[eng] Objective: In this meta-analysis, our goal was to examine the diagnostic utility of bilirubin in identifying complicated from uncomplicated pediatric appendicitis. Materials and methods: Using the databases Embase, PubMed, Scopus, and Cochrane, we carried out a thorough literature search up to 2022. Studies comparing complicated appendicitis (CA) and simple appendicitis (SA) in terms of hyperbilirubinemia in the pediatric population were included. Results: A total of 5 studies with 2740 acute appendicitis patients (1097 complicated appendicitis and 1643 simple appendicitis) were included in this meta-analysis. Five studies have discussed the diagnostic value of total bilirubin (TB). When compared to simple appendicitis, complicated appendicitis had a significantly higher TB count (I2=94%), (WMD=0.18, 95% CI -0.00 to 0.37; P=0.05), DB count (I2=0%), (WMD=0.11, 95% CI 0.04 to 0.18; P=0.002), and IB count (I2= not applicable), (WMD=0.04, 95% CI 0.01 to 0.07; P=0.02). Conclusions: In conclusion, in this meta-analysis, total bilirubin, direct bilirubin, and indirect bilirubin values were higher in complicated appendicitis compared to simple appendicitis. Both total bilirubin and direct bilirubin can be used as diagnostic parameters in childhood appendicitis to differentiate complicated appendicitis from simple appendicitis., [spa] Objetivo: En este metanálisis, nuestro objetivo fue examinar la utilidad diagnóstica de la bilirrubina para identificar la apendicitis pediátrica complicada frente a la no complicada. Materiales y métodos: Utilizando las bases de datos Embase, PubMed, Scopus y Cochrane, realizamos una búsqueda bibliográfica exhaustiva hasta 2022. Se incluyeron estudios que compararan apendicitis complicada (AC) y apendicitis simple (AS) en términos de hiperbilirrubinemia en la población pediátrica. Resultados: Se incluyeron en este metanálisis un total de 5 estudios con 2740 pacientes con apendicitis aguda (1097 complicada y 1643 simple). Cinco estudios han discutido el valor diagnóstico de la bilirrubina total (TB). En comparación con la apendicitis simple, la apendicitis complicada tuvo un recuento de TB significativamente mayor (I2=94 %), (DMP=0,18, IC del 95 %: -0,00 a 0,37; P=0,05), recuento de DB (I2=0 %), (DMP = 0,11, IC del 95 %: 0,04 a 0,18; P = 0,002) y recuento de BI (I2 = no aplicable), (DMP = 0,04, IC del 95 %: 0,01 a 0,07; P = 0,02). Conclusiones: Tanto la bilirrubina total como la directa pueden usarse como parámetros de diagnóstico en la apendicitis infantil para diferenciar la apendicitis complicada de la apendicitis simple.

Amaç: Omfalosel yenidoğanlarda en sık görülen konjenital karın duvarıdefektlerindendir. Tedavisi için faklı yaklaşımlar mevcuttur. Bu çalışmadaomfalosel tanısı konulan hastaların tedavi yaklaşımları ile ilgili 10 yıllıkdeneyimlerimizi literatür eşliğinde sunmayı hedefledik.Gereç ve Yöntemler: 2009-2018 yılları arasında omfalosel nedeni ileameliyat edilen hastaların cinsiyeti, doğum ağırlığı, eşlik eden ek konjenitalanomaliler, defekt çapı ve hastanede yatış süresinin morbidite-mortaliteaçısından uygulanılan tedavi metodları üzerine etkisi retrospektif olarakdeğerlendirildi.Bulgular: Omfalosel tanılı 6’sı erkek, 6’sı kız toplam 12 hastaçalışmaya dahil edildi. Hastaların ortalama doğum ağırlığı 2330±890 grambulundu. Hastaların %58’inde eşlik eden ek konjenital anomaliler vardı. Defektçapı ortalama 6,3±2,3 cm bulundu. Defekt çapı ve eşlik eden ek konjenitalanomalilerin sayısı arttıkça mortalite oranlarının arttığı görüldü. Tedavide ensık primer kapama yöntemi tercih edildi. Silo yöntemi ile tedavi ettiğimiz tümhastalar kaybedildi. Gümüş sülfadiazin kullanarak konservatif yaklaştığımızbüyük defekte sahip üç hastanın tedavileri sonrası ise herhangi birkomplikasyon görülmedi.Sonuç: Literatürde omfalosel tedavisinde birçok yöntemin kullanıldığıbildirilmektedir. Tedavide kullanılan yöntemlerin sağkalım üzerine etkilerivardır. Özellikle silo yöntemi ile tedavi edilen büyük defekte sahip hastalardamortalite oranları daha yüksektir., Aim: Omphalocele is the most common congenital abdominal wall defect innewborns. There are several approaches to treatment. In this study, we aimed topresent our 10 years experience about treatment approaches of omphalocelepatients with the literature.Materials and Methods: The effect of gender, birth weight, concomitantcongenital anomalies, defect size and length of hospital stay on the treatmentmodalities of patients who underwent surgery for omphalocele between 2009-2018was assessed retrospectively.Results: Twelve patients (6 males, 6 females) with omphalocele wereincluded in the study. The mean birth weight of the patients was 2330 ± 890grams. 58% of the patients had concomitant congenital anomalies. The meandiameter of the defect was found to be 6.3 ± 2.3 cm. It was observed that themortality rates increased as the defect diameter and the number of accompanyingcongenital anomalies increased. The most common method of primer closure waspreferred. All the patients treated with the silo method were lost. There wereno complications after the treatment of three patients with large defects whichwe conservatively treated using silver sulphadiazine.Discussion and Conclusion: Several methods have been reported in theliterature in the treatment of omphalocele. The methods used in the treatmenthave effects on survival. Mortality rates are higher in patients with largedefects especially treated by the silo method.

Henoch-Schonlein purpura are many and varied symptoms, also it was followed in children policlinics and hospitalized diseases for an important role. We aimed to determination of the patients with Henoch-Schonlein purpura by examination of their clinical analyses. Material and Method: The medical records of 212 patients with Henoch-Schonlein purpura in our polyclinics have been retrospectively studied. These patients were analyzed age groups, diagnosed seasons, held systems, laboratory findings, the treatments, complications and prognosis. SPSS 13 statistical software package was used for data analysis. Results: The mean age of the patients was 9.33 +/- 3.20 years. The disease was more frequently demonstrated in fall and winter seasons. There was no statistically assosiation between age groups with season, joint involvement, central neryeous system involvement, gastrointestinal system involvement and scrotal involvement but there was a positive correlation between age groups and renal involvement Conclusion: The treatment duration increases with the age of the patient and the necessity to use a combined treatment (antihistaminic, non-steroid anti-inflammatory treatment and steroids) increases. Familial Mediterranean Fever (FMF) is an autosomal recessive disease that clinically characterized by periodic abdominal pain, fever and arthralgia. Wide variety of mutations have been described in MEFV gene which is known to be responsible from FMF. In present study, 12 MEFV mutations [E148Q, P369S, F479L, M680I (G/C), M680I (G/A), I692del, M694V, M694I, K695R, V726A, A744S, R761H] have been screened. DNA samples were obtained from a total of 332 children, who were clinically suspected as FMF. Mutation analyses of MEFV gene were carried out with reverse hybridization method. In 113 children (mean age 11.5 years), mutations have been detected. In 60 children E148Q (4 homozygous), in 19 M694V (4 homozygous), in 16 P369S (none), and in 13 children V726A (none homozygous) mutations were detected. Among Familial Mediterranean Fever -suspected 332 children 104 fulfi lled diagnostic criteria for FMF and detected MEFV gene mutations with decreasing frequency were; E148Q (30.8%), M694V (18.3%), P369S (10.6%), V726A (8.6%), A744S (2.9%), R761H (2.9%), M694I (1.9%), K695R (1.9%) and I692del (1.0%), respectively. No mutation was detected in 15 (14.4%) FMF children while, two different MEFV mutations were detected together in 13 (12.5%) patients and these patients were accepted as a compound heterozygous. In conclusion, in our patients E148Q mutation was found higher than M694V mutation that has been detected commonly in patients with FMF. Ancak Ailevi Akdeniz Atesi süphesi ile mutasyon analizi istenen 332 çocuktan toplam olarak 104\'ünde tanı kriterlerine göre hastalıgın tanısı klinik olarak dogrulandı. Ailevi Akdeniz Atesi tanısı alan hastalarda gen mutasyonları sıklık sırasına göre; E148Q (%30.8), M694V (%18.3), P369S (%10.6), V726A (%8.6), A744S (%2.9), R761H (%2.9), M694I (%1.9), K695R (%1.9) ve I692del (%1.0). Ailevi Akdeniz Atesi tanısı konan hastaların 15\'inde (%14.4) hiçbir mutasyon saptanmazken, 13\'ünde (%12.5) iki farklı mutasyon aynı anda belirlenerek bilesik heterezigot olarak tanımlandı. Sonuç olarak hastalarımızda E148Q mutasyonuna, normalde en sık görülen M694V mutasyonundan daha sık rastlandı. Bu farklılık demografi k yapı veya metot ile iliskili olabilir.

The aim of this study was to compare hot water baths with “dressing a wound with povidin-iodin” at wound healing after primer excision of sacrococciygeal pilonidal sinus (SPS). Thirty six patients with SPS are randomly treated prospective by primary excision and marsupialization. During postoperative period, 18 patients (Group 1) are followed with hot-water and other 18 patients (Group 2) with povidine-iodine dressing. Healing periods (length, wideness and deepness), complications, recurrence and wound closure times between the two groups analyzed. Average patient ages are 25.5(15-45) in group 1 and 22.6(16-37) in group 2. On the postoperative fi rst day, mean wound lengths are 3.85 cm and 3.68 cm, mean wound broadness are 1.77 cm and 1.5 cm and mean wound deepness are 1.47 cm and 1.37 cm in order for fi rst and second groups. Wound closure times are 38.7 (27-59) and 40.8 (23-62) days for fi rst and second groups. There is no signifi cant difference between the two groups (p>0.05). In conclusion, hot water baths may be a alternative treatment method contributing positively to wound healing. Key words: Pilonidal sinus, hot water baths, povidon-iodin wound dress, Bu çalısmada sakrokoksigeal pilonidal sinüsler (SPS)\'de primer eksizyon ve marsupializasyon sonrası yara iyilesmesinde sıcak su oturma banyosu ve povidiniodin pansumanının etkinliginin karsılastırılması amaçlandı. SPS nedeni basvuran 36 hastaya primer eksizyon ve marsupiyalizasyon yapıldı ve postoperatif dönemde randomize prospektif olarak 18 hastaya günde iki kez sıcaksu oturma banyosu (Grup 1), diger 18 hastaya (Grup 2) iki günde bir povidon-iodinle pansuman yapıldı. Gruplar arasında yara iyilesme hızları (uzunluk, genislik, derinlik), komplikasyon, nüks ve kapanma süreleri karsılastırıldı. Ortalama hasta yasları; Grup 1\'de 25.5 (15-45) ve Grup 2\'de 22.6 (16-37) idi. Postoperatif 1. gün; Grup 1\'in ortalama yara uzunlugu 3.85 cm, ortalama yara genisligi 1.77 cm, ortalama yara derinligi 1.47 cm iken, Grup 2\'de sırasıyla 3.68 cm, 1.5 cm ve 1.37 cm\'di. Ortalama yara kapanma süreleri, Grup 1\'de 38.7 (27-59) gün, Grup 2\'de 40.8 (23-62) gündü. Degerlendirmede gruplar arasında istatistiksel olarak anlamlı fark saptanmadı (P>0.05). Sıcak su oturma banyosu her ne kadar istatiksel olarak anlamlı bir fark görülmemis olsada yara iyilesmesinde olumlu yönde etkileyen, pratik alternatif bir tedavi yöntemi olabilir.

Giriş: Ciddi hipospadias komplikasyonları çok sayıda operasyon geçirmesine rağmen hâlâ geniş üretral fistülleri, kordinüksü, penisten üretra rekonstrüksiyonunu sınırlandıracak ciddi skarlarıyada penis cildinde defekti bulunan hastalar olarak tanımlanabilir.Bu yazıda ciddi hipospadiaskomplikasyonu bulunan 6 hastaya uygulanan yeni flep teknikleri anlatılmaktadır. Yöntem: Ortalama yaşları 15,5 olan hipospadias cerrahisi komplikasyonu bulunan 6 hasta daha önce ortalama 4,5 defa hipospadiasla ilgili operasyon geçirmişlerdi. Hastaların 4’üne Kutlay tekniği ile onarım yapılırken, bir hastaya randompaternli ada şeklinde penil skin flebiile ve diğer hastaya ise deri greftiile prefabrike edilmiş groinflebi ile onarım yapıldı. Sonuçlar: Hastaların birinde ameliyat sonrası 1. ayda meatal darlık tespit edildi. Devamlı ve aralıklı üretralkateterizasyonlameatal darlığın tedavisi yapıldı. Bir başka hastada ise ameliyat sonrası gelişen yumuşak doku enfeksiyonu nedeniyle yara kenarlarından pürülan akıntı gelmeye başlandı. Bu hastada ameliyat sonrası 6. ay da subkoronal bölgede 0,3cm liküretrokütanöz fistül gözlendi. Diğer hastalarda kordinüksü, fistül, üretral darlık yada başka bir komplikasyon gözlenmedi. Çıkarımlar: Ciddi hipospadias komplikasyonu bulunan hastalarda deformitenin her bir komponentini düzeltebilecek, olguya özgü ve güvenilir tedavi seçenekleri kullanılmalıdır. Deformitenin şiddeti arttıkça tedavide daha komplike ve çok seanslı teknikler tercih edilebilir., Background: Severe hypospadias complications are defined as large urethral fistulae, chordee recurrence, and severe scar limiting urethral reconstruction through the penis, penis skin defects and complications persisting in spite of multiple operations. In this study, new flap techniques applied on six patients with severe hypospadias complications have been introduced. Methods: Six patients with hypospadias surgery complications, had undergone hypospadias repair operations on average 4,5 times. Four cases had undergone repair using the Kutlay technique, one was repaired by island-formed penile skin flap, and the other was repaired using a groin flap prefabricated by skin graft. Results: In one of the patients, meatal stenosis was found in the first postoperative month. It was treated by continuous and intermittent urethral catheterization. In another patient, purulent drainage was observed secondary to postoperative soft tissue infection. A 0.2 cm urethra-cutaneous fistula was observed on the subcoronal region in the postoperative sixth month in this patient. Other patients had no chordee recurrence, fistula, urethral stenosis or other complications. Conclusions: In patients with severe hypospadias complications, case-specific and reliable treatment options that may repair each component of the deformity have to be used. As the severity of the deformity increases, more complicated and multiple-session techniques may be preferred for treatment.

Abstract. Isolated fallopian tube torsion is seen rarely in sexually inactive early-adolescent girls. It can present with lower abdominal pain and pelvic mass. In this report, we aimed to draw attention to this rare clinical condition in childhood by presenting a 14-year-old girl. Isolated fallopian tube torsion should be considered in adolescent patients who have colic pain, especially unilateral pelvic localization with abrupt onset. Early diagnosis and prompt surgical intervention can save fallopian tubes in tubal torsion. Key words: Fallopian Tube, pelvic mass, torsion

Ingestion of foreign bodies (FBs) is a significant problem that causes morbidity and mortality in childhood. The aim of this retrospective study was to report our experience of foreign body ingestion in pediatric patients. The medical records of 165 patients who were hospitalized for foreign body (FB) ingestion in pediatric and chest surgery departments between 2005 and January 2010 were evaluated retrospectively. X-ray films and abdominal ultrasound scan were used for the diagnostic approach of the patients. The common complaints were odynophagia-dysphagia (n=107), hypersalivation (n=81), cough (n=21), vomitting (n=20) and asymptomatic in 34 patients. Radiological examinations showed that FB was located in the esophagus in 81.2% (n=134) of the patients, in the stomach of 6.74% (n=11) patients, in the intestinal segments in 10.4% (n=17), in the rectum in 1.21% (n=2) and in the liver parenchyma 0.6% (n=1) patients. Endoscopic examination performed in 134 (81.2%), FB proceeded uneventfully in 23 (13.9%) in follow up period and 8 (4.8%) patients underwent surgery. The type of ingested FB varied widely. The coins (n=54, 32.7%) and pieces of plastic toys (n=29, 17.5%) were the most frequently ingested FBs. Foreign body ingestion is a major problem in childhood. Management depends on carefully and close follow up for complications and favorable treatment choice.Key words: Foreign body, childhood, esophagus, dysphagia

Amac: Bu calismada, spesifik klinik ozellikleri ve ilginc klinik ve radyolojik tablolari olan ve son evrede tani konulduktan sonra ameliyat edilen diyafragmatik hernili hastalar analiz edildi. Ca­lis­ma­pla­ni:­Ocak 2006 ve Agustos 2010 tarihleri arasinda son evrede tani konan ve klinigimizde ameliyat edilen ucu eriskin ve besi cocuk toplam sekiz diyafragmatik hernili olgu (4 erkek, 4 bayan; ort. yas 12.4±24 yas; dagilim 9 ay-28 yil) retrospektif olarak degerlendirildi. Tum hastalarda laparotomi ve torakotomi sonrasinda herni reduksiyonu ve primer onarimi yapildi. Bul gu lar: Cocuklarin tanisi neonatal donem sonrasinda konuldu. Uc olguya eriskin donemde tani kondu. Bes hastada sag tarafli dogustan Morgagni hernisi; bir hastada hiatal herni ve iki hastada sol tarafli Bochdalek hernisi mevcuttu. Ameliyattan sonra 5 ila 19. gunler arasinda tum hastalar taburcu edildi. Hastanede ortalama kalis suresi 9.8 gundu (dagilim 7-23 gun). Hastalar alti ay ila iki yil sureyle takip edildi ve anlamli bir solunum veya gastrointestinal yakinma kaydedilmedi. Morgagni hernisi olan uc olguda da herhangi bir sorun tespit edilmedi. So­nuc:­Dogustan olan diyafram hernileri, cogunlukla neonatal donemde gorulen patolojiler olup, sonrasinda bu hernilerin tanisi guclesebilir. Takip edilen ve deneyimli uzmanlar tarafindan klinik olarak tedavi edilen olgularda mortalite ve morbidite oranlari dusuktur.

Background: In this clinical retrospective study, we aimed to investigate our experinces and whether capitonnage is an effective therapy method for a pulmonary hydatid cyst or not. Material and methods: A total of 412 patients with hydatid cyst operated in our hospitals were evaluated retrospectively between January 2003 and January 2011. In order to create a study group to compare the hydatid cyst operations with and without capitonnage in our department, 60 uncomplicated patients with the diagnosis of hydatid cyst who had undergone operations in the previous two years, were divided into two groups; while no capitonnage was performed and bronchial leaks were closed in one group, standard cystotomy plus the capitonnage operation was performed in the second group. All patients underwent surgery. Results: In many patients, one or more symptoms were present on admission (339 cases, 82%). Perforated cysts/nonperfore cysts rate was statistically significant (p = 0.001). There was no statistical difference between patients with or without capitonnage in terms of morbidity rates between patients with or without capitonnage (p = 0.041). However, morbidity rates were higher in the group without capitonnage. There were found statistically significant between capitonnage and non capitonnage groups in terms of length of hospital stay (p = 0.001). Conclusions: In the surgical treatment, resection should be avoided as much as possible. An average time of 3-5 minutes should be allocated and capitonnage should be performed. Capitonnage should always be performed in the surgical treatment of hydatid cyst. We believe that povidone iodine per se provides sufficient disinfection.

Objective: Clinical approach and therapeutic methods are important in cases with complicated hydatid cysts of the lung. This study was designed to retrospectively investigate cases with hydatid cysts, thereby discussing diagnostic methods, treatment modalities, and rates of morbidity and mortality in line with the literature. Methods: 176 cases with perforated hydatid cysts, who presented to our clinic and underwent surgery between 2003 and 2011, were included in the study. There were 71 (40.34%) females and 105 males (59.66%) with a mean age of 27.80 +/- 14.07. The most common symptom was dyspnea (44.31%) and the most common radiological finding was the water lily sign (21.02%). 88.06% of the cases were surgically treated by Cystotomy+closure of bronchial opening+capitonnage, 3.97% by wedge resection, 4.54% by segmentectomy and 3.40% by lobectomy. Results: The cysts exhibited multiple localization in 24 cases (13.63%), bilateral localization in 14 cases (7.95%), with the most common localization (43.75%) being the right lower lobe. While the hydatid cyst rupture occurred due to delivery in three (1.70%), trauma in 11 (6.25%), and iatrogenic causes in seven (3.97%) cases, it occurred spontaneously in the rest of the cases (88.08%). Fourteen of the cases with spontaneously occurring rupture (7.95%) were detected to have received anthelmintic treatment for hydatid cyst during the preoperative period (albendazole). The rate of morbidity was 27.27% and the rate of mortality was 1.13% in our study. Two cases recurred during a one-year follow-up (1.13%). Conclusion: Hydatid cyst rupture should be considered in the differential diagnosis of cases with pleural effusion, empyema, pneumothorax and pneumonia occurring in endemic regions. Symptoms occurring during and after perforation lead to errors in differential diagnosis. Performing the surgery without delay favorably affects postoperative morbidity and mortality. While parenchyma-preserving surgery is preferential, there is a need for resection in perforated hydatid cysts.

Familial Mediterranean Fever (FMF) is an autosomal recessive disease that clinically characterized by periodic abdominal pain, fever and arthralgia. Wide variety of mutations have been described in MEFV gene which is known to be responsible from FMF. In present study, 12 MEFV mutations [E148Q, P369S, F479L, M680I (G/C), M680I (G/A), I692del, M694V, M694I, K695R, V726A, A744S, R761H] have been screened. DNA samples were obtained from a total of 332 children, who were clinically suspected as FMF. Mutation analyses of MEFV gene were carried out with reverse hybridization method. In 113 children (mean age 11.5 years), mutations have been detected. In 60 children E148Q (4 homozygous), in 19 M694V (4 homozygous), in 16 P369S (none), and in 13 children V726A (none homozygous) mutations were detected. Among Familial Mediterranean Fever -suspected 332 children 104 fulfi lled diagnostic criteria for FMF and detected MEFV gene mutations with decreasing frequency were; E148Q (30.8%), M694V (18.3%), P369S (10.6%), V726A (8.6%), A744S (2.9%), R761H (2.9%), M694I (1.9%), K695R (1.9%) and I692del (1.0%), respectively. No mutation was detected in 15 (14.4%) FMF children while, two different MEFV mutations were detected together in 13 (12.5%) patients and these patients were accepted as a compound heterozygous. In conclusion, in our patients E148Q mutation was found higher than M694V mutation that has been detected commonly in patients with FMF. The difference may result from demographic features or used methods.
