1. Update on Salivary Gland Fine-Needle Aspiration and the Milan System for Reporting Salivary Gland Cytopathology

Abstract

* Context.-Fine-needle aspiration (FNA) is a well-established procedure for the diagnosis and management of salivary gland lesions, despite challenges imposed by salivary gland tumor diversity, complexity, and cytomorphologic overlap. Until recently, the reporting of salivary gland FNA specimens was inconsistent among different institutions throughout the world, leading to diagnostic confusion among pathologists and clinicians. In 2015, an international group of pathologists initiated the development of an evidence-based tiered classification system for reporting salivary gland FNA specimens, the Milan System for Reporting Salivary Gland Cytopathology (MSRSGC). The MSRSGC consists of 6 diagnostic categories, which incorporate the morphologic heterogeneity and overlap among various nonneoplastic, benign, and malignant lesions of the salivary glands. In addition, each MSRSGC diagnostic category is associated with a risk of malignancy and management recommendations. Objective.-To review the current status of salivary gland FNA, core needle biopsies, ancillary studies, and the beneficial role of the MSRSGC in providing a framework for reporting salivary gland lesions and guiding clinical management. Data Sources.-Literature review and personal institutional experience. Conclusions.-The main goal of the MSRSGC is to improve communication between cytopathologists and treating clinicians, while also facilitating cytologic-histologic correlation, quality improvement, and research. Since its implementation, the MSRSGC has gained international acceptance as a tool to improve reporting standards and consistency in this complex diagnostic area, and it has been endorsed by the 2021 American Society of Clinical Oncology management guidelines for salivary gland cancer. The large amount of data from published studies using MSRSGC served as a basis for the recent update of the MSRSGC. As per the data [...], Background: In 2016, the International Academy of Cytology (IAC) established a reporting system for breast fine-needle aspiration (FNA) cytology. This system proposed five categories in its classification: category for insufficient; category 2 for benign; category 3 for atypical, probably benign; category 4 indicating suspicious for malignancy; and category 6 for malignant. Aims and Objectives: The aim of the present study was to review and categorize the breast FNA samples according to the new system of reporting, to assess the risk of malignancy (ROM) for each category, and to assess the diagnostic yield of breast FNA biopsy (FNAB). Materials and Methods: The present retrospective study was conducted in the department of pathology, IGMC, Shimla from January 2019 to December 2019. All the samples during this period were included in the study. Results: A total of 405 breast cytology cases were included in the present study. The FNA findings were correlated with histopathology in 82 cases. The ROM for each category was 16.6 for insufficient, 3.7 for benign, 0 for atypical for malignancy, 100 for suspicious for malignancy, and 92.3 for malignant category. Conclusion: The IAC Yokohama system for reporting breast FNA cytology helps in standardizing reports and easier communication of the pathologists with treating surgeons. It offers a ROM for each category which helps during follow-up of patients. Utilizing the system in day-to-day reporting will improve diagnostic clarity and management. Background: Fine-needle aspiration cytology (FNAC) for salivary gland tumors can be challenging to due to the diversity of lesions and cytomorphological convergence between the tumors. The Milan System for Reporting Salivary Gland Cytopathology (MSRSGC) was hence developed and introduced to provide enhanced communication in salivary gland cytopathology. To evaluate the diagnostic utility and validity of MSRSGC in combination with ancillary radiological investigations, we aim to find correlation between Milan system and the radiological impression comparing with final histopathological diagnosis. Aims and Objective: Correlate the Milan category with the radiological and final histopathological diagnosis of salivary gland lesions. Review the FNAC diagnosis of salivary gland lesions and identify cytomorphological predictors of malignancy. Correlate the FNAC and the radiological diagnosis with final diagnosis in histopathology. Material and Methods: A five year retrospective study, comprising fifty four cases of salivary gland FNAC were included in the study. Results: Majority of the cases belong to Milan VI--Malignant followed by Milan IVa--Benign and rest of cases were among other categories. The sensitivity, specificity, positive predictive value, and negative predictive value of adjuvant radiological diagnosis in differentiating benign and malignant salivary gland lesions was found to be 80, 62.5, 72.7, and 71.4, respectively. We could observe that the concurrent radiological assessment along with Milan system of reporting in salivary gland FNAC especially under suspicious categories (Milan Category IVb as well as Milan Category V) is a useful and sensitive predictor of malignancy. Conclusion: A correlation with any form of ancillary radiological assessment is a helpful adjuvant with Milan system to derive a relatable diagnosis in salivary gland neoplasm especially those in categories describing the suspicious entities. Introduction: The Milan System for Reporting Salivary Gland Cytopathology (MSRSGC) was proposed by American Society of Cytopathology and International Academy of Cytology to standardize reporting system ensuring uniformity and better communication between clinicians and cytopathologists. Aims and Objectives: 1. To categorize salivary lesions as per MSRSGC. 2. To establish diagnostic accuracy of MSRSGC and calculate risk of malignancy for each diagnostic category. Materials and Methods: An ambispective study was conducted over 5 years including 45 FNAC cases of salivary lesions. The cases were categorized according to Milan System, and diagnostic accuracy of MSRSGC was established. Risk of malignancy for each category was calculated. Results: The patients' age ranged from 13 to 77 years with maximum swellings in parotid (53.3) followed by submandibular (15.6) and submental (2.2). Histopathological follow-up was done in all cases. The number of cases in each category was as follows: category I: 03 (6.7), category II: 06 (13.3), category III: 4 (8.9), category IVA: 25 (55.6), category IVB: 04 (8.9), category V: 02 (4.4), and category VI: 01 (2.2). The calculated ROM was as follows: category I: 0, category II: 0, category III: 0 category IVA: 8.0, category IVB: 25, category V: 50, and category VI: 100. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy with (without) indeterminate categories for malignancy were 88.9(96.9), 66.7(50.0), 94.1(94.1), 50.0(66.7), and 85.7(91.9), respectively. Conclusion: MSRSGC is an effective and standardized reporting system for categorization and risk stratification of salivary swellings which helps in enhancing patient care. Introduction: Ultrasound- and CT-guided fine needle aspiration cytology (FNAC) increases the accessibility of intra-abdominal masses to the liver and gall bladder with the advantages of low cost and high diagnostic yield. Cell block technique has been known for further increasing the diagnostic accuracy. Aims and Objectives: We aimed to study the effectiveness of FNAC and the cell block method in cytological diagnosis of liver and gall bladder masses. We also followed a step-wise approach to increase the success rate. Materials and Methods: A 2-year observational study was done from July 2020 to June 2022. Total 80 guided (CT and ultrasound) aspirations were done from space occupying/mass lesions in the liver [74 (92.5)] and gall bladder [6 (7.5)], out of which cell blocks by the plasma thrombin method were prepared in 12 cases (15). The on-site radiological details were noted, and rapid on-site evaluation was done in 65 cases (81.25). The prepared cytology slides were stained with Papanicolaou, H and E and May-Grunwald Giemsa (MGG) stain. The cytological diagnosis was noted, and the uses and limitations (if any) were observed in each case. A step-wise structured questionnaire format was developed to assist the reporting pathologist so as not to miss out on important diagnostic observations, if present. Results: FNAC in 71 cases (88.7) gave a conclusive diagnosis. The maximum number of cases were of adenocarcinoma [38 (51.3)] from the liver followed by hepatocellular carcinoma in 10 cases (13.5). In gall bladder masses, all 6 cases (100) were positive for malignancy, out of which 4 cases (66.7) could be characterized as adenocarcinoma. The cell block preparation was helpful in reaching the diagnosis as well as typing the malignancy in 10 cases (83.3). The chief limitation observed on conventional cytology smears was inadequate cellularity, which caused inconclusive diagnosis in 9 cases (11.25). The reporting questionnaire was helpful chiefly in terms of time-efficient reporting in 34 cases (42.5), increasing the ease and confidence in 69 cases (86.25) and the advantage of reproducibility of data in all cases (100) according to the case-by-case evaluation by the reporting pathologists. Conclusion: Guided FNAC in conjunction with the cell block technique is extremely helpful in the evaluation of mass lesions of the liver and gall bladder for cytological diagnosis. A proper step-wise approach may be useful to reach a quick and effective diagnosis. Abstract

Context.--Mediastinal tumors/lesions are frequently encountered in daily cytopathology practice. These lesions are accessible through endoscopic/endobronchial ultrasound-guided or computed tomography-guided fine-needle aspiration cytology and represent a wide range of primary and metastatic tumors. This often poses diagnostic challenges because of the complexity of the mediastinal anatomic structures. Tumors metastatic to mediastinal lymph nodes represent the most common mediastinal lesions and must be differentiated from primary lesions. Objective.--To provide an updated review on the fine-needle aspiration cytology of mediastinal tumors/lesions, with an emphasis on diagnostic challenges. This review encompasses thymic epithelial neoplasms, mediastinal lymphoproliferative disorders, germ cell tumors, neuroendocrine tumors, soft tissue tumors, and metastatic tumors. Differential diagnoses; useful ancillary studies, including targeted immunohistochemical panels; and diagnostic pitfalls are discussed. Data Sources.--Data were gathered from a PubMed search of peer-reviewed literature on mediastinal tumors. Data were also collected from the authors' own practices. Conclusions.--Fine-needle aspiration cytology plays a vital role in evaluation of mediastinal lesions. Being familiar with the clinical and cytomorphologic features of these lesions, appropriately triaging the diagnostic material for ancillary testing, and correlating with radiologic findings are important in arriving at correct diagnoses and guiding management. Context.-The use of whole slide images (WSIs) in diagnostic pathology presents special challenges for the cytopathologist. Informative areas on a direct smear from a thyroid fine-needle aspiration biopsy (FNAB) smear may be spread across a large area comprising blood and dead space. Manually navigating through these areas makes screening and evaluation of FNA smears on a digital platform time-consuming and laborious. We designed a machine learning algorithm that can identify regions of interest (ROIs) on thyroid fine-needle aspiration biopsy WSIs. Objective.-To evaluate the ability of the machine learning algorithm and screening software to identify and screen for a subset of informative ROIs on a thyroid FNA WSI that can be used for final diagnosis. Design.-A representative slide from each of 109 consecutive thyroid fine-needle aspiration biopsies was scanned. A cytopathologist reviewed each WSI and recorded a diagnosis. The machine learning algorithm screened and selected a subset of 100 ROIs from each WSI to present as an image gallery to the same cytopathologist after a washout period of 117 days. Results.-Concordance between the diagnoses using WSIs and those using the machine learning algorithmgenerated ROI image gallery was evaluated using pairwise weighted k statistics. Almost perfect concordance was seen between the 2 methods with a k score of 0.924. Abstract

Bleb-based cell motility proceeds by the successive inflation and retraction of large spherical membrane protrusions ('blebs') coupled with substrate adhesion. In addition to their role in motility, cellular blebs constitute a remarkable illustration of the dynamical interactions between the cytoskeletal cortex and the plasma membrane. Here we study the bleb-based motions of Entamoeba histolytica in the constrained geometry of a micropipette. We construct a generic theoretical model that combines the polymerization of an actin cortex underneath the plasma membrane with the myosin-generated contractile stress in the cortex and the stress-induced failure of membrane-cortex adhesion. One major parameter dictating the cell response to micropipette suction is the stationary cortex thickness, controlled by actin polymerization and depolymerization. The other relevant physical parameters can be combined into two characteristic cortex thicknesses for which the myosin stress (i) balances the suction pressure and (ii) provokes membrane-cortex unbinding. We propose a general phase diagram for cell motions inside a micropipette by comparing these three thicknesses.
