Abstract

COVID-19 negatively affected surgical activity, but the potential benefits resulting from adopted measures remain unclear. The aim of this study was to evaluate the change in surgical activity and potential benefit from COVID-19 measures in perspective of Italian surgeons on behalf of SPIGC. A nationwide online survey on surgical practice before, during, and after COVID-19 pandemic was conducted in March–April 2022 (NCT:05323851). Effects of COVID-19 hospital-related measures on surgical patients’ management and personal professional development across surgical specialties were explored. Data on demographics, pre-operative/peri-operative/post-operative management, and professional development were collected. Outcomes were matched with the corresponding volume. Four hundred and seventy-three respondents were included in final analysis across 14 surgical specialties. Since SARS-CoV-2 pandemic, application of telematic consultations (4.1% vs. 21.6%; p < 0.0001) and diagnostic evaluations (16.4% vs. 42.2%; p < 0.0001) increased. Elective surgical activities significantly reduced and surgeons opted more frequently for conservative management with a possible indication for elective (26.3% vs. 35.7%; p < 0.0001) or urgent (20.4% vs. 38.5%; p < 0.0001) surgery. All new COVID-related measures are perceived to be maintained in the future. Surgeons’ personal education online increased from 12.6% (pre-COVID) to 86.6% (post-COVID; p < 0.0001). Online educational activities are considered a beneficial effect from COVID pandemic (56.4%). COVID-19 had a great impact on surgical specialties, with significant reduction of operation volume. However, some forced changes turned out to be benefits. Isolation measures pushed the use of telemedicine and telemetric devices for outpatient practice and favored communication for educational purposes and surgeon–patient/family communication. Abstract

COVID-19 negatively affected surgical activity, but the potential benefits resulting from adopted measures remain unclear. The aim of this study was to evaluate the change in surgical activity and potential benefit from COVID-19 measures in perspective of Italian surgeons on behalf of SPIGC. A nationwide online survey on surgical practice before, during, and after COVID-19 pandemic was conducted in March–April 2022 (NCT:05323851). Effects of COVID-19 hospital-related measures on surgical patients’ management and personal professional development across surgical specialties were explored. Data on demographics, pre-operative/peri-operative/post-operative management, and professional development were collected. Outcomes were matched with the corresponding volume. Four hundred and seventy-three respondents were included in final analysis across 14 surgical specialties. Since SARS-CoV-2 pandemic, application of telematic consultations (4.1% vs. 21.6%; p < 0.0001) and diagnostic evaluations (16.4% vs. 42.2%; p < 0.0001) increased. Elective surgical activities significantly reduced and surgeons opted more frequently for conservative management with a possible indication for elective (26.3% vs. 35.7%; p < 0.0001) or urgent (20.4% vs. 38.5%; p < 0.0001) surgery. All new COVID-related measures are perceived to be maintained in the future. Surgeons’ personal education online increased from 12.6% (pre-COVID) to 86.6% (post-COVID; p < 0.0001). Online educational activities are considered a beneficial effect from COVID pandemic (56.4%). COVID-19 had a great impact on surgical specialties, with significant reduction of operation volume. However, some forced changes turned out to be benefits. Isolation measures pushed the use of telemedicine and telemetric devices for outpatient practice and favored communication for educational purposes and surgeon–patient/family communication. Abstract

Le ben note proprietà chimico-fisiche dei liquidi ionici (ILs), unite alla versatilità strutturale, sono alla base delle loro numerose e differenti applicazioni. In questo lavoro, si presenta un’interazione termocromica reversibile, sia in soluzione che in un film polimerico, tra un IL portante sul catione un residuo gluconammidico e il sale di cobalto Co(NTf2)2. I gruppi ossidrilici sul catione e l’anione bromuro dell’IL coordinano il Co al variare della temperatura applicata. A temperatura ambiente il complesso OH-Co risulta ottaedrico (rosa) mentre, intorno ai 60 °C, la geometria diviene tetraedrica con lo ione bromuro che coordina il Co (blu). Lo studio VT UV-vis (Figura 1) ha permesso di identificare le bande relative ai complessi e il loro andamento con la temperatura, determinando anche la temperatura di switch delle geometrie. Incorporando metallo e legante in un polimero, è stato ottenuto un film polimerico avente le stesse proprietà della soluzione. Abstract

The removal of emergent pollutants like endocrine disrupting compounds (EDC) from wastewaters is a pressing issue of contemporary times.1 An efficient and low-cost strategy involves the adsorption of pollutant onto suitable nanostructured materials. In this context, supramolecular gels, originated by the self-assembly of small molecules in solution, are receiving increasing interest, due to their high adsorption ability, selectivity and often high recyclability.2 In the light of this, in the framework of our interest in supramolecular gels in nonconventional solvents,3 we obtained and characterized supramolecular gels of 1,3:2,4- dibenzylidene-D-sorbitol (DBS) in aromatic and aliphatic ionic liquids, reported in Figure 1.

Abstract

For the valorization of lignocellulosic biomass, a key step is unlocking the conversion of glucose to suitable chemical platforms. To this aim, we herein describe the sequential chemo-enzymatic conversion of glucose into 5-hydroxymethylfurfural (5-HMF), in two steps: glucose isomerization to fructose, catalyzed by glucose isomerase (GI) and fructose dehydration to 5-HMF promoted by Amberlyst 15. The novelty of our approach lies in the use of crude cell extracts of Streptomyces coelicolor showing GI activity, as isomerization catalysts, along with a commercial immobilized GI. Under optimized reaction conditions, we obtained a conversion to 5-HMF (C5‑HMF) from glucose of 50% in the H2O/[bmim]­[Cl]0.5[BF4]0.5 1:3 w-w ternary mixture. We also carried out both reactions under sonochemical activation, obtaining a conversion to fructose (Cfructose) of 50% and C5‑HMF 74% for the isomerization and dehydration respectively, with a drastic reduction in reaction time and temperature. In this case, the amount of substrate processed was 50 times higher than in silent conditions.

Abstract

Desulfurization of fuel that avoids the introduction in the atmosphere of toxic sulfur volatile compounds, usually formed during combustion, is a viable strategy to reduce pollution. To find new efficient and eco-sustainable adsorbent materials, bio-based polymers such as chitosan (CS), cellulose (CE) and cellulose acetate (CEOAc) were dissolved in ionic liquids (ILs) to form ionic liquid gels (ILGs). Gelling and swelling ability, porosity, morphology, rheology and mechanical response were studied as a function of the polymer and ionic liquid (IL) used. The best performing gels were tested for fuel desulfurization using a fuel mimicking solution of thiophene (T), benzothiophene (BT) and dibenzothiophene (DBT) in hexane. T was the sulfur compound that adsorbed the fastest on gels, and the effect of different parameters affecting the adsorption process, such as the total concentration of sulfur compounds, volume of fuel, and the contact surface area of the gel, under static or dynamic conditions, was analyzed. Bio-based polymer ILGs proved to be promising adsorbents for fuel desulfurization allowing 84% removal efficiency to be reached for T and 60% for DBT and BT in a multistage apparatus. In addition, gels could be reused up to four times without any leaching of IL or polymer in fuel, proving to be competitive with other adsorbent materials currently used.

Abstract

Hypothesis: Performances of materials are frequently affected by the action of radicals that can induce their degradation. To overcome the above issue, natural antioxidants (AOs) can be added during manufacturing. Considering the high instability of AOs, they have been adsorbed on carbon nanomaterials surface. However, the inclusion of functionalized carbon nanomaterials into gel matrix could enhance the antioxidant efficiency and represent an easy way to disperse and handle the active species.Experiments: Carboxypropyl functionalized carbon nanotubes (f-CNT), pure or with physically adsorbed alpha-tocopherol (f-CNT-VE) and quercetin (f-CNT-Q), were incorporated in some ionic liquid gels (ILGs) formed by 1,3-didodecylimidazolium-based salts. Temperature of gel-sol transition, morphology, response to external stimuli and rheology of hybrid ILGs (HILGs) were investigated before their use as radical scavengers. To this purpose a free radical test was performed on gels.Findings: The properties of HILGs significantly differ from the ones of pure ILGs, as CNTs appreciably improved gel rheological response. Besides, the gel network is able to enhance radical scavenging activity of both natural AOs and f-CNTs, achieving a complete radical reduction in less than 1 h. Furthermore, the activity is also preserved in thin films obtained from HILGs, opening the way to the application of these systems as material coatings. (C) 2019 Elsevier Inc. All rights reserved.

Abstract

With the aim of obtaining thermochromic systems with potential applications in solar energy storage, we evaluated the behavior of some sugar-based ionic liquids (ILs)–Co(NTf₂)₂ complexes, in IL solution, as a function of temperature. Different structural changes on the cation, the nature of the anion, and the nature of the IL used as the solvent were considered. The analysis of the above factors was carried out through a combined approach of different techniques, that is, variable temperature UV–vis and NMR spectroscopies, conductivity, and thermal gravimetric analysis. The thermochromic systems were analyzed both as solutions and as thin films, and the data collected highlight the defining role played by both the cation structure and the solvent nature in determining their performance. Most of the investigated systems show a chromogenic transition from pink to blue, occurring in a temperature range suitable for practical applications (40–60 °C). Interestingly, when embedded in a polymeric matrix, thin films with high recyclability and long life are also described.

Abstract

Supramolecular gels are a fascinating class of materials, originated by the self-assembly of low molecular weight molecules. Underpinned by non-covalent interactions, they find application in a diverse range of fields. Among these, supramolecular gels can be considered as organized, non-conventional reaction media, able to influence reactivity in a radically different way, compared with what happens in solution. This short review will focus on this aspect, covering literature from 2010 onwards, addressing the application of supramolecular gels as reaction media. In particular, in the first section we explore organocatalytic reactions in gel phase, with wide synthetic relevance, such as aldol and Mannich reactions as well as Friedel Crafts or ester hydrolysis processes. Subsequently, focus is laid on metal-catalysed reactions, among which relevance is given to widely used reactions like cross-coupling and click reactions. Then, the final sections describe the use of supramolecular gels as reaction media for photo- and biocatalysed processes.

Abstract

Ionic liquids (ILs) are a class of non-conventional solvents, which, for almost two decades, have continued to generate burgeoning interest in different fields of present-day chemical research with few similar precedents. Among the various aspects related to ILs, a topic worthy of in-depth analysis is their influence on organic reactivity and reaction rates. In light of this, the present short review aims to provide an overview of the literature from 2010 to the present day that addresses this issue. In particular, we herein present two main different viewpoints by which the solvent effect of ILs is explained: the first is mainly based on considering the bulk polarity of ILs and linear solvation energy relationships, while the other treats ILs as nanostructured fluids. In both cases, studies dealing with IL mixtures are also covered. Finally, literature addressing the area of supramolecular catalysis "by" or "in" ILs is also reported. This is one of the few reviews covering these specific aspects, aiming to provide a useful framework to guide future research into the effects of ILs on organic reactivity.

Abstract

A new ionic liquid with sugar-based ligand has been prepared and used to coordinate cobalt(II), at low loading, in an ionic liquid solution. The system obtained reveals magnetic switching and marked thermochromism in solution/polymer films over a convenient temperature range (20–60 °C). This system is self-contained, devoid of any volatile substances, and reproducible over multiple thermal cycles. The colour change can be attributed to a change in the coordination geometry from octahedral (pink) at room temperature to tetrahedral (blue) on warming, monitored by 59Co NMR spectroscopy, which was used for the first time to study the change. This material may find applications in stimuli-responsive smart systems aimed at energy storage applications.

Abstract

Searching for new fluorescent organic salts to be used in biomedical and electrochemical field, we conjugated properties of imidazolium salts with the ones of naphthalene diimide core and we obtained some N,N′-bis-(1-alkyl-3-propylimidazolium)naphthalene diimide diiodides. We took into account alkyl chains going from hexyl to dodecyl, as well as hydrogenated and fluorinated alkyl tails. After determination of their thermal behaviour by differential scanning calorimetry, concentration-, and temperature-dependent spectroscopic studies (UV-vis and fluorescence) were performed evidencing the occurrence of isodesmic and enthalpy-driven self-assembly processes. Properties of aggregates were also investigated in the solid state by fluorescence spectroscopy and scanning electron microscopy (SEM). Information obtained evidenced the formation of H-aggregates that are organized in spherical or disk-like structures.

Abstract

Naphthalene diimide diimidazolium salts differing in the anion nature were synthesized and their properties as well as their self-assembly behaviour were investigated. In particular, we took into consideration the N,N′-bis-(1-octyl-3-propylimidazolium)-naphthalene diimide cations and anions differing in size, shape and coordination abilities like [I−], [BF4−] and [NTf2−]. After determination of thermal behaviour, using differential scanning calorimetry and thermal gravimetric analysis, the electrochemical stability and redox properties were assessed using cyclic voltammetry. The self-assembly behaviour of the salts was investigated using concentration and temperature-dependent spectroscopic studies (UV-vis and fluorescence). Data collected demonstrate the ability of salts used to aggregate through an isodesmic pathway. Interestingly, the aggregation is featured by an enhanced emission event. The properties of aggregates were also investigated in the solid state using fluorescence measurements and scanning electron microscopy (SEM). On the whole, data collected show that the properties of salts and of aggregates that they are able to form are heavily dependent on the anion nature. Salts used combine the properties of a semiconductor, like the naphthalene diimide core, with the ones of imidazolium salts and could have interesting applications as materials for optoelectronic and conservation of energy.

Abstract

Motivation: The rapid increase of bio-medical literature makes it harder and harder for scientists to keep pace with the discoveries on which they build their studies. Therefore, computational tools have become more widespread, among which network analysis plays a crucial role in several life-science contexts. Nevertheless, building correct and complete networks about some user-defined biomedical topics on top of the available literature is still challenging., Results: We introduce NetMe 2.0, a web-based platform that automatically extracts relevant biomedical entities and their relations from a set of input texts-i.e. in the form of full-text or abstract of PubMed Central's papers, free texts, or PDFs uploaded by users-and models them as a BioMedical Knowledge Graph (BKG). NetMe 2.0 also implements an innovative Retrieval Augmented Generation module (Graph-RAG) that works on top of the relationships modeled by the BKG and allows the distilling of well-formed sentences that explain their content. Abstract

BACKGROUND Bariatric surgery (BS) has a lower percentage of complications than other abdominal surgeries. Hemorrhage in one of the most common complications and can be life-threatening. Hereditary factor XI (FXI) deficiency is a coagulation disorder that can result in excessive bleeding requiring intervention to restore hemostasis. Risks over benefits in patients with morbid obesity with BS indication, as well as those with FXI deficiency, should be carefully evaluated. This article reports the case of an obese woman with FXI deficiency -undergoing SG. CASE REPORT A 49-year-old woman with a BMI of 51 kg/m² was diagnosed as having severe FXI deficiency during preoperative exams prior to bariatric surgery. Virus-inactivated homo-group plasma 10 ml/kg infusion was administrated 1 h before surgery, during the entire procedure, and continuing until postoperative day (POD) 4. A very low-calorie ketogenic diet (VLCKD) was proposed to the patient 4 weeks before surgery. Laparoscopic sleeve gastrectomy was performed with staple-line reinforcement by oversewing the seromuscular layer using continuous suture. Subcutaneous enoxaparin 4000 U.I. was administered from POD 1 until POD 25 to prevent any thromboembolic event. The patient was discharged on POD 5 in good clinical condition. CONCLUSIONS Risks of bleeding andor thromboembolic events before or after BS are increased in patient with FXI deficiency. Bariatric surgery in these patients is safe in experienced BS centers, and the risks associated with the obesity seem to exceed those of the coagulopathy and surgery. Careful preoperative counseling, extensive hematological checks, and meticulous surgery are essential to reduce BS risks. Sleeve gastrectomy oversewing the stapler line seems a reasonable choice.

Abstract

With the aim to obtain targeted chemotherapeutic agents, imidazolium and ammonium-based folate salts were synthesized. Their photophysical behavior was investigated both in buffer and buffer/DMSO solution as well as in solid phase, performing UV-vis and fluorescence investigations. Properties of the aggregates were also analyzed by dynamic light scattering. Gelation ability of the salts was analyzed in biocompatible solvents, and gel phases obtained were characterized by determining critical gelation concentrations and gel-solution transition temperatures. Insights about gelator interactions in the tridimensional network were also gained performing ATR-FTIR investigation. Properties of soft materials were further analyzed performing rheology measurements, scanning electron microscopy, fluorescence and resonance light scattering investigations. Antiproliferative activity of organic salts was tested towards two breast cancer cell lines, expressing different levels of folate receptor, namely MDA-MB-231 and MCF-7, and a normal epithelial cell line, like h-TER T-RPE-1, by using MTT assay. Dichlodihydrofluorescein acetate test was performed to verify the role of oxidative stress in cell death. Finally, antiproliferative activity was also evaluated in gel phase, to verify if salts were able to retain biological activity also after the entrapment in the gelatinous network. Results collected evidence that folate based organic salts were able to behave as targeted chemotherapeutic agents both in solution and gel phase, showing uptake mechanism and selectivity indexes that depend on both cancer cell line nature and salt structure.

30. Pancreatic Adeno-MiNEN, a Rare Newly Defined Entity with Challenging Diagnosis and Treatment: A Case Report with Systematic Literature Review and Pooled Analysis.

Abstract

Mixed neuroendocrine non-neuroendocrine neoplasms (MiNEN) are a peculiar entity that can occur throughout the whole gastrointestinal trait, and pancreatic localization is rare. Their main characteristic is the presence of at least a neuroendocrine and an epithelial component, each accounting for at least 30% of the tumour mass. The presence of epithelial ductal component defines adeno-MiNEN. We report a case of a 59-year-old woman affected by pancreatic adeno-MiNEN with challenging diagnosis and successfully treated. A systematic literature review and pooled analysis was also performed, aiming to define the management and outcomes of pancreatic adeno-MiNEN. Out of 190 identified records, 15 studies including 28 patients affected by pancreatic-adeno-MiNEN were included in the analysis. Pancreatic adeno-MiNEN occurred mainly in males (82.8%) and at a mean age of 61.7 (range: 24-82) years. Pre-operative diagnosis was possible only in 14.2% of cases. At presentation, the majority had already advanced disease (TNM stage III (53.8%) and stage IV 19.3%). Adjuvant therapy was performed in 55% of patients, and the tumour recurrence rate was in 30% of cases. Median disease-free survival (DFS) was 12 months (range: 0-216 months) with a 5-year DFS of 16.6%, while the median overall survival (OS) was 12 months (range: 0-288 months) with a 5-year OS of 23.5%. Pancreatic adeno-MiNENs are rare; as they have very heterogenous behaviour, they are rarely diagnosed preoperatively and have poor prognosis. Treatment of localised MiNEN still relies on radical surgical resection, which seems essential to achieve a good oncological prognosis. International registry on MiNEN is necessary to improve the knowledge on this rare tumour and to improve its outcomes.

31. BioTAGME: A Comprehensive Platform for Biological Knowledge Network Analysis.

Abstract

The inference of novel knowledge and new hypotheses from the current literature analysis is crucial in making new scientific discoveries. In bio-medicine, given the enormous amount of literature and knowledge bases available, the automatic gain of knowledge concerning relationships among biological elements, in the form of semantically related terms (or entities), is rising novel research challenges and corresponding applications. In this regard, we propose BioTAGME, a system that combines an entity-annotation framework based on Wikipedia corpus (i.e., TAGME tool) with a network-based inference methodology (i.e., DT-Hybrid). This integration aims to create an extensive Knowledge Graph modeling relations among biological terms and phrases extracted from titles and abstracts of papers available in PubMed. The framework consists of a back-end and a front-end. The back-end is entirely implemented in Scala and runs on top of a Spark cluster that distributes the computing effort among several machines. Background: The rapidly increasing biological literature is a key resource to automatically extract and gain knowledge concerning biological elements and their relations. Knowledge Networks are helpful tools in the context of biological knowledge discovery and modeling., Results: We introduce a novel system called NETME , which, starting from a set of full-texts obtained from PubMed , through an easy-to-use web interface, interactively extracts biological elements from ontological databases and then synthesizes a network inferring relations among such elements. 33. Ionic liquids: "normal" solvents or nanostructured fluids?

Abstract

Ionic liquids (ILs) are a class of non-conventional solvents, which, for almost two decades, have continued to generate burgeoning interest in different fields of present-day chemical research with few similar precedents. Among the various aspects related to ILs, a topic worthy of in-depth analysis is their influence on organic reactivity and reaction rates. In light of this, the present short review aims to provide an overview of the literature from 2010 to the present day that addresses this issue. In particular, we herein present two main different viewpoints by which the solvent effect of ILs is explained: the first is mainly based on considering the bulk polarity of ILs and linear solvation energy relationships, while the other treats ILs as nanostructured fluids. In both cases, studies dealing with IL mixtures are also covered. Finally, literature addressing the area of supramolecular catalysis "by" or "in" ILs is also reported. This is one of the few reviews covering these specific aspects, aiming to provide a useful framework to guide future research into the effects of ILs on organic reactivity.

34. Ionic liquid gels and antioxidant carbon nanotubes: Hybrid soft materials with improved radical scavenging activity.

Abstract

Hypothesis: Performances of materials are frequently affected by the action of radicals that can induce their degradation. To overcome the above issue, natural antioxidants (AOs) can be added during manufacturing. Considering the high instability of AOs, they have been adsorbed on carbon nanomaterials surface. However, the inclusion of functionalized carbon nanomaterials into gel matrix could enhance the antioxidant efficiency and represent an easy way to disperse and handle the active species., Experiments: Carboxypropyl functionalized carbon nanotubes (f-CNT), pure or with physically adsorbed α-tocopherol (f-CNT-VE) and quercetin (f-CNT-Q), were incorporated in some ionic liquid gels (ILGs) formed by 1,3-didodecylimidazolium-based salts. Temperature of gel-sol transition, morphology, response to external stimuli and rheology of hybrid ILGs (HILGs) were investigated before their use as radical scavengers. To this purpose a free radical test was performed on gels., Findings: The properties of HILGs significantly differ from the ones of pure ILGs, as CNTs appreciably improved gel rheological response. Besides, the gel network is able to enhance radical scavenging activity of both natural AOs and f-CNTs, achieving a complete radical reduction in less than 1 h.
