Purpose: The purpose of this study was to evaluate the diagnostic accuracy of full-body linear X-ray scanning (LS) in multiple trauma patients in comparison to 128-multislice computed tomography (MSCT)., Materials and Methods: 106 multiple trauma patients (female: 33; male: 73) were retrospectively included in this study. All patients underwent LS of the whole body, including extremities, and MSCT covering the neck, thorax, abdomen, and pelvis. The diagnostic accuracy of LS for the detection of fractures of the truncal skeleton and pneumothoraces was evaluated in comparison to MSCT by two observers in consensus. Extremity fractures detected by LS were documented., Results: The overall sensitivity of LS was 49.2 %, the specificity was 93.3 %, the positive predictive value was 91 %, and the negative predictive value was 57.5 %. The overall sensitivity for vertebral fractures was 16.7 %, and the specificity was 100 %. The sensitivity was 48.7 % and the specificity 98.2 % for all other fractures. Pneumothoraces were detected in 12 patients by CT, but not by LS. 40 extremity fractures were detected by LS, of which 4 fractures were dislocated, and 2 were fully covered by MSCT., Conclusion: The diagnostic accuracy of LS is limited in the evaluation of acute trauma of the truncal skeleton. LS allows fast whole-body X-ray imaging, and may be valuable for detecting extremity fractures in trauma patients in addition to MSCT., Key Points:  The overall sensitivity of LS for truncal skeleton injuries in multiple-trauma patients was < 50 %. The diagnostic reference standard MSCT is the preferred and reliable imaging modality. Pneumatosis cystoides intestinalis (PCI) is a rare illness in adults with gas filled blebs found in the submucosa or subserosa of the bowel wall. The main localization is the terminal ileum although all parts of the intestine can be affected. Clinical symptoms can vary from aqueous-slimy, bloody diarrhea to constipation and/or vague abdominal pain. Patients can also be completely asymptomatic. In symptomatic patients the therapy of PI is based on the assumed pathogenesis, so that a combined treatment of metronidazole 1500 mg daily during a period of 6-8 weeks additionally and oxygen application (PaO2 of 200-350 mmHg) for 7 days is suggested. In addition, elemental diets are recommended. Complications are indicated in the literature with 3%. In particular mechanical ileus, invagination and perforation as well as substantial intestinal bleeding up to the volvolus lead to further diagnostic and therapeutic steps. A surgical intervention is reserved for rare cases.

The acute compartment syndrome describes a posttraumatic or inflammatory edema, which leads to a painful constraint of muscular movement and paresthesia. An increase in pressure in the anatomical compartment is postulated. The main symptoms include local swelling, sensory loss, local muscle weakness as well as late livid discoloration. Therapy of choice is an early fasciotomy with decompression to avoid serious complications like muscle necrosis. Here we report a 22 year old patient who postoperatively suffered from a bilateral paresis of the foot jack. Further examinations by electromyography and magnetic resonance imaging (MRI) led to the diagnosis of an acute bilateral compartment syndrome.

The giant cell arteritis and its symptoms are usually non-specific and accompanied with symptoms of polymyalgia rheumatica. As complications of the giant cell arteritis ischemia, infarction or rupture of the damaged vessel can occur. We report on a 56-year-old female patient, who suffered for one year about weight loss, tiredness and intolerance as well as symptoms of polymyalgia rheumatica. Gastroscopy and colonoscopy showed normal findings. In the context of the malignancy search we made a computer tomography and magnet resonance tomography. The data showed an enlargement and an enhancement of the aorta, which led us to the suspicion of a giant cell arteritis. We started immediately with a medical treatment. The biopsy of the arteries temporales supported histological the diagnosis.

The objectives of this study were to determine sex-specific differences of the subacromial space width during active and passive arm abduction and to analyze the correlation of this space with general and regional anthropometric variables. Fourteen healthy subjects (7 men, 7 women) were examined with an open magnetic resonance system at 30 degrees and 90 degrees of abduction (with and without muscle activity). After 3-dimensional reconstruction, the minimal acromiohumeral distance, the glenoid size, and the humeral head radius were determined. At 30 degrees of abduction, a significant difference of the acromiohumeral distance was observed between men (8.18 +/- 1.0 mm) and women (6.98 +/- 0.75 mm) (P < .05), but not at 90 degrees (6.7 +/- 2.0 mm versus 5.9 +/- 1.0 mm) or under muscle activity (4.9 +/- 2.4 mm versus 3.5 +/- 2.1 mm). Significant correlations between the acromiohumeral distance and anthropometric variables were found at 30 degrees of abduction (r = 0.48 to 0.72), but not at 90 degrees, with or without muscle activity (r = 0.21 to 0.55). The results demonstrate that at physical rest, the subacromial space width is dependent on sex, but the interindividual variability increases substantially during abduction and under muscle activity.

Purpose: The purpose of this work was to evaluate patients with carpal tunnel syndrome (CTS) using a low-field extremity MR system (E-MRI: 0.2 T)., Method: Twenty-two patients with typical findings of CTS and 30 control persons were imaged on an E-MRI. Axial T2-weighted turbo SE (TSE), T1-weighted SE sequences, and 2D GRE magnetization transfer (MTC) sequences were compared. SE and MTC sequences were obtained before and after contrast agent administration (0.1 mmol/kg body wt of Gd-DTPA). Two readers evaluated typical MR findings of CTS independently., Results: Patients with CTS demonstrated palmar bowing of the flexor retinaculum significantly more often. The normal or edematous median nerve was best identified on TSE and MTC scans (kappa = 0.59 and 0.8). The MTC sequences showed perineural enhancement significantly better than respective T1-weighted SE sequences but were rated second in comparison with T2-weighted TSE scans., Conclusion: At low-field strength, median nerve edema is best depicted on T2-weighted TSE sequences, whereas MTC sequences are most sensitive to perineural contrast enhancement.

Purpose: Evaluation of time efficiency in softcopy reading versus hardcopy reading of digital chest x-rays., Material and Methods: 130 normal and pathologic chest x-rays in two plains were analyzed by 4 experienced radiologist at both a digital workstation and the light box. Reading time and switch time between two patient folders were measured as well as the frequency of post-processing at the monitor., Results: Reading time at the workstation slightly exceeded reading time at the light box; differences were not statistically significant. Post-processing (frequency between 2% and 83%) did not significantly prolong reading time. Mean switch time between two patient folders was 4.3 sec at the workstation and 13.7 sec at the light box., Conclusion: As compared to hardcopy reading, softcopy reading of digital chest x-rays does not significantly increase reading time. Switch time between patient folders can be reduced at the workstation by a factor of two to four.

The aim of our study was to develop a 3-D MR-based technique for the analysis of meniscal and femoral translations during flexion of the knee, and under the influence of antagonistic muscle forces in healthy subjects. In an open MR system, 5 knees were examined at 30 degrees and 90 degrees flexion using a T1-weighted 3-D gradient echo sequence. A force of 30 Newtons, first in the extending and then in the flexing direction, was applied to the distal lower leg. After three-dimensional reconstruction, the minimal distances between the centre of the tibial plateau and the posterior edge of the menisci and femoral condyles were determined. At 30 degrees flexion, the minimum distance for the meniscus was larger medially than laterally (23.2 +/- 1.8 mm vs. 16.2 +/- 3.3 mm), and this also applied to the condyles (25.1 +/- 1.5 vs. 19.0 +/- 3.0 mm). During flexion to 90 degrees, a posterior translation of 0.5 +/- 0.2 mm was observed for the lateral, and of 3.4 +/- 1.2 mm for the medial, meniscus. The condyles demonstrated a different posterior translation (lateral 2.2 +/- 0.56 mm; medial 1.8 +/- 1.9 mm). No obvious differences were found between extension and flexion muscle activity for the different positions of the knee. In the present study, a new 3-D technique is presented for the analysis of the femoral and meniscal translation at various positions of the knee, and under muscle activity. The results suggest different translation for the menisci and condyles.

Objective: The aim of this prospective study was to summarize all of the qualitative and quantitative imaging criteria for the differentiation of solitary pulmonary lesions (SPLs) as malignant (MSPLs) or benign (BSPLs) described in the literature and to critically analyze the different characteristics in order to evaluate their clinical importance and usefulness as criteria for a discrimination during the primary diagnostic assessment of SPLs using chest radiography, spiral computed tomography (SCT) and high-resolution computed tomography (HRCT)., Materials and Methods: SPLs were examined, evaluated and then completely removed by surgery in 104 consecutive patients (MSPLs n=81, BSPLs n=23). No SPL was excluded by size. Chest radiography was performed with frontal and lateral views, SCT was carried out with a slice thickness of 8 mm and HRCT with a slice thickness of 1 mm and a 12-cm field of view., Results: All the characteristics which enabled a reliable differentiation of MSPLs from BSPLs were characteristics which were observed significantly more frequently in MSPLs than BSPLs. Useful characteristics for the differentiation of MSPLs from BSPLs (1) using chest radiography were the indistinct edge (P<0.0001) and a ground-glass opacity of the lung parenchyma adjacent to the SPL (P<0. 05); (2) using SCT the presence of spicules (P<0.0005), the vessel sign (P<0.0005), necrotic areas (P<0.001), spicules extending to the visceral pleura (P<0.005), circumscribed pleural thickening (P<0. 005), inhomogeneity (P<0.01), a ground-glass opacity of the lung parenchyma adjacent to the SPL (P<0.01), the lesion density (P<0.05), pleural retraction (P<0.05) and the bronchus sign (P<0.05); and (3) using HRCT the presence of spicules (P<0.00005), spicules extending to the visceral pleura (P<0.0005), the vessel sign (P<0.0005), pleural retraction (P<0.001), circumscribed pleural thickening (P<0. 001), the bronchus sign (P<0.005), a ground-glass opacity of the lung parenchyma adjacent to the SPL (P<0.01), the lesion density (P<0.05) and the length of spicules (P<0.05). Using any one of the characteristics with a significance level of P<0.01, the identification of MSPLs (1) using chest radiography showed a sensitivity of 64.2% and a specificity of 82.6% (accuracy of 68.3%); (2) using SCT a sensitivity of 88.9% and a specificity of 60.9% (accuracy of 82.7%); and (3) using HRCT a sensitivity of 91.4% and a specificity of 56.5% (accuracy of 83.7%)., Conclusions: Using chest radiography, SCT and HRCT, a precise morphological assessment of the periphery of the pulmonary lesion and the adjacent visceral pleura is necessary to distinguish MSPLs from BSPLs. In this respect SCT and HRCT are useful in differentiation of MSPLs from BSPLs. However, metastases strongly resembled benign lesions in terms of size and edge type and chronic inflammatory pseudotumors as a group mimic MSPLs.

Purpose: To determine if, in a patient with an endometrial cancer, in addition to the knowledge of tumor grade, preoperative magnetic resonance (MR) imaging findings contribute to treatment stratification and specialist referral., Materials and Methods: By using a MEDLINE literature search and institutional pathology reports, pretest probabilities for myometrial invasion were correlated with tumor grade. Likelihood ratios (LRs) were obtained through summary receiver operating characteristics., Results: The mean pretest probabilities of deep myometrial invasion were derived from seven articles (1,875 patients) and from 125 institutional pathology reports. LRs for the prediction of myometrial invasion with contrast-enhanced MR imaging were derived from nine studies (742 patients); positive and negative LRs were 10.11 and 0.1, respectively. The mean weighted pretest probabilities of deep myometrial invasion in patients with tumor grades 1, 2, or 3 were 13%, 35%, or 54%, respectively. Posttest probabilities of deep myometrial invasion for grades 1, 2, or 3 increased to 60%, 84%, or 92%, respectively, for positive and decreased to 1%, 5%, or 10%, respectively, for negative MR imaging findings., Conclusion: Use of contrast-enhanced MR imaging significantly affects the posttest probability of deep myometrial invasion in patients with all grades of endometrial cancer and could be used to select patients for specialist referral.

Aim: To assess prospectively the sensitivity of a high-end ultrasound system in comparison to a spiral computed tomographic scanner in screening for liver metastases., Method: 88 consecutive patients with malignant diseases were examined using gray-scale sonography and contrast enhanced two-phase spiral computed tomography during arterial dominant and portal venous phase. For both methods all focal lesions of the liver were reported., Results: 99 focal lesions of the liver were detected by gray-scale sonography and/or two-phase spiral computed tomography. The sensitivity for gray-scale sonography was 93.9% (93 of 99 lesions) and for two-phase spiral computed tomography (71.7% (71 of 99 lesions), respectively., Conclusion: Screening for liver metastases should be performed by using a diagnostic method with a high sensitivity. Gray-scale sonography with a high-end ultrasound system fulfill' these requirements.

Sports-related injuries of the hand and wrist include lesions of the bones, joints, ligaments, tendons and muscles. In many cases, radiologic imaging procedures are indispensible for the depiction, localization and accurate diagnosis of these lesions. This article reviews the role and potential of current imaging modalities such as conventional radiography, fluoroscopic examinations, arthrography, computed tomography (CT), ultrasound, and magnetic resonance (MR) imaging for the diagnostic work-up of typical osseous and soft tissue injuries in sports medicine.
