Risk factors for discharge from an AGU to post-acute care (PAC) have not previously been investigated in detail.Methods: The objective is to identify risk factors for PAC transfer. The DAMAGE (prospective multicenter cohort) consecutively included more than 3500 subjects aged 75 or older and admitted to an AGU. The patients underwent a comprehensive geriatric assessment (CGA) during their stay in the AGU. Only community-dwelling patients admitted to the AGU from the emergency department were included in the analysis. We recorded the characteristics of the care pathway and identified risk factors for discharge to home or to a PAC facility.Results: 1928 patients were included. Loss of functional independence (a decrease in the Katz activities of daily living (ADL) score between 1 month prior to admission and AGU admission), living alone, social isolation, a high Katz ADL score at home, a low Katz ADL on admission, and delirium on admission were risk factors for transfer to PAC. Obesity, an elevated serum albumin level, and community-acquired infection were associated with discharge to home. Neither sex nor age was a risk factor for home discharge or transfer to PAC.Conclusion: The present results might help clinicians and discharge planning teams to identify patients at risk of transfer to PAC more reliably and promptly in AGUs.Keywords: care transitions, health services, rehabilitation, risk factors, acute geriatric unit

Patients aged 75 and over hospitalized initially in an acute geriatric unit (AGU) were included and followed up for 12 months. The risk of hospital readmission was analyzed using a Cox model, and its extension for recurrent events and the risk of death were analyzed using a Cox model for time-dependent variables.Results: A total of 3081 patients were included (mean (SD) age: 86.4 (5.5)). In the multivariate analysis, the relative risk (95% confidence interval [CI]) of hospital readmission rose progressively to 2.66 (1.44; 5.14), and the risk of death rose to 2.01 (1.23; 3.32) after five hospital admissions, relative to a patient with no hospital readmissions. The number of hospital readmissions during the follow-up period was the primary risk factor and the best predictor of the risk of hospital readmission and the risk of death.Conclusion: Hospital readmission is the primary risk factor for further hospital readmissions and for death in older subjects discharged from an AGU.Keywords: hospital readmission, death, older patients, acute geriatric unit, risk factors, recurrent events

Abstract

We have extracted QCD matrix elements from our data on double polarized inelastic scattering of electrons on nuclei. We find the higher twist matrix element \tilde{d_2}, which arises strictly from quark- gluon interactions, to be unambiguously non zero. The data also reveal an isospin dependence of higher twist effects if we assume that the Burkhardt-Cottingham Sum rule is valid. The fundamental Bjorken sum rule obtained from the a0 matrix element is satisfied at our low momentum transfer., Comment: formerly "Nachtmann Moments of the Proton and Deuteron Spin Structure Functions"

Abstract

The photoproduction of pi0pi0 and pi0pi+/- pairs off 40Ca has been investigated with the TAPS detector using the Glasgow photon tagging spectrometer at the Mainz MAMI accelerator. Data have been taken for incident photon energies in the energy range from 200 - 820 MeV. Total cross sections have been extracted from threshold up to the maximum photon energy and invariant mass distributions of the pion pairs have been obtained for incident photon energies between 400 - 500 MeV and 500 - 550 MeV. The double pi0 invariant mass distributions show some relative enhancement with respect to the mixed charge channel at small invariant masses. The effects are smaller than previously observed for lead nuclei and the distributions do not significantly deviate from carbon data. The data are in good agreement with the results of recent calculations in the framework of the BUU model, with careful treatment of final state interaction effects but without an explicit in-medium modification of scalar, iso-scalar pion pairs. This means that for Ca most of the experimentally observed effect can be explained by final state interactions. Only at low incident photon energies there is still a small low mass enhancement of the double pi0 data over the BUU results., Comment: accepted for publication in Eur. Phys. J. A

Abstract

We have examined the spin structure of the proton in the region of the nucleon resonances (1.085 GeV < W < 1.910 GeV) at an average four momentum transfer of Q^2 = 1.3 GeV^2. Using the Jefferson Lab polarized electron beam, a spectrometer, and a polarized solid target, we measured the asymmetries A_parallel and A_perp to high precision, and extracted the asymmetries A_1 and A_2, and the spin structure functions g_1 and g_2. Abstract

Recent experimental results for meson photoproduction from nuclei obtained with TAPS at MAMI are analyzed in view of the suppression of the second nucleon resonance region in total photoabsorption. The cross sections can be split into a component from the low density surface region of nuclei and a component which scales more like the nuclear volume. The energy dependence of the surface component is similar to the deuteron cross section, it shows a clear signal for the second resonance peak assigned to the excitation of the P11(1440), D13(1520), and S11(1535). The volume component behaves differently, it is lacking the second resonance peak and shows an enhancement at intermediate photon energies., Comment: accepted for publication in Eur. J. Phys. A

Abstract

Photoproduction of neutral pions from nuclei (carbon, calcium, niobium, lead) has been studied for incident photon energies from 200 MeV to 800 MeV with the TAPS detector using the Glasgow photon tagging spectrometer at the Mainz MAMI accelerator. Data were obtained for the inclusive photoproduction of neutral pions and the partial channels of quasifree single pi0, double pi0, and pi0pi+/- photoproduction. They have been analyzed in terms of the in-medium behavior of nucleon resonances and the pion - nucleus interaction. Abstract

Differential cross sections of the reactions $(\gamma,\pi^\circ\pi^\circ)$ and $(\gamma,\pi^\circ\pi^++\pi^\circ\pi^-)$ have been measured for several nuclei ($^1$H,$^{12}$C, and $^{\rm nat}$Pb) at an incident-photon energy of $E_{\gamma}$=400-460 MeV at the tagged-photon facility at MAMI-B using the TAPS spectrometer. A significant nuclear-mass dependence of the $\pi\pi$ invariant-mass distribution is found in the $\pi^\circ\pi^\circ$ channel. This dependence is not observed in the $\pi^\circ\pi^{+/-}$ channel and is consistent with an in-medium modification of the $\pi\pi$ interaction in the $I$=$J$=0 channel. The data are compared to $\pi$-induced measurements and to calculations within a chiral-unitary approach.

Abstract

An original permanent magnet flux source is designed in order to generate a magnetic field of several Tesla. The magnet configuration and discretization of the structure are optimized with the help of numerical simulation software developped at LEG (DIPOLE-3D, FLUX2D & FLUX3D). The model of spheroidal flux source presented in the paper creates a field in excess of 4.3 T in a central volume of 0 6 mm x h 2.8 mm with external diameter [approximately equal to] 0 100 mm. A prototype is in the final assembly stage, designed for X-ray dichroism experiments at the European Synchrotron Radiation Facility (ESRF) in Grenoble. Prevention of fragility fractures in older people has become a public health priority, although the most appropriate and cost-effective strategy remains unclear. In the present statement, the Interest group on falls and fracture prevention of the European union geriatric medicine society (EUGMS), in collaboration with the International association of gerontology and geriatrics for the European region (IAGG-ER), the European union of medical specialists (EUMS), the Fragility fracture network (FFN), the International osteoporosis foundation (IOF) – European society for clinical and economic aspects of osteoporosis and osteoarthritis (ECCEO), outlines its views on the main points in the current debate in relation to the primary and secondary prevention of falls, the diagnosis and treatment of bone fragility, and the place of combined falls and fracture liaison services for fracture prevention in older people.

Abstract

The retest effect-improvement of performance on second exposure to a task-may impede the detection of cognitive decline in clinical trials for neurodegenerative diseases. We assessed the impact of the retest effect in Huntington's disease trials, and investigated its possible neutralization. We enrolled 54 patients in the Multicentric Intracerebral Grafting in Huntington's Disease (MIG-HD) trial and 39 in the placebo arm of the Riluzole trial in Huntington's Disease (RIL-HD). All were assessed with the Unified Huntington's Disease Rating Scale (UHDRS) plus additional cognitive tasks at baseline (A1), shortly after baseline (A2) and one year later (A3). We used paired t-tests to analyze the retest effect between A1 and A2. For each task of the MIG-HD study, we used a stepwise algorithm to design models predictive of patient performance at A3, which we applied to the RIL-HD trial for external validation. We observed a retest effect in most cognitive tasks. A decline in performance at one year was detected in 3 of the 15 cognitive tasks with A1 as the baseline, and 9 of the 15 cognitive tasks with A2 as the baseline. We also included the retest effect in performance modeling and showed that it facilitated performance prediction one year later for 14 of the 15 cognitive tasks. The retest effect may mask cognitive decline in patients with neurodegenerative diseases. The dual baseline can improve clinical trial design, and better prediction should homogenize patient groups, resulting in smaller numbers of participants being required.
