Abstract

Context Drug induced pancreatitis are rare but potentially serious. Thus, drug withdrawal is warranted. Case report A 79-year-old woman who was treated with antituberculosis therapy for 5 weeks was admitted to our unit for pancreatitis. Usual etiologies of pancreatitis were eliminated. Because of vomiting, antituberculosis therapy was withdrawn and symptoms disappeared. Eight days later, the same treatment was reintroduced and the patient presented recurrent pancreatitis; thus, treatment was withheld again followed by disappearance of clinical and biological abnormalities. Two days later, a treatment without isoniazid was reintroduced and no recurrence of symptoms was observed. Conclusions We have experienced a case of isoniazid induced pancreatitis. This is a rare cause of pancreatitis but potentially fatal thus recognition of drug induced pancreatitis and definitive withdrawal of the drug is required., JOP. 19. Acute Pancreatitis in the Course of Meprobamate Poisoning

Abstract

Context We report a case of massive poisoning with meprobamate leading to acute pancreatitis. Case report A 43-year-old patient with a history of schizophrenia and multiple suicide attempts was admitted to the intensive care unit for severe poisoning with meprobamate (voluntary ingestion of 60 g). On admission, the patient was deeply comatose with low blood pressure and hypothermia. Laboratory analysis revealed leukocytosis and high lipase and amylase serum levels. There was no eosinophilia. Abdominal computed tomography showed pancreatitis grade A. The patient was intubated and ventilated, and intravenous dopamine was infused. The patient regained consciousness and was extubated five days later. Improvement in pancreatic tests was noted several days later. The outcome was favorable. Discussion According to the Naranjo probability scale, meprobamate-induced acute pancreatitis was probable. Acute pancreatitis in meprobamate poisoning is exceptional. The pathogenesis of pancreatitis-induced meprobamate poisoning may be explained by two mechanisms: stimulation of pancreatic secretion secondary to cholinergic activation and pancreatic ductal hypertension. Conclusions The signs of severe meprobamate toxicity are numerous including cardiovascular and central nervous symptoms. The article presents a case study that illustrates two cases of reversible Fanconi syndrome induced by lenalidomide. It highlights the case of a 71-year-old female and 52-year-old female with their respective diagnosis based on their laboratory examinations. It outlines the result which indicates that the disease is a rare event and should be watched for ionic abnormalities that can reveal potential complications.

40. β and αAdrenoceptor Antagonism by Medroxalol in Healthy Volunteers

Abstract

We studied β- and α-adrenoceptor blockades by medroxalol in eight normal, male volunteers using exercise and isoproterenol tests for β-antagonism and a phenylephrine test for α-antagonism. Each volunteer received three oral doses (400–1,200 mg) and two intravenous doses (range 0.5–1.5 mg/kg) at 1− to 6-week intervals. Venous blood was sampled for determination of the medroxalol plasma concentration (Cp) at fixed intervals up to 25 h after each dose. Ergometric exercise and adrenergic agonist injections (isoproterenol) or infusions (phenylephrine) were repeated at 1, 4, and 24 h after medroxalol and results compared to pretreatment control values. In addition, isoproterenol and phenylephrine tests were performed 5 min after the intravenous infusion of medroxalol. After oral administration, peak Cp of medroxalol occurred between 2 and 3 h and the mean t1/2elimination was 11.1 ± 0.6 h. Heart rate at 75 maximum load (HR 75) decreased significantly after the three oral doses with a peak dose-dependent effect at 4 h. At this time, the dose of isoproterenol which raised heart rate by 25 beats/minute (CD 25) was increased 27 times by 400 mg of medroxalol, 51 times by 800 mg, and 94 times by 1,200 mg. The dose of phenylephrine necessary to increase mean blood pressure by 15 mm Hg (PD 15) increased 1.3-fold and 2.4-fold after 400 mg and 1,200 mg of medroxalol, respectively. β-Blockade remained significant for at least 24 h after medroxalol. Reduction in HR 75 and increase in CD 25 were both significantly correlated with log Cp of medroxalol, whereas the increase in PD 15 did not correlate with log Cp. After intravenous administration of medroxalol. β-blockade was maximal at the end of infusion and was proportional to log Cp. but there was no significant change in PD 15. Medroxalol administration was well tolerated, and minor gastrointestinal disturbances appeared 1 - 2 h only after the 1,200 mg oral dose in five out of seven subjects. We conclude that medroxalol (a) has a long plasma elimination half-life, (b) exerts a marked and prolonged β-adrenoceptor blockade, and (c) has significant but less marked α-adrenoceptor blocking properties, which appear only after oral administration of the drug.

Abstract

Selective inhibitors of cyclooxygenase-2 have less gastroduodenal toxicity than non selective anti-inflammatory drugs. However, there is little information on their effect on the distal gut. A 91 year old woman presented with acute diarrhoea 5 weeks after beginning celecoxib treatment. Laboratory results showed an inflammatory syndrome and increased alanine aminotransferase (ALT) to 13 N. Endoscopic examination of the colon showed diffuse erythematous lesions of the sigmoid and of part of the right colon. No aetiology has been found for colitis or hepatitis. Diarrhea and blood test anomalies disappeared one week after celecoxib was stopped. The role of celecoxib in the etiology of colitis was considered plausible but not for liver damage. This report and a few other cases in the literature suggest that cyclooxygenase-2 selective non-steroidal anti-inflammatory drug inhibitor toxicity should be investigated in case of unexplained acute colitis.
