Abstract

Les données de consommation en acides gras polyinsaturés (AGPI) de la femme allaitante sont rares bien qu’essentielles pour évaluer le niveau d’apport en acides gras du nourrisson et éventuellement orienter la mère vers une alimentation adaptée voire vers une supplémentation en acides gras essentiels. Les données INCA 2 concernant les femmes allaitantes ont été combinées aux données de composition nutritionnelle des aliments de la base de données Ciqual 2013 pour évaluer leurs apports en acides gras. Bien qu’il s’agisse d’un échantillon limité d’individus enquêtés il y a 12 ans, ces données restent malgré tout intéressantes à analyser et à comparer aux recommandations actuelles de l’Anses. Elles montrent que la consommation en AGPI n-3 n’est pas suffisante au regard des recommandations. Par ailleurs, il a été démontré que la composition en acides gras du lait maternel est un marqueur reconnu de la consommation des lipides alimentaires et un bon reflet du niveau de consommation des AGPI n-6 et n-3. Or, les données épidémiologiques relatives à la quantité et à la qualité des lipides consommés en France sont rares. Dans le cadre de différentes études cliniques conduites sur le lait maternel, l’ITERG a pu ainsi estimer l’évolution des habitudes alimentaires des Français depuis près de 17 ans. Un travail de synthèse a repris l’ensemble des données acquises au cours de 4 études cliniques menées entre 1997 et 2014. Cette synthèse a mis en évidence une évolution positive des proportions en AGPI n-3 en 17 ans (1997–2014). Toutefois, les proportions d’acide alpha-linolénique (ALA) et d’acide docosahexaénoïque (DHA) dans le lait maternel restent en deçà des recommandations nutritionnelles pour le nouveau-né.

Abstract

Subclinical mastitis (SCM) is an inflammatory state of the lactating mammary gland, which is asymptomatic and may have negative consequences for child growth. The objectives of this study were to: (1) test the association between the dietary inflammatory index (DII®) and SCM and (2) assess the differences in nutrient intakes between women without SCM and those with SCM. One hundred and seventy-seven women with available data on human milk (HM) sodium potassium ratio (Na:K) and dietary intake data were included for analysis. Multivariable logistic regression was used to examine the association between nutrient intake and the DII score in relation to SCM. Women without SCM had a lower median DII score (0.60) than women with moderate (1.12) or severe (1.74) SCM (p < 0.01). A one-unit increase in DII was associated with about 41% increased odds of having SCM, adjusting for country and mode of delivery, p = 0.001. Women with SCM had lower mean intakes of several anti-inflammatory nutrients. We show for the first time exploratory evidence that SCM may be associated with a pro-inflammatory diet and women with SCM have lower intakes of several antioxidant and anti-inflammatory nutrients.

Abstract

Background: Long-chain polyunsaturated fatty acids (LC-PUFA) are nutritionally important constituents of breast milk to support normal growth, immune function and central nervous system development of newborn infants. Both linoleic acid (18:2 n-6; LA) and alpha-linolenic acid (18:3 n-3; ALA), the essential fatty acids, precursors of n-6 and n-3 LC-PUFA, are also present in breast milk. Although the total amount of fat in human milk is fairly constant, the fatty acid composition can vary substantially depending on the diet of the mother. Large variations in the LA, ALA and DHA (22:6 n-3) contents are observed among countries. As reported by previous studies (1993-2001), the ALA consumption of the French population was 2-3 times lower than the recommended value. Since this period, the food industry offers more food products enriched in n-3 fatty acids. Objective: The present study aimed to investigate whether current recommendations and improved n-3 PUFA content of food products comply with the actual breast-milk PUFA composition. For this purpose, the PUFA content of a large number of human milk samples collected in 2007 in different French regions were compared with previous data obtained in studies conducted in the nineties years (1993-1998). Moreover, this study aimed to confirm expected reduction of the trans fatty acid (TFA) intake resulting from decreased level of TFA in margarine and other dietary fats during the same period. Study design and methods: Mature (postpartum day ≥ 30) human milk samples (n = 145) were provided in 2007 by eight French regional human milk banks (Bordeaux : n = 20; Dijon: n = 21; Ile-de-France: n = 18; Lyon: n = 20; Montpellier: n = 18; Nantes: n = 21; St Etienne: n = 6; Tours: n = 21), from healthy women volunteers with no particular health history. The fatty acid composition of breast milk was analysed by high-resolution gas-liquid chromatography. Statistical analysis was performed with Kruskall Wallis and Mann and Whitney U tests (p < 0.05). Results: Fatty acid composition of human milk samples did not differ significantly among the eight French regions. The proportions of saturated fatty acids were relatively similar (means ranging from 44.9% to 48.7% of total fatty acids). Monounsaturated fatty acids also varied little, from 36.9% to 39.7%. Total n-6 and n-3 PUFA accounted for 12.5%-15.2%, with an n-6/n-3 PUFA ratio close to 10, similar among the collect centers. Considering the overall results, LA represented between 10.4% and 12.7%, and ALA varied from 0.72% to 0.95%. Comparison of our results to previous data (1993-1998) showed opposite changes for LA and ALA. During the last decade, the LA level has decreased by 17%, whereas the ALA level has increased by 45%, and hence in 2007, the LA/ ALA ratio was close to 14 versus 24 in 1993-1998. The LC-PUFA content remained unchanged. Concerning total TFA, their content was lower (– 37%; p < 0.0001) in 2007 than in 1997, respectively 1.3% and 2.1% of total fatty acids of breast milk. Conclusion: Basing on the PUFA composition of human milk, this large French study shows favourable changes in consumption of both omega-6 and omega-3 fatty acid precursors, and in TFA, in the course of last decade.

Abstract

Some studies have demonstrated the transfer of trans fatty acids (TFA) across the human placenta. It was suggested that TFA might disturb the metabolism of essential fatty acids (EFA) in fetus and consequently might affect intrauterine human growth more or less according to the TFA intake level of the mother. In this context, the objective of this study was to assess, for French pregnant women, possible impact of their TFA intake on parameters of their term infants: 1/ TFA composition of the umbilical cord (plasma lipids and parietal phospholipids), 2/ birth weight and head circumference. The TFA composition of maternal and umbilical plasma lipids, and parietal phospholipids of umbilical cord were determined by gas chromatography, associated with a thin-layer chromatography for the phospholipids. Because TFA content of adipose tissue is a reliable biochemical indicator of the usual TFA intake level, maternal adipose tissue was also analyzed. Trans fatty acid (TFA) percentage was significantly higher (p = 0.001) in maternal (0.9% of total fatty acids) than in umbilical plasma total lipids (0.6%) thus confirming their placental passage. Moreover, trans isomer pattern of cord plasma lipids was different from the maternal’s one, especially regarding trans isomers of linoleic acid (9c,12c-18:2). Percentage values of 9c,13t + 9t,12t and 9t,12c isomers were respectively, 2 and 3 times higher (p < 0.001) in umbilical than in maternal plasma (Except trans 16:1 acids). All trans isomers observed in umbilical plasma lipids were detected in both parietal and vessel (vein and arteries) phospholipids of umbilical cord. Nevertheless, 9c13t + 9t12t isomer mix level was significantly higher (p < 0.001) in artery than in vein TPL. Moreover, in artery TPL, percentage value of 9c13t+9t12t isomer mix was inversely correlated (r = - 0.703, p = 0.003) with arachidonic acid content. Nevertheless, for this French population, there was no relation between either weight or head circumference of newborn and the TFA levels in both adipose tissue and plasma lipids of their mother.

Abstract

Eating habits of lactating women can influence the nutrient composition of human milk, which in turn influences nutrient intake of breastfed infants. The aim of the present study was to identify food patterns and nutritional adequacy among lactating women in Europe. Data from a multicentre European longitudinal cohort (ATLAS study) were analysed to identify dietary patterns using cluster analysis. Dietary information from 180 lactating women was obtained using 3-d food diaries over the first 4 months of lactation. Four dietary patterns were identified: ‘vege-oils’, ‘fish-poultry’, ‘confectionery-salads’ and ‘mixed dishes’. Nutrition adequacy was not significantly different between clusters, but the ‘vege-oils’ cluster tended to yield the highest nutrition adequacy measured by Mean Adequacy Ratio. Compared with European dietary reference values (DRVs) for lactating women, women in all clusters had inadequate intakes of energy, pantothenic acid, folate, vitamin C, vitamin A, vitamin D, zinc, iodine, potassium and linoleic acid. Adequate intake for fibre and α-linolenic acid was only achieved in the ‘vege-oils’ cluster. Overall, fat intake was above DRVs. The present study showed that various dietary patterns do not adequately supply all nutrients, indicating a need to promote overall healthy dietary habits for European lactating women.

Abstract

Background: Subclinical mastitis (SCM) is an inflammatory condition of the mammary gland. We examined the effects of SCM on human milk (HM) composition, infant growth, and HM intake in a mother–infant cohort from seven European countries. Methods: HM samples were obtained from 305 mothers at 2, 17, 30, 60, 90, and 120 days postpartum. SCM status was assessed using HM Sodium (Na): Potassium (K) ratio >0.6. Levels of different macro- and micronutrients were analyzed in HM. Results: SCM prevalence in the first month of lactation was 35.4%. Mean gestational age at delivery was lower and birth by C-section higher in SCM mothers (p ≤ 0.001). HM concentrations of lactose, DHA, linolenic acid, calcium, and phosphorous (p < 0.05 for all) was lower, while total protein, alpha-lactalbumin, lactoferrin, albumin, arachidonic acid to DHA ratio, n-6 to n-3 ratio and minerals (iron, selenium, manganese, zinc, and copper) were higher (p < 0.001 for all) in mothers with SCM. There were no differences in infant growth and HM intake between non-SCM and SCM groups. Conclusion: We document, for the first time, in a large European standardized and longitudinal study, a high prevalence of SCM in early lactation and demonstrate that SCM is associated with significant changes in the macro- and micronutrient composition of HM. Future studies exploring the relation of SCM with breastfeeding behaviors and developmental outcomes are warranted.

Abstract

Polyunsaturated fatty acid (PUFA) composition data of the breast milk are rare but essential for assessing the adequate intake of fatty acids in the newborn and possibly directing the mother to a suitable diet or even supplementation with essential fatty acids. The raw data for lactating women from the national study on individual food consumption 2006-2007 (INCA 2) were combined with the French food composition table (Ciqual 2013) to assess their fatty acid intakes. Although this is a limited number of subjects, these data are still interesting to analyze and compare with current Anses recommendations. According to these results, omega 3 PUFA consumption is not sufficient for the purposes of the Recommended Dietary Intakes (RDI). In addition, it has been shown that the fatty acid composition of breast milk is a proven marker of dietary lipid consumption and a good reflection of the consumption's level of omega-6 and omega-3 PUFAs. Epidemiological data on quantity and quality of lipids consumed in France are rare. In the context of different observational and interventional studies conducted on breast milk, ITERG has been able to estimate the evolution of French food habits for nearly 17 years. A synthesis work has taken over all the data acquired during 3 clinical studies carried out between 1997 and 2014. This synthesis showed a positive evolution of the proportions in omega-3 PUFA in 17 years (1997-2014). However, the proportions of alpha-linolenic acid (ALA) and docosahexaenoic acid (DHA) in breast milk remain below nutritional recommendations for the newborn.

Abstract

Aim: This study examined the influence of different human milk fortifiers on biomarkers of gastrointestinal immaturity and inflammation in preterm infants.Methods: We report secondary outcomes from a controlled, double-blind, randomised, parallel group study conducted from 2011 to 2014 in neonatal intensive care units at 11 metropolitan hospitals in France, Belgium, Germany, Switzerland and Italy. Preterm infants born at up to 32 weeks or weighing up to 1500 g were randomised to a new powdered human milk fortifier (n = 77) or a control fortifier (n = 76) for a minimum of 21 days. We analysed faecal markers of gut inflammation, namely alpha-1 antitrypsin and calprotectin, and maturity, namely elastase-1.Results: Faecal alpha-1 antitrypsin was slightly lower in the new than control fortifier group after 21 days of full enteral feeding, with a geometric mean and standard deviation of 1.52 ± 1.32 vs 1.82 ± 1.44 mg/g stools (P = .01). There was no significant difference in faecal calprotectin (median [Q1-Q3] of 296 [136-565] μg/g stools in both groups combined at study day 21). Faecal elastase-1 was lower in the new fortifier than control fortifier group (202.5 ± 1.6 vs 257.7 ± 1.5 μg/g stools, P = .016).Conclusion: Mean values for each parameter were within the ranges in healthy term infants, indicating favourable markers of gastrointestinal status in both groups. In addition, for faecal calprotectin, the relatively high concentration observed in preterm infants fed fortified human milk suggests that the threshold level for detecting necrotising enterocolitis should be revised. 46. Nutritional implications of trans fatty acids during perinatal period, in French pregnant women

Abstract

Some studies have demonstrated the transfer of trans fatty acids (TFA) across the human placenta. It was suggested that TFA might disturb the metabolism of essential fatty acids (EFA) in fetus and consequently might affect intrauterine human growth more or less according to the TFA intake level of the mother. In this context, the objective of this study was to assess, for French pregnant women, possible impact of their TFA intake on parameters of their term infants: 1/ TFA composition of the umbilical cord (plasma lipids and parietal phospholipids), 2/ birth weight and head circumference. The TFA composition of maternal and umbilical plasma lipids, and parietal phospholipids of umbilical cord were determined by gas chromatography, associated with a thin-layer chromatography for the phospholipids. Because TFA content of adipose tissue is a reliable biochemical indicator of the usual TFA intake level, maternal adipose tissue was also analyzed. Trans fatty acid (TFA) percentage was significantly higher (p = 0.001) in maternal (0.9% of total fatty acids) than in umbilical plasma total lipids (0.6%) thus confirming their placental passage. Moreover, trans isomer pattern of cord plasma lipids was different from the maternal’s one, especially regarding trans isomers of linoleic acid (9c,12c-18:2). Percentage values of 9c,13t + 9t,12t and 9t,12c isomers were respectively, 2 and 3 times higher (p < 0.001) in umbilical than in maternal plasma (Except trans 16:1 acids). All trans isomers observed in umbilical plasma lipids were detected in both parietal and vessel (vein and arteries) phospholipids of umbilical cord. Nevertheless, 9c13t + 9t12t isomer mix level was significantly higher (p < 0.001) in artery than in vein TPL. Moreover, in artery TPL, percentage value of 9c13t+9t12t isomer mix was inversely correlated (r = - 0.703, p = 0.003) with arachidonic acid content. Nevertheless, for this French population, there was no relation between either weight or head circumference of newborn and the TFA levels in both adipose tissue and plasma lipids of their mother.
