Összefoglaló. A járvánnyal szembeni védekezés során mindennapossá vált a maszkhasználat, amely jelentős kihívást jelent a hétköznapi kommunikációs helyzetekben. Az egészségügy bizonyos területein korábban is jelen volt ez a probléma, azonban a most általános érvényű, folyamatos maszkviselés és védőruha-használat szimbolikus és gyakorlati értelemben is akadályként jelenik meg az orvos és a páciens között, ami több szinten nehezíti a hatékony kapcsolatteremtést és kommunikációt. Ezért szükségessé vált olyan egységes kommunikációs protokollok kidolgozása, amelyek segítik a bizalmi kapcsolat kiépítését az egészségügyben úgy, hogy az ellátók érzelmi elérhetősége és a biztonságos fizikai távolságtartás egyszerre valósuljon meg. Tanulmányunk célja, hogy a kurrens szakirodalom áttekintése révén összefoglaljuk a maszkviselés és a védőruha-használat hatásait az orvos-beteg kommunikációra és a kapcsolatban megjelenő bizalomra és empátiás folyamatokra, valamint fel kívánjuk hívni a figyelmet az egészségügyi személyzetet érő hatásokra is. Célunk továbbá, hogy a hazai és nemzetközi tapasztalatokat figyelembe véve olyan ajánlásokat fogalmazzunk meg, melyek segítségével javítható az egészségügyi dolgozók kommunikációja, azon keresztül pedig az ellátás legfontosabb szereplőinek, a betegeknek és az ellátóknak az elégedettsége a kapcsolattal. Orv Hetil. 2021; 162(23): 897–904. Summary. In the fight against the epidemic, the use of masks has become commonplace, which is a significant challenge in everyday communication situations. This problem has been present in some areas of healthcare in the past, but today’s universal use of masks and personal protective equipment (PPE) is both a symbolic and practical barrier between doctor and patient, making it difficult to establish an effective communication and connectivity on several levels. Therefore, it has become necessary to develop unified communication protocols that help building a relationship of trust in healthcare so that emotional availability of providers and safe physical distance are achieved simultaneously. The aim of our study is to summarize the effects of mask wearing and PPE usage on the physician-patient communication and also the trust and empathy processes that emerge within the relationship through the review of the current literature. We would also like to draw attention to the effects on healthcare staff. Our goal is also to formulate recommendations, taking into account domestic and international experience, that can be used to improve the communication of healthcare professionals, and through that, the satisfaction of the most important actors in care, patients and caregivers. People's attitudes toward time, the time perspectives (TPs) rooted in past experiences, might be crucial for the emotional evaluation of life events, such as painful situations. In a survey-based study (N = 353), we investigated the relationship of TPs with the self-perceived affective and physical aspects of pain. Specifically, we hypothesised that past-negative TP would be associated with a negative emotional appraisal of pain characterised by higher pain-related catastrophizing, fear, and physical sensitivity to pain. We tested this hypothesis with hierarchical regression models, adding depression, anxiety, and variables of early life circumstances to the model stepwise. Early life circumstances (i.e., socioeconomic status, household unpredictability, and family resources) were measured retrospectively using questionnaires. In addition, based on the results of the regression analyses, we built a structural equation model (SEM) and tested whether past-negative mediates the effect of household unpredictability toward emotional and physical appraisal of pain. We found that individuals scoring high on past-negative TP had greater pain catastrophizing, pain-related fear, and sensitivity to pain. Further, there was evidence that these associations are independent from depression, anxiety, and early life circumstances. Finally, SEM analysis suggested that unpredictable early life circumstances were directed toward intense, negatively emotionalised pain perception via the mediation of the past-negative TP. The findings generally support the view that pain behaviour is formed by emotional states and attitudes that are linked to past experiences that are not necessarily specific to the context of pain.

The increasing incidence of infertility and contrary the remaining levels of success rate of assisted reproductive therapies (ART) justify expanding the biomedical approach and underline the importance of psychosocial and lifestyle factors. The aim of the study was to examine the effects of pre-treatment physical activity (PA) on the quality of life (QoL) in infertile women. A cross sectional cohort study was carried out involving 50 female patients with infertility diagnosis. Standardized QoL questionnaires and pre-treatment PA patterns were administrated during December 2018 – April 2019 in a university hospital-based fertility centre in South-Hungary. 43% of the women reported regular exercise. Regarding GPAQ moderate intensity PA in work and recreation was preferred. They spent 4,2 hours with active transportation weekly, and 4.5 hours sedentary daily. 68,89% reached the 150min/week MVPA recommendation and 18,2% spent more than 4 hours with exercise Respondents were less satisfied with Physical Domain (57.7±7.5) and most with Psychological Domain (72.2±12.1) of WHOQOL-BREF. Lower QoL by Emotional (56.46±18.12) and Mind-Body (56.84±19.67), mediate by Social (62.76±17.40) and better values by Relational Subscale (76.48±19.62) were measured with FertiQoL. Significant difference between active and inactive group was described for Psychological domain (Z=-2.240, p=0.025). Positive correlation was found between MVPA vs Total MET and Emotional Subscale (p=0.014, p=0.009) and between work-related PA and Mind-body Subscale (p=0.006). Pre-treatment PA could positively affect QoL undergoing ART. However, for more impressive results, objective assessment of physical activity, increased number of participants, and further examinations are needed.

Background Individuals frequently show habituation to repeated noxious heat. However, given the defensive function of human pain processing, it is reasonable to assume that individuals anticipate that they would become increasingly sensitive to repeated thermal pain stimuli. No previous studies have, however, been addressed to this assumption. Therefore, in the current study, we investigated how healthy human individuals imagine the intensity of repeated thermal pain stimulations, and compared this with the intensity ratings given after physically induced thermal pain trials. Methods Healthy participants (N = 20) gave pain intensity ratings in two conditions: imagined and real thermal pain. In the real pain condition, thermal pain stimuli of two intensities (minimal and moderate pain) were delivered in four consecutive trials. The duration of the peak temperature was 20 s, and stimulation was always delivered to the same location. In each trial, participants rated the pain intensity twice, 5 and 15 s after the onset of the peak temperature. In the imagined pain condition, participants were subjected to a reference pain stimulus and then asked to imagine and rate the same sequence of stimulations as in the induced pain condition. Results Ratings of imagined pain and physically induced pain followed opposite courses over repeated stimulations: Ratings of imagined pain indicated sensitization, whereas ratings for physically induced pain indicated habituation. The findings were similar for minimal and moderate pain intensities. Conclusions The findings suggest that, rather than habituating to pain, healthy individuals imagine that they would become increasingly sensitive to repeated thermal pain stimuli. Significance This study identified opposite patterns of change in perception of imagined pain (sensitization) and physically induced pain (habituation). The findings show that individuals anticipate that they would become increasingly sensitive to repeated pain stimuli, which might also have clinical implications.

Former studies have revealed the impact of a negative view of the past on postoperative pain intensity and pain catastrophizing (i.e. pain vulnerability). In this study, we extensively examined the association between time perspective (TP) and pain vulnerability in a healthy sample. The Zimbardo Time Perspective Inventory, Pain Catastrophizing Scale and Pain Sensitivity Questionnaire were administered, along with evaluations of pictures about different painful situations (N = 226). Data analysis included examination of the impact of the unique contribution of the five TPs, the deviation from a balanced TP profile and defining different TP clusters on pain vulnerability. Results imply that a negative view of the past might increase sensitivity against physical pain and disrupt the affective processing of painful events, suggesting the relevance of TP in pain therapy.

Recent theoretical work on the Life History Theory and empirical findings on Machiavellianism suggest that Machiavellian individuals are motivated to acquire short-term benefits and prioritize situations with high potential rewards. Accordingly, in our study, we investigated the associations between reward/punishment sensitivity and Machiavellian interpersonal tactics with self-report measures. Moreover, as a first attempt, we investigated the correlates of Machiavellianism and the behavioral preference for rewards with the IOWA Gambling task (IGT). The results showed robust positive associations of Machiavellian behavioral characteristics with Sensitivity to Reward, and a moderately negative correlation with Sensitivity to Punishment. This finding was further supported by IGT: Machiavellians tended to make reward-oriented decisions.

Background Health literacy (HL) is low for 40–50 % of the population in developed nations, and is strongly linked to many undesirable health outcomes. Older adults are particularly at risk. The intervention research on health literacy in ageing populations project systematically created a large inventory of HL interventions targeting adults age 50+ , to support practical production of policy and practice guidelines for promoting health literacy in European populations. Methods We comprehensively surveyed international scientific literature, grey literature and other sources (published 2003+) for implemented HL interventions that involved older adults. Studies were screened for eligibility criteria and further selected for aspects important in European public health policy, including priority diseases, risk factors and vulnerable target groups. Interventions were prioritised using a multiple criteria tool to select final interventions that also featured strong evidence of efficacy and a broad range of strategies. Results From nearly 7000 written summaries, 1097 met inclusion criteria, of which 233 were chosen for scoring and ranking. Of these, seven had the highest multi-criteria scores. Eight more articles were selected based on rounded criteria including a high multi-criteria score as well as elements of innovation. Final selections were 18 articles describing 15 programmes, which feature strong evidence of efficacy among important diseases or risk factors and vulnerable groups, or that had success with elements of innovation were identified. Most programmes tried to increase skills in communication, self-management and understanding healthcare or lifestyle choices. Conclusions These programmes have multiple positive attributes which could be used as guidance for developing innovative intervention programmes to trial on European older adults. They provide evidence of efficacy in addressing high priority diseases and risk factors. Human observers can recognize natural images very effectively. Yet, in the literature there is a debate about the extent to which the recognition of natural images requires controlled attentional processing. In the present study we address this topic by testing whether natural scene recognition is affected by mental fatigue. Mental fatigue is known to particularly compromise high-level, controlled attentional processing of local features. Effortless, automatic processing of more global features of an image stays relatively intact, however. We conducted a natural image categorization experiment ( N = 20) in which mental fatigue was induced by time-on-task (ToT). Stimuli were images from 5 natural scene categories. Semantic typicality (high or low) and the magnitude of 7 global image properties were determined for each image in separate rating experiments. Significant performance effects of typicality and global properties on scene recognition were found, but, despite a general decline in performance, these effects remained unchanged with increasing ToT. The findings support the importance of the global property processing in natural scene recognition and suggest that this process is insensitive to mental fatigue.
