1. Angular distribution of electron emission from ultrafast nanotip sources

Abstract

We investigate the angular distribution of ultrafast laser-induced electron emission from a tungsten nanotip in the multiphoton regime. A theoretical model allows precise determination of the relative contribution of different electron emission mechanisms, revealing connections between emission mechanism and the angular distribution of emitted electrons. We infer a continuous map of the work function across the surface of the tip, which in combination with the model can be used to predict values including the number of electrons per pulse and the angular divergence of the resulting beam as a function of laser power and tip voltage for (310)-oriented tungsten nanotips. The model is straightforward to implement and can be used to optimize the performance of instruments using ultrafast nanotip electron sources., Comment: 9 pages, 7 figures

4. Selecting and Using Authentic Videos for Intentional Second Language Learning: Nine Considerations

Abstract

Although using video to facilitate language learning is an exceedingly common practice, both for in-class and out-of-class learning, at present the literature exploring the use of authentic videos for intentional language learning is void of a user-friendly framework that educators and learners can refer to when selecting and using authentic videos to optimise language learning. To bridge this gap in the literature, the authors of this paper have drawn from a range of relevant research pertaining to cognitive neuroscience, educational psychology, and second language acquisition, as well as over two decades of combined personal experience as professional language educators, to develop a theoretical framework delineating nine research-based considerations educators and learners should bear in mind when selecting, using, and/ or prescribing authentic videos to facilitate optimal language learning.

5. Breathlessness without borders: a call to action for global breathlessness research

Abstract

It is likely that the burden of breathlessness in low and middle-income countries (LMICs) is much higher than has been estimated using calculations of disease burden and expected prevalence of the symptom. However, most breathlessness research has been conducted in high-income countries and may not be relevant to LMICs. To address this issue, we convened an international breathlessness and global health workshop. Our multidisciplinary team of experts (global palliative care, respiratory medicine, epidemiology, palliative medicine, psychiatry, sport science, global public health and health economics) met at the University of Hull for a two-day workshop in May 2024. We had 8 presentations on key issues relevant to global breathlessness research. Our discussions focussed on unexplored questions and links between breathlessness and other health and social issues, in order to develop an agenda for global breathlessness research. Our discussions highlighted (1) the global burden of breathlessness generated by a range of lifestyle, environmental, disease and poverty-related factors, (2) the need for a global healthcare workforce that can address modifiable causes and the symptoms of breathlessness together using an integrated approach, (3) the value of information over clinical effectiveness when considering implementation of breathlessness self-management interventions, (4) Addressing non-clinical outcomes which are meaningful to individuals and families and (5) Developing a language for global breathlessness research which does not assume that the cause of breathlessness is diagnosed or treated. We present our discussions and recommendations for new approaches and paradigms for global breathlessness research to generate discussion—not to provide empirical evidence.

6. The research activities of Ontario’s large community hospitals: an updated scoping review

Abstract

Abstract Background Community hospitals provide the majority of patient care in Canada but traditionally do not participate in clinical research. The disconnect between where most patients receive their health care and where health research is conducted leads to decreased study recruitment, reduced generalizability of study results, and inequitable patient access to novel therapies. A scoping review of the research activities of Ontario’s large community hospitals (LCHs) between 2013 and 2015 reported an annualized output of 266 publications. In the last decade, efforts have been made to engage more community hospitals in research. In this updated scoping review, we provide a snapshot of the research activities of Ontario’s LCHs between 2016 and 2022, describing the number and type of research publications as well as the frequency of collaboration within and between LCHs. Methods Three medical databases (PubMed, Embase, and CINAHL) were searched for publications that included at least one author affiliated with one of Ontario’s 47 LCHs, and for which the topic was hospital or health related. Screening and extraction occurred concurrently. Results We identified 3,719 publications from 2016 to 22 with at least one Ontario LCH-affiliated author, representing an annualized output of 531 publications. The most frequent publication type was observational study (n = 1,654; 45%), quality improvement (n = 355; 10%), systematic reviews (n = 352; 9%) and randomized controlled trials (n = 325; 9%). The most common disciplines were outpatient care (n = 1,144; 31%), health systems research (n = 806; 22%), inpatient care (n = 437; 12%) and surgery (n = 403; 11%). LCH-affiliated first authors were identified in 997 (27%) publications, representing 755 unique authors, while LCH-affiliated senior authors were identified in 962 (26%) publications, representing 583 unique authors. Among the 1,565 studies with an LCH-affiliated first or senior author, 574 (37%) included collaborators from the same LCH and 86 (5%) included collaborators from other Ontario LCHs. Conclusions Health research by LCH-affiliated clinicians and researchers increased significantly in 2016–2022 relative to 2013–2015. Participation in randomized controlled trials however, remains low, suggesting that further efforts are required to build clinical research infrastructure in LCHs.

Abstract

Ptychographic Coherent Diffractive Imaging enables diffraction-limited imaging of nanoscale structures at extreme ultraviolet and x-ray wavelengths, where high-quality image-forming optics are not available. However, its reliance on a set of diverse diffraction patterns makes it challenging to use ptychography to image highly periodic samples, limiting its application to defect inspection for electronic and photonic devices. Here, we use a vortex high harmonic light beam driven by a laser carrying orbital angular momentum to implement extreme ultraviolet ptychographic imaging of highly periodic samples with high fidelity and reliability. We also demonstrate, for the first time to our knowledge, ptychographic imaging of an isolated, near-diffraction-limited defect in an otherwise periodic sample using vortex high harmonic beams. This enhanced metrology technique can enable high-fidelity imaging and inspection of highly periodic structures for next-generation nano, energy, photonic and quantum devices., Comment: 20 pages, 4 figures, 4 supplementary figures

10. Powering Toward Los Angeles: Comparing Power Output and Pacing Approach Between Maximal 2000- and 1500-m On-Water Racing in Elite Rowers.

Abstract

Purpose: To compare power output and pacing between maximal 1500- and 2000-m on-water rowing performances. Methods: Twenty-six (female n = 4, male n = 22) international rowers, across 6 boat classes, completed maximal 1500- and 2000-m on-water races, separated by 24 to 48 hours. Crew combinations and seat orders remained consistent between races. Peach PowerLine instrumentation measured power output and stroke rate. Differences in completion time, mean power output (MPO), percentage prognostic velocity (PPV; percentage of world record velocity in each boat class), stroke rate, and pacing variance were assessed using linear mixed modeling. Results: Compared with 2000-m, completion times were 90.4 (6.1) seconds shorter over 1500 m (−24.7% [0.7%]). Both MPO (P =.255, η p 2 =.06) and PPV (P =.340, η p 2 =.18) were not different between distances. Broadly, crews adopted a reverse-J-shaped pacing across both distances, demonstrating a reduced variance over 1500 m (P =.035, η p 2 =.62). Percentage change in MPO from 2000 to 1500 m demonstrated a strong negative association with pacing variance over 1500 m (R2 =.74, P =.027). Conclusions: International rowing crews did not increase MPO or PPV when racing maximally over 1500 m compared to 2000 m. Comparable strategies were adopted over both distances, with less variance in pacing observed over 1500 m. The crews that demonstrated greater increases in MPO over the shorter race employed a flatter pacing strategy. To improve 1500-m on-water performance, rowers may need to adopt a more even pacing approach. [ABSTRACT FROM AUTHOR]

14. Visualisation and tooling for interactive molecular dynamics in virtual reality

Abstract

Interactive molecular dynamics (IMD) is a powerful technique which allows user intuition to guide and perturb a simulation. I do not believe that we have taken full advantage of this medium as by merely duplicating existing methods we do not address the new issues nor take advantage of the possibilities. It is vital to ensure that the system can be rendered efficiently to avoid motion sickness. We must use the latest techniques to allow the visualisation of large systems at a range of magnification levels. I discuss how we achieve this using raycasting imposters, as well as proposing some new primitives that can find use in molecular visualisation. Sudden visual discontinuities can be immersion-breaking and distracting when using virtual reality. I have developed two techniques to avoid these issues when visualising systems. For periodic systems, I propose a technique to crop atoms within the simulation box to ensure a smooth transition across periodic boundaries. I also correctly depict bonds across periodic boundaries. Another visual discontinuity occurs when viewing proteins using ribbon diagrams, with discrete flips in the direction the ribbon twists prevalent at the time resolution used in IMD. I address this with a novel double-normal interpolation approach, which when combined with smoothly interpolating the colour and scale yields diagrams that are no longer visually jarring. In addition to visualisation techniques, we can also exploit the additional freedom afforded in virtual reality to apply interactive forces. By leveraging the rotational degrees of freedom of a handheld virtual reality controller, I propose a new class of interactive forces that can rotate and translate a molecule without internal perturbations. By including damping forces as well, a user can now have precise control over a molecule's trajectory. This technique also extends to more complex coarse-grained systems with asymmetric particles.

Abstract

ObjectivesFew surveys have focused on physician moral distress, burnout, and professional fulfilment. We assessed physician wellness and coping during the COVID-19 pandemic.DesignCross-sectional survey using four validated instruments.SettingSixty-two sites in Canada and the United States.SubjectsAttending physicians (adult, pediatric; intensivist, nonintensivist) who worked in North American ICUs.InterventionNone.Measurements and main resultsWe analysed 431 questionnaires (43.3% response rate) from 25 states and eight provinces. Respondents were predominantly male (229 [55.6%]) and in practice for 11.8 ± 9.8 years. Compared with prepandemic, respondents reported significant intrapandemic increases in days worked/mo, ICU bed occupancy, and self-reported moral distress (240 [56.9%]) and burnout (259 [63.8%]). Of the 10 top-ranked items that incited moral distress, most pertained to regulatory/organizational ( n = 6) or local/institutional ( n = 2) issues or both ( n = 2). Average moral distress (95.6 ± 66.9), professional fulfilment (6.5 ± 2.1), and burnout scores (3.6 ± 2.0) were moderate with 227 physicians (54.6%) meeting burnout criteria. A significant dose-response existed between COVID-19 patient volume and moral distress scores. Physicians who worked more days/mo and more scheduled in-house nightshifts, especially combined with more unscheduled in-house nightshifts, experienced significantly more moral distress. One in five physicians used at least one maladaptive coping strategy. We identified four coping profiles (active/social, avoidant, mixed/ambivalent, infrequent) that were associated with significant differences across all wellness measures.ConclusionsDespite moderate intrapandemic moral distress and burnout, physicians experienced moderate professional fulfilment. However, one in five physicians used at least one maladaptive coping strategy. We highlight potentially modifiable factors at individual, institutional, and regulatory levels to enhance physician wellness.

18. The Impact of Decorative Images on Student Performance: A Two-Year Study of Online Library Modules

Abstract

During a two-year period, over 1,900 undergraduate students completed a version of an online asynchronous library module either with or without decorative images. Two instruction librarians compared quiz scores and affective feedback from both versions to determine the impact decorative images had on student performance and analyzed the results through the lenses of multimedia and user experience studies. The study finds that decorative images have little impact on student performance and expounds on how these findings impact design concerns of future online library modules.

Abstract

Abstract Background Informal carers of terminally ill patients play a vital role in providing palliative care at home, which impacts on their pre- and post-death bereavement experience and presents an up to 50% greater risk for mental-health problems. However, developing and implementing effective bereavement support remains challenging. There is a need to build the evidence base for music therapy as a potentially promising bereavement support for this vulnerable population. This study aimed to co-design an international best practice agenda for research into music therapy for informal carers of patients pre- and post-death bereavement. Methods Online half day workshop using a World Café approach; an innovative method for harnessing group intelligence within a group of international expert stakeholders (music therapy clinicians and academics with experience of music therapy with informal carers at end-of-life). Demographics, experience, key priorities and methodological challenges were gathered during a pre-workshop survey to inform workshop discussions. The online workshop involved four rounds of rotating, 25-minute, small group parallel discussions using Padlet. One final large group discussion involved a consensus building activity. All data were analysed thematically to identify patterns to inform priorities and recommendations. Results Twenty-two consented and completed the pre-event survey (response rate 44%), from countries representing 10 different time zones. Sixteen participated in the workshop and developed the following best practice agenda. The effectiveness of music therapy in supporting informal carers across the bereavement continuum should be prioritised. This should be done using a mixed methods design to draw on the strengths of different methodological approaches to building the evidence base. It should involve service users throughout and should use a core outcome set to guide the choice of clinically important bereavement outcome measures in efficacy/effectiveness research. Conclusions Findings should inform future pre- and post-death bereavement support research for informal caregivers of terminally ill patients. This is an important step in building the evidence base for commissioners and service providers on how to incorporate more innovative approaches in palliative care bereavement services.

Abstract

One of the key components of achieving sustainability and energy efficiency in power utility networks is accurate short-term load forecasting (STLF). STLF is essential for effective operational planning. Overestimation leads to unnecessary energy consumption and costs, while underestimation leads to energy shortages and potential blackouts, severely impacting the community and the economy. This study highlights the potential of regularized greedy forest (RGF) algorithm for STLF, which integrates the underlying tree structure with regularization to learn a decision forest. In particular, we propose the RGF model combined with two gradient-boosting frameworks, namely eXtreme gradient boosting and light gradient boosting machine models, to create a more robust ensemble model using Bayesian optimization techniques. The proposed ensemble model is evaluated in the real-world case study. It performs better than the existing models in terms of mean absolute percentage error (MAPE) and mean absolute error (MAE) in various scenarios. The experimental results show that the MAPE of the proposed model on average is 1.60% in the daily operation scenario, which provides accurate day-ahead (24 h) load forecasting with a half-day gap. The study demonstrates that the proposed model is suitable and practical for the STLF problem, and thus it contributes to the overall sustainability and resilience of power utility networks.

Abstract

This open access book analyzes barriers to inclusion in academia and details ways to create a more diverse, inclusive environment. It first describes what the barriers to inclusion are and how they function within the broader society, focusing on concept of implicit bias: what it is, how it develops, and the importance of training organizational members to recognize and challenge it. It then discusses the limitations of data collection that is guided by the conventional assumption that being diverse automatically means being inclusive. Lastly, it highlights the importance of creating a collaborative, interdisciplinary and institution-wide vision of an inclusive community.

Abstract

The extreme nonlinear optical process of high-harmonic generation (HHG) makes it possible to map the properties of a laser beam onto a radiating electron wavefunction, and in turn, onto the emitted x-ray light. Bright HHG beams typically emerge from a longitudinal phased distribution of atomic-scale quantum antennae. Here, we form a transverse necklace-shaped phased array of HHG emitters, where orbital angular momentum conservation allows us to tune the line spacing and divergence properties of extreme-ultraviolet and soft X-ray high harmonic combs. The on-axis HHG emission has extremely low divergence, well below that obtained when using Gaussian driving beams, which further decreases with harmonic order. This work provides a new degree of freedom for the design of harmonic combs, particularly in the soft X-ray regime, where very limited options are available. Such harmonic beams can enable more sensitive probes of the fastest correlated charge and spin dynamics in molecules, nanoparticles and materials., Comment: Submitted to Science Advances

Abstract

Upcoming measurements of the 21-cm line of neutral hydrogen will open a new observational window into the early stages of structure growth, providing a unique opportunity for probing large-scale cosmological signatures using the small-scale signals from the first stars. In this paper we evaluate the detection significance of compensated isocurvature perturbations (CIPs) from observations of the 21-cm hydrogen-line during the cosmic-dawn era. CIPs are modulations of the relative baryon and dark-matter density that leave the total matter density unchanged. We find that, under different assumptions for feedback and foregrounds, the ongoing HERA and upcoming SKA1-low experiments will provide constraints on uncorrelated CIPs at the level of $\sigma(A_{\rm CIP})= 10^{-3}-10^{-4}$, comparable to the sensitivity of upcoming CMB experiments, and potentially exceeding the constraints from cosmic-variance limited BAO surveys., Comment: 13 pages, 5 figures, comments welcome

Abstract

Abstract Background Critically ill patients commonly receive proton pump inhibitors (PPIs) to prevent gastrointestinal (GI) bleeding from stress-induced ulceration. Despite widespread use in the intensive care unit (ICU), observational data suggest that PPIs may be associated with adverse outcomes in patients with COVID-19 infection. This preplanned study is nested within a large randomized trial evaluating pantoprazole versus placebo in invasively ventilated patients. The 3 objectives are as follows: (1) to describe the characteristics of patients with COVID-19 in terms of demographics, biomarkers, venous thromboembolism, tracheostomy incidence and timing, and other clinical outcomes; (2) to evaluate the impact of COVID-19 infection on clinically important GI bleeding, 90-day mortality, and other outcomes compared to a propensity-matched non-infected cohort; and (3) to explore whether pantoprazole has a differential treatment effect on clinically important GI bleeding, 90-day mortality, and other outcomes in patients with and without COVID-19 infection. Methods The ongoing trial Re-EValuating the Inhibition of Stress Erosions (REVISE) compares pantoprazole 40 mg IV to placebo on the primary efficacy outcome of clinically important GI bleeding and the primary safety outcome of 90-day mortality. The protocol described in this report is for a substudy focused on patients with COVID-19 infection that was not in the original pre-pandemic trial protocol. We developed a one-page case report form to characterize these patients including data related to biomarkers, venous thromboembolism, COVID-19 therapies, tracheostomy incidence and timing, duration of mechanical ventilation, and ICU and hospital stay. Our analysis will describe the trajectory of patients with COVID-19 infection, a propensity-matched analysis of infected and non-infected patients, and an extended subgroup analysis comparing the effect of PPI among patients with and without COVID-19 infection. Discussion Prophylactic acid suppression in invasively ventilated critically ill patients with COVID-19 infection has unknown consequences. The results of these investigations will inform practice, guidelines, and future research. Abstract

OBJECTIVES:. DNA methylation can be used to determine an individual’s biological age, as opposed to chronological age, an indicator of underlying health status. This study aimed to assess epigenetic age in critically ill patients with and without sepsis to determine if higher epigenetic age is associated with admission diagnosis or mortality. DESIGN:. Secondary analysis of whole blood DNA methylation data generated from a nested case–control study of critically ill septic and nonseptic patients. SETTING:. Four tertiary care hospitals in Canada. INTERVENTIONS:. None. PATIENTS:. Critically ill patients with and without sepsis. MEASUREMENTS AND MAIN RESULTS:. Epigenetic age was derived from DNA methylation data using the Hannum and PhenoAge algorithms and deviation from the patient’s chronological age in years was determined. Of the 66 patients with sepsis, 34 were male (51.5%), the mean age was 65.03 years and 25 patients (37.8%) died before discharge. Of the 68 nonseptic patients, 47 were male (69.1%), the mean age was 64.92 years and 25 (36.7%) died before discharge. Epigenetic age calculated using the PhenoAge algorithm showed a significant age acceleration of 4.97 years in septic patients (p = 0.045), but no significant acceleration in nonseptic patients. Epigenetic age calculated using the Hannum algorithm showed no significant acceleration in the septic or nonseptic patients. Similarly, in the combined septic and nonseptic cohorts, nonsurvivors showed an epigenetic age acceleration of 7.62 years (p = 0.004) using the PhenoAge algorithm while survivors showed no significant age acceleration. Survivor status was not associated with age acceleration using the Hannum algorithm. CONCLUSIONS:. In critically ill patients, epigenetic age acceleration, as calculated by the PhenoAge algorithm, was associated with sepsis diagnosis and mortality.

Abstract

Oscillometry is an emerging pulmonary function testing tool that is conducted during tidal breaths with minimal patient effort. It is highly sensitive to changes in lung mechanics. Oscillometry was recently shown to be highly associated with disease severity in idiopathic pulmonary fibrosis (IPF). The usefulness of oscillometry after single lung transplant in IPF patients is not well understood. Our study demonstrated that oscillometry can detect changes in the graft despite presence of a native fibrotic lung to provide useful information to complement spirometry.

Abstract

Intensive care unit healthcare workers (ICU HCW) are at risk of mental health disorders during emerging disease outbreaks. Numerous cross-sectional studies have reported psychological distress, anxiety, and depression amongst ICU HCW during the COVID-19 pandemic. However, few studies have followed HCW longitudinally, and none of these have examined the association between COVID-19 workload and mental health. We conducted a longitudinal cohort study of 309 Canadian ICU HCW from April 2020 to August 2020, during the 1st wave of the COVID-19 pandemic. Psychological distress was assessed using the General Health Questionnaire 12-item scale (GHQ-12) at 3 timepoints: during the acceleration phase of the 1st wave (T1), the deceleration phase of the 1st wave (T2), and after the 1st wave had passed (T3). Clinically relevant psychological distress, defined as a GHQ-12 score ≥ 3, was identified in 64.7% of participants at T1, 41.0% at T2, and 34.6% at T3. Psychological distress was not associated with COVID-19 workload at T1. At T2, psychological distress was associated with the number of COVID-19 patients in the ICU (odds ratio [OR]: 1.06, 95% confidence interval [CI]: 1.00, 1.13) while at T3, when COVID-19 patient numbers were low, it was associated with the number of weekly hospital shifts with COVID-19 exposure (OR: 1.33, 95% CI: 1.09, 1.64). When analyzed longitudinally in a mixed effects model, pandemic timepoint was a stronger predictor of psychological distress (OR: 0.24, 95% CI: 0.15, 0.40 for T2 and OR: 0.16, 95% CI: 0.09, 0.27 for T3) than COVID-19 workload. Participants who showed persistent psychological distress at T3 were compared with those who showed recovery at T3. Persistent psychological distress was associated with a higher number of weekly shifts with COVID-19 exposure (OR: 1.97, 95% CI:1.33, 3.09) but not with a higher number of COVID-19 patients in the ICU (OR: 0.86, 95% CI: 0.76, 0.95). In summary, clinically relevant psychological distress was observed in a majority of ICU HCW during the acceleration phase of the 1st wave of the COVID-19 pandemic but decreased rapidly as the 1st wave progressed. Persistent psychological distress was associated with working more weekly shifts with COVID-19 exposure but not with higher numbers of COVID-19 patients in the ICU. In future emerging disease outbreaks, minimizing shifts with direct disease exposure may help alleviate symptoms for individuals with persistent psychological distress.

Abstract

During VR demos we have performed over last few years, many participants (in the absence of any haptic feedback) have commented on their perceived ability to 'feel' differences between simulated molecular objects. The mechanisms for such 'feeling' are not entirely clear: observing from outside VR, one can see that there is nothing physical for participants to 'feel'. Here we outline exploratory user studies designed to evaluate the extent to which participants can distinguish quantitative differences in the flexibility of VR-simulated molecular objects. The results suggest that an individual's capacity to detect differences in molecular flexibility is enhanced when they can interact with and manipulate the molecules, as opposed to merely observing the same interaction. Building on these results, we intend to carry out further studies investigating humans' ability to sense quantitative properties of VR simulations without haptic technology.

Abstract

We are witnessing the evolution of cosmology into a precision science. When the universe cools after the big bang, hydrogen atoms eventually form releasing the cosmic microwave background. Measurements of this, combined with direct observations of fully formed galaxies through telescopes inform us about the seeding of structure across the cosmos. In-between these milestones there is little-to-no observational data, particularly regarding the first stars and their growth into mature galaxies. Primordial hydrogen must eventually progress into everything we see today, and we know from local observations of galaxies that the remaining hydrogen must also become ionised again. A new generation of radio telescopes aim to detect the cosmological 21cm line of these hydrogen atoms in order to observe these events directly. This will provide unmatched insight into the formation of the first stars and their progression into the early galaxies responsible for reionisation. As never seen before, these new instruments will provide tomographical maps of our universe that fill in a significant proportion of the unobserved universe. Scientists across the globe hope to witness the cosmic dawn within the next decade. This work is focused on improving the statistical analyses techniques that probe the first star formation and when the early universe is being reionised - the cosmic dawn and epoch of reionisation respectively. The majority of this work looks at Bayesian model selection, which is a robust method for quantifying how well models fit observations. We apply it within the context of high redshift 21cm power spectrum observations, comparing a variety of scale and morpho- logical implementations of the reionisation process. We also use it to look into distinguishing effects caused by the remnants of the first stars and the statistical benefits of including UV observations. We then turn to improving the likelihood statistic, required for Bayesian analyses. The Morlet transform is implemented as an alternative to the Fourier transform within the power spectrum to enable the whole observational light-cone to be analysed at once. Bayesian analyses are incredibly promising and with this work we are a step closer to statistical machinery that provides objective conclusions about the quality of models when presented with data. Synergy between observational methods is important to maximise the discerning power of any Bayesian method. Now that observations are becoming more precise within cosmology, we can begin to constrain primordial astrophysics.
