7. Validation of a Bayesian Learning Model to Predict the Risk for Cannabis Use Disorder

Abstract

Background: Cannabis use disorder (CUD) is a growing public health problem. Early identification of adolescents and young adults at risk of developing CUD in the future may help stem this trend. A logistic regression model fitted using a Bayesian learning approach was developed recently to predict the risk of future CUD based on seven risk factors in adolescence and youth. A nationally representative longitudinal dataset, Add Health was used to train the model (henceforth referred as Add Health model). Methods: We validated the Add Health model on two cohorts, namely, Michigan Longitudinal Study (MLS) and Christchurch Health and Development Study (CHDS) using longitudinal data from participants until they were approximately 30 years old (to be consistent with the training data from Add Health). If a participant was diagnosed with CUD at any age during this period, they were considered a case. We calculated the area under the curve (AUC) and the ratio of expected and observed number of cases (E/O). We also explored re-calibrating the model to account for differences in population prevalence. Results: The cohort sizes used for validation were 424 (53 cases) for MLS and 637 (105 cases) for CHDS. AUCs for the two cohorts were 0.66 (MLS) and 0.73 (CHDS) and the corresponding E/O ratios (after recalibration) were 0.995 and 0.999. Abstract

BackgroundVariation in liability to cannabis use disorder has a strong genetic component (estimated twin and family heritability about 50-70%) and is associated with negative outcomes, including increased risk of psychopathology. The aim of the study was to conduct a large genome-wide association study (GWAS) to identify novel genetic variants associated with cannabis use disorder.MethodsTo conduct this GWAS meta-analysis of cannabis use disorder and identify associations with genetic loci, we used samples from the Psychiatric Genomics Consortium Substance Use Disorders working group, iPSYCH, and deCODE (20 916 case samples, 363 116 control samples in total), contrasting cannabis use disorder cases with controls. To examine the genetic overlap between cannabis use disorder and 22 traits of interest (chosen because of previously published phenotypic correlations [eg, psychiatric disorders] or hypothesised associations [eg, chronotype] with cannabis use disorder), we used linkage disequilibrium score regression to calculate genetic correlations.FindingsWe identified two genome-wide significant loci: a novel chromosome 7 locus (FOXP2, lead single-nucleotide polymorphism [SNP] rs7783012; odds ratio [OR] 1·11, 95% CI 1·07-1·15, p=1·84 × 10-9) and the previously identified chromosome 8 locus (near CHRNA2 and EPHX2, lead SNP rs4732724; OR 0·89, 95% CI 0·86-0·93, p=6·46 × 10-9). Cannabis use disorder and cannabis use were genetically correlated (rg 0·50, p=1·50 × 10-21), but they showed significantly different genetic correlations with 12 of the 22 traits we tested, suggesting at least partially different genetic underpinnings of cannabis use and cannabis use disorder. Abstract

Objective: On 15 March 2019, a white supremacist terrorist carried out sequential attacks on two mosques in Christchurch, New Zealand during Friday prayers. This resulted in the loss of 51 lives, 40 others sustained gunshot injuries, and there were approximately 250 survivors. This study aimed to evaluate the impacts on community members, assess clinical needs, facilitate access to appropriate interventions and provide insights into working with a traumatised and diverse population. Methods: This cross-sectional study used semi-structured clinical interviews and self-report measures to assess social and demographic factors, mental health disorders and well-being for adult Muslims 11–32 months post-attack. Results: A total of 189 participants completed assessments. The sample was diverse, representing 34 different ethnicities and participant proximity to the attack was complex, with personal and familial exposures. Elevated levels of psychological distress and psychopathology were found with 38% of participants reporting moderate/severe psychological distress on the Kessler-10, 39% reporting post-traumatic stress disorder on the post-traumatic stress disorder checklist-5, and 40% reporting poor well-being or possible depression on the World Health Organization-5 Well Being Index. Secondary stressors were also documented, as well as high scores for post-traumatic growth and the importance of faith. Conclusion: This study provides valuable insights into the repercussions of the Christchurch mosque attack on the affected community, describing the complexity of exposure and the substantial burden of morbidity experienced. It also highlights the high levels of social connectedness and the role of faith in promoting positive outcomes in the recovery process for this population. [ABSTRACT FROM AUTHOR]

36. Adolescent Firesetting as a Risk Marker for Adult Externalizing and Internalizing Problems: Analysis of a 40-Year Birth Cohort Study.

Abstract

Within the Christchurch Health and Development Study (N = 1,265), we examine the prevalence and continuity of firesetting across the life course and explore whether adolescent firesetting is a risk marker for adult internalizing and externalizing behaviors. Life course continuity of firesetting was low. Adolescent firesetting (3%) correlated with antisocial behaviors and adverse family circumstances. Independent of conduct disorder, adolescent firesetting showed limited associations with adulthood internalizing outcomes, but associated with most externalizing outcomes (e.g., cannabis use disorder, OR = 2.08, 95% CI = [1.14, 3.78]). Further adjustment for antisocial tendencies and family circumstances reduced associations; the largest effect to remain was for alcohol use disorder (OR = 2.08, 95% CI = [1.19, 3.63]). Therefore, for most externalizing behaviors, adolescent firesetting is a marker (i.e., indicates future risk), but not an independent risk marker (i.e., does not have independent specificity). For adolescents who set fires, clinical treatment should assess comorbid conduct disorder and family instability and consider a range of problematic outcomes. [ABSTRACT FROM AUTHOR]

48. Early Childhood Education and Later Educational Attainment and Socioeconomic Wellbeing Outcomes to Age 30

Abstract

This paper examines the associations between attendance at early childhood education (ECE) and longer-term academic attainment and socioeconomic wellbeing in a New Zealand birth cohort studied to age 30. Nearly 95% of the cohort had attended ECE by age 5. After adjustment for family background and child characteristics, results showed statistically significant trends for increasing duration of ECE attendance to be associated with: greater attainment of high school and university qualifications and higher adult socioeconomic wellbeing (income, employment, occupational status); effect size estimates were small to moderate. No evidence was found for differential effects of ECE attendance across subgroups defined by child: ethnicity, gender, parental education or socioeconomic status. The implications of these findings are discussed.

49. Improving Educational Outcomes for At-Risk Students

Abstract

Drawing on data from a longitudinal study of at-risk youth (n = 593), this article reports on the analysis of factors that enabled these youth to succeed at school. It considers the impact of three baseline factors (age, gender, ethnicity) and a number of time-dynamic factors [positive school environment, additional educational support, positive peer and parent relationships, exclusion/expulsion from school, depression and externalising individual risk, as well as the involvement of a range of services (mental health, justice, welfare)] upon educational progress. Over time, the educational status of this group of youth deteriorated. Differences were observed for indigenous, older and male youth who had poorer outcomes on average. Positive peer groups and a positive school environment predicted better outcomes, while the use of harsh disciplinary practices such as expulsion was the strongest predictor of poorer educational outcomes and had a pervasive negative impact on all three educational progress measures. Formal services did not make an appreciable difference to educational outcomes, while the provision of additional educational support only contributed to keeping youth enrolled in educational programmes but did not appreciably improve their educational outcomes. Improving educational outcomes for at-risk youth requires a pan-system response, whereby schools reduce the use of expulsion and create a positive school climate, other professionals support schools to retain challenging students at school and the positive resources generated by pro-social peer groups are harnessed.
