Abstract: Abstract Data privacy is playing a vital role while facing the digital life aspects. Today, the world is being extensively inter-connected through the internet of things (IoT) technologies. This huge interconnectivity is bringing very wonderful capabilities for improving the quality of life (QoL) with itself, for instance, in distributed healthcare. On the other hand, there are new challenges in the interconnectivity per use. One of the most challenging issues of IoT use in social systems and digital life is secure, trustable, and reliable interactions over IoT networks such that safety, security, and privacy in both aspects of cyber and physical worlds for humankind should be planned and controlled. Due to the less physical activity of most people in the current world, fitness and aerobic sports are now an important need at any age to help them keep healthy in their cyber-physical life, specifically, for the younger student that are still in the growth ages. However, these sport activities need to be monitored seriously and closely to not put their life in danger. Herewith, healthcare services through IoT is becoming more applicable. Therefore, health information privacy for athletes is now a hot topic of investigation as we present the topic here. We propose an IoT-based physique healthcare system considering private information sharing for athletes based on data hiding at the edge of a collaborative system. The proposed system pays attention to the key factors of healthcare IoT infrastructure but it is bringing its new suggestions for more safety. Moreover, many evaluations based on different kinds of healthcare data are provided.

Abstract: Abstract With the rapid advancement of sports analytics and fan engagement technologies, the volume and diversity of physique data generated by smart devices across various distributed sports platforms have grown significantly. Extracting insights and enhancing fan experiences from such data offer considerable benefits. Yet, this process unveils two primary challenges. Firstly, efficiently utilizing the vast datasets in sports analytics is daunting due to the complex nature of the sports industry. Secondly, the data collected from diverse sources and stored in distributed platforms contain sensitive information like fan preferences and athlete performance metrics, posing risks of privacy breaches. To address these challenges, we leverage an advanced Locality-Sensitive Hashing technique, known as PSDFP $$_{\text {ALSH}}$$ ALSH , tailored for the sports domain. This paper presents a new privacy-preserving method for sports data fusion and prediction in distributed environments, utilizing enhanced Locality-Sensitive Hashing to protect sensitive information while maintaining high data utility. Through extensive experimentation, our approach demonstrates superior performance over existing methods in terms of Mean Absolute Error (MAE), Root Mean Squared Error (RMSE), and computational efficiency.

3. Prolonged versus intermittent β-lactam infusion in sepsis: a systematic review and meta-analysis of randomized controlled trials

Abstract: Abstract Background The two latest studies on prolonged versus intermittent use of β-lactam antibiotics in patients with sepsis did not reach consistent conclusions, further contributing to the controversy surrounding the effectiveness of the prolonged β-lactam antibiotics infusion strategy. We conducted a systemic review and meta-analysis to evaluate the efficacy and safety of prolonged and intermittent β-lactam infusion in adult patients with sepsis. Methods We systematically searched PubMed, EMBASE, and Cochrane Library databases for original randomized controlled trials comparing prolonged and intermittent β-lactam infusion in sepsis patients. A random-effects model was used to evaluate mortality, clinical success, microbiological success, and adverse events. We also conducted subgroup analyses to explore the impact of various factors on the mortality rates. Relative risk (RR) and corresponding 95% confidence intervals (CIs) were used to calculate the overall effect sizes for dichotomous outcomes. This meta-analysis was registered in PROSPERO (CRD42023463905). Results We assessed 15 studies involving 2130 patients. In our comprehensive assessment, we found a significant reduction in all-cause mortality (RR, 0.83; 95% CI 0.72–0.97; P = 0.02) and a notable improvement in clinical success (RR, 1.16; 95% CI 1.03–1.31; P = 0.02) in the prolonged infusion group compared to the intermittent infusion group, whereas microbiological success did not yield statistically significant results (RR, 1.10; 95% CI 0.98–1.23; P = 0.11). No significant differences in adverse events were observed between the two groups (RR, 0.91; 95% CI 0.64–1.29; P = 0.60). Additionally, remarkable conclusions were drawn from subgroup analyses including studies with sample sizes exceeding 20 individuals per group (RR, 0.84; 95%CI 0.72–0.98; P = 0.03), research conducted post-2010 (RR, 0.84; 95%CI 0.72–0.98; P = 0.03), cases involving infections predominantly caused by Gram-negative bacteria (RR, 0.81; 95%CI 0.68–0.96; P = 0.02), as well as the administration of a loading dose (RR, 0.84; 95% CI 0.72–0.97; P = 0.02) and the use of penicillin (RR, 0.61; 95% CI 0.38–0.98; P = 0.04). Conclusions Compared to intermittent infusion, prolonged infusion of β-lactam antibiotics significantly decreases all-cause mortality among patients with sepsis and enhances clinical success without increasing adverse events.

4. Experimental Investigation of Rotor Noise in Reverse Non-Axial Inflow

Abstract: This paper experimentally characterises the far-field noise emissions of a rotor operating in reverse non-axial inflow conditions. Specifically, experiments were undertaken at a range of rotor tilting angles and inflow velocities to investigate the effects of negative tilting on rotor acoustics and their correlation with aerodynamic performance. The results show that the forces and moments experienced by the rotor blades change significantly with increasing inflow velocity and increasing negative tilting angle. Correspondingly, distinct modifications to the far-field acoustic spectra are observed for the negatively tilted rotor when compared to the edgewise condition, with the broadband noise content notably increasing. Moreover, for a given tilting angle, the broadband noise component is accentuated with increasing inflow velocity, similar to when the negative tilting angle is increased. With reference to the flow-field surveys conducted in the literature and a preliminary in-house flow measurement, the increase in broadband content can possibly be attributed to the heightened level of ingestion of blade self-turbulence, i.e., the ingestion of turbulent wake generated by the upstream portion of the rotor by the downstream portion. At lower inflow velocities, the magnitude of the blade passing frequency at each of the observer angles is found to change minimally with negative tilt. In contrast, at higher inflow velocities, the directivity pattern and intensity of both the blade passing frequency and the overall sound pressure level are shown to change with increases in magnitude, particularly at downstream observer locations with negative tilt. These findings have important ramifications for the design and suitable operational profile of aerial vehicles for future urban air mobility applications.

5. Safety and efficacy of ciprofol vs. propofol for sedation in intensive care unit patients with mechanical ventilation: a multi-center, open label, randomized, phase 2 trial

Ciprofol (HSK3486; Haisco Pharmaceutical Group Co., Ltd., Chengdu, China), developed as a novel 2,6-disubstituted phenol derivative showed similar tolerability and efficacy characteristics as propofol when applicated as continuous intravenous infusion for 12 h maintenance sedation in a previous phase 1 trial. The phase 2 trial was designed to investigate the safety, efficacy, and pharmacokinetic characteristics of ciprofol for sedation of patients undergoing mechanical ventilation. Methods:. In this multicenter, open label, randomized, propofol positive-controlled, phase 2 trial, 39 Chinese intensive care unit patients receiving mechanical ventilation were enrolled and randomly assigned to a ciprofol or propofol group in a 2:1 ratio. The ciprofol infusion was started with a loading infusion of 0.1–0.2 mg/kg for 0.5–5.0 min, followed by an initial maintenance infusion rate of 0.30 mg·kg−1·h−1, which could be adjusted to an infusion rate of 0.06 to 0.80 mg·kg−1·h−1, whereas for propofol the loading infusion dose was 0.5–1.0 mg/kg for 0.5–5.0 min, followed by an initial maintenance infusion rate of 1.50 mg·kg−1·h−1, which could be adjusted to 0.30–4.00 mg·kg−1·h−1 to achieve −2 to +1 Richmond Agitation-Sedation Scale sedation within 6–24 h of drug administration. Results:. Of the 39 enrolled patients, 36 completed the trial. The median (min, max) of the average time to sedation compliance values for ciprofol and propofol were 60.0 (52.6, 60.0) min and 60.0 (55.2, 60.0) min, with median difference of 0.00 (95% confidence interval: 0.00, 0.00). In total, 29 (74.4%) patients comprising 18 (69.2%) in the ciprofol and 11 (84.6%) in the propofol group experienced 86 treatment emergent adverse events (TEAEs), the majority being of severity grade 1 or 2. Drug- and sedation-related TEAEs were hypotension (7.7% vs. 23.1%, P = 0.310) and sinus bradycardia (3.8% vs. 7.7%, P = 1.000) in the ciprofol and propofol groups, respectively. The plasma concentration-time curves for ciprofol and propofol were similar. Conclusions:. ciprofol is comparable to propofol with good tolerance and efficacy for sedation of Chinese intensive care unit patients undergoing mechanical ventilation in the present study setting. Abstract: Abstract Background Pain relief for patients in the intensive care unit (ICU) can improve treatment outcomes and reduce the burden on doctors and nurses. This study aims to report the clinical analgesic and sedative effects of nalbuphine and sufentanil on ICU patients. Methods This study retrospectively analyzed the medical records of 87 critically ill patients who received nalbuphine or sufentanil infusion in the ICU, including demographic data, diagnosis, Acute Physiology and Chronic Health Evaluation (APACHE) II, Critical Care Pain Observation Tool (CPOT), Richmond Agitation-Sedation Scale (RASS), systolic and diastolic blood pressure, heart rate and blood oxygen saturation (SpO2). The primary outcomes of this study were CPOT and RASS scores. The secondary outcomes were hemodynamic changes, including systolic blood pressure, diastolic blood pressure, heart rate, and SpO2. The adverse events recorded during pain management, such as hypoxemia, respiration depression and bradycardia, were also collected and analyzed. Results None of the patients in both groups experienced episode of hypoxemia, respiration depression and bradycardia. Abstract: Abstract Background Previous cluster-randomized controlled trials evaluating the impact of implementing evidence-based guidelines for nutrition therapy in critical illness do not consistently demonstrate patient benefits. A large-scale, sufficiently powered study is therefore warranted to ascertain the effects of guideline implementation on patient-centered outcomes. Methods We conducted a multicenter, cluster-randomized, parallel-controlled trial in intensive care units (ICUs) across China. We developed an evidence-based feeding guideline. ICUs randomly allocated to the guideline group formed a local "intervention team", which actively implemented the guideline using standardized educational materials, a graphical feeding protocol, and live online education outreach meetings conducted by members of the study management committee. ICUs assigned to the control group remained unaware of the guideline content. All ICUs enrolled patients who were expected to stay in the ICU longer than seven days. The primary outcome was all-cause mortality within 28 days of enrollment. Results Forty-eight ICUs were randomized to the guideline group and 49 to the control group. From March 2018 to July 2019, the guideline ICUs enrolled 1399 patients, and the control ICUs enrolled 1373 patients. Implementation of the guideline resulted in significantly earlier EN initiation (1.20 vs. 1.55 mean days to initiation of EN; difference − 0.40 [95% CI − 0.71 to − 0.09]; P = 0.01) and delayed PN initiation (1.29 vs. 0.80 mean days to start of PN; difference 1.06 [95% CI 0.44 to 1.67]; P = 0.001). There was no significant difference in 28-day mortality (14.2% vs. 15.2%; difference − 1.6% [95% CI − 4.3% to 1.2%]; P = 0.42) between groups. Conclusions In this large-scale, multicenter trial, active implementation of an evidence-based feeding guideline reduced the time to commencement of EN and overall PN use but did not translate to a reduction in mortality from critical illness. 8. Metabolomic Characterization Reveals ILF2 and ILF3 Affected Metabolic Adaptions in Esophageal Squamous Cell Carcinoma

Abstract: Esophageal cancer (EC) is a common malignant disease in eastern countries. However, a study of the metabolomic characteristics associated with other biological factors in esophageal squamous cell carcinoma (ESCC) is limited. Interleukin enhancer binding factor 2 (ILF2) and ILF3, double-stranded RNA-binding proteins, have been reported to contribute to the occurrence and development of various types of malignancy. Nevertheless, the underlying functions of ILF2 and ILF3 in ESCC metabolic reprogramming have never been reported. This study aimed to contribute to the metabolic characterization of ESCC and to investigate the metabolomic alterations associated with ILF2 and ILF3 in ESCC tissues. Here, we identified 112 differential metabolites, which were mainly enriched in phosphatidylcholine biosynthesis, fatty acid metabolism, and amino acid metabolism pathways, based on liquid chromatography–mass spectrometry and capillary electrophoresis–mass spectrometry approaches using ESCC tissues and paired para-cancer tissues from twenty-eight ESCC patients. In addition, ILF2 and ILF3 expression were significantly elevated in EC tissues compared to the histologically normal samples, and closely associated with PI3K/AKT and MAPK signaling pathways in ESCC. Moreover, in ESCC tissues with a high ILF2 expression, several short-chain acyl-carnitines (C3:0, C4:0, and C5:0) related to the BCAA metabolic pathway and long-chain acyl-carnitines (C14:0, C16:0, C16:0-OH, and C18:0) involved in the oxidation of fatty acids were obviously upregulated. Additionally, a series of intermediate metabolites involved in the glycolysis pathway, including G6P/F6P, F1,6BP, DHAP, G3P, and 2,3BPG, were remarkably downregulated in highly ILF3-expressed ESCC tissues compared with the corresponding para-cancer tissues. Overall, these findings may provide evidence for the roles of ILF2 and ILF3 during the process of ESCC metabolic alterations, and new insights into the development of early diagnosis and treatment for ESCC. Further investigation is needed to clarify the underlying mechanism of ILF2 and ILF3 on acyl-carnitines and the glycolysis pathway, respectively.

10. A New Mapped WENO Method for Hyperbolic Problems

Abstract: In this study, a new family of rational mapping functions gRM(ω;k,m,s) is introduced for seventh-order WENO schemes. gRM is a more general family of mapping functions, which includes other mapping functions such as gM and gIM as special cases. The mapped WENO scheme WENO-IM(2,0.1), which uses gIM, performs excellently at fifth order but rather poorly at seventh order. The reason for this loss of accuracy was found to be the over-amplification of very small weights by the mapping process, which can be traced back to the large slope of gIM at ω = 0. For m > 1, gRM can be designed to have a unit slope at ω = 0, which will preserve small weights with little to no amplification. It has been demonstrated through several one-dimensional linear advection test cases that the mapped WENO scheme WENO-RM(6,3,2 × 103), which uses the mapping function gRM(ω;6,3,2 × 103), outperforms both WENO-M and WENO-IM(2,0.1) at seventh order. The proposed scheme also performs better at a number of one-dimensional inviscid gas flow problems compared to other popular WENO schemes such as the WENO-Z scheme.

11. Anti-inflammation and anti-apoptosis effects of growth arrest-specific protein 6 in acute liver injury induced by LPS/D-GalN in mice

Abstract: Abstract Purpose To investigate the effect of growth arrest-specific protein 6 (Gas6) on acute liver injury in mice and related mechanisms. Methods Thirty C57BL/6 (6-8 weeks old) mice were randomly divided into control, LPS/D-GalN, and LPS/D-GalN+Gas6 groups (10 mice in each group). The LPS/D-GalN group was intraperitoneally administered with LPS (0.25 mg/Kg) and D-GalN (400 mg/Kg) for 5h. The LPS/D-GalN+Gas6 group was intraperitoneally administered with rmGas6 one hour before intraperitoneal application of LPS/D-GalN. All subjects were sacrificed at 5 h for blood and tissue analysis. The expression of protein and mRNA was assessed by western blotting and RT-PCR, respectively. Results Compared with the control group, AST, ALT, IL-1β, TNF-α, IL-6 IL-10, MPO activity were increased in the LPS/D-GalN group. However, they were significantly inhibited by Gas6. Gas6 markedly suppressed the expression of apoptosis-related protein induced by LPS/D-GalN. Moreover, Gas6 attenuated the activation of the NF-κB signaling pathway in acute liver injury induced by LPS/D-GalN. Conclusions Gas6 alleviates acute liver injury in mice through regulating NF-κB signaling pathways. Gas6 can be a potential therapeutic agent in treating LPS/D-GalN-induced acute liver injury in the future.

Abstract: Abstract Background There is a lack of large-scale epidemiological data on the clinical practice of enteral nutrition (EN) feeding in China. This study aimed to provide such data on Chinese hospitals and to investigate factors associated with EN delivery. Methods This cross-sectional study was launched in 118 intensive care units (ICUs) of 116 mainland hospitals and conducted on April 26, 2017. At 00:00 on April 26, all patients in these ICUs were included. Demographic and clinical variables of patients on April 25 were obtained. The dates of hospitalization, ICU admission and nutrition initiation were reviewed. The outcome status 28 days after the day of investigation was obtained. Results A total of 1953 patients were included for analysis, including 1483 survivors and 312 nonsurvivors. The median study day was day 7 (IQR 2–19 days) after ICU entry. The proportions of subjects starting EN within 24, 48 and 72 h after ICU entry was 24.8% (84/352), 32.7% (150/459) and 40.0% (200/541), respectively. The proportion of subjects receiving > 80% estimated energy target within 24, 48, 72 h and 7 days after ICU entry was 10.5% (37/352), 10.9% (50/459), 11.8% (64/541) and 17.8% (162/910), respectively. 13. Autophagy Enhancing Contributes to the Organ Protective Effect of Alpha-Lipoic Acid in Septic Rats

Abstract: Alpha-lipoic acid (ALA) reportedly has protective effects against sepsis, which is a leading cause of mortality worldwide and is associated with multiple organ dysfunction. The present study aimed to investigate further the possible action mechanisms of ALA. Male Sprague-Dawley rats were subjected to cecal ligation and puncture (CLP) in order to establish a sepsis model. The rats received an oral gavage of 200 mg/kg ALA or saline immediately after surgery. The heart rate (HR), left ventricular systolic pressure (LVSP), left ventricular end-diastolic pressure (LVEDP) and maximum rising and lowering rates of left ventricular pressure (±dp/dt) were examined for assessing the cardiac function. Blood urea nitrogen (BUN) and serum creatinine levels were assessed for evaluating renal function. Neutrophil gelatinase-associated lipocalin (NAGL) was examined for reflecting acute renal injury. Histopathological alterations of the small intestine were examined by hematoxylin-eosin staining. The ultrastructure of the small intestine and kidney was observed under electron microscopy. The levels of autophagy- and inflammation-associated proteins were determined via western blot analysis. The binding of nuclear factor-kappa B (NF-κB) to DNA was tested via an electrophoretic mobility shift assay. Cell apoptosis was examined using TUNEL staining. ALA treatment improved the survival rate, restored the loss of body weight and pro-inflammatory cytokines production in the serum of CLP-induced septic rats. ALA improved the cardiac and renal functions, downregulated the expression levels of interleukin-1β, tumor necrosis factor-α, and inducible nitric oxide synthase in the myocardium and small intestine of septic rats. ALA treatment also inactivated the NF-κB signaling pathway in the small intestine. An examination of autophagy showed that ALA increased the LC3II/I ratio, upregulated Atg5, Atg7, and beclin-1 and downregulated p62 protein levels in the myocardium, kidney, and small intestine of septic rats, and further promoted autophagosome accumulation in the kidney and small intestine. In addition, ALA could also reduce cell apoptosis in myocardium, kidney and small intestine tissues. These effects can be completely or party inhibited by 3-MA. Our findings suggest that autophagy enhancing may contribute to the organ protective effect of ALA in septic rats.

14. Milk fat globule-epidermal growth factor 8 (MFG-E8) attenuates sepsis-induced acute kidney injury by inhibiting NF-κB signaling pathway

Abstract: Abstract Purpose: To explore the effect of milk fat globule-epidermal growth factor 8 (MFG-E8) on sepsis-induced acute kidney injury (SAKI). Methods: Male C57BL/6 mice were randomized to control, sham, CLP, CLP+PBS, and CLP+rmMFG-E8 groups. SAKI was induced by cecal ligation and puncture (CLP). Recombinant mouse MFG-E8 (rmMFG-E8) (20 μg/kg) or PBS (vehicle) was administered intraperitoneally. Blood, urine and renal tissue were collected at 24 h after CLP. Blood samples were tested for serum kidney injury biomarker and cytokines. Urine samples were collected to detect KIM-1, and NGAL. Real-time PCR was tested for Bax and Bcl-2. TUNEL staining was used to determine renal apoptosis. Western blot was used to detect the expression of Bax, Bcl-2, and proteins in the NF-κB pathway. Results: MFG-E8 alleviated SAKI by decreasing serum Cre, BUN, urine KIM-1 and NGAL and by mitigating renal pathological changes significant (p < 0.05). IL-1β, IL-6, TNF-α were significantly inhibited by MFG-E8 (p < 0.05). Apoptosis induced by SAKI was markedly suppressed by MFG-E8. Finally, MFG-E8 attenuated the activation of the NF-��B signaling pathway in SAKI. Conclusion: MFG-E8 has beneficial effects on SAKI, which may be achieved by inhibiting the NF-κB pathway.

15. Effects of high-tidal-volume mechanical ventilation on the expression of PX receptor and inflammatory response in lung tissue of rats

Abstract: Ventilator-induced lung injury is a severe complication mainly caused from mechanical ventilation (MV), associated with the upregulation of inflammation response. The mechanism still remains unclear. This study aims to explore the effects of pathological damage, neutrophil infiltration, expression of P 2 X 7 receptor, and activation of Caspase-1 in lung tissue using a rat model. Sprague Dawley (SD) rats were randomly divided into sham group, conventional MV group, and high-tidal-volume ventilation group and fed with clean water and rat food. The sham group received tracheotomy without MV; conventional MV group was given 7 mL/kg tidal volume ventilation, and high-tidal-volume MV group was given 28 mL/kg tidal volume ventilation. All the rats were sacrificed after 4 h of ventilation or spontaneous breath. Lung wet/dry ratio was measured, and paraffin sections were prepared for pathological injury assessment and immunohistochemistry of P 2 X 7 and myeloperoxidase levels. Lung homogenate was used for Western blot analysis of P 2 X 7 receptor and Caspase-1 levels and real-time polymerase chain reaction (PCR) analysis of P 2 X 7 gene expression level. Compared to sham group and conventional MV group, high-tidal-volume MV led to an increase in lung wet/dry ratio and histology score. High-tidal-volume ventilation also led to chemotaxis of neutrophils. The expression levels of protein and messenger RNA (mRNA) of P 2 X 7 receptor were significantly upregulated. Cleaved-caspase-1 expression was also upregulated. All data provide the evidence that high-tidal-volume MV can lead to lung injury, neutrophils infiltration, and upregulation of cleaved-Caspase-1 level. This result may be related to the upregulation of P 2 X 7 receptor expression.

Abstract: BackgroundOssabaw pigs are unique miniature swine with genetic predisposition to develop metabolic syndrome and coronary atherosclerosis after extended periods receiving atherogenic diets. We have hypothesized that transgenic Ossabaw swine expressing chimp PCSK9 (proprotein convertase subtilisin‐like/kexin type 9) containing the D374Y gain of function would develop familial hypercholesterolemia and coronary artery plaques more rapidly than Landrace swine with the same transgene. Methods and ResultsOssabaw and Landrace PCSK9 gain‐of‐function founders were generated by Sleeping Beauty transposition and cloning. Histopathologic findings in the Ossabaw founder animal showed more advanced plaques and higher stenosis than in the Landrace founder, underscoring the Ossabaw genetic predisposition to atherosclerosis. We chose to further characterize the Ossabaw PCSK9 gain‐of‐function animals receiving standard or atherogenic diets in a 6‐month longitudinal study using computed tomography, magnetic resonance (MR) imaging, intravascular ultrasound, and optical coherence tomography, followed by pathological analysis of atherosclerosis focused on the coronary arteries. The Ossabaw model was consistently hypercholesterolemic, with or without dietary challenge, and by 6 months had consistent and diffuse fibrofatty or fibroatheromatous plaques with necrosis, overlying fibrous caps, and calcification in up to 10% of coronary plaques. ConclusionsThe Ossabaw PCSK9 gain‐of‐function model provides consistent and robust disease development in a time frame that is practical for use in preclinical therapeutic evaluation to drive innovation. Although no animal model perfectly mimics the human condition, this genetic large‐animal model is a novel tool for testing therapeutic interventions in the context of developing and advanced coronary artery disease.

24. Acoustic Shielding and Scattering Effects of a Propeller Mounted Above a Flat Plate

Abstract: To reduce the overall noise pollution generated by aircraft, design choices often attempt to incorporate acoustic shielding often using aircraft wings to minimise the noise generated by engines and propellers. By mounting a propeller above a wing, increasing the noise shielding, acoustic scattering effects can be introduced which change the directivity and magnitude of the noise generated. Experiments were carried out to investigate the acoustic shielding and scattering effects of a propeller mounted above a flat plate trailing edge. Two propellers were tested at constant rotational rates of 5000 RPM and 7000 RPM respectively. The tests were conducted within an anechoic wind tunnel facility with loading and far-field noise data collected by microphone arrays at a parametric set of propeller locations relative to the flat plate trailing edge. Analysis of the spectral content of the noise along with the tonal content of the signals are presented. The levels of coherence and cross spectrum phase are considered to give greater insights into the levels of noise attenuation due to shielding and scattering effects. Analysis of the acoustic data shows locations with notable levels of noise attenuation for both the tonal and broadband noise content.

Abstract: Background: Current management practices and outcomes in weaning from invasive mechanical ventilation are poorly understood. We aimed to describe the epidemiology, management, timings, risk for failure, and outcomes of weaning in patients requiring at least 2 days of invasive mechanical ventilation. Methods: WEAN SAFE was an international, multicentre, prospective, observational cohort study done in 481 intensive care units in 50 countries. Eligible participants were older than 16 years, admitted to a participating intensive care unit, and receiving mechanical ventilation for 2 calendar days or longer. We defined weaning initiation as the first attempt to separate a patient from the ventilator, successful weaning as no reintubation or death within 7 days of extubation, and weaning eligibility criteria based on positive end-expiratory pressure, fractional concentration of oxygen in inspired air, and vasopressors. The primary outcome was the proportion of patients successfully weaned at 90 days. Key secondary outcomes included weaning duration, timing of weaning events, factors associated with weaning delay and weaning failure, and hospital outcomes. This study is registered with ClinicalTrials.gov, NCT03255109. Findings: Between Oct 4, 2017, and June 25, 2018, 10 232 patients were screened for eligibility, of whom 5869 were enrolled. 4523 (77·1%) patients underwent at least one separation attempt and 3817 (65·0%) patients were successfully weaned from ventilation at day 90. 237 (4·0%) patients were transferred before any separation attempt, 153 (2·6%) were transferred after at least one separation attempt and not successfully weaned, and 1662 (28·3%) died while invasively ventilated. The median time from fulfilling weaning eligibility criteria to first separation attempt was 1 day (IQR 0-4), and 1013 (22·4%) patients had a delay in initiating first separation of 5 or more days. Abstract: Biologically inspired finlet treatments have been shown to effectively reduce the trailing-edge noise of a flat plate and hence are a viable noise-suppression technology for engineering applications. The present work performs a thorough experimental investigation on the near-field dynamics of finlet surface treatments applied to a flat plate. To examine the underlying noise-reduction mechanism, the manipulated flow field is analysed using data from detailed static, unsteady wall-pressure as well as velocity measurements and their correlations. Specifically, the densely populated dynamic transducers allow for the tracking of the turbulent boundary-layer development from upstream to the wake of the finlet-treated area (see supplementary movies), which elucidates the formation of ‘finlet-induced turbulence’ through flow–finlet interaction. Associated turbulence structures are found to further develop within the treated area and structures shed from the top of the finlets are observed to mix and merge with the turbulence being channelled through the space between the finlets in the finlet wake. While the mixing process increases the spanwise turbulence length scale, it significantly attenuates the unsteady wall-pressure fluctuation at the trailing edge and thus leads to broadband reduction of the trailing-edge noise. Moreover, it corroborates the findings of earlier studies suggesting that there exists an optimal distance between finlets and trailing-edge where the mixing effects are most beneficial.

Abstract: A numerical investigation is conducted on aerodynamic noise of flow past a circular cylinder with different spanwise lengths (0.5πD, πD, 2πD, and 4πD) at Re = 10 000, where D is the diameter of the cylinder. The near-field pressure and velocity fields are predicted through Large Eddy Simulation, and then, the acoustic analogy is used to obtain the far-field noise. The results show good agreements for both the near and far field with the data from in-house experiments and the literature. Though the spanwise length has limited influence on the power spectral density of the near-field velocity and pressure fluctuations at different spanwise locations, substantial differences are observed for the spanwise pressure coherence and near-wake structures. The 0.5πD case shows primarily two-dimensional flow features immediately behind the cylinder compared to the other three cases, resulting in the overprediction of the spanwise pressure coherence, which has strong implications for the far-field noise prediction. With the spanwise length correction, the differences in overall noise magnitudes of the different cases diminish. Nevertheless, the 2πD and 4πD cases better capture the first and second harmonics of the vortex shedding and its associated directivities than the other two cases, showing the importance of sufficient spanwise lengths in predicting noise from flow past a cylinder.

Abstract: To detect the expression of multiple myeloma-associated antigen (MMSA)-8 and MMSA-1 in bone marrow mononuclear cells of patients with acute myeloid leukemia, and explore their roles in acute myeloid leukemia.A total of 83 patients with M2 acute myeloid leukemia in our hospital from January 2019 to January 2020 were selected as research group, during the same period, 15 patients diagnosed iron deficiency anemia were selected as control group. Real-time fluorescence quantitative PCR was used to detect the levels of MMSA-8 and MMSA-1 in bone marrow mononuclear cells. Patients in the research group were divided into remission and non remission according to the clinical curative effect, and were divided into good prognosis, medium prognosis, and poor prognosis according to the prognosis. The relationship between MMSA-8, MMSA-1 and clinical efficacy, prognosis was analyzed. In addition, the general data of patients in the research group were collected, including white blood cell count (WBC), hemoglobin (Hb), platelet count (PLT), and percentage of bone marrow progenitor cells at admission. Pearson method was used to analyze the correlation between MMSA-8, MMSA-1 and clinical data, and MMSA-8 and MMSA-1.The analysis results about mRNA levels of MMSA-8 and MMSA-1 in bone marrow mononuclear cells of patients showed that patients in the research group were significantly higher than those in the control group (P0.05); In the research group, patients without remission were also significantly higher than those with remission, as well as those with medium and poor prognosis than with good prognosis, while only mRNA level of MMSA-1 in patients with poor prognosis was significantly higher than those with medium prognosis (P0.05). Pearson analysis showed that MMSA-8, MMSA-1 were positively correlated with WBC (r=0.468, r=0.516), and MMSA-8 was positively correlated with MMSA-1 (r=0.318).The levels of MMSA-8 and MMSA-1 in bone marrow mononuclear cells of patients with M2 acute myeloid leukemia are increased, which are closely related to the occurrence and development of the disease, and have certain value for the prognosis.M2型急性髓系白血病患者骨髓中MMSA-8、MMSA-1表达及临床意义.检测急性髓系白血病患者骨髓单个核细胞中多发性骨髓瘤相关抗原（MMSA）-8、MMSA-1表达情况，探讨二者在急性髓系白血病中的临床意义。.纳入2019年1月-2020年1月本院M2型急性髓系白血病患者83例为研究组，同期血液科诊断为单纯缺铁性贫血患者15例为对照组，实时荧光定量PCR检测骨髓单个核细胞中MMSA-8、 MMSA-1表达水平。将研究组患者根据疗效分为缓解、未缓解，根据预后状况分为预后良好、预后中等和预后不良，分析MMSA-8、MMSA-1与疗效、预后的关系。另收集研究组患者的一般资料，包括入院时血常规白细胞数、血红蛋白、血小板数和骨髓原始细胞百分比，Pearson法分析MMSA-8、MMSA-1与患者临床资料以及MMSA-8与MMSA-1的相关性。.对患者骨髓单个核细胞中MMSA-8、MMSA-1 mRNA水平分析结果显示，研究组患者明显高于对照组患者（P0.05）；研究组中未缓解患者明显高于缓解患者（P0.05），预后中等、预后不良患者均明显高于预后良好患者（P0.05），预后不良患者仅MMSA-1 mRNA水平明显高于预后中等患者（P0.05）。Pearson法分析结果显示，研究组患者MMSA-8、MMSA-1与白细胞数呈正相关（r=0.468，r=0.516），MMSA-8与MMSA-1呈正相关（r=0.318）。.M2型急性髓系白血病患者骨髓单个核细胞中MMSA-8、MMSA-1水平升高，二者均与疾病发生发展关系密切，且对预后有一定预测价值。.

Abstract: This paper studies vibrissa shaped cylinder as a passive control method for reducing the aerodynamic noise of the flow past a cylinder. One elliptical cylinder and a circular cylinder are also investigated for comparison. The far-field noise results show that compared with the circular cylinder case, the tonal peak almost disappears in the vibrissa shaped cylinder case and the sound pressure level (SPL) could be reduced by about 20dB at Re=36,000. The elliptical cylinder only shows a slight SPL reduction by about 4 dB with a shift in the tonal peak frequency from St=0.2 to St=0.36. Furthermore, some noticeable variations occur in the noise directivity pattern for the vibrissa shaped cylinder case. The shift in the tonal peak frequency, the reduction in noise and modifications in the noise directivity are related to the changing and breakdown of the vortex shedding caused by the interference with the elliptical and vibrissa shaped cylinder surfaces. The PIV experiments are conducted to the three cases for the flow field information to further understand the mechanism of the noise reduction caused by the vibrissa shaped cylinder and the elliptical cylinder. Abstract: Experiments were carried out within an aeroacoustic wind tunnel facility to investigate acoustic scattering and shielding effects of a propeller being placed above a semi-infinite flat plate. The position of the flat plate trailing edge was adjusted relative to the propeller which was tested in static conditions with a constant rotational rate of 5000 RPM. Far-field noise was collected by a pair of microphone arcs in the reflection and shadow zone regions respectively. Effects of installation on the overall sound pressure level and first blade passing frequency tone was studied. Moreover the magnitude-squared coherence and cross spectrum phase data between selected microphone pairs were analysed. Significant amplification and attenuation can be observed with changing relative locations due to complex acoustic interactions between the propeller and flat plate surface and trailing edge. There are locations with significant potential for noise attenuation and directivity manipulation in terms of tonal and broadband noise content of the propeller. Appreciable changes in the first blade passing frequency phase was seen when installed cases were compared to the baseline isolated propeller case. Reductions of up to 16.3 dB in the first blade passing frequency tone due to the presence of notable shielding were observed. [ABSTRACT FROM AUTHOR]

Abstract: A set of experiments were carried out within an aeroacoustic wind tunnel facility to investigate parametrically the acoustic interactions of a propeller positioned above two flat plates of varying dimensions. A propeller was tested at a constant rotational rate of 7000 RPM in static conditions in three test configurations; an isolated propeller, a propeller interacting with a semi-infinite flat plate and a propeller interacting with a finite flat plate. The aerodynamic loading and far-field noise data were collected. The changes in magnitude and directivity of the overall sound pressure level as well as the first blade passing frequency tone were explored comprehensively for the different cases. The spectral content of selected cases of interest were examined in greater detail showing the impacts of acoustic scattering, diffraction, reflection and shielding when both flat plates are introduced. The results show that locations exist at which there is significant potential for noise attenuation in both the tonal and broadband noise content. Reductions of up to 11.6 dB in the overall sound pressure level and 19.9 dB in the first blade passing frequency were discovered.

46. Steady and unsteady loading and radiated far-field sound of three NACA series airfoils in a uniform flow

Abstract: This paper investigates the hydrodynamic near-field and predicted radiated acoustic far-field of three NACA-series airfoils, namely the NACA 16-616, NACA 16-506, and NACA 0024 profiles, over a range of angles of attack, encompassing the pre-stall, stall, and post-stall flow regimes. In both the static pressure and the pressure fluctuation results, it is shown that each flow regime for each airfoil is easily distinguished, that each regime has different spectral behavior and boundary layer characteristics, and it is further shown that the behavior is different for each airfoil. In particular, it is shown that airfoil shape has a significant impact on both the angle of attack at which stall occurs and on the nature of the stalling mechanism. In the predicted radiated acoustic field, it is found that the variation of sound pressure level with frequency and with angle of attack is different for each airfoil, with airfoil thickness shown to have a significant impact on the abrupt effect of stall on the predicted radiated far-field sound. This is of significance due to the widespread use of aeroacoustic models, such as that proposed by Brooks, Pope, and Marcolini, derived empirically from only one airfoil.
