Abstract

Elevated atmospheric nitrogen (N) deposition can alter forest biogeochemistry leading to adverse impacts on terrestrial ecosystems. Experimental studies often apply N to the forest understorey at greatly elevated loads, bypassing important canopy interactions. Using a narrow N deposition gradient (0, 5, 10, 15, 20, and 25 kg N * [ha.sup.-1] * [year.sup.-1]), we measured changes in jack pine (Pinus banksiana Lamb.) forest biogeochemistry in the bituminous sands region of northern Alberta, Canada after 5 years of above-canopy N additions. The canopy intercepted approximately 46% of applied N across all treatments, but during the final year, N interception was reduced to 7% in the highest treatment. Nitrogen concentrations in nonvascular organisms increased with treatment and N was also immobilized in decomposing litter in the highest treatment. Otherwise, vascular plant biomass, foliar chemistry, and soil processes exhibited no relationship with treatment over 5 years. This work suggests that jack pine forests in the region have a high capacity to immobilize N inputs over the short-term (5 years), which restricts other biogeochemical responses traditionally associated with elevated N deposition. Les etudes experimentales appliquent souvent de l'azote au sous-etage des forets a des charges tres elevees, sans tenir compte des interactions importantes avec la canopee. En utilisant un gradient etroit de depot d'azote (0, 5, 10,15, 20 et 25 kgN * [ha.sup.-1]an-1), nousavons mesure les changements dans la biogeochimie de laforetde pingris (Pinus banksiana Lamb.) dans la region des sables bitumineux du nord de l'Alberta, au Canada, apres cinq ans d'ajouts d'azote au-dessus de la canopee. La canopee a intercepte environ 46 % de l'azote applique dans tous les traitements, mais au cours de la derniere annee, l'interception de l'azote a ete reduite a 7 % dans le traitement le plus eleve. Les concentrations d'azote dans les organismes non vasculaires ont augmente avec le traitement et l'azote etait egalement immobilise dans la litiere en decomposition dans le traitement le plus eleve. Autrement, la biomasse des plantes vasculaires, la chimie foliaire et les processus du sol n'ont montre aucune relation avec le traitement sur 5 ans. Ce travail suggere que les forets de pins gris de la region ont une grande capacite a immobiliser les apports d'azote a court terme (5 ans), ce qui limite les autres reponses biogeochimiques traditionnellement associees aux depots eleves d'azote. 10. Allied Health Student Engagement in Telehealth Clinical Placements: A Rapid Review

Abstract

Purpose: The engagement of allied health students in telehealth consultations whilst on clinical placement has rapidly increased due to the disruption caused by COVID-19. While this is understandable given the circumstances, it has occurred on the largely unfounded assumption that utilising telehealth is pedagogically appropriate to supplement or replace assessed placements. Method: This rapid review used the Cochrane rapid review methodology to synthesise the literature relating to the perspectives of allied health students whilst on clinical placements utilising telehealth. Results: A systematic search and selection process found three studies. In summary, the combined findings of the research suggest that student perspectives were generally positive, and several important benefits were reported, including being able to improve knowledge and skills, and reduce anxiety for some consultations. However, students acknowledged that some consultations were considerably more difficult to undertake without being in the same physical space as clients. Conclusions: The methodological quality of these three papers was inconsistent, and collectively the area clearly needs more evidence to support the transition of face-to-face to telehealth environments. Recommendations: Based on broader telehealth literature, the authors propose several recommendations to meet the immediate challenge of insufficient guiding research evidence. These include deconstructing telehealth placements into discrete clinical placement elements, using education theory to support student clinicians to implement a proactive, integrated approach to adopting telehealth as a standard service modality, and investing in and developing infrastructure, policy, systems, education, and training in an explicit, systematic manner. The use of telehealth within allied health clinical placements is here to stay, but considerable work is needed to prepare students for this environment as well as triaging and progressively scaffolding their experiences. 16. Implementing Shared Memory Parallelism in MCBEND

Abstract

MCBEND is a general purpose radiation transport Monte Carlo code from AMEC Foster Wheelers’s ANSWERS® Software Service. MCBEND is well established in the UK shielding community for radiation shielding and dosimetry assessments. The existing MCBEND parallel capability effectively involves running the same calculation on many processors. This works very well except when the memory requirements of a model restrict the number of instances of a calculation that will fit on a machine. To more effectively utilise parallel hardware OpenMP has been used to implement shared memory parallelism in MCBEND. This paper describes the reasoning behind the choice of OpenMP, notes some of the challenges of multi-threading an established code such as MCBEND and assesses the performance of the parallel method implemented in MCBEND.

Abstract

RANKERN 16 is the latest version of the point-kernel gamma radiation transport Monte Carlo code from AMEC Foster Wheeler’s ANSWERS Software Service. RANKERN is well established in the UK shielding community for radiation shielding and dosimetry assessments. Many important developments have been made available to users in this latest release of RANKERN. The existing general 3D geometry capability has been extended to include import of CAD files in the IGES format providing efficient full CAD modelling capability without geometric approximation. Import of tetrahedral mesh and polygon surface formats has also been provided. An efficient voxel geometry type has been added suitable for representing CT data. There have been numerous input syntax enhancements and an extended actinide gamma source library. This paper describes some of the new features and compares the performance of the new geometry capabilities.

19. The effect of different depths of medial heel skive on plantar pressures

Abstract

Background: Foot orthoses are often used to treat lower limb injuries associated with excessive pronation. There are many orthotic modifications available for this purpose, with one being the medial heel skive. However, empirical evidence for the mechanical effects of the medial heel skive modification is limited. This study aimed to evaluate the effect that different depths of medial heel skive have on plantar pressures.Methods: Thirty healthy adults (mean age 24 years, range 18-46) with a flat-arched or pronated foot posture and no current foot pain or deformity participated in this study. Using the in-shoe pedar-X® system, plantar pressure data were collected for the rearfoot, midfoot and forefoot while participants walked along an 8 metre walkway wearing a standardised shoe. Experimental conditions included a customised foot orthosis with the following 4 orthotic modifications: (i) no medial heel skive, (ii) a 2 mm medial heel skive, (iii) a 4 mm medial heel skive and (iv) a 6 mm medial heel skive.Results: Compared to the foot orthosis with no medial heel skive, statistically significant increases in peak pressure were observed at the medial rearfoot - there was a 15% increase (p = 0.001) with the 4 mm skive and a 29% increase (p < 0.001) with the 6 mm skive. No significant change was observed with the 2 mm medial heel skive. With respect to the midfoot and forefoot, there were no significant differences between the orthoses.Conclusions: This study found that a medial heel skive of 4 mm or 6 mm increases peak pressure under the medial rearfoot in asymptomatic adults with a flat-arched or pronated foot posture. Plantar pressures at the midfoot and forefoot were not altered by a medial heel skive of 2, 4 or 6 mm. These findings provide some evidence for the effects of the medial heel skive orthotic modification. © 2012 Bonanno et al.; licensee BioMed Central Ltd.

Abstract

Abstract Background Foot orthoses are often used to treat lower limb injuries associated with excessive pronation. There are many orthotic modifications available for this purpose, with one being the medial heel skive. However, empirical evidence for the mechanical effects of the medial heel skive modification is limited. This study aimed to evaluate the effect that different depths of medial heel skive have on plantar pressures. Methods Thirty healthy adults (mean age 24 years, range 18–46) with a flat-arched or pronated foot posture and no current foot pain or deformity participated in this study. Using the in-shoe pedar-X® system, plantar pressure data were collected for the rearfoot, midfoot and forefoot while participants walked along an 8 metre walkway wearing a standardised shoe. Experimental conditions included a customised foot orthosis with the following 4 orthotic modifications: (i) no medial heel skive, (ii) a 2 mm medial heel skive, (iii) a 4 mm medial heel skive and (iv) a 6 mm medial heel skive. Results Compared to the foot orthosis with no medial heel skive, statistically significant increases in peak pressure were observed at the medial rearfoot – there was a 15% increase (p = 0.001) with the 4 mm skive and a 29% increase (p Conclusions This study found that a medial heel skive of 4 mm or 6 mm increases peak pressure under the medial rearfoot in asymptomatic adults with a flat-arched or pronated foot posture. Plantar pressures at the midfoot and forefoot were not altered by a medial heel skive of 2, 4 or 6 mm. These findings provide some evidence for the effects of the medial heel skive orthotic modification.

Abstract

Abstract Background Plantar fasciitis is the most commonly reported cause of chronic pain beneath the heel. Management of this condition commonly involves the use of corticosteroid injection in cases where less invasive treatments have failed. However, despite widespread use, only two randomised trials have tested the effect of this treatment in comparison to placebo. These trials currently offer the best available evidence by which to guide clinical practice, though both were limited by methodological issues such as insufficient statistical power. Therefore, the aim of this randomised trial is to compare the effect of ultrasound-guided corticosteroid injection versus placebo for treatment of plantar fasciitis. Methods The trial will be conducted at the La Trobe University Podiatry Clinic and will recruit 80 community-dwelling participants. Diagnostic ultrasound will be used to diagnose plantar fasciitis and participants will be required to meet a range of selection criteria. Participants will be randomly allocated to one of two treatment arms: (i) ultrasound-guided injection of the plantar fascia with 1 mL of 4 mg/mL dexamethasone sodium phosphate (experimental group), or (ii) ultrasound-guided injection of the plantar fascia with 1 mL normal saline (control group). Blinding will be applied to participants and the investigator performing procedures, measuring outcomes and analysing data. Primary outcomes will be pain measured by the Foot Health Status Questionnaire and plantar fascia thickness measured by ultrasound at 4, 8 and 12 weeks. All data analyses will be conducted on an intention-to-treat basis. Conclusion This will be a randomised trial investigating the effect of dexamethasone injection on pre-specified treatment outcomes in people with plantar fasciitis. Within the parameters of this protocol, the trial findings will be used to make evidence-based recommendations regarding the use of corticosteroid injection for treatment of this condition. 31. Diagnostic imaging for chronic plantar heel pain: a systematic review and meta-analysis

Abstract

Abstract Background Chronic plantar heel pain (CPHP) is a generalised term used to describe a range of undifferentiated conditions affecting the plantar heel. Plantar fasciitis is reported as the most common cause and the terms are frequently used interchangeably in the literature. Diagnostic imaging has been used by many researchers and practitioners to investigate the involvement of specific anatomical structures in CPHP. These observations help to explain the underlying pathology of the disorder, and are of benefit in forming an accurate diagnosis and targeted treatment plan. The purpose of this systematic review was to investigate the diagnostic imaging features associated with CPHP, and evaluate study findings by meta-analysis where appropriate. Methods Bibliographic databases including Medline, Embase, CINAHL, SportDiscus and The Cochrane Library were searched electronically on March 25, 2009. Eligible articles were required to report imaging findings in participants with CPHP unrelated to inflammatory arthritis, and to compare these findings with a control group. Methodological quality was evaluated by use of the Quality Index as described by Downs and Black. Meta-analysis of study data was conducted where appropriate. Results Plantar fascia thickness as measured by ultrasonography was the most widely reported imaging feature. Meta-analysis revealed that the plantar fascia of CPHP participants was 2.16 mm thicker than control participants (95% CI = 1.60 to 2.71 mm, P < 0.001) and that CPHP participants were more likely to have plantar fascia thickness values greater than 4.0 mm (OR = 105.11, 95% CI = 3.09 to 3577.28, P = 0.01). CPHP participants were also more likely to show radiographic evidence of subcalcaneal spur than control participants (OR = 8.52, 95% CI = 4.08 to 17.77, P < 0.001). Conclusion This systematic review has identified 23 studies investigating the diagnostic imaging appearance of the plantar fascia and inferior calcaneum in people with CPHP. Analysis of these studies found that people with CPHP are likely to have a thickened plantar fascia with associated fluid collection, and that thickness values >4.0 mm are diagnostic of plantar fasciitis. Additionally, subcalcaneal spur formation is strongly associated with pain beneath the heel.

Abstract

MCBEND 11 is the latest version of the general radiation transport Monte Carlo code from AMEC Foster Wheeler’s ANSWERS® Software Service. MCBEND is well established in the UK shielding community for radiation shielding and dosimetry assessments. MCBEND supports a number of acceleration techniques, for example the use of an importance map in conjunction with Splitting/Russian Roulette. MCBEND has a well established automated tool to generate this importance map, commonly referred to as the MAGIC module using a diffusion adjoint solution. This method is fully integrated with the MCBEND geometry and material specification, and can easily be run as part of a normal MCBEND calculation. An often overlooked feature of MCBEND is the ability to use this method for forward scoping calculations, which can be run as a very quick deterministic method. Additionally, the development of the Visual Workshop environment for results display provides new capabilities for the use of the forward calculation as a productivity tool. In this paper, we illustrate the use of the combination of the old and new in order to provide an enhanced analysis capability. We also explore the use of more advanced deterministic methods for scoping calculations used in conjunction with MCBEND, with a view to providing a suite of methods to accompany the main Monte Carlo solver.
