- Abstract
A self-mixing laser interferometer has been combined with a compact and robust optical system including an adaptive optical element in the form of a voltage controllable liquid lens. The use of the liquid lens enables the self-mixing interferometer to adjust the optical focus position and the beam spot diameter on the target surface, and subsequently the feedback level within the laser cavity. The optical system has been designed to focus the beam at distances from a few centimetres from the front facet of the laser diode to infinity. With such a simple arrangement, it is possible to modify and control the intensity of the back reflected light from the target surface into the laser cavity, by simply changing the voltage applied to the lens to modify the focus condition on the target surface. The final effect obtained is full active control of the feedback level of the self-mixing effect taking place. This has allowed keeping the feedback level of the interferometer in the desired regime for measurements along very long distances and for different measurement situations, so extending the capabilities of a classical self-mixing interferometer. The advantage of the proposed adaptive optical head is thus its combination of precise metrology capabilities plus a great potential in automated feedback control and operator-free industrial applications. Signal reconstruction of the target vibration amplitude presents a maximum error of ¿/16 as compared with a commercial capacitive sensor in the whole focusable range for displacement measurements. 23. Lack of interaction between valaciclovir, the L-valyl ester of acyclovir, and Maalox antacid.

- Abstract
Valaciclovir is rapidly and extensively converted to acyclovir. In this study we investigated the potential interaction between oral valaciclovir and Maalox. On each of three occasions 18 healthy volunteers received a single oral dose of 1000 mg valaciclovir, or 30 mL Maalox 65 min after valaciclovir administration, or 30 mL Maalox 30 min before valaciclovir. Acyclovir plasma concentrations and pharmacokinetic parameters were not significantly affected by administration of Maalox before or after valaciclovir. Therefore, there is no need for restriction of valaciclovir dosing in patients receiving antacid medication. [ABSTRACT FROM AUTHOR]

- Abstract
System level simulation results of a planar electrostatic micromotor, based on analytical models of the static and dynamic torque behaviours, are presented. A planar variable capacitance (VC) electrostatic micromotor designed, fabricated and tested at LAAS (Toulouse) in 1995 is simulated using the high level language VHDLAMS (VHSIC (very high speed integrated circuits) hardware description languageanalog mixed signal). The analytical torque model is obtained by first calculating the overlaps and capacitances between different electrodes based on a conformal mapping transformation. Capacitance values in the order of 10−16 F and torque values in the order of 10−11 N m have been calculated in agreement with previous measurements and simulations from this type of motor. A dynamic model has been developed for the motor by calculating the inertia coefficient and estimating the friction-coefficient-based values calculated previously for other similar devices. Starting voltage results obtained from experimental measurement are in good agreement with our proposed simulation model. Simulation results of starting voltage values, step response, switching response and continuous operation of the micromotor, based on the dynamic model of the torque, are also presented. Four VHDLAMS blocks were created, validated and simulated for power supply, excitation control, micromotor torque creation and micromotor dynamics. These blocks can be considered as the initial phase towards the creation of intellectual property (IP) blocks for microsystems in general and electrostatic micromotors in particular.

25. Pharmacokinetics of Oral Acyclovir in Neonates and in Infants: a Population Analysis

- Abstract
ABSTRACTAcyclovir is approved for the treatment of herpes simplex virus (HSV) and varicella-zoster virus (VZV) infections in children by the intravenous and oral routes. However, its use by the oral route in children younger than 2 years of age is limited due to a lack of pharmacokinetic data. The objectives of the present study were to determine the typical pharmacokinetics of an oral suspension of acyclovir given to children younger than 2 years of age and the interindividual variabilities in the values of the pharmacokinetic parameters in order to support the proposed dosing regimen (24 mg/kg of body weight three times a day for patients younger than 1 month of age or four times a day otherwise). Children younger than age 2 years with HSV or VZV infections were enrolled in a multicenter study. Children were treated for at least 5 days with an acyclovir oral suspension. Plasma samples were obtained at steady state, before acyclovir administration, and at 2, 3, 5, and 8 h after acyclovir administration. Acyclovir concentrations were measured by radioimmunoassay. The data were analyzed by a population approach. Data for 79 children were considered in the pharmacokinetic study (212 samples, 1 to 5 samples per patient). Acyclovir clearance was related to the estimated glomerular filtration rate, body surface area, and serum creatinine level. The volume of distribution was related to body weight. The elimination half-life decreased sharply during the first month after birth, from 10 to 15 h to 2.5 h. Bioavailability was 0.12. The interindividual variability was less pronounced when the parameters were normalized with respect to body weight. Hence, dosage adjustment by body weight is recommended for this population. Simulations showed that the length of time that acyclovir remains above the 50% inhibitory concentration during a 24-h period was more than 12 h for HSV but not for VZV. The proposed dosing regimen seems adequate for the treatment of HSV infections, while for the treatment of VZV infections, a twofold increase in the dose seems necessary for children older than age 3 months.

Eser, Sultan (Balikesir Author), Background In 2015, the second cycle of the CONCORD programme established global surveillance of cancer survival as a metric of the effectiveness of health systems and to inform global policy on cancer control. CONCORD-3 updates the worldwide surveillance of cancer survival to 2014. Methods CONCORD-3 includes individual records for 37.5 million patients diagnosed with cancer during the 15-year period 2000-14. Data were provided by 322 population-based cancer registries in 71 countries and territories, 47 of which provided data with 100% population coverage. The study includes 18 cancers or groups of cancers: oesophagus, stomach, colon, rectum, liver, pancreas, lung, breast (women), cervix, ovary, prostate, and melanoma of the skin in adults, and brain tumours, leukaemias, and lymphomas in both adults and children. Standardised quality control procedures were applied; errors were rectified by the registry concerned. We estimated 5-year net survival. Estimates were age-standardised with the International Cancer Survival Standard weights.Findings For most cancers, 5-year net survival remains among the highest in the world in the USA and Canada, in Australia and New Zealand, and in Finland, Iceland, Norway, and Sweden. For many cancers, Denmark is closing the survival gap with the other Nordic countries. Survival trends are generally increasing, even for some of the more lethal cancers: in some countries, survival has increased by up to 5% for cancers of the liver, pancreas, and lung. For women diagnosed during 2010-14, 5-year survival for breast cancer is now 89.5% in Australia and 90.2% in the USA, but international differences remain very wide, with levels as low as 66.1% in India. For gastrointestinal cancers, the highest levels of 5-year survival are seen in southeast Asia: in South Korea for cancers of the stomach (68.9%), colon (71.8%), and rectum (71.1%); in Japan for oesophageal cancer (36.0%); and in Taiwan for liver cancer (27.9%). By contrast, in the same world region, survival is generally lower than elsewhere for melanoma of the skin (59.9% in South Korea, 52.1% in Taiwan, and 49.6% in China), and for both lymphoid malignancies (52.5%, 50.5%, and 38.3%) and myeloid malignancies (45.9%, 33.4%, and 24.8%). For children diagnosed during 2010-14, 5-year survival for acute lymphoblastic leukaemia ranged from 49.8% in Ecuador to 95.2% in Finland. 5-year survival from brain tumours in children is higher than for adults but the global range is very wide (from 28.9% in Brazil to nearly 80% in Sweden and Denmark). Interpretation The CONCORD programme enables timely comparisons of the overall effectiveness of health systems in providing care for 18 cancers that collectively represent 75% of all cancers diagnosed worldwide every year. It contributes to the evidence base for global policy on cancer control. Since 2017, the Organisation for Economic Co-operation and Development has used findings from the CONCORD programme as the official benchmark of cancer survival, among their indicators of the quality of health care in 48 countries worldwide. 27. Disappearance of fringes in the self-mixing interferometry sensing scheme: impact of the initial laser mode solution.

- Abstract
We report here the first-ever, to the best of our knowledge, observation of an inconsistency in the fringe disappearance that occurs in self-mixing interferometers. The disappearance of fringes has been observed in vibration and absolute distance sensing schemes under moderate/strong feedback regimes, and it has a major impact on the design of self-mixing sensors. The number of missing fringes that mostly depends on the feedback strength is also linked to the establishment of the initial stable solution, and, as a consequence, the first modulation period will result in more fringes than expected in the case of an already permanent modulation. We demonstrate that this phenomenon is entirely predicted by the well-admitted dynamic rate equation model of the laser under optical feedback followed by the perfect agreement with experimental results.

28. Speckle-insensitive fringe detection method based on Hilbert transform for self-mixing interferometry.

- Abstract
A robust phase measurement method to detect self-mixing signals from rough surface targets is addressed. Using the Hilbert transform to create the analytic signal of the monitored optical output power, an accurate instantaneous phase can be extracted without being affected by amplitude variations induced by the speckle phenomenon. In case of fringe fading, a running average filter removes the local bias offset improving the calculations. This also allows one to observe the phase at dynamic feedback levels within the same calculation, avoiding the need of adding external optical, mechanical or electronic components. This method provides confidence for the fringe detection required in subsequent steps to reach subwavelength precision of a reconstructed displacement or for velocimetry applications.

- Abstract
In this Letter, we experimentally show that transient phenomenons in self-mixing signals from a moving target contain information about the target reflectivity and distance. These transient phenomenons are well explained with a dynamical model of the laser diode, which is used to trace an abacus giving the target reflectivity and distance from a measured high-bandwidth, self-mixing signal.

- Abstract
A self-mixing (SM) laser displacement sensor coupled with a microelectromechanical system (MEMS) accelerometer is presented that enables reliable displacement measurements even in the case of a nonstationary laser head. The proposed technique allows the use of SM-based sensors for embedded applications. The system resolution is currently limited to approximately 300 nm due to the noise characteristics of the currently used accelerometer. It is shown that this resolution can be greatly improved by the use of a low noise accelerometer.

- Abstract
The authors report 21 cases of pulmonary metastases from operated cases of rectal and colonic carcinoma. Half the patients were aged between 60 and 70 years. In all cases except one the primary tumour was known and treated surgically before the metastasis occurred. The authors emphasize the necessity of radiological supervision of the chest after operation and recall that neither chemotherapy nor radiotherapy should be undertaken without prior histological confirmation. It is possible to remove the metastases by surgery but a high percentage require pneumonectomy i.e. it is necessary to take the same precautions before operation as for primary carcinoma of the bronchus. The operative mortality was nil, and the five year survival rate 18 p. 100.

- Abstract
Oeso-tracheal-bronchial fistulas are rare in adults and their precise etiology is often difficult to determine, apart from the traumatic causes. Their diagnosis, based on symptoms, is confirmed by tracheal, bronchial and esophageal endoscopy, esophageal opacification and esophageal cineradiography. Treatment should permit one to avoid progressive pulmonary deterioration. Fatal accidents of bronchial inudation are rare but have been observed, especially in tracheotomised subjects. Surgical cure is possible and should be carried out under conditions of security which are defined here. In cases of irreversible pulmonary damage, one should carry out during the same stage, resection of part of the lung. Whereas the prognosis of fistulas after tracheotomy or intubation is always reserved, that of primary fistulas is good, providing the surgical cure is carried out early.
