ÖZETAmaç: Bu çalışmamızda rutin DJ stend kullanımının lazer litotripsi uygulanan komplike olmayan üreterorenoskopiolgularında postoperatif ağrı üzerine etkilerini araştırmayı amaçladık.Materyal Method: Kanuni Sultan Suleyman Eğitim Araştırma Hastanesi üroloji kliniğinde Aralık 2011ve Şubat 2013 tarihleri arasında Holmium-YAG laser (365 micron; 0.5-1.4J/5-10 Hz) ile lithotripsiuygulanan 139 üreterorenoskopi olgusu retrospektif olarak incelendi. Komplike olmayan üreterorenoskopiuygulanmış 98 hasta çalışmaya dahil edildi. Random olarak 24-26 F DJ stend yerleştirilenhastalar grup 1 ve DJ stend uygulanmayan diğer hastalar ise grup 2 olacak şekilde iki hasta grubuoluşturuldu. Tüm hastalarda postoperatif ilk gün, 5. ve 7. günlerde visuel ağrı skoru (VAS) ile ağrışiddeti değerlendirmesi uygulandı.Bulgular: Toplam 48 hastada (% 48.9) DJ stend uygulandı. Hastaların 59’ u kadın 39’ u erkekti (1.5:1).Ortalama yaş 36.7 (18-74) saptandı. Taş lokalizasyonu olguların 34’ ünde (%34.6) proksimal yerleşimliyken,64’ ünde (% 65.4) distal yerleşimliydi. Ortalama taş çapı 12.3 mm (8-22) olarak belirlendi.Ortalama yaş, cinsiyet dağılımı ve ortalama taş çapı her iki grupta benzer saptandı. Postoperatif ilkgün, 5. ve 7. günlerde hesaplanan VAS skorları grup 1 hastalarında anlamlı derecede daha düşüksaptandı (sırasıyla p: 0.01, 0.01, 0.03).Sonuç: Lazer litotripsi uygulanan komplike olmayan üreterorenoskopi olgularında özellikle cerrahidensonra ilerleyen günlerde postoperatif ağrının önlenmesi veya azaltılması amacıyla rutin DJ stenduygulanması tercih edilebilir.Anahtar Sözcükler: Double J stent; Lazer lithotripsi; Ağrı; Komplike olmayan üreterorenoskopi

The purpose of our study was to evaluate the utility of the apparent diffusion coefficient (ADC) value measurement in the diagnosis of peritoneal metastases on diffusion weighted magnetic resonance imaging.Diffusion weighted imaging with conventional magnetic resonance imaging sequences was performed on twenty consecutive oncology patients (group I) with peritoneal metastases. The ADC values of the metastases, the peritoneal fat around the metastases (group I) and the peritoneal fat in patients with no malignancy (group II) at b(0-100), b(0-600), and (b 0-1000) s/mm(2) gradients were measured and compared.The apparent diffusion coefficient values of three gradients in peritoneal metastases (2.27±0.4; 1.67±0.7 and 1.09±0.4×10(-3) mm(2)/s at b 100, 600 and 1000 gradients, respectively) were significantly lower than the ADC values of the peritoneal fat around metastases (3.07±0.4; 2.07±0.4; 1.33±0.3×10(-3) mm(2)/s at b 100, 600 and 1000 gradients, respectively) (p0.05). There was no significant difference between the ADC values of peritoneal fat in the patients of group I and group II at the 3 diffusion gradients (p0.05).The measurement of ADC values may be used as a complementary diagnostic method in differentiating peritoneal metastases from peritoneal fat on Diffusion Weighted MRI (DWMRI DWMRI).Bu çalışmanın amacı difüzyon ağırlıklı manyetik rezonans görüntülemede (MRG) peritoneal metastazların tanısında görünür difüzyon katsayısı (ADC) değerlerinin yararlılığını değerlendirmektir.Peritoneal metastazı bulunan 20 onkoloji hastasına (grup 1) konvansiyonel MRG sekansları ile birlikte difüzyon ağırlıklı görüntüleme gerçekleştirildi. Duplex collecting systems are the most commonly encountered anomaly of the urinary system. Complete duplex system with an H shaped ureter is a very rare situation. There are only two reported H ureter cases in the literature. Herein, we aimed to present an H shaped ureter case, which was identified while performing ureterorenoscopy to a 48-year-old female patient due to a right distal ureteral stone.

The aim of the current study was to evaluate the use of fresh-frozen concurrently with embalmed cadavers as initial training models for flexible ureteroscopy (fURS) in a group of urologists who were inexperienced in retrograde intrarenal surgery (RIRS). Twelve urologists involved in a cadaveric fURS training course were enrolled into this prospective study. All the participants were inexperienced in fURS. Theoretical lectures and step-by-step tips and tricks video presentations on fURS were used to incorporate the technical background of the procedure to the hands-on-training course and to standardize the operating steps of the procedure. An 8-item survey was administered to the participants upon initiation and at the end of the course. Pre- and post-training scores were similar for each question. All the participants successfully completed the hands-on-training tasks. Mean pre-training duration [3.56 +/- A 2.0 min (range 1.21-7.46)] was significantly higher than mean post-training duration [1.76 +/- A 1.54 min (range 1.00-6.34)] (p = 0.008). At the end of the day, the trainers checked the integrity of the collecting system both by endoscopy and by fluoroscopy and could not detect any injury of the upper ureteral wall or pelvicalyceal structures. The functionality of the scopes was also checked, and no scope injury (including a reduction in the deflection capacity) was noted. The fURS simulation training model using soft human cadavers has the unique advantage of perfectly mimicking the living human tissues. This similarity makes this model one of the best if not the perfect simulator for an effective endourologic training.
