Abstract

A growing number of low- and middle-income country (LMIC) institutions have developed and implemented formal programs to support mentorship. Although the individual-level benefits of mentorship are well established, such activities can also sustainably build institutional capacity, bridge inequities in health care, and catalyze scientific advancement. To date, however, evaluation of these programs remains limited, representing an important gap in our understanding about the impact of mentoring. Without rigorous and ongoing evaluation, there may be missed opportunities for identifying best practices, iteratively improving program activities, and demonstrating the returns on investment in mentorship. In this report, we propose a framework for evaluating mentorship programs in LMIC settings where resources may be constrained. We identify six domains: 1) mentor-mentee relationship, 2) career guidance, 3) academic productivity, 4) networking, 5) wellness, and 6) organizational capacity. Within each, we describe specific metrics and how they may be considered as part of evaluation plans. We emphasize the role of measurement and evaluation at the institutional level, so that programs may enhance their mentoring capacity and optimize the management of their resources. Although we advocate for a comprehensive approach to evaluation, we recognize that-depending on stage and relative maturity-some domains may be prioritized to address short- and medium-term program goals.

Abstract

Loreto, in the Peruvian Amazon, has one of the highest adolescent pregnancy rates in the country. However, underlying causes of adolescent pregnancy are not fully understood as data are limited in Indigenous and remote Amazonian communities. This study investigated adolescent reproductive health within Loreto using an ecological systems framework. Forty-one semi-structured interviews were conducted in June 2022: community leaders (n = 12) and adolescent participants between 15 and 17 years of age (pregnant girls, n = 11; never pregnant girls, n = 9; and boys, n = 9). We also conducted focus group discussions with community health workers and educators in October 2022 (three focus groups, n = 15). Adolescent reproductive health is complex with multi-layered factors that put girls at higher risk of pregnancy. We found a paradoxical relationship between expected social and gender norms and individual desires. This research provides a contextual understanding of the lived experience of adolescents and young people in the Amazon region of Peru. Our findings suggest the need for greater exploration of the contradictory ideas surrounding adolescent pregnancy and female sexuality. [ABSTRACT FROM AUTHOR]

Abstract

Human papillomavirus (HPV) is causally associated with anal cancer, as HPV DNA is detected in up to 90% of anal intraepithelial neoplasias and anal cancers. With the gradual increase of anal cancer rates, there is a growing need to establish reliable and clinically relevant methods to detect anal cancer precursors. In resource-limited settings, HPV DNA detection is a potentially relevant tool for anal cancer screening. Here, we evaluated the performance of the Hybribio GenoArray (GA) for genotyping HPV in anal samples, against the reference standard Roche Linear Array (LA). Anal swab samples were obtained from sexually active men who have sex with men. Following DNA extraction, each sample was genotyped using GA and LA. The overall interassay agreement, type-specific, and single and multiple genotype agreements were evaluated by kappa statistics and McNemar's χ(2) tests. Using GA and LA, 68% and 76% of samples were HPV DNA positive, respectively. There was substantial interassay agreements for the detection of all HPV genotypes (κ = 0.70, 86% agreement). Although LA was able to detect more genotypes per sample, the interassay agreement was acceptable (κ = 0.53, 63% agreement). GA had poorer specific detection of HPV genotypes 35, 42, and 51 (κ < 0.60). In conclusion, GA and LA showed good interassay agreement for the detection of most HPV genotypes in anal samples. However, the detection of HPV DNA in up to 76% of anal samples warrants further evaluation of its clinical significance.

10. Barriers and Facilitators in the Recruitment and Retention of Peruvian Female Sex Workers in a Randomized HPV Vaccine Trial

Abstract

Objective: To share the lessons learned when recruiting and retaining Peruvian female sex workers (FSWs) in a clinical trial.Methods: Peruvian FSWs 18-26 years of age were asked to join a clinical study of HPV vaccine starting in August 2009. Condoms, lubricants, and health services were given as an incentive to join the study, as well as a gift valued at three US dollars at each study visit for retention purposes.Results: 120 participants completed the survey. Barriers to non-enrollment recruiting included the false association of our clinical trial with an ineffective HIV vaccine study, plans to become pregnant during the vaccine study, not identifying as sex workers, pushback from husbands with fear of vaccine related birth defects, questioning motives for a free vaccine, not wanting to use birth control, lack of high perceived value of incentives, and limited time availability. Barriers to retention included periodic travel out of Lima, high costs for commuting, the requirement of no clients one day before the visit, and misinformation by health care providers regarding associations between the vaccine and illnesses.Conclusions: Working with health promoters and brothel managers, making periodic phone call reminders of appointments, and identifying participants who were peer leaders helped facilitate study participation and retention. Despite hardships in recruiting and retaining female sex workers, this group will participate in a study they deem useful for their health. Before recruiting female sex workers in clinical trials, potential barriers need to be addressed by study investigators.

16. Social innovation in health: a critical but overlooked component of the COVID-19 pandemic response

Abstract

The COVID-19 pandemic has profoundly disrupted the provision of health services across the globe. Travel to hospitals is restricted and many health facilities have limited services, deepening financial problems for some clinics and hospitals.1 Vulnerable groups who are already marginalised by their gender, race or nationality have been disproportionately affected by COVID-19.2 In response, social innovations have been developed to protect vulnerable groups and rapidly pivot health systems towards COVID-19. Social innovations in health are inclusive solutions that meet the needs of end users through a multistakeholder, community-engaged process to address the healthcare delivery gap.3 Social innovation is particularly well suited for the COVID-19 response because it focuses on local needs, develops low-cost solutions and builds on community strengths (figure 1). In partnership with partner academic institutions, the Special Programme for Research and Training in Tropical Diseases launched the Social Innovation in Health Initiative (SIHI) in 2014. SIHI focuses on research, training and advocacy related to social innovation in health. In collaboration with partners, the network organised an online open event to discuss social innovation responses to emergencies.4 This piece highlights how social innovation has contributed to the COVID-19 response and presents three examples of social innovation projects that have adapted to the pandemic. These examples demonstrate how social innovation during COVID-19 has mobilised local communities, swiftly adapted existing health services and built strong partnerships.

Abstract

The aim of the study was to describe the process of designing and producing a video to promote HIV testing in Peruvian men who have sex with men (MSM). The process involved the following steps: identification of the theories of behavior change; identifying key messages and video features; developing a script that would captivate the target audience; working with an experienced production company; and piloting the video. A video with everyday situations of riskassociated with HIV infection was the one preferred by participants. Key messages identified, and theoretical constructs models chosen were used to create the video scenes. Participants identified with the main, 9 minute video which they considered to be clear and dynamic. It is necessary to work with the target population to design a video according to their preferences. El objetivo del estudio fue describir el proceso de diseñar y producir un video para promover la prueba del VIH en hombres que tienen sexo con hombres (HSH) peruanos. El proceso involucró los siguientes pasos: identificación de las teorías de cambio de comportamiento; identificación de los mensajes clave y características del video; elaboración de un guion que cautive al público objetivo; trabajar con una compañía de producción con experiencia; y el piloto del video. Un video con situaciones cotidianas de riesgo asociadas con la infección por el VIH fue el preferido por los participantes. Los mensajes clave identificados, y los constructos de los modelos teóricos elegidos, fueron empleados para crear las escenas del video. Los participantes se identificaron con el video principal de 9 minutos de duración el cual lo consideraron claro y dinámico. Es necesario trabajar con la población objetivo para diseñar un video acorde con sus preferencias.

Abstract

Training in Biomedical Informatics is essential to meet the challenges of a globalized world. However, the development of postgraduate training and research programs in this area are scarce in Latin America. Through QUIPU: Andean Center for Training and research in Iformatics for Global Health, has developed the first Certificate and Master’s Program on Biomedical Informatics in the Andean Region. The aim of this article is to describe the experience of the program. To date, 51 students from Peru, Chile, Ecuador, Colombia and Venezuela have participated; they come from health ministries, hospitals, universities, research centers, professional associations and private companies. Seventeen courses were offered with theparticipation of faculty from Argentina, Chile, Colombia, USA, Mexico and Peru. This program is already institutionalized at the School of Public Health and Administration from the Universidad Peruana Cayetano Heredia. El entrenamiento en Informática Biomédica es fundamental para enfrentar los desafíos de un mundo globalizado. Sin embargo, el desarrollo de programas de entrenamiento e investigación en posgrado en esta área son escasos en América Latina. A través del QUIPU: Centro Andino de Investigación y Entrenamiento en Informática para la Salud Global, se ha desarrollado el primer programa de Diplomado y Maestría en Informática Biomédica en la Región Andina. El objetivo de este artículo es describir la experiencia del programa. A la fecha han participado 51 alumnos de Perú, Chile, Ecuador, Colombia y Venezuela; procedentes de ministerios de salud, hospitales, universidades, centros de investigación, colegios profesionales y empresas privadas. Diecisiete cursos se impartieron con la participación presencial y virtual de profesores de Argentina, Chile, Colombia, Estados Unidos, México y Perú. Este programa está ya institucionalizado en la Facultad de Salud Pública y Administración de la Universidad Peruana Cayetano Heredia.

Lessons learned in designing the Kuskaya program include: 1) involving mentors in the fellow selection process, 2) involving fellows in existing lines of research to increase mentor involvement, 3) institutionalizing mentoring through regular works-in-progress meetings and providing mentoring materials, and 4) defining a core curriculum for all fellows while providing additional supplementary materials to meet each cohort’s needs, and evaluating their progress. Kuskaya provides an innovative model for bi-national, global health training to engage and provide a public health career pathway for all professionals. K

Objetivo: Evaluar el uso de Internet para buscar parejas sexuales e información sobre VIH/SIDA o infecciones de transmisión sexual (ITS) entre personas viviendo con VIH/SIDA (PVVS). Materiales y métodos: Entrevistamos a las PVVS que acudieron consecutivamente a Vía Libre en Lima Perú entre mayo y junio de 2004. Resultados: De las 100 PVVS (46 hombres que tienen sexo con hombres [HSH], 18 hombres heterosexuales y 36 mujeres), 59% reportó el uso de Internet en los últimos 12 meses. De ellos, la mayoría (73%) accedió a Internet en un lugar público; 16 (27,1%) reportaron haber buscado parejas sexuales en línea y cinco (8,5%) reportaron haber tenido sexo con una pareja que conocieron por Internet. Todas las PVVS que buscaron sexo en Internet fueron hombres, siendo el porcentaje de HSH significativamente mayor que la de heterosexuales (94% frente a 6%; p=0,032). Los cinco que reportaron haber tenido sexo con una pareja que conocieron en línea fueron HSH. De los que accedieron a Internet en el último año, 76% (45/59) lo hizo para buscar información sobre VIH/SIDA y 39% (23/59) para buscar información sobre otras ITS. Conclusiones: Entre las PVVS encuestadas, más de la mitad reportó acceso a Internet. La mayoría lo hizo para buscar información sobre VIH/SIDA y más de la cuarta parte - casi exclusivamente HSH - lo hizo para buscar parejas sexuales. Estos hallazgos sugieren que Internet es una herramienta conveniente para brindar mensajes de prevención en línea sobre VIH/ITS a HSH de alto riesgo. Objective: To evaluate use of the Internet for seeking sex partners and information on HIV and/or sexually transmitted infections (STIs) among people living with HIV/AIDS (PLWHA). Materials and methods: We interviewed consecutive PLWHA attending at Vía Libre, in Lima, Peru, between May and June 2004. Results: Of 100 PLWHA, (46 men who had sex with men [MSM], 18 heterosexual men and 36 women), 59% reported using the Internet in the last 12 months. Of these, the majority (73%) accessed the Internet at a public place; 16 (27,1%) reported having gone online to search for sexual partners; and five (8,5%) reported having had sex with a partner found over the Internet. All sex seekers were men, of them a greater percentage were MSM rather than heterosexual (94% vs. 6%, p= 0,032). All five respondents who reported having had sex with a partner found online were MSM. Of those who accessed the Internet in the past year, 76% (47/59) had used it to find information on HIV/AIDS, and 39% (23/59) had used it to seek information on other STIs. Conclusions: Among PLWHA interviewed more than half reported Internet access. More than a quarter -almost exclusively MSM- had used it to search for sex partners, however, the majority had used the Internet to look for information about HIV/AIDS. These findings suggest that the Internet offers a convenient tool to engage high-risk MSM in online HIV/STI prevention.

The Universidad Peruana Cayetano Heredia and University of Washington created the NIH-funded Kuskaya training program in response to the need for transformative global health training for talented graduates from all disciplines. Kuskaya is a 1-year, interdisciplinary training program that teaches Peruvian and US graduates critical skills related to public health research through the design and implementation of a collaborative research project in Peru. Between 2014 and 2018, the program has trained 33 fellows, of which one third were from non-health disciplines. The program is unique because it targets junior trainees from disciplines outside of the health field, the program's curriculum is adapted to fit the fellows' backgrounds and professional aspirations, and the structure of the program allows for collaboration within the cohort and encourages fellows to apply for additional funding and pursue advanced degrees. Abstract

We aimed to gather information among gay men regarding their preferences for online sexual health information; 1,160 Peruvian MSM, 18 years or older, completed an online survey hosted on www.tunexo.org. The mean age was 26.8 years. Around 90% had post-high school education. The self-reported HIV prevalence was 12.3%. The acceptability of sexual health content was greater in the most highly educated group. The highest rated topics and services of interest were those related to improving sexual and mental health. The least educated group was significantly more interested in “getting prevention messages on mobiles” compared to men with the highest level of education (71% vs. 52%; p < 0.001). Men’s sexual health was of more interest to the 30-39-year-old group compared to the 18-24-year-old one (97% vs. 87%; p = 0.005). Future Web-based interventions related to sexual health among targeted groups of MSM in Peru can be tailored to meet their preferences. [ABSTRACT FROM AUTHOR]

Abstract

To improve global health and the welfare of a population, skilled human resources are required, not only in medicine and health, but also in the field of informatics. Unfortunately, training and research programs specific to biomedical informatics in developing countries are both scarce and poorly documented. The aim of this paper is to report the results from the first Informatics Expert Meeting for the Andean Region, including, nine Latin American based institutional case studies. This two-day event occurred in March 2010 and brought together twenty-three leaders in biomedical informatics from around the world. The blend of practical and experiential advice from these experts contributed to rich discussions addressing both challenges and applications of informatics within Latin American. In addition, to address the needs emphasized at the meeting, the QUIPU Network was established to expand the research consortium in the Andean Region, Latin America, and internationally. The use of these new technologies in existing public health training and research programs will be key to improving the health of populations in the Andean Region and around the globe. Abstract

Innovative tools such as the Internet, personal digital assistants, tablet computers, cell phones, and other technologies are a growing arsenal in the global effort to prevent and control HIV and other sexually transmitted infections (STIs). While a variety of information and communication technology tools are in various stages of use for HIV/STI prevention, relatively few areas have accumulated a critical mass of evidence-based data about the most effective approaches. However, some of that evidence is compelling, and the potential for future uses appears large. Application to some areas of practice and research are nascent, the impact on disease incidence and economic evaluation data are still very limited, and evaluation of these tools would benefit from rigorous study designs. In this article we review the published literature regarding the use of information and communication technology applications to HIV/STI control. Appropriately utilized technologies may improve HIV/STI screening, prevention, surveillance, and care for patients and populations in both resource-constrained and resource-rich settings. Avances tecnológicos innovadores como Internet, computadoras personales de bolsillo, teléfonos celulares y otros equipos son un arsenal en crecimiento en el esfuerzo de impedir y controlar el VIH y otras infecciones de transmisión sexual (ITS). A pesar que existe una diversidad de tecnologías de información y comunicación en diferentes etapas de desarrollo para la prevención del VIH e ITS, la investigación en esta área se encuentra aún en crecimiento, y el impacto en la incidencia de enfermedad, las evaluaciones con diseños rigurosos y los estudios económicos todavía son muy limitados. Sin embargo, algunas de estas evidencias son prometedoras y poseen un gran potencial para su uso en nuestro medio. En este artículo hemos realizado una revisión sistemática de la literatura relacionada con el uso de la tecnología aplicada a la prevención y control del VIH e ITS. De ser usada apropiadamente, esta tecnología podría mejorar la vigilancia del VIH y otras ITS, diagnóstico, notificación de parejas, prevención, manejo clínico, y capacitación de profesionales de la salud.
