Abstract

There is some evidence that early colonization of the intestine affects the composition of the intestinal microbiota after weaning. In the present study, the effect of prebiotics administered from the first day of life on fecal counts of bifidobacteria and lactobacilli were studied during and after the administration of the prebiotics. In this double-blind, randomized, placebo-controlled, explorative study, 20 newborns of hepatitis C virus-infected mothers who decided not to breast feed due to their concerns regarding their plasma viral load were randomly assigned to either a formula with 8 g/L of a specific prebiotic mixture (short-chain galacto-oligosaccharides and long-chain fructo-oligosaccharides, ratio 9:1) or a formula containing the same amount of maltodextrin (placebo). Clinical examination including anthropometric measurements, microbiological analysis of fecal samples, and blood leukocyte population analysis were performed at birth and 3, 6, and 12 mo age. At the age of 12 mo, hepatitis B vaccine-specific IgG serum titers (Hepatitis B virus surface antibodies) were also measured. Prebiotic supplementation resulted in more fecal bifidobacteria (P < 0.0001) and lactobacilli (P = 0.0044) compared with the placebo group. These differences between the groups were maintained during the second half of the first year without any prebiotic supplementation. There was no influence of the different diets on anthropometric data or the measured immunological variables. The data from this small explorative study indicate that early colonization of the intestine might have long-lasting effects on the composition of the intestinal microbiota.

Abstract

This is a follow up study of a multicenter randomised placebo-controlled trial in seven centres in five West European countries. The RCT assessed the effect of infant formula supplemented with a mixture of prebiotics (with neutral short-chain and long-chain oligosaccharides and pectin-derived acidic oligosaccharides) during infancy in term-born children (n=1130). In the follow-up study 672 children (60% of the study population) participated: 232 (56%) from the prebiotics group (PG), 243 (58%) from the control group (CG), and 197 (66%) from the non-randomised breast-fed group (BG). The primary outcome was the occurrence of febrile episodes at three to five years of age prospectively documented by the parents: in the PG 1.17 (interquartile range 0.50-2.08) episodes per year versus 1.20 (0.52-2.57) in the CG; and 1.48 (0.65-2.60) in the BG. This specific prebiotics mixture given during infancy in healthy non-atopic subjects does not decrease febrile episodes and therefore seems not to prevent infection between their third and fifth birthday.

The INFAT-study examines the effect of a reduction in the ratio of n-6/n-3 long-chain polyunsaturated fatty acids in the maternal diet during pregnancy and lactation on adipose tissue development of the infant. The aim of this thesis was to study the effect of the dietary intervention on offspring body composition up to 1 year of age. We found no evidence that supplementation with n-3 fatty acids and instructions to reduce arachidonic acid intake during pregnancy and lactation relevantly affects fat mass or fat distribution in the offspring during the first year of life., Die INFAT-Studie untersucht den Einfluss einer Senkung des n-6/n-3 Fettsäure-Quotienten in der mütterlichen Ernährung während der Schwangerschaft und Stillzeit auf die Fettgewebsentwicklung beim Neugeborenen. Das Ziel dieser Arbeit war den Einfluss der Ernährungsintervention auf die auf die Körperzusammensetzung der Kinder im 1. Lebensjahr zu untersuchen. Wir fanden keine Evidenz dafür, dass eine Reduktion des Verhältnisses von n-6 zu n-3 Fettsäuren in der Schwangerschaft und Stillzeit das Fettgewebswachstum bei den Nachkommen im 1. Lebensjahr günstig beeinflusst.
