10. Enhancing host cell responses via lipid-functionalisation of orthopaedic titanium

Total joint replacement surgery is an excellent treatment for arthritis. Unfortunately around 10% of these prostheses will fail due to aseptic loosening and require revision surgery that is both expensive and less successful for patients. One way of reducing revision surgery could include steps to enhance human osteoblast (hOB) maturation at biomaterial surfaces by modifying existing implant materials. The aim of this research therefore was to create a lipid-functionalised titanium surface that encourages maturation of hOBs, a key event for successfuI osseo integration. Lysophosphatidic acid (LPA) refers to a family of structurally-related bioactive lipids. One of these molecules, oleoyl-LPA, had already been shown to enhance vitamin D3-induced hOB formation and maturation in vitro. Novel results presented within this thesis indicate that LPA members and structurally related synthetic lipids exhibit different potencies with regard to vitamin D3-induced hOB maturation. In summary the finding ofthis thesis indicate that the synthetic compounds are especially efficacious in securing vitamin D3-induced hOB maturation. When developing bioactive titanium surfaces for orthopaedic applications it is important to examine hOB responses to the selected agent(s) in combination with factors central to skeletal health. In this thesis, initial studies focused on an important local mediator, transforming growth factor ~ (TGF~) . In addition research extended to two widely recognised systemic factors, parathyroid hormone (PTH) and oestradiol. Interestingly TGF~ bolstered the maturation of hOBs to LPA and vitamin D3 via a mechanism that may involve an autocrine/paracrine platelet-derived growth factor (PDGF) loop. In contrast oestradiol and parathyroid hormone (PTH) appeared to be without influence on the maturation responses of hOBs to LPA. In light of the risk of prosthetic joint infections, it is also important to consider whether any surface modification promotes or deters the growth of microorganisms. Whilst oleoyl-LPA lacked appreciable antimicrobial actions, it did not promote bacterial growth. In contrast, palmitoyl-LPA appeared to have greater antimicrobial activity and may therefore be an interesting molecule to investigate further in developing multifunctional bone biomaterials that combine host tissue biocompatibility with an antibacterial surface finish . In considering titanium surface functionalisation with LPA (or indeed related lipids) the potential for self-assembly of lipid bi-Iayered structure presented itself as an attractive and facile means of modification. To this end titanium was initially treated with an alkanephosphonate (octadecylphosphonic acid) that served as a tethering point for LPA via tail -tail - hydrophobic interactions akin to the arrangement of phospholipids in cell membranes. The findings of this thesis support the successful functionalisation oftitanium using this simple approach . Future research should continue in exploring the stability and biocompatibility of these novel titanium modifications.

A recent survey-based experiment published in Clinical Orthopaedics and Related Research® found that musculoskeletal surgeons have an implicit bias in favor of specific diseases over nonspecific illnesses. This bias can affect the care provided to patients with conditions like radial tunnel syndrome, fibromyalgia, and repetitive strain injury. The study suggests that surgeons should acknowledge their biases and try communication strategies that allow patients with nonspecific illnesses to feel heard, without resorting to potentially harmful investigations and surgeries. Further research is needed to confirm the presence of implicit bias among surgeons and to develop effective strategies for better communication with these patients. Additionally, providing resources to improve the psychosocial aspects of care and creating a forum for surgeons to share communication strategies could be beneficial. [Extracted from the article]

The town of Hay River, located in the Northwest Territories, Canada, is vulnerable to ice jam flooding occurring in the adjacent Hay River delta. The most extreme flooding events have occurred when ice jams in the channels of the delta were pushed downstream towards the mouth at Great Slave Lake. This movement has been linked to incoming waves from ice jam release in the upstream reaches of the Hay River. This study incorporated the effect of an upstream ice jam release wave into the prediction of ice jam caused flood levels in the delta by integrating three one-dimensional models. The method was validated with observed breakup events and then used to simulate various combinations of ice and water conditions in the river and the delta. Multiple linear regression analyses were applied to the results to develop a prediction tool for assessing ice jam flood risk. 23. Numerical Modelling of River Ice in Complex River Systems

Rivers in cold regions experience ice conditions for a significant part of the year. River ice can cause ice jam flooding, impact hydropower generation operations, and affect a river’s ecological and morphological conditions. Many ice processes are highly dynamic and are affected by meteorological / hydrodynamic conditions, and river geomorphology. River ice can be very challenging to study due to the risks and costs associated with data collection in harsh winter conditions. One of the most economical and efficient approaches to study river ice processes and to evaluate the effects of ice on a river’s regime is to use numerical model simulations. At present, most existing one-dimensional (1D) river ice models are based on an implicit finite difference solution to the Saint-Venant equations. As a result, highly dynamic events such as rapid ice jam formation or sudden ice jam release are difficult to model due to numerical instabilities that can arise if the flow approaches supercritical. Also, river ice models with network modelling capabilities reduce conservation of mass and energy principles to continuity of discharge and equality of water levels at the junctions, which may not be reasonable when the ice and flow conditions are rapidly changing. There is a need for a comprehensive 1D river ice process model that is capable of simulating the full ice regime in rivers with complex natural channel geometry where mixed flow regimes are anticipated. The ultimate goal of this research is to develop a robust public-domain comprehensive 1D river ice process model, capable of handling complex natural channel geometry and channel networks for the full spectrum of scenarios from simple known steady ice conditions to highly dynamic cases such as ice jam formation or release. In this study, a number of developments were made to the University of Alberta’s public-domain hydrodynamic and river ice process model, River1D, as steps towards realizing this ultimate long-term goal. Firstly, the model was reformulated to accommodate natural channel geometry and enhanced to include previously excluded ice processes. Previous versions of the model allowed for a rectangular channel approximation only. The new natural channel geometry version of the model was then enhanced to include new ice processes: water supercooling, frazil accretion, frazil re-entrainment, anchor ice formation and release, border ice formation, under-cover transport of frazil, and ice cover formation based on leading edge stability criteria. The model was validated with freeze-up data from the Susitna River, Alaska. Secondly, the model was modified to simulate flow in channel networks using a momentum based approach to simulate junctions that includes important physical effects at junctions but without the need to adjust model parameters or redefine junctions should a flow reversal occur. A series of steady and unsteady tests were used to assess this new approach. The results were compared to and agreed favourably with results simulated with a two-dimensional (2D) model. The unsteady test results demonstrated the model’s capability of handling transient flow reversals. The model was then applied to a network of channels in the Mackenzie Delta for both open water and ice jam conditions. Model results agreed well with observed water level data. Modelled ice jam conditions indicated a flow reversal in the Peel Mackenzie Connector, which is consistent with observations in this channel during breakup. Lastly, the model was enhanced to simulate ice jam profiles in multi-channel networks. The enhancements include provisions for handling junctions when solving the ice jam stability equation within a channel network. The model was compared to a series of idealized test cases from a previous study that sought to investigate the impacts of islands on ice jam profiles. Model results agreed very favourably with the results from the previous study. The model was then applied to the Hay River Delta. The model was validated for both open water and ice jam conditions.

Background: The clinical picture of ulnar-sided wrist pain is oftentimes confusing because various pathologies may be coexistent. In this study, we aimed: (1) to compare the prevalence of potential causes of ulnar-sided wrist pain on magnetic resonance imaging (MRI) in patients who underwent triangular fibrocartilage complex (TFCC) repair and control subjects: and (2) to evaluate whether inferior clinical results were associated with specific patient characteristics or other potential causes of ulnar-sided wrist pain. Methods: We included 67 patients who underwent a TFCC repair and 67 control subjects. The MRI scans were examined for sources of ulnar-sided wrist pain. After TFCC repair, 42 patients (63%) completed surveys, including Quick Disabilities of the Arm, Shoulder and Hand and pain scores. Bivariate analysis was performed to compare our groups and to identify factors associated with our outcomes. Results: We found significantly higher rates of distal radioulnar joint (DRUJ) arthritis (P =.033), extensor carpi ulnaris (ECU) pathology (P =.028), and ulnar styloid fractures (P =.028) in patients with TFCC repairs. With increasing age, increasing pathology in the pisotriquetral joint (P =.040), more ulnocarpal abutment (P =.0081), and more degenerative tears (P <.001) were seen in both groups. No demographic characteristics or MRI findings were significantly associated with our outcomes. Conclusions: We observed higher rates of DRUJ arthritis and ECU pathology in patients with TFCC tears undergoing repair compared with age- and sex-matched controls. This may be due to damage to the TFCC itself altering relationships of the DRUJ and the ECU subsheath, or it may reflect various pathologies that cause ulnar-sided wrist pain and drive patients toward surgery. Background: Psychological factors such as depression, pain catastrophizing, kinesiophobia, pain anxiety, and more negative illness perceptions are associated with worse pain and function in patients at the start of treatment for de Quervain's tenosynovitis. Longitudinal studies have found symptoms of depression and pain catastrophizing at baseline were associated with worse pain after treatment. It is important to study patients opting for surgery for their condition because patients should choose surgical treatment based on their values rather than misconceptions. Psychological factors associated with worse patient-reported outcomes from surgery for de Quervain's tenosynovitis should be identified and addressed preoperatively so surgeons can correct any misunderstandings about the condition.Question/purpose: What preoperative psychosocial factors (depression, anxiety, pain catastrophizing, illness perception, and patient expectations) are associated with pain and function 3 months after surgical treatment of de Quervain's tenosynovitis after controlling for demographic characteristics?Methods: This was a prospective cohort study of 164 patients who underwent surgery for de Quervain's tenosynovitis between September 2017 and October 2018 performed by 20 hand surgeons at 18 centers. Our database included 326 patients who underwent surgery for de Quervain's tenosynovitis during the study period. Of these, 62% (201 of 326) completed all baseline questionnaires and 50% (164 of 326) also completed patient-reported outcomes at 3 months postoperatively. We found no difference between those included and those not analyzed in terms of age, sex, duration of symptoms, smoking status, and workload. The mean ± SD age of the patients was 52 ± 14 years, 86% (141 of 164) were women, and the mean duration of symptoms was 13 ± 19 months. Patients completed the Patient-Rated Wrist Evaluation (PRWE), the VAS for pain and function, the Patient Health Questionnaire for symptoms of anxiety and depression, the Pain Catastrophizing Scale, the Credibility/Expectations Questionnaire, and the Brief Illness Perceptions questionnaire at baseline. Patients also completed the PRWE and VAS for pain and function at 3 months postoperatively. We used a hierarchical multivariable linear regression model to investigate the relative contribution of patient demographics and psychosocial factors to the pain and functional outcome at 3 months postoperatively.Results: After adjusting for demographic characteristics, psychosocial factors, and baseline PRWE score, we found that only the patient's expectations of treatment and how long their illness would last were associated with the total PRWE score at 3 months postoperatively. More positive patient expectations of treatment were associated with better patient-reported pain and function at 3 months postoperatively (ß = -2.0; p < 0.01), while more negative patient perceptions of how long their condition would last were associated with worse patient-reported pain and function (timeline ß = 2.7; p < 0.01). The final model accounted for 31% of the variance in the patient-reported outcome at 3 months postoperatively.Conclusion: Patient expectations and illness perceptions are associated with patient-reported pain and functional outcomes after surgical decompression for de Quervain's tenosynovitis. Addressing misconceptions about de Quervain's tenosynovitis in terms of the consequences for patients and how long their symptoms will last should allow patients to make informed decisions about the treatment that best matches their values. Prospective studies are needed to investigate whether addressing patient expectations and illness perceptions, with decision aids for example, can improve patient-reported pain and function postoperatively in those patients who still choose surgery for de Quervain's tenosynovitis.Level Of Evidence: Level III, therapeutic study. Sixty-five patients were invited for a follow-up visit at a mean of 17 years (range 15–20) postoperatively. Twenty-eight patients attended, who had 34 operations, 14 trapeziectomy alone and 20 with LRTI. There were no statistically significant differences between the two groups in terms of satisfaction with surgery or functional outcomes, with most measurements showing minimal or no differences in means between the two groups. There was no difference in the space between the metacarpal and scaphoid. Radial abduction was the only parameter that was significantly greater in the patients with simple trapeziectomy (median 79°) compared with trapeziectomy with LRTI (median 71°) (p = 0.04). Even at 17 years there is no significant benefit of LRTI over trapeziectomy alone for thumb carpometacarpal joint osteoarthritis. Level of evidence: I [ABSTRACT FROM AUTHOR]

In this cohort study, we report on short-term outcomes of 203 patients (203 wrists) who underwent scapholunate ligament reconstruction using a three-ligament tenodesis technique between December 2013 and December 2017. Patients completed the Patient Rated Wrist Evaluation questionnaire and a visual analogue scale for pain online before surgery and at 3 months and 1 year after surgery. We also measured the range of movement before surgery, in 150 patients 3 months after surgery, and in 90 patients 1 year after surgery. The patients reported clinically significant improvements in both pain and function postoperatively, with continuing improvement at both 3 and 12 months. We conclude that the short-term outcomes of this procedure are generally good in term of patients' function, satisfaction, and pain relief, but about 20% of the operated wrists did not improve. Introduction Most total hip arthroplasties (THAs) in the UK are performed through a posterior or lateral surgical approach. We aimed to investigate any difference in outcome from revision THA according to the approach at primary and revision THA surgery. Methods A retrospective cohort study of 205 patients who underwent revision THA for aseptic loosening. Patients rated their pain from 0-10 and completed the Self-Administered Patient Satisfaction Scale (SAPS), Oxford Hip Score (OHS), WOMAC and Short form-12 questionnaires. Results 205 patients (209 hips) from a cohort of 238 patients (243 hips, 86%) were available for analysis. The mean follow-up was 5 years (SD 1.71). Grouping by approach 20% (43/209) had both primary and revision procedures via a lateral approach, 20% (43/209) had their primary surgery via a lateral approach and their revision surgery via a posterior approach, whilst 60% (123/209) had both procedures via a posterior approach. The WOMAC and OHS were significantly better in patients who had a posterior approach for both primary and revision surgery, compared to those that did not (OHS p = 0.028, WOMAC p = 0.026). We found no significant differences in pain, satisfaction or health-related quality of life between the groups. Discussion Choice of approach for revision hip arthroplasty is influenced by a number of factors, but in clinical situations where either a lateral or posterior approach could be used, the posterior approach appears to be associated with better joint-specific outcomes.
