Context and objective. Drug-resistant tuberculosis (DR-TB) has a major impact on patient treatment outcomes. The strategies put in place for its control involve early diagnosis and appropriate treatment of patients. The objective of the present study was to determine the phenotypic and genotypic profile of Mycobacterium tuberculosis strains from family contacts and their index cases in Kinshasa, Democratic Republic of Congo (DRC). Methods. This was a descriptive cross-sectional study that took place in Kinshasa (DRC) from July 2011 to August 2017. It consisted in analyzing strains of M. tuberculosis from family contact cases and their index cases. The strains were isolated on the Löwenstein-Jensens (LJ) medium, the antibiogram was carried out by the technique of proportions and the genotyping by Spolygotyping. Results. Thirty strains of M. tuberculosis (34.1%) out of 88 isolated from Multidrug-resistant TB (MDR-TB) contacts and their index cases were analyzed. The median age of patients was 31 ± 13.55 years with extremes ranging from 4 to 78 years. Two strains from a contact case had a different genotypic profile from the index case whereas 13 showed a profile identical to that of the index case. A diversity of lineages was noted. Regarding the phenotypic profile, a strain of the case-contact showed a different profile from the index case, notably the Extensively drug-resistant tuberculosis (XDR-TB) and the pre-XDR-TB. Conclusion The survey showed the circulation of complicated forms of TB with a diversity of genotypic strains in contacts as well in index cases. French Abstract Contexte & objectif. L’émergence de la tuberculose pharmacorésistante (TB-PR) est un véritable défi pour la lutte contre la tuberculose. Les stratégies mises en place pour son contrôle impliquent un diagnostic précoce et le traitement adapté des patients. La présente étude avait pour objectif de déterminer le profil phénotypique et génotypique des souches de Mycobacterium tuberculosis (M.t) issus des contacts familiaux et leurs cas-index. Méthodes. Il s’agissait d’une étude transversale à visée descriptive qui s’est déroulée à Kinshasa (RDC) de juillet 2011 à août 2017. Elle a consisté à analyser des souches de M.t provenant des cas-contacts familiaux en comparaison aux cas-index. Les souches ont été isolées sur le milieu de Löwenstein-Jensens (LJ), l’antibiogramme était réalisé par la technique des proportions et le génotypage par Spolygotyping. Résultats. Trente souches de M.t (34,1%) sur 88 isolées de contacts TB-MR et leurs cas-index ont été analysées. L’âge médian des patients était de 31±13,55 ans (extrême 4 et 78 ans). Deux souches provenant d’un cas-contact avaient un profil génotypique différent du cas-index. Tandis que 13 ont montré un profil identique à celui du cas-index et on a noté une diversité des lignées. S’agissant du profil phénotypique, une seule souche du cas-contact était différente du cas-index (XDRTB et preXDRTB). Conclusion L’enquête a montré la circulation de formes compliquées de la TB avec une diversité des souches génotypiques chez les contacts familiaux comme chez les cas-index.
