Patients with COVID-19 may experience multiple conditions (e.g., fever, hyperventilation, anorexia, gastroenteritis, acid-base disorder) that may cause electrolyte imbalances. Hypokalemia is a concerning electrolyte disorder that may increase the susceptibility to various kinds of arrhythmia. This study aimed to estimate prevalence, risk factors and outcome of hypokalemia in a cohort of non-critically ill patients. A retrospective analysis was conducted on 290 hospitalized patients with confirmed COVID-19 infection at the tertiary teaching hospital of Modena, Italy.Hypokalemia (Hypokalemia is a frequent disorder in COVID-19 patients and urinary potassium loss may be the main cause of hypokalemia. The disorder was mild in the majority of the patients and was unrelated to poor outcomes. Nevertheless, hypokalemic patients required potassium supplements to dampen the risk of arrhythmias.

A previous Delphi survey from the Rational Use of Analgesics (RUA) project involving Italian palliative care specialists revealed some discrepancies between current guidelines and clinical practice with a lack of consensus on items regarding the use of strong opioids in treating cancer pain. Those results represented the basis for a new Delphi study addressing a better approach to pain treatment in patients with cancer. The study consisted of a two-round multidisciplinary Delphi study. Specialists rated their agreement with a set of 17 statements using a 5-point Likert scale (0 = totally disagree and 4 = totally agree). Consensus on a statement was achieved if the median consensus score (MCS) (expressed as value at which at least 50% of participants agreed) was at least 4 and the interquartile range (IQR) was 3–4. This survey included input from 186 palliative care specialists representing all Italian territory. Consensus was reached on seven statements. More than 70% of participants agreed with the use of low dose of strong opioids in moderate pain treatment and valued transdermal route as an effective option when the oral route is not available. There was strong consensus on the importance of knowing opioid pharmacokinetics for therapy personalization and on identifying immediate-release opioids as key for tailoring therapy to patients’ needs. Limited agreement was reached on items regarding breakthrough pain and the management of opioid-induced bowel dysfunction. These findings may assist clinicians in applying clinical evidence to routine care settings and call for a reappraisal of current pain treatment recommendations with the final aim of optimizing the clinical use of strong opioids in patients with cancer.

Most reports of survival of very-low-birth-weight (VLBW) babies focus on infants in tertiary care centres in large metropolitan areas.1-5 Extrapolating data from major centres to peripheral hospitals is problematic because of various factors, including poor socio-economic conditions, absence of intensive/special-care facilities and equipment, and limited number of staff with knowledge and skills in neonatal care in smaller hospitals.3 We were able to raise the survival rate (SR) from 21% to 40% following the use of low-cost measures between 2002 and 2005.

Se la scelta di collocare la finzione utopica nel tempo (immaginandosi un futuro) piuttosto che nello spazio (immaginandosi un altrove) coincide per alcuni autori con la contingenza temporale (e biografica) di un’importante novità storica, culturale e sociale, che investe un’intera comunità – i racconti che ne scaturiranno saranno segnati più dalla speranza o dalla paura, dalle aspettative o dal disincanto verso il progresso, dal desiderio di prolungare l’ebbrezza di quella novità storica o dalla nostalgia di ciò che essa ha soppresso? Queste e altre domande innervano una ricerca che intende analizzare gli sguardi su un remoto futuro (2000, 2073, 2222, 3004, etc.) offertici in un gruppo di opere proto-fantascientifiche pubblicate nei decenni immediatamente successivi l’unità d’Italia. Nipotini di Mercier e Verne, scapigliati delusi dall’avventura risorgimentale, protagonisti di una nascente letteratura di consumo, i vari Ghislanzoni, Mantegazza, Della Sala Spada, Barucchi, e i più noti Dossi, Imbriani, Nievo e Salgari, si impegnarono con minor o maggior convinzione (nonché successo) a descrivere l’esperienza di un meraviglioso viaggio di avvicinamento a un paese immaginato, e quindi immaginario, com’era per loro l’Italia in via di definizione, proiettando in un’ucronia e/o in un’utopia i sogni di sviluppo scientifico-tecnologico e di emancipazione sociale, ma anche l’incubo di rinvenire in esse gli stessi problemi irrisolti del presente (identità nazionale, guerra, istruzione, lavoro, disuguaglianze, etc.). L’individuazione e l’analisi delle costanti tematiche che caratterizzano queste esperienze narrative, nonché l’attenzione riservata alle singole opzioni espressive di scrittura del futuro, intendono pertanto verificare caso per caso in che misura il paesaggio utopico o antiutopico tratteggiato sia del presente un fiducioso ritratto in prospettiva o una disillusa deformazione anamorfica.

ObjectiveMultiple endocrine neoplasia type 2 (MEN 2) is a genetic disease characterized by medullary thyroid carcinoma (MTC) associated (MEN 2A and 2B) or not familial MTC (FMTC) with other endocrine neoplasia due to germline RET gene mutations. The prevalence of these rare genetic diseases and their corresponding RET mutations are unknown due to the small size of the study population.MethodsWe collected data on germline RET mutations of 250 families with hereditary MTC followed in 20 different Italian centres.Results and conclusionsThe most frequent RET amino acid substitution was Val804Met (19.6%) followed by Cys634Arg (13.6%). A total of 40 different germline RET mutations were present. Six families (2.4%) were negative for germline RET mutations. The comparison of the prevalence of RET germline mutations in the present study with those published by other European studies showed a higher prevalence of Val804Met and Ser891Ala mutations and a lower prevalence of Leu790Phe and Tyr791Phe (PPFurthermore, the phenotype data collection showed an unexpected higher prevalence of FMTC (57.6%) with respect to other MEN 2 syndromes (34% MEN 2A and 6.8% of MEN 2B). In conclusion, we observed a statistically significant different pattern of RET mutations in Italian MEN 2 families with respect to other European studies and a higher prevalence of FMTC phenotype. The different ethnic origins of the patients and the particular attention given to analysing apparently sporadic MTC for RET germline mutations may explain these findings.

Il saggio intende affrontare il concetto di ferita come traccia semantica e mnestica, indagandone la presenza e il senso nel quadro della letteratura religiosa (da Jacopone alla retorica sacra barocca), laddove la pagina mentale che si offre alla scrittura è il corpo piagato di Cristo, lo stilo la lancia di ogni peccatore, e i caratteri le ferite che indelebilmente fissano il ricordo del Sacrificio. L’immagine della ferita costituisce soprattutto una valida rappresentazione metaforica dell’idea di traccia mnemonica; o meglio, nei vari stadi che temporalmente la caratterizzano si offre come espressione dell’intera dinamica memoriale. Qualsiasi segno che portiamo sul corpo può esser letto come la registrazione – su di un supporto, al pari della memoria, deperibile – di tracce del nostro vissuto. La violenza (linguistica, iconica, fisica) rappresenta d’altronde una funzione primaria nelle pratiche di memorizzazione e reminiscenza. Ogni forma di artificiale incisione del corpo – subìta o inflitta – può divenire mediatrice del ricordo; e, soprattutto, comunicare la necessaria dimensione passionale del ricordo in quanto emozione che ha impresso violentemente l’animo. Il ferire (e i suoi strumenti) traduce appieno sub specie metaphorae la prassi delle annotazioni memoriali, di una dimensione mentale dell’atto di scrittura, poiché porta con sé l’idea della permanenza (di fronte al flusso temporale), della resistenza (alle perturbanti passioni psico-fisiche) e della profondità (a cui conducono violentemente il dato memoriale). All’interno del discorso sacro è ad esempio l’immagine della stigmate che, presentandosi nella forma di un concetto sintetico, per valore e finalità stratifica in sé più variazioni figurali dell’idea di segno (di memoria).
