2. Analyse der mehrphasigen 64-Zeilen-Multidetektor-Computertomographie zur präoperativen angiographischen Evaluation potenzieller Lebendnierenspender

Abstract

Zusammenfassung: Hintergrund: Die anatomische Darstellung und Erfassung m�glicher Anomalien der Nierengef��e und Ureteren ist f�r die Planung einer Lebendnierenspende von essenzieller Bedeutung. Die vorliegende Untersuchung soll die Wertigkeit der nichtinvasiven Evaluation mit der 64-Zeilen-Multidetektor-CT untersuchen. Material und Methoden: In die Auswertung wurden 63 Lebendnierenspender (LNS) eingeschlossen, die im Zeitraum 12.2004 bis 01.2007 mit der CT pr�operativ untersucht wurden. Die Untersuchungen erfolgten mit einem Somatom-Sensation-Cardiac-64 (Siemens Medical Solutions, Deutschland). Neben einer CT-Angiographie (CTA) wurden eine ven�se Phase des Abdomens und eine Sp�tphase nach 15 min in Low-dose-Technik f�r die CT-Urographie (CTU) durchgef�hrt. Die radiologischen Befunde wurden mit dem intraoperativen Befund oder in 2 F�llen mit der konventionellen Angiographie verglichen, au�erdem wurden Sensitivit�t, Spezifit�t, negativer und positiver Vorhersagewert ermittelt. Ergebnisse: Bei den ausgewerteten 63 LNS (31 weibliche, 32 m�nnliche) erreichte die CTA eine Sensitivit�t von 100% f�r die Arterien und Polarterien und 98,3% f�r die Nierenvenenanatomie. Die Low-dose-CTU erreichte ebenfalls eine Sensitivit�t von 100%. Schlussfolgerung: Die Ergebnisse zeigen, dass eine nichtinvasive pr�operative Planung bei potenziellen Lebendnierenspendern anhand einer 64-Zeilen-Multidetektor-CT im Sinne des „On-stop-Shopping“ zuverl�ssig m�glich ist.

35. Intrahepatic type II gall bladder perforation by a gall stone in a CAPD patient

Abstract

Abstract Introduction Perforation of the gall bladder represents a rare, but life-threatening complication of cholecystitis. Clinical presentation may vary between severe peritonism in acute perforation and absence of symptoms in subacute or chronic progression of perforation. Abdominal imaging like ultrasound or CT-scan are important tools for immediate diagnose of gall bladder perforation. Case presentation We report a case of a 30-year old female patient with end-stage kidney disease treated by continuous ambulatory peritoneal dialysis (CAPD) who was admitted to the emergency room with fever and mild abdominal pain. A type II gall bladder perforation by a solitary gall stone with development of a liver abscess was detected by abdominal ultrasound. Conclusion Gall bladder perforations are rare but have to be considered in patients with abdominal pain and fever. Abdominal ultrasound is a reliable tool to establish diagnosis.

Abstract

Intrakranielle vaskuläre Malformationen (IVM) des Neugeborenen sind insgesamt selten. Hierbei können und müssen durale arteriovenöse Shunts (DAVS) und aneurysmatische Malformationen der V. Galeni (VGAM) unterschieden werden, letztere stellt die wohl häufigste IVM des Neugeborenen dar. Für beide existieren mehrere Klassifizierungen. Die Mortalität der DVAS ist im Neugeborenenalter überdurchschnittlich hoch. Erst wenn durch eine erhöhte Flussbelastung sich die dem Gefäßkurzschluss nachgeschalteten Gefäße im Verlauf der Schwangerschaft erweitern, können VGAM und DVAS im Ultraschall diagnostiziert werden. Daher werden sie in der Regel erst im letzten Trimenon oder postnatal entdeckt. Die Prognose einer IVM wird durch das Shuntvolumen und der daraus resultierenden kardialen Belastung sowie durch das Alter des Kindes beeinflusst. Zur Planung der meist interventionellen Therapie werden neben postnataler digitaler Subtraktionsangiographie (DSA) und Magnetresonanztomographie (MRT) auch zunehmend pränatale MRT-Untersuchungen eingesetzt.

Abstract

Objectives: To prospectively evaluate the effect of hyoscine butylbromide (HBB) on visualisation of anatomical details and motion-related artefacts in mp-MRI of the prostate at 3.0 Tesla.Methods: One hundred and three consecutive patients (65 ± 10 years) were included in this trial, powered to demonstrate an improvement of image quality after HBB administration, assessed on a 5-point scale by two blinded readers. All patients received high-spatial resolution axial T2-weighted TSE sequences at 3.0 T without spasmolytic agent, repeated after application of 40 mg HBB and followed by routine mp-MRI. Secondary endpoints were (1) susceptibility to side effects, (2) dependence of spasmolytic effect on patients´ weight, and (3) prostate volume.Results: In 68% of patients, HBB significantly improved the anatomic score (mean 3.4 ± 0.9 before and 4.4 ± 0.7 after HBB for both readers, p = <0.001). In 67%, HBB significantly enhanced the artefact score (mean 3.2 ± 1 before and 4.2 ± 0.8 after HBB for reader 1, p = <0.001; 3.2 ± 1 and 4.1 ± 0.8 for reader 2, p = <0.001). Subgroup analysis revealed no statistically significant difference between patients with different bodyweight or prostate volume. Inter-reader agreement was excellent (k = 0.95-0.98).Conclusions: Hyoscine butylbromide significantly improves image quality and reduces motion-related artefacts in mp-MRI of the prostate independent of bodyweight or prostate volume. No side effects were reported.Key Points: • Hyoscine butylbromide (HBB) improved image quality in over 2/3 of patients. • Severe artefacts were reduced after HBB in more than 20%. • The number of non-diagnostic MRI was reduced to <1% after HBB. • HBB effect was independent of bodyweight and prostate volume. • No side effects of HBB were reported in this study population. Abstract

Introduction: The purpose of the present study was to evaluate the feasibility of delayed gadolinium-enhanced magnetic resonance imaging of cartilage (dGEMRIC) in the detection of cartilage changes versus morphologic imaging in the long-term course of Legg-Calvé-Perthes disease (LCPD). Methods: A total of 31 hips in 26 patients (mean age, 30.0years; range, 18-54years) who were diagnosed with LCPD in childhood were included. Twenty-one radiographically normal contralateral hips served as controls. dGEMRIC indices of femoral and acetabular cartilage in the weight-bearing zone. Cartilage morphology was classified on radial PD-weighted images according to the modified Outerbridge classification. Results: Mean dGEMRIC values of cartilage were significantly lower in hips after LCPD than in the radiographically normal contralateral hips (513±100 ms vs. 579±103 ms; P=0.026). In 24 out of 31 LCPD hips and in 4 out of 21 radiographically normal contralateral hips, morphological cartilage changes were noted. Analysis of variance analysis revealed a significant influence of Outerbridge grading on decreased T1-values (P=0.031). Conclusion: Our results suggest that dGEMRIC at 1.5 T is suitable to assess cartilage quality changes in the long-term follow-up after LCPD. The evaluation of biochemical cartilage quality with dGEMRIC may provide additional information about early cartilage changes occurring without visible alterations of cartilage morphology.
