Abstract

Simple Summary: Anastomotic leakage (AL) has a reported incidence of up to 53% and contributes to a high morbidity and mortality after oesophagectomy. Because of its high success rates, endoluminal vacuum-assisted closure therapy (EVAC) is nowadays the standard treatment for AL. However, its effectiveness depends upon different factors. As most patients receive trimodal therapy (neoadjuvant therapy, followed by surgery and adjuvant therapy) due to their preoperative cancer staging, the question arises as to whether neoadjuvant therapy impacts the success rate of EVAC. Therefore, our aim was to identify any adverse effects of a neoadjuvant therapy on EVAC compared to no prior treatment, in order to improve patient care and treatment algorithms. We found no significant influence of neoadjuvant therapy on EVAC. Furthermore, EVAC proved to be an effective and secure treatment option, leading to no changes in the management of AL for now. Background: Anastomotic leakage (AL) is a dreaded complication after oesophagectomy. Endoluminal vacuum-assisted closure therapy (EVAC) has been increasingly used as a first-line treatment for AL. We aimed to identify any potential adverse effects of a neoadjuvant therapy (chemotherapy (CT) or radiochemotherapy (RCT)) on EVAC. Methods: We performed a retrospective cohort study at our tertiary centre between 2013 and 2024. All patients who underwent EVAC for AL after oesophagectomy were included in this study. Parameters such as success rate, length of therapy, number of sponges needed, changes in treatment, and survival were analysed. Results: A total of 29 patients were included, 19 of whom received CT/RCT and 10 of whom received no neoadjuvant treatment (NT). There was no significant difference in patient survival (30-day survival rate CT/RCT n = 1 (5.3%) vs. NT n = 1 (10%), p = 0.632), success rate (CT/RCT n = 15 (78.9%) vs. NT n = 9 (90%), p = 0.454), and length of therapy (CT/RCT vs. NT 24.11 vs. 23.8, p = 0.681), the number of sponges required (CT/RCT vs. NT 6.26 vs. 6.6, p = 0.835), and the need for changing treatment (CT/RCT n = 5 (26.3%) vs. NT n = 1 (10%), p = 0.303). Conclusions: NT did not affect the success rate or length of therapy. Thus, we found no significant influence of CT/RCT on EVAC for AL after oesophagectomy. [ABSTRACT FROM AUTHOR]

Abstract

Simple Summary: Pancreatic cancer is a particularly aggressive form of cancer characterized by poor patient survival, highlighting the need for preventive strategies and interventions to improve long-term survival. While physical activity is known to lower the risk of many different forms of cancer and reduce cancer-specific mortality, its effect on pancreatic cancer is less clear. Therefore, this systematic review and meta-analysis aimed to summarize epidemiological evidence on the relationship between physical activity and pancreatic cancer risk and mortality. A meta-analysis of seven case–control and eighteen prospective cohort studies demonstrated that higher levels of physical activity were associated with a lower risk of developing pancreatic cancer. No significant association was found between pre-diagnosis physical activity and pancreatic cancer mortality, based on data from only six prospective cohort studies. These results could help guide further research and contribute to public health recommendations on physical activity for cancer prevention. Methods: PubMed and Embase were searched until May 2024 for studies examining physical activity in relation to pancreatic cancer incidence and mortality. Summary risk estimates for highest vs. lowest physical activity levels were calculated using a random-effects model. The risk of publication bias was assessed with a funnel plot and Egger's regression test. Results: A total of seven case–control and eighteen prospective cohort studies were included that investigated the association between physical activity and pancreatic cancer incidence. Our meta-analysis showed a summary estimate of 0.75 (95% CI 0.64–0.88) for case–control studies (I2 = 23%, n = 7) and a summary estimate of 0.91 (95% CI 0.86–0.97) for prospective cohort studies (I2 = 5%, n = 18). Among the six prospective cohort studies that assessed pancreatic cancer mortality, the summary estimate was 1.03 (95% CI 0.83–1.27), I2 = 50%. Conclusions: Higher levels of physical activity were associated with reduced pancreatic cancer risk. Abstract

Introduction: Colorectal cancer (CRC) ranks as the third most prevalent malignancy globally, with a concerning rise in incidence among young adults. Despite progress in understanding genetic predispositions and lifestyle risk factors, the intricate molecular mechanisms of CRC demand exploration. MicroRNAs (miRNAs) emerge as key regulators of gene expression and their deregulation in tumor cells play pivotal roles in cancer progression. Methods: NanoString's nCounter technology was utilized tomeasure the expression of 827 cancer-related miRNAs in tumor tissue and adjacent non-involved normal colon tissue from five patients with locoregional CRC progression. These expression profiles were then compared to those from the primary colon adenocarcinoma (COAD) cohort in The Cancer Genome Atlas (TCGA). Results and discussion: Intriguingly, 156 miRNAs showed a contrasting dysregulation pattern in reccurent tumor compared to their expression in the TCGA COAD cohort. This observation implies dynamic alterations in miRNA expression patterns throughout disease progression. Our exploratory study contributes to understanding the regulatory landscape of recurrent CRC, emphasizing the role of miRNAs in disease relapse. Notable findings include the prominence of let-7 miRNA family, dysregulation of key target genes, and dynamic changes in miRNA expression patterns during progression. Univariate Cox proportional hazard models highlighted miRNAs associated with adverse outcomes and potential protective factors. Abstract

Truncated O - N -acetylgalactosamine (O -GalNAc) glycosylation is an important feature of pancreatic ductal adenocarcinoma (PDAC), and the expression of truncated O -GalNAc glycans is strongly associated with decreased survival and poor prognosis. It has been proven that aberrant O -GalNAc glycosylation influences PDAC signaling to promote oncogenic properties, but elucidation of the influence of truncated O -GalNAc glycosylation on different signaling molecules has just been started. We herein elucidated the impact of aberrant O -GalNAc glycosylation on two important PDAC signaling pathways, namely, AKT/mTOR and RAS/MAPK. In PDAC cells expressing truncated O -GalNAc glycans, we identified differentially expressed proteins associated with AKT/mTOR and RAS/MAPK pathways using quantitative proteomics. Since AKT, a key-signaling molecule in PDAC, was among the identified proteins, we analyzed AKT and found a strikingly enhanced S473 phosphorylation and identified a previously unknown O -GalNAc modification. Consecutive analysis of COSMC knockdowns in PDAC revealed strong effects on AKT upstream and downstream effector molecules. Interestingly, truncated O -GalNAc glycans could facilitate an mTORC1 inhibitor resistance using AZD8055. In addition, as AKT/mTOR pathway has extensive cross talks with RAS/MAPK pathway, we analyzed the pathways and found it to be negatively regulated. Finally, we found that the expression of epithelial-mesenchymal transition markers, key features of aggressive PDACs cells, are enhanced and truncated O -GalNAc glycans enhance pancreatic cancer cell growth in a xenograft mouse model. Our study demonstrates that truncated O -GalNAc glycans have a strong impact on AKT/mTOR and RAS/MAPK signaling pathways, are modulated by EGF or IGF-1 signaling and should be considered for targeted therapy of these pathways in PDAC.
