A large proportion of health care costs in the UK are spent on emergency care. This study examined the prospective relationship between depression and use of emergency care in patients with COPD managed in primary care.Methods: This was a twelve-month, prospective longitudinal study of 355 patients with COPD in six primary care practices in the UK. Baseline measures included demographic characteristics, depression and anxiety, severity of COPD, presence or absence of other chronic diseases, and prior use of emergency care. Outcome measures were (a) number of emergency department (ED) visits; or (b) an emergency hospital admission in the follow-up year.Results: Older age, number of comorbid physical health conditions, severity of COPD, prior use of emergency care, and depression were all independently associated with both ED attendance and an emergency hospital admission in the follow-up year. Subthreshold depression (HADS depression score 4–7) was associated with a 2.8 times increased odds of emergency hospital admission, and HADS depression >8 was associated with 4.8 times increased odds.Conclusion: Depression is a predictor of emergency care in COPD, independent of severity of disease or physical comorbidity. Even mild (subthreshold) symptoms of depression more than double the risk of using emergency care, suggesting there is a strong case to develop and deploy integrated preventive strategies in primary care that can promote mental health in people with COPD.Keywords: COPD, depression, anxiety, emergency care, United Kingdom, hospital admission, primary care

Abstract

BackgroundThe digital revolution has led to a boom in the number of available online health care resources. To navigate these resources successfully, digital literacy education is required. Learners who can evaluate the reliability and validity of online health care information are likely to be more effective at avoiding potentially dangerous misinformation. In addition to providing health care education, massive open online courses (MOOCs) are well positioned to play a role in providing digital literacy education in this context. ObjectiveThis study focused on learners enrolled in a MOOC on cancer genomics. The aim of this study was to evaluate the efficacy of a series of digital literacy–related activities within this course. This was an iterative study, with changes made to digital literacy–related activities in 4 of the 8 runs of the course. MethodsThis mixed methods study focused on learner engagement with the digital literacy–related activities, including the final course written assignment. Quantitative data including the number of references listed in each written assignment were compared between successive runs. Qualitative data in the form of learner comments on discussion forums for digital literacy–related tasks were evaluated to determine the impact of these educational activities. ResultsUsing the number of references included for each final course assignment as an indicator of digital literacy skills, the digital literacy–related activities in the final 2 runs were judged to be the most successful. We found a statistically significant increase in the number of references cited by learners in their final written assignments. The average number of references cited in Run 8 was significantly higher (3.5) than in Run 1 (1.8) of the MOOC (P=.001). Learner comments in Runs 7 and 8 showed that a poll in which learners were asked to select which of 4 online resources was reliable was effective in stimulating learner discussion about how to evaluate resource reliability. ConclusionsSimilar to many health care MOOCs, the course studied here had a heterogeneous group of learners, including patients (and their families), the public, health care students, and practitioners. Carefully designing a range of digital literacy–related activities that would be beneficial to this heterogenous group of learners enabled learners to become more effective at evaluating and citing appropriate online resources within their written assignments.

Abstract

he Kuril Islands are a volcanic archipelago located between Hokkaido and Kamchatka. In this study we investigated earthworm fauna of three of the Southern Kuril Islands, Kunashir, Shikotan, and Yuri, using both morphological analysis and DNA barcoding. Our results highlight the potential of DNA barcoding for studying earthworm fauna: while previous studies reported only six earthworm species and subspecies on the Southern Kurils, we detected 15 genetic clusters. Six of them correspond to European cosmopolites; six, to Asian species, and three, to unidentified species. While no European earthworms were found on Yuri that is uninhabited since WWII, they dominated on larger and inhabited Kunashir and Shikotan, suggesting that they are recent invaders. Of the six Asian species, five had cox1 sequences identical or very closely related to published sequences from the mainland or the Japanese islands and thus are recent invaders.

Abstract

A chance visit to Darwin allowed inspection of and addition to Northern Territory (NT) Museum’s earthworm collection. Native Diplotrema zicsii sp. nov. from Alligator River, Kakadu NP is described. Town samples were dominated by cosmopolitan exotic Metaphire bahli (Gates, 1945) herein keyed and compared morpho-molecularly with M. peguana (Rosa, 1890) requiring revision of allied species including Filipino Pheretima philippina (Rosa, 1891), P. p. lipa and P. p. victorias sub-spp. nov. A new P. philippina-group now replaces the dubia-group of Sims & Easton, 1972 and Amynthas carinensis (Rosa, 1890) further replaces their sieboldi-group. Lumbricid Eisenia fetida (Savigny, 1826) and Glossoscolecid Pontoscolex corethrurus (Müller, 1857) are confirmed introductions to the NT. mtDNA barcodes newly include Metaphire houlleti (Perrier, 1872) and Polypheretima elongata (Perrier, 1872) spp.-complexes from the Philippines. Pithemera philippinensis James & Hong, 2004 and Pi. glandis Hong & James, 2011 are new synonyms of Pi. bicincta (Perrier, 1875) that is common in Luzon. Vietnamese homonym Pheretima thaii Nguyen, 2011 (non P. thaii Hong & James, 2011) is replaced with Pheretima baii nom. nov. Two new Filipino taxa are also described: Pleionogaster adya sp. nov. from southern Luzon and Pl. miagao sp. nov. from western Visayas.

This review aims to provide an overview of the burden of depression and anxiety in those with COPD and to outline the contemporary advances and challenges in the management of depression and anxiety in COPD. Symptoms of depression and anxiety in COPD lead to worse health outcomes, including impaired health-related quality of life and increased mortality risk. Depression and anxiety also increase health care utilization rates and costs. Although the quality of the data varies considerably, the cumulative evidence shows that complex interventions consisting of pulmonary rehabilitation interventions with or without psychological components improve symptoms of depression and anxiety in COPD. Cognitive behavioral therapy is also an effective intervention for managing depression in COPD, but treatment effects are small. Cognitive behavioral therapy could potentially lead to greater benefits in depression and anxiety in people with COPD if embedded in multidisciplinary collaborative care frameworks, but this hypothesis has not yet been empirically assessed. Mindfulness-based treatments are an alternative option for the management of depression and anxiety in people with long-term conditions, but their efficacy is unproven in COPD. Beyond pulmonary rehabilitation, the evidence about optimal approaches for managing depression and anxiety in COPD remains unclear and largely speculative. We therefore conducted a systematic review of prospective cohort studies that measured depression, anxiety, and HRQoL in COPD. Methods: Electronic databases (Medline, Embase, Cumulative Index to Nursing and Allied Health Literature [CINAHL], British Nursing Index and Archive, PsycINFO and Cochrane database) were searched from inception to June 18, 2013. Studies were eligible for inclusion if they: used a nonexperimental prospective cohort design; included patients with a diagnosis of COPD confirmed by spirometry; and used validated measures of depression, anxiety, and HRQoL. Data were extracted and pooled using random effects models. Results: Six studies were included in the systematic review; of these, three were included in the meta-analysis for depression and two were included for the meta-analysis for anxiety. 40. Does a combined dialogic and print referencing reading program improve the vocabulary and print knowledge of children aged 4-5?

Abstract

Dialogic reading (Whitehurst et al., 1988) has a positive effect on the vocabulary of young children (Whitehurst et al., 1994). Knowledge of print is a strong predictor of later reading ability (McCardle et al., 2001). Adults using a print referencing style refer explicitly to aspects of the printed word (Justice & Ezell, 2004). Justice et al. (2009a) recommended coupling the reading styles to see if their simultaneous use could improve both the vocabulary and print knowledge of young children. This current study is the first assessment of a combined approach. Within the between-subjects design 55 children aged 4-5 were recruited from 2 schools. The children in the experimental condition read with the researcher using the print referencing/ dialogic reading program in small groups over a 5 week period. The children in the control condition were read to in a more didactic way. The children’s receptive vocabulary was assessed, both before and after the program, using the PPVT-4 scale. They were also tested on their expressive and receptive vocabulary, and print knowledge using researcher-designed tests. A Quade’s rank analysis of covariance indicated there was a significant improvement in children’s print knowledge but no improvement in vocabulary. Correlational analysis suggested there were significant relationships between both receptive and expressive vocabulary, and print knowledge. Future studies of this program may be useful to determine whether a more long-term or intensive program might improve vocabulary and if the program could be adapted for use with pre-schoolers.

A critique of recent clado-molecular phylogenies notes shortcomings of starting materials, methods applied, and,therefore, their conclusions; hence this review. A new group, Exquisiclitellata, is newly defined as those ‘non-crassiclitellate’members of the superorder Megadrilacea (viz., Moniligastridae Claus, 1880, plus Alluroididae Michaelsen, 1900 andSyngenodrilidae Smith & Green, 1919). Support for restitution and elevation of American Diplocardiinae Michaelsen, 1900and Argilophilini Fender & McKey-Fender, 1990 are again raised. ICZN priority requires revival of Typhoeus Beddard, 1883over synonym Eutyphoeus Michaelsen, 1900 and the sub-family Typhoeinae (corr. of Typhaeinae Benham, 1890) is reestablished.Hoplochaetellinae sub-family nov. is proposed as a development of Octochaetidae s. lato in India.Wegeneriellinae sub-fam. nov. accommodates the holoic members of a restricted Neogastrini Csuzdi, 1996 from W. Africaand S. America. Caribbean family Exxidae Blakemore, 2000 and related Trigastrinae Michaelsen, 1900 are both retained. Abstract

A continental European earthworm, Kenleenus armadas sp. nov., is newly described from an introduction to a farm in Dublin, Ireland. Previously recognized as Aquitainian Prosellodrilus amplisetosus Bouché, 1972, the morphology and molecular characteristics rather indicate it as a taxon new to science. Its description has required revision of all prosellodrilid genera including Algerian subgenus Maghrebiella Baha & Bera, 2001 that is a permanently invalid homonym, here replaced with Cadanera nom. nov. A revised checklist is appended that holds Allolobophora festae Rosa, 1892 as a species incertae sedis pending review of the Cadanera type. This report brings the total earthworms known from Britain & Ireland to 72 species, including a new record of cosmopolitan Pithemera bicincta (Perrier, 1875) (Megascolecidae) from Dublin’s Botanic Gardens

Abstract

Two independent cases of oligochaete worms recovered from Korean women’s vaginas are reported. Both specimens were non-parasitic cosmopolitan exotics identified as: microdrile tubificid Limnodrilus hoffmeisteri Claparède, 1862 (Tubificidae) and megadrile dichogastrid Dichogaster bolaui (Michaelsen, 1891) (Octochaetidae) – a new record for Korea. The tubificid is a freshwater euryhaline species that may reach high numbers in organically rich water e.g. in a paddy field, whereas the earthworm is commonly intercepted by quarantine in plant or vegetable cargos and also appears in bathtubs when it inhabits drainage sys¬tems. Thus, bathing/douching, field working or picnicking without a blanket are suggested as possible modes of ingress. Oligo¬chaetes rarely occur in live human bodies but the few previous historical records are reviewed.

44. Increased threat learning after social isolation in human adolescents

Abstract

In animal models, social isolation impacts threat responding and threat learning, especially during development. This study examined the effects of acute social isolation on threat learning in human adolescents using an experimental, within-participant design. Participants aged 16–19 years underwent a session of complete isolation and a separate session of isolation with virtual social interactions, counterbalanced between participants, as well as a baseline session. At baseline and following each isolation session, participants reported their psychological state and completed a threat learning task in which self-report ratings and physiological responses to learned threat and safety cues were measured. Threat learning increased after both isolation sessions in two ways. First, participants found the learned threat cue more anxiety-inducing and unpleasant after isolation compared with baseline. Second, during threat extinction, electrodermal activity was partially elevated after isolation compared with baseline. Further, the results suggested that isolation influenced threat learning through state loneliness. Threat learning is central to threat-related disorders including anxiety, phobias, obsessive-compulsive disorder (OCD) and post-traumatic stress disorder (PTSD), and our findings suggest that isolation and loneliness in adolescence might increase vulnerability to the emergence of these disorders through increased threat learning.

Abstract

Metaphire tanbode sp. nov. is found in rice paddy in Kinki plain at Lake Biwa and Amynthas yamade sp. nov. isfrom Hira range to the West. M. tanbode belongs to the M. hilgendorfi / A. tokioensis species-complex, while montane A. yamadeis comparable to both Amynthas aeruginosus-group and Duplodicodrilus schmardae-group. Genetic barcoding (mtDNA COI) viatypes is attempted. Taxonomic ‘housekeeping’ requires replacement of invalid homonyms: e.g. Pheretima montana Ishizuka, 1999(non type-species P. montana Kinberg, 1867) is renamed Amynthas nonmontanus; others are A. nonsilvestris, A. noninvisus, A.nonmonticolus and A. nonsetosus, noms. et combs. novae. Thus Pheretima Kinberg, 1867 s. stricto remains unrecorded from Japanwhile prior Amynthas Kinberg, 1867, and its derivative Metaphire Sims & Easton, 1972, are abundant and diverse. Family andgeneric level definition and placement of Oriental pheretimoids are restated for the benefit of current workers and for novicefield-ecologists. Surveys of below-ground biodiversity of rice paddy in Lake Biwa is compared to more natural habitats aroundLake Pedder in Western Tasmanian Wilderness Area, and co-incidentally, both have 21 recorded earthworm species. Thus claimsfrom various countries of less than six species per location are contraindicated by thorough eco-taxonomic methods yieldingmore representative results.

Abstract

Abstract. A new species, Drawida eda Blakemore, is proposed for an earthworm from rice paddy near Lake Biwa in centralJapan. It is compared with both Drawida barwelli (Beddard, 1886) – the cosmopolitan type of the genus – and with sympatric D.japonica (Michaelsen, 1892) for which a new synonym, D. propatula Gates, 1935, is added. Parasitic origin theory of diagnostic‘genital markings’ in D. japonica is extended to other taxa and their conspecificity is mooted. Definitive resolution of thetaxonomic complexities within Drawida via DNA analysis is pending, although the COI barcode for the type of D. eda is initiallyprovided, the first time for a new earthworm species. Polygiceriate similarity of Oriental ‘exquisiticlitellate’ Drawida tomegascolecid genera like Nexogaster Blakemore, 1997 (type Nexogaster sexies Blakemore, 1997) is briefly noted. Using thisopportunity, replacement names are given for two preoccupied Tasmanian Lake Pedder taxa as a normal part of taxonomic‘housekeeping’, viz. Abstract

Palaearctic family Criodrilidae Vejdovsky, 1884 is briefly reviewed and revised to reabsorb Biwadrilidae Brinkhurst & Jamieson, 1971, monotypic for lacustrine/limnic Biwadrilus bathybates (Stephenson, 1917) based on examination of new Lake Biwa material from recent earthworm surveys. Comparison is with type species: Criodrilus lacuum Hoffmeister, 1845. Synonymy in Criodrilus of monotypic genus Hydrilus Qiu & Bouché, 1998 from Algeria is accepted but with provisional restoration of its type as Criodrilus ghaniae (Qiu & Bouché, 1998) comb. nov., if indeed it belongs in the Criodrilidae. Another recent taxon Guarani camaqua Rodríguez & Lima, 2007 from rice fields of Rio Grande do Sul, Brazil is syn. nov. of Criodrilus lacuum at species and genus level since its characteristics are easily embraced within interspecific variability. Moreover, the prior taxon, C. lacuum, was already known from its type-locality. Distribution, ecology and species associations of the criodrilids are briefly summarised including a report of C. lacuum maintained in a laboratory culture for >42 years (T. Timm pers. comm.). A key to species is provided.
