Abstract

Abstract

BACKGROUND: Patients with systolic heart failure and anemia have worse symptoms, functional capacity, and outcomes than those without anemia. We evaluated the effects of darbepoetin alfa on clinical outcomes in patients with systolic heart failure and anemia. METHODS: In this randomized, double-blind trial, we assigned 2278 patients with systolic heart failure and mild-to-moderate anemia (hemoglobin level, 9.0 to 12.0 g per deciliter) to receive either darbepoetin alfa (to achieve a hemoglobin target of 13 g per deciliter) or placebo. The primary outcome was a composite of death from any cause or hospitalization for worsening heart failure. RESULTS: The primary outcome occurred in 576 of 1136 patients (50.7%) in the darbepoetin alfa group and 565 of 1142 patients (49.5%) in the placebo group (hazard ratio in the darbepoetin alfa group, 1.01; 95% confidence interval, 0.90 to 1.13; P=0.87). There was no significant between-group difference in any of the secondary outcomes. The neutral effect of darbepoetin alfa was consistent across all prespecified subgroups. Fatal or nonfatal stroke occurred in 42 patients (3.7%) in the darbepoetin alfa group and 31 patients (2.7%) in the placebo group (P=0.23). Thromboembolic adverse events were reported in 153 patients (13.5%) in the darbepoetin alfa group and 114 patients (10.0%) in the placebo group (P=0.01). Cancer-related adverse events were similar in the two study groups. CONCLUSIONS: Treatment with darbepoetin alfa did not improve clinical outcomes in patients with systolic heart failure and mild-to-moderate anemia. Abstract

CEBAF has developed a comprehensive conceptual design of an industrial user facility based on a kilowatt ultraviolet (UV) (160-1000 mm) and infrared (IR) (2-25 micron) free electron laser (FEL) driven by a recirculating, energy recovering 200 MeV superconducting radio frequency (SRF) accelerator. FEL users, CEBAF's partners in the Lase Processing Consortium, including AT&T, DuPont, IBM, Northrop Grumman, 3M, and Xerox, are developing applications such as metal, ceramic, and electronic material micro-fabrication and polymer and metal surface processing, with the overall effort leading to later scale-up to industrial systems at 50-100 kW. Representative applications are described. The proposed high-average-power FEL overcomes limitations of conventional laser sources in available power, cost-effectiveness, tunability, and pulse structure.

Abstract

Previously we presented a design for a kiloWatt demonstration industrial UV FEL. Progress has been made in resolving several design issues identified in that work. More exact simulations of the injector have resulted in a better estimate of the injector performance. A more compact lattice has been designed meeting the design requirements for the UV FEL, and a new design point has been studied which greatly increases the threshold for longitudinal instabilities. A stability analysis of the RF control system has found that only minor modifications from existing CEBAF controls will be necessary to allow them to be used with a high-current, energy-recovery accelerator. Designs for the optical cavity length and figure systems have been conceptualized and a model of the corner-cube resonator is being built and tested. Finally, three-dimensional simulations of the FEL have been carried out which show that the laser should exceed its minimum design goals for average power.

Abstract

The Synchrotron Radiation Center (SRC) operates the Aladdin electron storage ring at energies of 800 MeV or 1 GeV in support of a broad range of national and international research programs. A low emittance configuration is in routine operation during 800-MeV shifts and offers improved photon flux density with about the same beam lifetime. An improved undulator compensation algorithm and new optical beam position monitors have been implemented improving beam stability and maintaining vertical beam size variations to < 2% peak-to-peak during undulator scanning. Instrumentation initiatives include construction of a modified Wadsworth beamline (7.8 – 50 eV) and a variable-line-spacing plane-grating monochromator (VLS-PGM, 75 – 2000 eV) to utilize radiation from a permanent magnet undulator. The Wadsworth beamline is being commissioned for photoelectron spectroscopy (PES) experiments using high-resolution Scienta analyzers. The VLS-PGM is being constructed for experiments that require higher photon energies and high flux density such as x-ray photoemission electron microscopy (X-PEEM) and x-ray absorption spectroscopy (XAS). It is scheduled to be available in early 2004. Recent research at the SRC has produced exciting results in a variety of fields, culminating in eight articles published in Physical Review Letters and three in Nature since October 2002, in addition to articles in many other publications. An outreach program offers research experiences for undergraduates and provides the general public with an awareness of synchrotron radiation. Hands-on workshops and activities on FTIR microscopy and X-PEEM are offered for graduate students and scientists. SRC sponsors a summer Research Experience for Undergraduates (REU) program and offers opportunities to non-research universities and high schools. Tours and educational events are coordinated with local civic groups and schools.
