2. Crusted scabies in an elderly woman

Abstract

Crusted scabies is a highly contagious variant of classic scabies. Affected individuals are often elderly or immunocompromised and disease is associated with significant morbidity and mortality. Herein, we report an elderly woman residing in an assisted living facility who presented with diffuse sand-on-skin scale on her trunk, proximal extremities, scalp, hands, and feet. She was diagnosed with crusted scabies, isolated from other patients, and subsequently treated with permethrin 5% lotion and ivermectin. She died two weeks after initial presentation owing to sepsis.

Abstract

A study published in the International Journal of Dermatology examined the diagnostic frameworks for pyoderma gangrenosum (PG), a condition that is often misdiagnosed due to the lack of a confirmatory test. The study surveyed dermatologists who had experience with PG and evaluated their agreement on the interpretation of different diagnostic criteria. The results showed that while the Su and Delphi criteria had higher diagnostic agreement, they had lower sensitivity compared to the PARACELSUS score. The study also found that the subjective aspects of the frameworks, such as physical examination findings and patient history, had the weakest agreement. The authors suggest that future revisions of the frameworks should focus on reducing reliance on subjective assessments and identifying more objective diagnostic measures.
