The presented literature review examines in detail the methods for diagnosing the auditory tube, the history of development of studies of the Eustachian tube and the treatment of its dysfunction in children. The issue of dysfunction of the auditory tube as one of the causes of the formation of chronic hearing loss in childhood is increasingly being identified because of the rapid development of technologies and methods for examining patients. Aim is to study the multiple etiologic factors of the disease and the need for an integrated approach to the study of this condition.

Objective: Is to study the effectiveness of the of balloon dilatation of the auditory tube in children with persistent dysfunction of the auditory tube. Examination and treatment of 28 children aged 5 to 16 years with persistent dysfunction of the auditory tube was performed. To diagnose and objectify the results of the treatment, taking of complaints and medical history of the patient, examination of ENT organs, endoscopic examination of the nose, nasopharynx and otoendoscopy, functional multispiral computed tomography of the auditory tubes, audiological examination (tympanometry, tonal threshold audiometry) were performed. Treatment included balloon dilatation of the auditory tubes. In 25 (89.3%) children, upon repeated examination after 2, 6, 12 months, the clinical, radiological, and audiological parameters were normal.

The macroscopic and histological methods were employed to examine the autopsy specimens of salivary lingual glands obtained from 299 patients of both sexes and various age ranging from newborn to longevity. The age-associated alterations of minor lingual and pharyngeal glands were revealed, and the topographical relations between the glands and lymphoid cells were described. The characteristic sparsity of the glands in infancy is caused by nutritional uniformity at this period, when diminished production of secretory IgA results in frequent inflammatory processes in oral and pharyngeal cavities. With age, the glandular orifices widen, and their number increases thereby augmenting local immunity in the oral cavity and in oral aspect of the pharynx. Starting from elderly and senile age, the involutive alterations were observed, which were accompanied by diminished production of secretory immunoglobulin A and related degradation of local and humoral immunity.

We present the review of the literature on the anatomical and physiological features of the structure of the auditory tube in children, epidemiology, etiology, and the pathogenesis of the formation of auditory tube dysfunction is presented. The first part of the article presents in more detail the structural and functional differences in the Eustachian tube in children, the classification and mechanism of the occurrence of the auditory tube dysfunction in pediatric practice.

The objective of the present study was to evaluate the effectiveness of the surgical treatment of otosclerosis in the subjects of advanced age. The medical histories of 19 patients aged above 65 years (20 ears) presenting with otosclerosis were available for the retrospective analysis. All of them had undergone surgical interventions (a total of 22 ones or 4.49% of the 489 operations performed between 2001 and 2012). The audiometric assessment was made at four wavelengths (0.5, 1, 2, and 4 kHz). Primary surgery (n=18) was preceded by the determination of the high air and bone conductivity thresholds (76.18 dB and 42.15 dB respectively), with the mean bone to air interval (BAI) being 33.99 dB. The surgical treatment resulted in the improvement of hearing; in 83.33% of the cases, BAI was reduced to 10 dB or below this value. It is concluded that the surgical intervention on the stirrup bone for the treatment of otosclerosis in the patients above 65 years of age is a safe and efficacious method despite marked hearing impairment in this group of patients.

This paper reports an observation illustrating the possibility of the successful surgical treatment of otosclerosis in a HIV-infected patient presenting with acquired immunodeficiency syndrome (AIDS) and positive response in the test for hepatitis C. The authors used the results of multispiral computed tomography (MSCT) of the temporal bones as a basis for the prediction of the outcome of the surgical intervention taking into consideration specific primary manifestations of HIV infection as well as immunological and virlogical responses to anti-retroviral therapy.

The first results of use of functional multi-detector computed tomography in diagnosis of orbital fractures with eye movement impairment are obtained. In addition to orbital bone fractures diagnosis this method allows to perform more detailed evaluation of orbital soft tissues particularly extraocular muscles. Proposed method allows differentiating paralytic and restrictive ophthalmoplegia, evaluate the function of extraocular muscles in severe orbital deformities.

Computed tomography images are analyzed in patients with and without lacrimal drainage system abnormalities. Potential of functional multidetector computed tomography in dacryostenosis is showed.

The present paper reports a case of congenital (juvenile) otosclerosis. This pathology is known to occur in 10% of the patients and cause severe hearing impairment. Precise diagnostics of the activity and extent of the otosclerotic process based on the results of functional multispiral computed tomography (MSCT) enables the practicing surgeons to identify indications for the surgical intervention, choose the adequate method and time for its performance, and make prognosis of its outcome. This inference is confirmed by the clinical observation described in this communication.

The objective of the present study was to estimate the potential of multislice computed tomography (MSCT) for the detection of specific anatomic and topographic features in the structure of the middle ear and the oval window region prior to stapedoplasty. We analysed the results of MSCT of the temporal bones in 20 patients presenting with otosclerosis (40 observations). All these patients underwent stapedoplasty, and the intraoperative findings were compared with the MSCT data. Specificity and sensitivity of MSCT for the detection of structural abnormalities in the middle ear were estimated at 91.3% and 84.6% respectively. It is concluded that the preoperative MSCT examination of the temporal bones yields the data that allow for the more accurate planning of the succeeding surgical intervention as well as for choosing its optimal strategy and extent, proper instruments, the type and length of the implant (prosthesis), and thereby for avoiding the possible complications and improving the prognosis of the treatment.

The objective of this communication is to demonstrate success of the surgical intervention on the sole hearing ear in the presence of absolute indications for such treatment, viz. the presence of cholesteatoma and labyrinth fistula (LF). The sensorineural loss of hearing is a rather common serious complication of LF surgery in the patients with cholesteatoma. The present observation concerns a 41 year-old woman with the history of an episode of suppurative otitis media experienced in the childhood. After a period of prolonged remission, she suffered systemic dizziness despite the intact tympanic membranes. The patient was examined with the use of multispiral computed tomography (MSCT) that revealed serious lesions in the middle ear (labyrinth fistula). It made possible careful planning and performing sanation surgery on the middle ear including tympano- and mastoidoplasty. This treatment produced good morphological and functional result.

The paper deals with computed tomography (CT) diagnosis of Eustachian tube dysfunction. A functional multislice spiral (MS) CT protocol has been first elaborated for the Eustachian tube. Twenty-three patients with unilateral conductive hearing loss and Eustachian tube dysfunction were examined. The normal functional anatomy of the Eustachian tube was evaluated on the side without evidence of pathology in 23 patients. Functional MSCT showed that the cartilaginous portion of the Eustachian tube opened partially and closed completely in 9 patients. Ten patients were found to have steady changes in the cartilaginous portion of the Eustachian tube. The developed method could confirm the cause of conductive hearing loss and optimally define a treatment policy.

This paper reports a rare clinical observation of congenital malformation of the internal and middle ear diagnosed with the help of functional multispiral computed tomography (fMSCT). The patient presenting with conductive hearing loss was suspected to have otosclerosis. However, the use of fMSCT made it possible to arrive at a more precise diagnosis, that is congenital malformation of the internal and middle ear that clinically manifested itself as the conductive loss of hearing. It was decided to refrain from the surgical intervention: first, because its efficacy in this case was considered to be doubtful and second, it was fraught with the high risk of development of postoperative complications.

The objective of the present study was to estimate the state of the stirrup prosthesis based on the results of functional multispiral computed tomography (fMSCT). A total of 30 patients suffering otosclerosis and treated by stapedoplasty were examined using the modified fMSCT technique developed by the authors. The data obtained were compared with the results of the audiologic study. It was shown that modified fMSCT makes it possible to objectively and visually estimate the state of the stirrup prosthesis, elucidate the causes of poor outcome of the surgical treatment and identify the indications for the second operation. The study has demonstrated that a more pronounced decrease of the air-bone gap (ABG) is associated with a rise in the amplitude of prosthesis movements whereas the degree of reduction of bone conduction does not significantly correlate with the depth to which the prosthesis reaches into the vestibulim.

The study deals with the capacities of functional multislice spiral computed tomography (FMSCT) in choosing a treatment policy and planning the tactic and scope of surgery for posttraumatic scar changes in the rectus muscle of the eye. Orbital MSCT and FMSCT were conducted in 15 patients (30 orbits). The findings showed that it was necessary to perform orbital FMSCT in posttaumatic scar changes to evaluate the contractility of the rectus muscles and their involvement in the area where a fracture occurs. Improved diagnosis achieved by FNSCT could optimally choose the tactics and scope of surgical intervention.

The objective of this work was to estimate the potential of functional multispiral computed tomography (fMSCT) for the choice and planning of the treatment strategy and the extent of surgical intervention in the patients presenting with fibroosseous diseases of the middle ear associated with the pathologically altered mobility of the auditory ossicles. Studies with the use of MSCT and fMSCT for the examination of temporal bones in 21 patients (25 observations) provided information about normal CT anatomy of the middle ear and a basis for the development of the fMSCT protocol; moreover they allowed the range of mobility of the auditory ossicles to be determined in healthy subjects and patients with middle ear disorders. It is concluded that fMSCT of temporal bones may be recommended to patients suffering otosclerosis, tympanosclerosis, and adhesive otitis media. The use of this technique improves the accuracy of diagnosis and facilitates the choice and planning of the treatment strategy and the extent of surgical intervention in the patients presenting with middle ear diseases.

The utility of the data obtained by multispiral computed tomography (MSCT) for the choice of surgical techniques is discussed with special reference to chronic suppurative otitis media. Specific features of bone tissue destruction are described in patients with cholesteatoma and without it. Results of MSCT were used to estimate the optimal extent of surgical intervention. Intraoperative findings were in excellent agreement with MSCT data.

The authors discuss the diagnostic significance of the blood formula and of indexes derived from the formula, used in clinical practice, for assessing the allergic reaction. The counts of eosinophils and lymphocytes increase in 56-86% of cases in allergic reactions. Analysis of the blood formula enabled the authors derive a new index for detection of allergic reaction: the allergization index (AI). AI fluctuations in normal subjects are from 0.68 to 1.08, whereas in the patients with various forms of allergic reactions it increases to 2.37-2.97. Allergic reactions were detected by AI in 82-91% of cases.
