Abstract

Existing studies of burden associated with caring for cognitively impaired older adults have centered on dementia caregivers. Characterizing burden experienced by caregivers to older adults with mild cognitive impairment (MCI) is critical as this may provide early indication of need for caregiver intervention and support. This study examined the psychometric properties of the Caregiver Burden Inventory (CBI), a multi-dimensional caregiver inventory, in a sample of caregivers to patients diagnosed with MCI. Participant recruitment occurred through collaborative partnership between the University of Michigan Alzheimer’s Disease Center and Wayne State University Institute of Gerontology. Participants included 109 caregivers (51.3% women, 62.4% spousal caregiver, Mage = 68.8 ± 11.6 years) to patients diagnosed with MCI as part of a longitudinal cohort study. Cronbach’s alpha (.778 - .858) was acceptable across CBI subscales. Principal Components Analysis with promax rotation identified a five factor structure (Emotional/Social Wellbeing, Time-Dependence, Physical Health, Expectations, Relationship Strain) which explained 72.0% of the variance and partially deviated from original CBI dimensions. Results suggest that the CBI may be a valid measure of the MCI caregiver’s experience of burden. The newly emerged constructs— “expectations of the future” and “relationship strain”—are relevant to caregivers intervening in the early phase of cognitive impairment. Future directions include additional CBI validation with confirmatory factor analysis in a separate MCI caregiver sample.

Abstract

Despite the increased number of persons living with Mild Cognitive Impairment (MCI), there is a lack of information on diverse caregiving experiences within this population. This study sought to compare demographic and service utilization. Caregivers were study partners of a diverse groups of older adults participating in a longitudinal cohort study of aging and dementia recruited through a University of Michigan (UM) Memory and Aging Project partnership with Wayne State University (WSU) Institute of Gerontology and WSU/UM Michigan Center for Urban African American Aging Research. Participants included 40 AA (M(age)= 68.18, SD=10.29) and 108 Caucasian (M(age)= 67.57, SD=12.83) caregivers and individuals with MCI, AA (M(age)= 71.66, SD=9.28) and Caucasian (M(age)= 73.21, SD=8.11). Results indicate AA caregivers (M = 14.85, SD= 10.83) have less years of education than Caucasians (M = 15.81, SD= 12.83), ANOVA F(11,20)=4.692, p=.032. 10. Program Components and Outcomes of Individuals With Dementia: Results From the Replication of an Evidence-Based Program.

Abstract

This study examines whether the delivery of three components, (a) exercise training, (b) behavior management skill building, and (c) dementia-related education, in an evidence-based program are differentially associated with changes in outcomes for individuals with dementia (IWDs) after 3 months. Data come from 508 IWDs participating in the community replication of the evidence-based "Reducing Disability in Alzheimer's Disease" program. Regression results indicate that after 3 months, more exercise sessions are associated with improvements in physical functioning, mobility, minutes exercising, and symptoms of depression; more dementia education sessions are related to fewer restricted activity days; and more behavior management sessions are related to more symptoms of depression. As resource-strapped agencies look to best serve participants, further analysis and consideration is needed to determine the ideal balance of program efficacy, feasibility, and resources, as well as program benefits for IWDs and caregivers.

Abstract

With a growing number of evidence-based programs, it is necessary to understand the translation activities, experiences, and challenges of program replication in a community setting. This article reviews the implementation tasks necessary for agencies to implement the Reducing Disability in Alzheimer's Disease (RDAD) intervention. It presents the importance of using original evidence-based program protocols and enhancing them to best fit service settings by reviewing the translation and implementation activities of (a) selecting and training program and supervisory staff; (b) recruiting, screening, and consenting participants to enroll in the program; and (c) developing a manual to guide community-based program replication. Furthermore, the process revealed that the replication of an evidence-based program can take place within the realities of a community setting with input from program oversight, implementation, and evaluation staff and the original researcher., (© The Author(s) 2013.)

Abstract

There have been few replications of efficacious evidence-based programs for dementia caregivers offered in community settings. This study highlights the replication of the evidence-based Reducing Disability in Alzheimer's Disease program and explores the changes in outcomes for participating caregivers and whether those changes are related to level of program utilization. With data from 219 caregivers, regression results indicate that more exercise sessions are associated with a decrease in caregiver strain and more behavior management sessions are associated with a decrease in unmet needs after 3 months. Findings demonstrate how a multicomponent program can have positive benefits for family caregivers.
