This work evaluated the contribution of the thrombin generation assay in cell culture models, in a mouse model of APS, and in patients with endothelial dysfunction leading to a hypercoagulable state. Antiphospholipid antibodies cause a decrease in ePCR, inducing resistance to activated protein C, found in our different models and patients. Treatment with hydroxychloroquine in the experimental model of APS showed a protective effect of this treatment on endothelial function, restoring vasorelaxation and normalizing thrombin generation after injection of purified antiphospholipid antibodies. A hypercoagulable state, associated with the development of pulmonary embolism, has been demonstrated in idiopathic pulmonary fibrosis. Circulating endothelial progenitors from patients with idiopathic pulmonary fibrosis induce a hypercoagulable state associated with hypoexpression of thrombomodulin. These results open a therapeutic avenue. Currently, clinical trials are testing thrombomodulin as a potential therapeutic. Recently, a pandemic by Sars-CoV-2, is associated with a hypercoagulable state, with the development of multiple thrombosis, found during autopsies of patients. We have demonstrated the value of the thrombin generation test as a prognostic factor during Sars-CoV-2 infection, predicting the hospitalization of patients in intensive care. The thrombin generation test seems to be an interesting tool during endothelial damage inducing hypercoagulability in cross-sectional models, and suggests possible therapeutic avenues, such as the modulation of the glycocalyx in APS., Ce travail a permis d’évaluer l’apport du test de génération de thrombine dans des modèles de cultures cellulaires, dans un modèle murin de SAPL, et chez des patients atteints d’une dysfonction endothéliale entrainant un état d’hypercoagulabilité. Les anticorps antiphospholipides entrainent une diminution de l’ePCR, induisant résistance à la protéine C activée, retrouvée dans nos différents modèles et patients. Le traitement par l'hydroxychloroquine dans le modèle expérimental de SAPL, a montré un effet protecteur de ce traitement sur la fonction endothéliale, en restaurant la vasorelaxation et une normalisation de la génération de thrombine après injection d'anticorps antiphospholipides purifiés. Il a été démontré un état hypercoagulable, associé au développement d’embolie pulmonaire, au cours de la fibrose pulmonaire idiopathique. Les progéniteurs endothéliaux circulants de patients atteints de fibrose pulmonaire idiopathique, induisent un état d’hypercoagulabilité, associé à une hypoexpression de thrombomoduline. Ces résultats ouvrent une piste thérapeutique. Actuellement des essais cliniques testent la thrombomoduline comme thérapeutique potentielle. Récemment, une pandémie par Sars-CoV-2, est associée à un état hypercoagulable, avec le développement de thromboses multiples, retrouvées lors d’autopsies de patients. Nous avons démontré l’intérêt du test de génération de thrombine comme facteur pronostique au cours de l’infection à Sars-CoV-2, prédisant l’hospitalisation en réanimation des patients. Le test de génération de thrombine semble être un outil intéressant au cours de l’atteinte endothéliale induisant une hypercoagulabilité dans des modèles transversaux, et laisse entrevoir de possibles pistes thérapeutiques, comme la modulation du glycocalyx dans le SAPL.

Le syndrome des antiphospholipides (SAPL) est un syndrome clinico-biologique caractérisé par une thrombophilie clinique ou une atteinte obstétricale, associée à la présence d’Ac antiphospholipides (APL) 2 fois à 12 semaines d’intervalle. L’objectif est d’évaluer l’intérêt du test de génération de thrombine (TGT) dans un modèle murin de SAPL traité par hydroxychloroquine (HCQ) et chez des patients SAPL. Matériel et méthodes : Une mise au point du TGT a été effectuée. L’impact de la protéine C activée (PCa) a été évalué. L’étude des patients SAPL est rétrospective et a inclus tous types de SAPL. Résultats : En présence de PCa, nous avons observé une différence significative avec une génération de thrombine plus importante chez les souris SAPL (ETP : 237,4±53,2 nM.min ; Pic : 45,2±12,7 nM) que les souris SAPL traitées par HCQ (ETP : 145,7±21,6 nM.min; Pic:23.9±8,0 nM) et les témoins (ETP: 116,3±29,6 nM.min; Pic: 21,9±3,1 nM). Chez les patients, le TGT était très augmenté dans le groupe SAPL obstétrical (ETP : 1804,47±521,04 nM.min ; Pic : 259,72±89,67 nM) et le groupe SAPL thrombotique (ETP : 1308,11±608,86 nM.min; Pic : 162,91±79,85 nM) comparé aux témoins (ETP : 784,91±149,69 nM.min; Pic : 82,84±17,86 nM). Chez 6 patients SAPL thrombotique sur 21, les paramètres du thrombinogramme étaient très faibles et associés à une négativation des APL dans les 2 ans. Conclusion : La génération de thrombine était plus basse chez les souris traitées par HCQ, ce qui confirme son intérêt vasculoprotecteur. Des taux plus faibles des paramètres du thrombinogramme sont corrélés à une négativation des APL. Une étude prospective est nécessaire pour valider l’intérêt du TGT dans le suivi de l’activité du SAPL.

In France, the first isolated case seems now to be reported in December 2019, stage 3 of the COVID-19 epidemic was triggered on March 14(th), the start of the planned containment exit from May 11(th). Healthcare services have faced a large influx of patients who may be beyond their capacity to receive and care, particularly in the Large-East and Ile-de-France regions. Some patients show an evolution of the disease never observed before with other coronaviruses and develop in a few days a very important inflammatory reaction, which can lead to death of patients. A working group of the French Society of Clinical Biology (SFBC) was set up with the objective of providing updated information on the current status of the biological prescriptions (focusing on biochemistry ones) and their evolution during the epidemic, and of analyzing the biological parameters associated with comorbidities and patient evolution in order to link biological results with medical events. The expanded working group covers all sectors of medical biology in France and extends to the French-speaking world: hospital sectors (CHU and CH, Army Training Hospitals) and the private sector opening a field of view on the biological situation in establishments for dependent elderly, social establishments and clinical medical institutions. Les services de soins ont fait face à un afflux massif de patients pouvant déborder leurs capacités d’accueil et de prise en charge, notamment dans les régions Grand-Est et Ile-de-France. Certains patients présentent une évolution de la maladie encore jamais observée avec les coronavirus et développent en quelques jours une réaction inflammatoire très importante, pouvant mener au décès. Un groupe de travail de la Société française de biologie clinique (SFBC) s’est constitué, ayant pour objectif de faire le point sur les prescriptions biologiques et leur évolution au cours de l’épidémie, d’analyser les paramètres biologiques, avec un focus biochimique, associés aux comorbidités et à l’évolution du patient, dans le but de relier les résultats biologiques avec des évènements du parcours de soins du patient. Ce groupe de travail recouvre tous les secteurs publics (CHU, CH, Hôpitaux d’instruction des armées) et privés de la biologie médicale en France métropolitaine et ultra-marine ; il s’étend également à la francophonie. Il permet une vision large sur la situation biologique en milieu hospitalier, établissements d’hébergements de personnes âgées dépendantes (Ehpad), établissements médicaux sociaux (EMS) et en cliniques. Le but de cet article est la présentation de ce groupe de travail et ses actions immédiates et à venir.
