Objectives: This prospective study aims to compare the diagnostic value of different preoperative examinations in patients who underwent TLE surgery for treatment-resistant temporal lobe epilepsy, following histopathological evaluations, and to evaluate the seizure-freedom rates observed during the postoperative follow-up.

Hipokampal skleroz dirençli temporal lob epilepsisinde en sık izlenen histopatolojik bulgudur. Hipokampal sklerozun izlendiği vakalarda etyolojide sıklıkla febril konvulziyon yer almaktadır. Hipokampal sklerozun saptanması cerrahi açısından iyi prognostik faktördür. Biz çalışmamızda retrospektif olarak ilaç tedavisine direçli temporal lob epilepsisi nedeniyle opere edilen, patoloji sonucu hipokampal sklerozla uyumlu 44 vakayı değerlendirdik. Çalışmamızda hipokampal sklerozda ortak etyolojiyi saptamak, preoperatif tetkiklerin birbirleri ve cerrahi odakla uyumunu değerlendirmeyi amaçladık. Bu amaçla iktal semiyoloji, interiktal, iktal ve postiktal EEG ile MR, MR Spekstroskopi ile PET değerlendirmeleri yapıldı. Varolan PET görüntüleri tekrardan SPM analiz yöntemiyle kantifiye edilerek bu yöntemin lokalizasyona olan katkısının değerlendirilmesi planlandı. Çalışmamızın sonucunda sırasıyla Kr MR ile SPM- PET cerrahi odakla en yüksek uyumu gösterdi. PET görüntüleri SPM yöntemi ile analiz edildiğinde cerrahi odağı saptama başarısı artmaktaydı. Cerrahi sonrası nöbetsizlik Engel sınıflamasına göre yapıldı. 6. ayda olguların % 85'i, 1. yılda % 83'ü, 2. yılda % 79'u Engel Sınıf 1 olarak izlendi. Sonuç olarak cerrahi odağın iyi belirlenebilmesi için preoperatif tetkikler multidisipliner yaklaşımla bir bütün olarak değerlendirilmelidir. Hipokampal skleroza bağlı TLE olgularında cerrahi odağın doğru belirlenmesiyle nöbet kontrolü iyi bir şekilde sağlanabilmektedir. The most common histopathological finding on temporal lobe epilepsy is hippocampal sclerosis and the etiology of hippocampal sclerosis is frequently FC. Hippocampal sclerosis is surgically good prognostic factor. This study is retrospective and formed by 44 drug-resistant TLE cases which operated and their pathology results show hippocampal sclerosis. In this study, our goal is to evaluate common etiological factors associated HS, the correlation among each preoperative test and surgical zone. From this perspective we perform ictal semiology, interictal, ictal, postictal EEG and MR, MR spectroscopy and PET evaluations. We quantify all of the PET screening with SPM analysis method and try to find the benefits of this method to localization value. In result, respectively Cr-MR and SPM-PET shows highest consistency by surgical site. PET screening analysed by SPM method shows better success on stating the surgical site. Post-surgical outcome classified by Engel's classsification. 85% of the cases on 6. month, 83% of first year, 79% of second year diagnosed as Engel class I. Consequently for the better determination of surgical site one must evaluate preoperative diagnostic methods by multidisciplinary approach as whole. TLE caused by hippocampal sclerosis could control effectively by proper determination of surgical area.

Objectives: Epileptic seizures diagnosed wth clinical and electroencephalographic ( EEG) signs were classified according to ILAE 1981, Luders, and ILAE 2001 systems in the present study, the aim of which was to compare classification systems by means of preictal and postictal consistency, and to determine deficiencies of the classification systems.

Epilepsi cerrahisi, ilaca dirençli nöbetleri olan hasta grubunda uygulanmaktadır. Hastaların yaklaşık %20'sinin ilaca dirençli nöbetleri olup, hastaların büyük çoğunluğu (%65-90) rezektif cerrahiden belirgin fayda görürler. Epileptik odak lokalizasyonu, klinik değerlendirme, uzun süreli video-EEG monitorizasyon, nörogörüntüleme yöntemlerinden Kr MR, MRS, PET, nöropsikolojik testleri ile yapılmaktadır. Biz çalışmamızda prospektif olarak, ilaç tedavisine dirençli MTLE nedeniyle temporal lob cerrahisi geçiren ve histopatolojik olarak değerlendirilen hastaların preoperatif tetkiklerinin korelasyonu ve postoperatif izlemlerinde nöbetsizlik oranlarını değerlendirmeyi amaçladık. Gazi Üniversitesi Tıp Fakültesi Nöroloji Anabilim Dalı, Erişkin Epilepsi Monitorizasyon Ünitesinde ilaca dirençli TLE tanısıyla izlenen ve cerrahi adayı olarak belirlenen 35 hasta prospektif olarak değerlendirilmiştir. Çalışmamızın sonucunda epileptik odağı tespit etmede preoperatif tetkiklerden PET, Kr MR tetkiklerinin video-EEG'ye benzer, rutin EEG ve MRS'in ise bunlara göre daha düşük oranda katkı sağladığını gözlemledik. Çalışmamızda görüntüleme yöntemlerini iktal EEG ile korele ettiğimizde epileptik odağı tespit etmede PET tetkikinin lateralizasyon değeri oldukça yüksek değerlerde olup duyarlılığı (%100) olarak tespit edilmiştir. Kr MR epileptik odağı tespit etmede duyarlılığı %97,1; rutin EEG'in duyarlılığı %82,9; MRS'in duyarlılığı %79,4 olarak tespit edilmiştir. Cerrahi sonrası olgularımızın %71,4'ünde hipokampal skleroz tespit edilmiştir. Çalışmamızda hastalarımızda cerrahi sonrası nöbetsizlik oranları 6. ay ve 1. yıl %82,8'si Engel Ia olarak sınıflanırken, 2. yılda bu oran %74,3 olarak belirlenmiştir. Sonuç olarak cerrahi adayın iyi belirlenmesi, preoperatif tetkiklerin ve sonuçların birbirleriyle uyumu iyi cerrahi prognoz açısından oldukça önemlidir. İlaca dirençli temporal lob epilepsi tanısı ile izlenen olgularda cerrahi ile nöbet sıklığının belirgin azaldığı, hastaların yaşam kalitesini artırdığı ve kullandıkları ilaç sayısında azalma sağladığı gözlenmiştir. Epilepsy surgery is an advanced treatment approach for patients with drug resistant seizures, who comprise 20% of epilepsy patients. Clinical evaluation, long-term video-EEG monitoring, neuroimaging modalities such as MRI, MRS and, PET CT and neuropsychological testing are used in order to localize the epileptic focus. In this study, we aimed to evaluate the correlation between preoperative tests and histopathological results, together with the clinical outcomes of intractable MTLE patients, who underwent temporal lobe resection surgery. Thirty-five patients with intractable TLE were included. As a result of our study; the contribution of PET CT and Brain MRI for the localization of the epileptic focus was similar to Video EEG, while routine EEG and MRS was found to have lesser benefit. PET CT had a high lateralization value and significant sensitivity (100%) while this value was 97.1% for Brain MRI, 82.9% for routine EEG, and 79.4% for MRS. Postoperative histopathological assessment of our series were compatible with hippocampal sclerosis in 71.4%. In our study, the outcome after 6 months and one year according to Engel classification was Ia in 82.8% of the patients, however it was 74.3% after two years.
