Primary central nervous system lymphoma (PCNSL) is a rare subtype of extranodal non-Hodgkin's lymphoma that accounts for 4% of newly diagnosed central nervous system (CNS) tumors. Most primary lymphomas of the central nervous system are of the subtype of diffuse large B-cell lymphomas, which have highly aggressive behavior and may involve the brain, leptomeninges, eyes or spinal cord without evidence of systemic disease. Primary CNS lymphomas are very rare in immunocompetent patients, but their rates are increasing. So far, only 11 primary Gasser ganglion lymphomas have been reported, with an incidence of 2.5 cases per 30,000,000 inhabitants. However, B cell lymphomas of the marginal zone of the Gasserian ganglion have been very rarely reported. We report here a clinical presentation characteristic of B cell lymphoma of the marginal zone of the Gasser ganglion in an immunocompetent patient who was treated with surgery and radiotherapy, evolving with improvement of symptoms and without recurrence in 3 months of follow-up.

Travelling salesman problem (TSP) is a well known combinatorial optimization problem which has drawn colossal attention due to its eclectic range of applications. In this article, we have proposed two modified versions of intelligent water drops (IWD) algorithm. The first one is the enhanced IWD (e-IWD) algorithm to solve single objective TSP. In the second modification, e-IWD algorithm has been extended to enhanced multi-objective IWD(e-MIWD) algorithm for solving multi-objective TSP. In order to achieve a better exploration capability in both of the proposed algorithms, the soil and velocity parameters of a randomly selected water drop are updated after every iteration of the algorithm when it traverses all the intermediate vertices for a tour. The proposed algorithms have been compared with some other existing similar algorithms on different benchmark instances of TSPs. Furthermore, we have addressed the TSP for both single and multiple objectives under uncertain environment.

For long, the cities depended on grey infrastructure for draining stormwater. However, incidences of pluvial flooding are increasing, and existing grey infrastructure is unable to take up the additional stormwater load. Consequently, planners are forced to think of new and sustainable alternatives for stormwater management. Natural channels can supplement the stormwater drainage systems, but these channels in cities are reclaimed to provide land for housing and other functions despite their crucial role. This study presents the case of a natural channel in Chandigarh (India) that is redeveloped as a greenspace without compromising its function of stormwater conveyance. We analytically discussed the non-intentional preservation of this seasonal natural channel and introduced a new term, greenswales, for similar arrangements. A greenswale is defined as the stretch of greenspaces laid over a natural channel, ephemeral or intermittent, having stormwater detention and conveyance as primary functions during precipitation. This study's significant finding is that the seasonal natural channels in a city can be safeguarded through the judicious superimposition of green spaces over them. Crucial lessons from this case can guide new developments in utilising natural seasonal channels as a nature-based solution for stormwater management, reducing the load on grey infrastructure and providing the city with a greenspace.

We report the case of a 16-year-old boy diagnosed to have Sotos syndrome, with rare association of bilateral primary optic atrophy and epilepsy. He presented with accelerated linear growth, facial gestalt, distinctive facial features, seizures and progressive diminution of vision in both eyes. He had features of gigantism from early childhood. An MRI showed that brain and endocrine functions were normal. This case is of interest, as we have to be aware of this not so rare disorder. In addition to the classic features, there were two unusual associations with Sotos syndrome in the patient.

This study investigated the application of mixed biofilms formed by two Pseudomonas strains (NAA22 and NAA23) for bio-decolorization of malachite green (MG) dye. The isolated strains displayed biofilm formation and MG decolorization capabilities. Mixed biofilms exhibited significantly greater biofilm formation and MG decolorization (94.3%) compared to individual strains, suggesting synergistic interactions. This decolorization efficiency surpassed previously reported values for single strain decolorization. The mixed biofilms tolerated a broad range of temperatures (20-40 °C) and pH (5-9), with optimal decolorization at neutral or slightly acidic conditions (pH7.0). Enzyme analysis revealed laccase, NADH-DCIP reductase, and azoreductase as key contributors to MG decolorization, with significantly higher activity in mixed biofilms. Importantly, the bio-decolorization process transformed MG into non-phytotoxic compounds, demonstrated by seed germination and growth assays. These findings propose a promising and environmentally safe approach for MG bioremediation using mixed Pseudomonas biofilms., Competing Interests: Declarations. Emicizumab is a monoclonal antibody that bridges activated factor IX (FIX) and factor X (FX) to replace the function of missing activated factor VIII (FVIII) in hemophilia A patients irrespective of FVIII inhibitor status. This study assessed the effectiveness of emicizumab in preventing bleeding episodes in patients with hemophilia A. This observational study included patients with moderate to severe hemophilia A who were undergoing episodic FVIII replacement therapy. The primary endpoint was the difference in annualized bleeding rates (ABR) and the secondary endpoint was the difference in Hemophilia Joint Health Score (HJHS) before and after emicizumab prophylaxis. A total of 30 male hemophilia patients were included, the mean age was 16.7 (SD: ±8.1) years, and most of them had moderate hemophilia A [63.3%]. Before prophylaxis, the median ABR was 48 (interquartile range [IQR]: 35-60), and 93.3% of patients had ABR greater than eight, whereas after prophylaxis the median ABR decreased significantly (median [IQR]: 0 [0.0-0.4], p  < 0.001), and 56.7% had zero bleeds. ABR was not significantly different in patient with and without FVIII inhibitors. The HJHS scores significantly improved after prophylaxis (10 vs. 2.5, p  < 0.001). The bleeding events were reduced significantly (23 vs. 0.0, p  < 0.001), and zero new target joints were reported after prophylaxis. Most of the patients [93.3%] did not face any serious adverse events after prophylaxis. Background: Hemophilia is one of the commonest inherited bleeding disorders which may lead to chronic bleeding tendencies and life-long disabilities if not properly managed. Knowing the pattern of the disease aids in the prevention of disability and improvement of quality of life in hemophilia. However, there is a dearth of literature on the issue in Bangladesh. So, this study was designed to explore the frequency and site of spontaneous bleeding in moderate and severe hemophilia patients visiting in a tertiary level hospital., Methods: This descriptive cross-sectional study was conducted at the department of Hematology and Bone Marrow Transplantation (BMT) Center in Dhaka Medical College Hospital, Dhaka between February 2020 and August 2020. A total of 44 diagnosed cases of moderate to severe hemophilia were included in the study according to inclusion criteria. A detailed inquiry of history, thorough physical examination and relevant investigations were done and were recorded in case-record form. Informed written consent was taken from patients or their guardians where appropriate. All procedures were done according to Declaration of Helsinki. After entry and checking, data was analysed using SPSS version 26., Results: Out of 44 participants, 25 (56.8%) and 19 (43.2%) had moderate and severe hemophilia. Mean age of the study population was 21.31 (± 9.78) years with the majority aged between 11 and 20 years (45.5%). All sociodemographic features were similar across severity. Hemophilia A and B was found in 90.9% and 9.1%, respectively. However, all type B patients severe hemophilia making it statistically significantly different from type A (p = 0.029). The median age of first bleeding was 3.5 years and median age of first diagnosis was 5 years. Nevertheless, approximately 67.4% patients were diagnosed as a case of hemophilia at the time of their first diagnosis. The median spontaneous bleedings episodes among all patients was 32 (range: 0-97) which did not different significantly between severe and moderate patients. The most common affected (target) joint was knee joint (88.6%) followed by elbow joint (64%) among all patients. The knee joint was more commonly involved in severe than moderate disease., Conclusion: This study observed the variations in pattern and frequency of spontaneous bleeding in patients with hemophilia. Severe disease was more frequent in hemophilia B than A and knee joint was the most frequent site of bleeding. However, further extensive studies are recommended., (© 2022. The Author(s).)

To evaluate the species diversity and strengthen the taxonomic identification of elasmobranchs in the Bay of Bengal, Bangladesh, a study was conducted in the southeast coastal region between January 2016 and March 2018. Using morphological and genetic identification techniques, this study presents 22 species from the region. Thirteen of these are new records. The new records consist of eight species from the family Dasyatidae, and one each from Mobulidae, Rhinobatidae, Narcinidae, Hemiscylliidae and Triakidae. Furthermore, four occurrences are first verified reports, and five are potential new records requiring further taxonomic investigation.

Trade involving elasmobranch products in Bangladesh is a four-decade-long practice in large scale and there is little understanding of its impact on species composition, population, and subsequent conservation. Capacity for monitoring and identification is lacking in landing and shark processing centres. A rapid survey and collection of tissue samples were performed in three landings and nine shark processing centres between 2016 and 2017 in the south-eastern coastal region of Bangladesh. Sequencing for a 707-bp fragment of the mitochondrial cytochrome c oxidase subunit I (COI) gene was used to assess the taxonomic status and species composition from 71 elasmobranch tissue samples collected from the shark processing centre only. Good quality COI sequences were obtained for 34 specimens representing 21 species-the majority of which are threatened with extinction. A total of ten species of sharks (Carcharhinus brevipinna, C. amboinensis, C. leucas, C. sorrah, C. amblyrhynchoides, Chiloscyllium burmensis, Galeocerdo cuvier, Rhincodon typus, Scoliodon laticaudus, and Sphyrna lewini), eleven species of rays (Aetomylaeus maculatus, Gymnura poecilura, Mobula mobular, M. kuhlii, Neotrygon indica, Pateobatis uarnacoides, Rhinoptera javanica, and R. jayakari), including three species of guitarfish (Glaucostegus granulatus, G. obtusus, and G. typus), were identified. Four species (14.7% of samples) were found to be listed in the Convention on International Trade in Endangered Species of Wild Fauna and Flora (CITES) in Appendix II. Sixteen species (59% of the specimens) were threatened with extinction according to IUCN Red List, whereas 41% were data deficient or not assessed. The results have important implications for the management of regional fisheries and the conservation of elasmobranchs as they 1) represent a preliminary understanding of elasmobranch diversity in trade; 2) depict a lack of awareness and monitoring; and 3) demonstrate a need for urgent monitoring and regulation of elasmobranch trade in Bangladesh., Competing Interests: The authors have declared that no competing interests exist.

Purpose: Treatment of malignant and nonmalignant hematologic diseases with hematopoietic stem-cell transplantation (HSCT) was first described almost 60 years ago, and its use has expanded significantly over the last 20 years. Whereas HSCT has become the standard of care for many patients in developed countries, the significant economic investment, infrastructure, and health care provider training that are required to provide such a service have prohibited it from being widely adopted, particularly in developing countries., Methods: Over the past two decades, however, efforts to bring HSCT to the developing world have increased, and several institutions have described their efforts to establish such a program. We aim to provide an overview of the current challenges and applications of HSCT in developing countries as well as to describe our experience in developing an HSCT program at Dhaka Medical College and Hospital in Bangladesh via a partnership with health care providers at Massachusetts General Hospital., Results and Conclusion: We discuss key steps of the program, including the formation of a collaborative partnership, infrastructure development, human resource capacity building, and financial considerations.

Background: The global burden from cancer is rising, especially as low-income countries like Bangladesh observe rapid aging. So far, there are no comprehensive descriptions reporting diagnosed cancer group that include hematological malignancies in Bangladesh., Methods: This was a multi-center hospital-based retrospective descriptive study of over 5000 confirmed hematological cancer cases in between January 2008 to December 2012. Morphological typing was carried out using the "French American British" classification system., Results: A total of 5013 patients aged between 2 to 90 years had been diagnosed with malignant hematological disorders. A 69.2% were males (n=3468) and 30.8% females (n=1545), with a male to female ratio of 2.2:1. The overall median age at diagnosis was 42 years. Acute myeloid leukemia was most frequent (28.3%) with a median age of 35 years, followed by chronic myeloid leukemia with 18.2% (median age 40 years), non-Hodgkin lymphoma (16.9%; median age 48 years), acute lymphoblastic leukemia (14.1%; median age 27 years), multiple myeloma (10.5%; median age 55 years), myelodysplastic syndromes (4.5%; median age 57 years) and Hodgkin's lymphoma (3.9%; median age 36 years). The least common was chronic lymphocytic leukemia (3.7%; median age 60 years). Below the age of 20 years, acute lymphoblastic leukemia was predominant (37.3%), followed by acute myeloid leukemia (34%). Chronic lymphocytic leukemia and multiple myeloma had mostly occurred among older patients, aged 50-over., Conclusions: For the first time, our study presents the pattern and distribution of diagnosed hematological cancers in Bangladesh. It shows differences in population distributions as compared to other settings with possibly a lower presence of non-Hodgkin lymphoma. There might be under-reporting of affected women. Further studies are necessary on the epidemiology, genetics and potential environmental risk factors within this rapidly aging country.

A 60 year old male presented with soreness and swelling of the oral cavity and throat for 2 weeks as well as fever and prostration. He was diagnosed as a case of acute myeloid leukaemia (AML) on the basis of blood and bone marrow morphology and immunophenotyping. Normal platelet count and normal bone marrow megakaryocytes were striking features as well as low total count of WBC and myeloblast. Being informed about the risk and benefit of the treatment with chemotherapy, he opted for taking palliative treatment rather than conventional chemotherapy for remission induction. Since then the patient was doing surprisingly well and he was alive for 38 months with few episodes of clinical deterioration which was successfully managed by palliative measures.

Manganese (Mn) intoxication is most often regarded as an occupational manifestation and occurs in places such as manganese mines, dry cell battery plants and ceramic industries. In the present study, the influence of genetic polymorphism in cytochrome P450 2D6 (CYP2D6∗2), glutathione S-transferase M1 (GSTM1) and NAD(P)H quinone oxidoreductase 1 (NQO1) genes on blood manganese and plasma prolactin concentrations in manganese miners was investigated. Genotyping of CYP2D6∗2 C→T 2850 and NQO1 C→T 609 was carried out using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) whereas the genotyping of GSTM1 was carried out by multiplex PCR using beta globin as an internal control. Manganese miners with CYP2D6∗2 C→T 2850 variant genotype had relatively low Mn concentration [GM: 21.4±8.9 μg L(-1)] than the subjects with wild (GM: 36.3±8.5 μg L(-1)) and heterozygous (GM: 34.4±6.9 μg L(-1)) genotypes. Miners with CYP2D6∗2 variant genotypes showed low prolactin levels (GM: 13.13±1.6 ng mL(-1)) compared to the wild (GM: 16.4.4±1.5 μg L(-1)) and heterozygous (GM: 18.7±1.6 ng mL(-1)) genotypes. Gene-gene interaction studies also revealed that the subjects with CYP2D6∗2 C→T 2850 variant genotypes had low levels of Mn and prolactin. Our new findings suggest that CYP2D6∗2 C→T 2850 variant genotypes can regulate plasma prolactin levels in manganese miners of Central India and could be involved in the fast metabolism of blood manganese, compared to wild and heterozygous genotypes., (Copyright © 2010 Elsevier Ltd. Objective: To assess the oxidative stress induced due to heavy metal exposure. Exposed populations are selected from an engine tuning station and control from the same area with no occupational exposure., Method: Standard methods were followed for enzymatic assay, and heavy metals in blood and urine were analyzed by using inductively coupled plasma-optical emission spectrophotometer after microwave digestion., Result: Changes in mean blood Pb, Cd, and Ni concentrations in blood and urine of exposed population of all age groups (20 to 35, 35 to 45, and 46 to 58 years) and exposure durations (< or =10, 11 to 20, and >20 years) were statistically not significant. However, exposed workers exhibited statistically significant higher antioxidant status in terms of serum glutathione-S-transferase activity, malondialdehyde level, and catalase activity., Conclusion: The findings in this article suggest that occupational exposure to diesel exhaust of engine tuning workers causes induction of oxidative stress, which cannot be correlated with the heavy metals status in blood and urine of an exposed population.

The uterotropic effect of clofibrate and phenylbutazone was studied in immature female rats. A significant increase in the uterine wet weight was observed following clofibrate and phenylbutazone administration. Clofibrate but not phenylbutazone synergized with the uterotropic effect of ethinyl-oestradiol. Phenylbutazone pretreatment significantly decreased the pentobarbitone sleeping time. The uterotropic effect of clofibrate and phenylbutazone might involve displacement of 17-beta oestradiol (or ethinyl oestradiol) from plasma albumin binding sites in rats.
