL-single agent as the referee treatment (median PFS: 7.2 months, 95% CI: 5.3–10.3), the longest median PFS were observed in VRL-combination regimens (9.5, 95%CI 88.8–11.3, HR = 0.72) and in CAPE-single agent (10.7, 95%CI 8.3–15.8, HR = 0.70). The VICTOR-6 study provides new data coming from the real-life setting, by adding new information regarding the use of mCHT as an option of treatment for MBC patients.

Abstract

We report the case of an 18-month-old girl who presented a complete III right cranial nerve paralysis which disappeared spontaneously in approximately three weeks without any medical therapy. Magnetic Resonance (MR) investigation performed three days after paralysis onset showed a focal area of contrast enhancement at the third cranial nerve emergence. MR performed three months later, with an absence of symptoms, showed a less evident but persistent contrast enhancement at the cranial nerve emergence. We briefly discuss the causes of inflammatory nerve involvement since a parainfective cause of nerve paralysis was implicated.

Abstract

Metronomic chemotherapy (mCHT) refers to the minimum biologically effective dose of a chemotherapy agent given as a continuous dosing regimen, with no prolonged drug-free breaks, that leads to antitumor activity. Aim of the present study is to describe the use of mCHT in a retrospective cohort of metastatic breast cancer (MBC) patients in order to collect data regarding the different types and regimens of drugs employed, their efficacy and safety. Between January 2011 and December 2016, data of 584 metastatic breast cancer patients treated with mCHT were collected. The use of VRL-based regimens increased during the time of observation (2011: 16.8% - 2016: 29.8%), as well as CTX-based ones (2011: 17.1% - 2016: 25.6%), whereas CAPE-based and MTX-based regimens remained stable. In the 1st-line setting, the highest ORR and DCR were observed for VRL-based regimens (single agent: 44% and 88%; combination: 36.7% and 82.4%, respectively). Assuming VRL-single agent as the referee treatment (median PFS: 7.2 months, 95% CI: 5.3–10.3), the longest median PFS were observed in VRL-combination regimens (9.5, 95%CI 88.8–11.3, HR = 0.72) and in CAPE-single agent (10.7, 95%CI 8.3–15.8, HR = 0.70). The VICTOR-6 study provides new data coming from the real-life setting, by adding new information regarding the use of mCHT as an option of treatment for MBC patients.

25. Exploring food security as a multidimensional topic: twenty years of scientific publications and recent developments.

Abstract

The scientific literature dealing with food security is vast and fragmented, making it difficult to understand the state of the art and potential development of scientific research on a central theme within sustainable development. The current article, starting from some milestone publications during the 1980s and 1990s about food poverty and good nutrition programmes, sets out the quantitative and qualitative aspects of a vast scientific production that could generate future food security research. It offers an overview of the topics that characterize the theoretical and empirical dimensions of food security, maps the state of the art, and highlights trends in publications' ascending and descending themes. To this end the paper applies quantitative/qualitative methods to analyse more than 20,000 scientific articles published in Scopus between 2000 and 2020. Evidence suggests the need to find more robust links between micro studies on food safety and nutrition poverty and macro changes in food security, such as the impact of climate change on agricultural production and global food crises. However, the potential inherent in the extensive and multidisciplinary research on food safety encounters limitations, particularly the difficulty of theoretically and empirically connecting the global and regional dimensions of change (crisis) with meso (policy) and micro (individual behaviour) dimensions., (© The Author(s) 2022, corrected publication 2022.)

Abstract

This study explores the parental role of transsexuals and compares this to common assumptions about transsexuality and parentage. We conducted semi-structured interviews with 14 male-to-female transsexuals and 14 men, half parents and half non-parents, in order to explore four thematic areas: self-representation of the parental role, the description of the transsexual as a parent, the common representations of transsexuals as a parent, and male and female parental stereotypes. We conducted thematic and lexical analyses of the interviews using Taltac2 software. The results indicate that social representations of transsexuality and parenthood have a strong influence on processes of self-representation. Transsexual parents accurately understood conventional male and female parental prototypes and saw themselves as competent, responsible parents. They constructed their role based on affection toward the child rather than on the complementary role of their wives. In contrast, men's descriptions of transsexual parental roles were simpler and the descriptions of their parental role coincided with their personal experiences. These results suggest that the transsexual journey toward parenthood involves a high degree of re-adjustment, because their parental role does not coincide with a conventional one.

Abstract

A field trial was carried out during a summer-fall period on a commercial beef cattle farm in Minas Gerais State, located in the Southeast of Brazil. In order to evaluate the prophylactic effect and the curative efficacy of fipronil in a 1% solution, 200 Zebu crossbred bulls, with ages varying from 20 to 30 months and weights from 233 to 362 kg, were selected. The bulls were assigned by ranked pair to an untreated control group (A) or to a treated group (B), resulting in 100 animals per group. All experimental animals were surgically castrated on day 0, following routine procedures. After castration all animals in the group B were treated with 10 mg/kg bw of a 1% fipronil solution, topically on the dorsal mid-line. The wounds were individually inspected on days: 2, 4, 7, 10, 14, 17, 21, 28 and 35. After castration the animals were naturally exposed to Cochliomyia hominivorax and remained in the same pasture throughout the trial. Among the animals in the control group, 83 were observed to harbor C. hominivorax eggs, with a total of 97 ovipositions, and among those 73 animals had active myiasis. In group B (fipronil 1%), 66 animals showed C. hominivorax eggs, with 92 ovipositions and five animals with active myiasis. Most ovipositions and active myiasis were detected until seven days post-castration for both groups. Wound parasite infestation evidenced bleeding, serous purulent exudation and presence of active C. hominivorax larvae. Treatment with fipronil 1% had a prophylactic effect on scrotal wounds against the development of C. hominivorax larvae in more than 95% of the treated animals for up to 17 days after castration. The treatment showed partial protection of 66% and 50% on days 21 and 28 post-treatment (pt), respectively. Three animals from the control group and one from the treated group showed active screwworms on day 21 pt, and one animal from the treated group and two from the control group also presented C. hominivorax larvae on scrotal wounds on day 28 pt. By the end of the observation period (day 35 pt), the castration wound had healed in all animals. All experimental animals presenting scrotal wounds infested with C. hominivorax larvae were treated with a 1% pour-on formulation of fipronil, on the same day that infestation was observed. Active C. hominivorax larvae were not seen during the monitoring period immediately after treatment. The curative efficacy of fipronil 1% against C. hominivorax larvae infestation in castration wounds was 100%.
