Çölyak hastalığı, temelde bağırsak emilim bozukluğuyla seyreden ve ortaya çıkan kronikotoimmün bir hastalıktır. Diğer otoimmün hastalıklar ile de birliktelik gösterebilir. Amaç: Bu çalışmada, Hashimoto tiroidit olgularında tiroid fonksiyonları ile kardiyovasküler hastalık (KVH) riski arasındaki ilişkinin araştırılması amaçlanmıştır. ÖZETAmaç: Helicobacter pylori (H. pylori), ilk keşfinden itibaren sindirim sistemi ve diğer sistemlerle ilgili bir çokhastalığın patogenezinde yer almakla suçlanmıştır. İlginç olarak, sindirim sistemi dışı hastalıklarla ilgili literatürünçoğunluğu, özellikle iskemik kalp hastalığına odaklanmıştır. Aterosklerozun ilerlemesinde oksitlenmişdüşük yoğunluklu lipoprotein (ox-LDL) önemli bir rol oynamaktadır. Bu çalışmada, H. pylori eradikasyonununserum ox-LDL düzeyleri üzerine etkisini değerlendirmek amaçlanmıştır.Yöntem: Çalışmaya H. pylori ile ilişkili dispepsisi olan 73 hasta ve 23 sağlıklı gönüllü kontrol grubu olarak dahiledildi. Hastalara bizmut içeren dörtlü tedavi 14 gün süreyle verildi. Serum ox-LDL, LDL ve yüksek yoğunluklulipoprotein (HDL) düzeyleri tedaviden önce ve 8 hafta sonra olmak üzere ölçüldü. H. pylori eradikasyonu14C-üre nefes testi ile doğrulandı.Bulgular: Hasta ve kontrol grubu arasında yaş ve cinsiyet açısından anlamlı fark bulunmadı (sırasıyla p = 0,066ve p = 0,475). H. pylori eradikasyonu öncesi hastalarda serum ox-LDL ve LDL düzeyleri sağlıklı kontrollere göreanlamlı olarak yüksek bulundu (sırasıyla p = 0,04 ve p = 0,046). Düzeltilebilir kardiyovasküler hastalık riskfaktörü taşımayanlarda eradikasyon başarısı sonrası ox-LDL düzeylerinde anlamlı azalma saptandı (p = 0.047).Ancak, diğer hastalar arasında tedavi öncesi ve sonrası serum ox-LDL düzeylerinde istatistiksel olarak anlamlıbir fark yoktu (p> 0.05).Sonuç: H. pylori ilişkili dispepsi hastalarında eradikasyon tedavisinin ateroskleroz progresyonuna sınırlıda olsayararlı etkisi olduğu kanaatindeyiz.Anahtar Sözcükler: Helicobacter pylori; Ateroskleroz; oksitlenmiş LDLABSTRACTObjectives: Since the discovery, Helicobacter pylori (H. pylori) it have been implicated in the pathogenesis ofseveral diseases located both in the digestive and extradigestive systems. Interestingly, the majority of theliterature on extradigestive-related disease mainly focuses on ischemic heart diseases. Oxidized low-densitylipoprotein (ox-LDL) is an important key point in the progression of atherogenesis. The aim of the presentstudy was to investigate whether the eradication of H. pylori infection affects serum ox-LDL levels.Methods: A total of 73 patients with H. Pylori–associated dyspepsia and 23 controls were included in thisstudy. Bismuth quadruple therapy was given for 14 days. Serum levels of ox-LDL, LDL and high-densitylipoprotein (HDL) were measured at baseline and 8 weeks after therapy. Successful eradication was provenwith the 14C-urea breath test.Results: There were no significant differences in terms of age and gender between the patients and controls(p=0.066 and p = 0.475, respectively). Comparing H. Pylori patients to healthy controls with respect to serumox-LDL and LDL levels were significantly higher in the patient group (p=0.04 and p=0.046 respectively). It wasnoticed that oxLDL levels were lower in eradicated patients with no correctable risk factors for cardiovasculardisease after therapy (p=0.047). In this study, we aimed to determine the role of nurses on helping the patients with end-stage renal disease (ESRD) make a good decision about the type of dialysis treatment.Total of 129 (73 women and 56 men) consecutive patients with ESRD who were recommended renal replacement therapy (RRT) were enrolled in the study. Firstly, patients were assessed for preemptive kidney transplantation and contraindications of peritoneal dialysis by a nephrologist. Eighty one patients who were eligible for PD have been trained by a nephrology specialist or a nurse regarding the options of dialysis treatments. The average results of patients according to acceptance of PD were evaluated statistically.Among the patients offered PD-first, only 47% accepted PD. It was seen that after the education given by nephrology specialist and nurse regard to dialysis treatment options, 10 (26%) and 28 (74%) patients have choosen to carry out PD respectively (p=0.004).We think that training by nurse who has educated specially for PD can increase the proportion of patients who will choose PD except special medical indications., Bu çalışmada son dönem böbrek yetmezliği tanısıyla diyaliz planlananlarda ilk tercih tedavisi olarak periton diyalizi seçiminde diyaliz hemşirelerinin rolünü değerlendirmeyi amaçladık.Çalışmaya nefroloji kliniğimizde Ocak 2016- Şubat 2017 tarihleri arasında son dönem böbrek yetmezliği tanısı konularak böbrek yerine koyma tedavisi kararı alınan toplam 129 (73 kadın, 56 erkek) hasta alındı. Periton diyalizi ilk tercih tedavisi için uygun olarak değerlendirilen toplam 81 hasta iki gruba ayrıldı. Hasta ve aileleri, diyaliz tedavi seçenekleri eğitimini nefroloji uzmanı veya eğitimli-tecrübeli hemşire tarafından almışlardır. Hastaların periton diyalizi tercih sonuçları istatistiksel olarak karşılaştırıldı.Periton diyalizi ilk tercih tedavisi için uygun olan 81 hastanın 38 (%47)’i periton diyalizi tercih ettiği görüldü. Periton diyalizi kabul eden hastalardan 28’nin (%74) hemşire eğitiminden sonra, 10’nun (%26) doktor eğitiminden sonra tercih ettiği bulundu (p=0.004).Periton diyalizi için uygun hastalarda diyaliz seçenekleri eğitiminin tecrübeli diyaliz hemşireleri tarafından yapılmasının periton diyalizi hasta oranını arttıracağını düşünmekteyiz.

I N F O R M A T I O N Date of Submission 30.03.2017 Revision: 06.04.2017 Accepted: 04.07.2017, Amaç: Periton diyalizi ünitemizde, son altı yıl içinde sürekli ayaktan periton diyalizi (SAPD) ilişkili peritonit gelişen hastaların peritonit sıklığı ve neden olan etkenlerin araştırılması amaçlanmıştır. Yöntemler: 2011-2017 yılları arasında üniversite hastanemiz periton diyalizi ünitesinde takip edilen toplam 65 SAPD hastası çalışmaya alındı. Bulgular: Çalışmanın kapsadığı 6 yıllık sürede toplam 65 SAPD hastası retrospektif olarak incelendi. Bu hastaların 37’sinde peritonit atağı saptanmazken, 28 hastada en az bir kez peritonit atağı geçirdi. Toplam 49 peritonit atağı görüldü. Takip süresi boyunca 5 hasta peritonit atağı tedavisi sırasında kaybedildi, 6 hasta da ise hemodiyalize geçildi. Sonuçlar: Merkezimizde SAPD peritonitin en sık etkeni gram pozitif mikroorganizmalardı. Takip süresince peritonit insidansı ve kültür negatif atak oranı Türkiye verileri ile benzerdi. Son 10 yıllık verileri değerlendirdiğimize peritonit sıklığı aynı düzeylerde saptandığı görülmektedir. Bu olumsuz durumu düzeltmek için daha çok araştırmaya ihtiyaç duyulmakla beraber hasta ve sağlık çalışanları ile birlikte toplumun hijyen ve akıllı ilaç kullanımı konusunda eğitimi sağlanmalıdır.

WOS: 000411458800018, Intravenous immunoglobulin (IVIG) has been developed for the treatment of hypogammaglobulinemia and is also used for many systemic inflammatory and autoimmune disorders. Acute kidney injury is a rare complication of the use of IVIG, and is mainly caused by stabilizing agents in the protein formulation. A complete recovery of renal function is seen within days. We present the case of an 81-year old woman, treated for acute inflammatory demyelinating polyneuropathy (Guillain-Barre syndrome) that caused loss of sensation and muscle weakness in the leg. She was treated with IVIG for five days. Renal failure developed during treatment. The patient's acute kidney injury resolved a month later. The patients should be monitored closely after IVIG therapy for renal injury.

Hereditary hemorrhagic telangiectasia (HHT), also known as OslerWeber-Rendu syndrome is an autosomal-dominant multi-organ vascular dysplasia and results in a tendency for bleeding. The worldwide prevalence is 1-2 cases per 100 000 population. However most patients have a normal life expectancy. HHT can lead to serious or even life-threatening complications such as stroke, cerebral abscess or massive haemorrhage in almost 10% of patients. Here, we reported a patient presented with HHT causing iron deficiency anemia, Herediter hemorajik telanjiektazi (HHT), diğer adıyla Osler Weber Rendu Sendromu otozomal dominant geçişli, ailesel, sistemik bir vasküler displazi hastalığıdır (1). Klinik tanı Curaçao kriterlerine dayanarak konulmaktadır. Ayda üçten fazla spontan epistaksis, mukokutanöz telanjiektaziler ve aile öyküsü olması tanı triadını oluşturur (2). Sıklığı 1-2/100 000 olarak bildirilmektedir (3). Tedavisi destekleyici tedavi şeklindedir ve komplikasyonlardan korunmak amaçlanmaktadır. Aim: In this study, we aimed to determine the relationship between inflammation and both malnutrition and atherosclerosis among the patients on hemodialysis and peritoneal dialysis Materials and Methods: Total of 74 (27 women and 47 men) consecutive hemodialysis (HD) and peritoneal dialysis (PD) patients were enrolled in the study. Patients with known previous malign neoplasms and chronic inflammatory diseases were excluded. Patients were groupped according to the presence of atherosclerosis and malnutrition. While evaluating acromegalic patients, a discordance can be seen between insulin like growth factor-1(IGF-1) and GH response to oral glucose tolerance test (OGTT). In this study, the supressions and discordances of GH between OGTT and intravenous glucose tolerance test (IVGTT) were invastigated in acromegalic patients. Oral GTT and IVGTT performed to 18 acromegalic subjects. Serum levels of GH and glucose were analysed at 0, 30, 60, 90 and 120 minutes during OGTT and IVGTT, and compared. The supression of the serum GH levels during IVGTT were insignificantly higher than the serum GH levels during OGTT (p> 0.05). At 30th minutes, serum levels of glucose in IVGTT was significantly higher than OGTT (p 0.05). There were obvious differences between OGTT and IVGTT in patients with acromegaly in terms of glucose elevation in time. Whereas, growth hormone levels did not reach statistical differences, even if growth hormones were found higher at all times in OGTT than in IVGTT, Akromegali hastaları değerlendirilirken, oral glukoz tolerans testine (OGTT) insülin benzeri büyüme faktörü-1 (IGF-1) ve büyüme hormonu (BH) yanıtlarında uyumsuzluk görülebilir. Bu çalışmada akromegali hastalarında, intravenöz glukoz tolerans testi (IVGTT) ve OGTT arasında BH baskılanması ve uyumsuzluğun değerlendirilmesi amaçlanmıştır. Onsekiz akromegali hastasına OGTT ve IVGTT uygulandı. OGTT ve IVGTT uygulanması takiben 0, 30, 60, 90 ve 120’inci dakikalarda serum BH ve glukoz düzeyleri çalışılarak istatistiksel karşılaştırılmalar yapıldı. IVGTT sonrası serum BH düzeyleri baskılanması, OGTT sonrası serum BH düzeyleri ile karşılaştırıldığında istatistiksel anlamlı olmayan bir yükseklik vardı (p> 0.05). IVGTT sonrası 30’uncu dakikadaki serum glukoz değerleri, OGTT sonrasına göre anlamlı olarak yüksek bulundu (p 0.05). Akromegalili olgularda IVGTT ile anlamlı yükseklikte glukoz artışları saptanmaktadır. Ancak OGTT’ye göre BH değerlerinde baskılanma fazla, ancak anlamlı düzeyde değildir. Yeterli glukoz artışı sağlanamayan OGTT yapılmış olgularda gastroenteropankreatik yolağı devreden çıkaran IVGTT tercih edilebilir.

Background/Aims: In our study, we aimed to assess the effect of vitamin E and C supplementation to triple and quadruple Helicobacter pylori eradication regimens. Materials and Methods: Four hundred patients with H. pylori infection were classified into four groups. Patients in group A (n=100) received amoxicillin, clarithromycin, and lansoprazole for 2 weeks. In group B, patients (n=100) received vitamins C and E for a month, in addition to amoxicillin, clarithromycin, and lansoprazole for 2 weeks. Patients in group C (n=100) received amoxicillin, clarithromycin, lansoprazole, and bismuth subcitrate for 2 weeks, whereas those in group D (n=100) received vitamins C and E for a month, in addition to amoxicillin, clarithromycin, lansoprazole, and bismuth subcitrate for 2 weeks. H. pylori eradication was assessed with the C14 urea breath test 2 months after the end of the therapy. The eradication rate was assessed using per-protocol (PP) and intention-to-treat (ITT) analyses. Results: Three hundred forty-eight patients finished the study. The eradication of H. pylori was achieved in 63 of 84 patients (75%) by PP and 63 of 100 (63%) by ITT analysis in group A, 60 of 84 (71.4%) by PP and 60 of 100 (60%) by ITT analysis in group B, 72 of 89 (80.9 %) by PP and 72 of 100 (72%) by ITT analysis in group C, and 76 of 91 (83.5%) by PP and 76 of 100 (76%) by ITT analysis in group D. There was no remarkable change between groups A and B (p>0.05). Similar results were also found between groups D and C (p>0.05).Conclusion: This study revealed that supplementing vitamins C and E to either the triple or quadruple therapies did not provide an additional advantage for achieving significantly higher eradication rates for H. pylori

Amaç: Venöz tromboembolizmin (VTE) kalıtsal etyolojik nedenleri arasında en sık görülen faktör V Leiden (FVL), Protrombin G20210A (PTM) ve Metilentetrahidrofolat redüktaz (MTHFR) C677T gen mutasyonları ile tromboz yerleşim bölgeleri arasındaki ilişkinin değerlendirilmesi. Yöntemler: Yüz kırk dört idiopatik VTE hastasında FVL, PTM ve MTHFR gen mutasyonu geriye dönük alarak tarandı. Hastalar tromboz bölgelerine göre pulmoner emboli (PE), derin ven trombozu (DVT), PE ile DVT birlikteliği, serebral (SVT) ve intraabdominal ven trombozu olarak gruplandırıldı. Genetik mutasyonlar ile VTE lokalizasyonları arasındaki ilişki analiz edildi. Bulgular: Hastaların ortalama yaşı 41,16±13,23 yıl ve erkek/kadın oranı 1,18 idi. VTE bölgelerine göre hastaların 44 (%30,6)’ü sadece DVT, 41 (%28,5)’i sadece PE, 26 (%18,1)’sı DVT ile PE birlikteliği, 23 (%16)’ü SVT ve 10 (%6,9)’u karın içi bölgesinde tromboz gelişmiş olanlar idi. Toplamda hastaların %46,5’şinde FVL mutasyonu,%13,2’sinde PTM ve %45,1’inde MTHFR gen mutasyonu tespit edildi. Genetik mutasyonlarla tromboz bölgesi arasında istatistiksel anlamlı bir ilişki gözlenmedi (p>0,05). Sonuç: Venöz tromboembolizmin için bilinen risk faktörlerinden genetik mutasyonlar, tromboz oluşum yerinin belirlenmesinde bağımsız birer risk faktörü değildir., Objective: In the present study, we aimed to consider the relation between the manifestations of venous thromboembolism (VTE) and gene mutations including factor V Leiden (FVL), prothrombin G20210A and MTHFR C677T. Methods: One hundred and forty four patients with idiopathic VTE were enrolled in this study. The data of patients were obtained from the medical records in hospital information system. The patients were grouped according to the location of VTE. In all subjects FVL, prothrombin G20210A, and MTHFR C677T were analyzed by specific polymerase chain reactions and restriction enzymes. Univariate and multivariate analysis were used to evaluate the relation between the groups and the gene mutations including factor V Leiden (FVL), prothrombin G20210A and MTHFR C677T. Results: The mean age of patients was 41.16 ± 13.23 years and the male / female ratio was 1.18. Among the patients with VTE, 44 (30.6%) had only DVT, 41 (28.5%) had only PE, 26 (18.1%) had both DVT and PE, 23 (16%) had cerebral veins thrombosis (CVT) and 10 (6.9%) had abdominal vein thrombosis The prevalence was found to be 46.5% for FVL, 13.2% for prothrombin G20210A and 45.1% for MTHFR C677T gene mutation among patients. There was no statistically difference between the manifestations of VTE regarding the gene mutations (p>0,05). Conclusion: The findings of this study suggest that gene mutations including factor V Leiden (FVL), prothrombin G20210A and MTHFR C677T are not sufficient to determine the location of VTE

ÖzetVitamin D, kemik metabolizması haricinde birçok hastalık ve kansere karşı potansiyel koruyucu etkileri vardır. Hücre proliferasyonu, apopitozu, inflamasyon ve angiogenez üzerinden kanser insidansı, progresyonu ve bazı kanser tiplerinde mortaliteye karşı vitamin D’nin koruyucu etiklerine ait veriler giderek artmaktadır. Meme kanseri, prostat kanseri, gastrointestinal kanserler açısından epidemiyolojik veriler ve preklinik bulgular Vitamin D’nin kanser gelişimi ve progresyonundaki rollerini desteklemektedir. Amaç: Behçet hastalığında kalp tutulumu seyrek olarak görülmekle birlikte, hastanın prognozunu önemli derecede etkilemesi ve mortalite riskini arttırması nedeni ile önemlidir. Çalışmamızda bilinen başka hastalığı olmayan Behçet hastalarında kardiyovasküler hastalıklarda önemli bir prediktör olan aortik elastik özellikler ve sol ventrikül diyastolik fonksiyonlarının değerlendirilmesi amaçlandı. Gereç ve Yöntem: Çalışmamıza Behçet hastalığı tanısı olan 25 hasta [14 K, ortanca yaş 40 (20-64) yıl] dahil edildi. Kontrol grubu olarak benzer demografik özellikleri olan 25 sağlıklı gönüllü [12 K, ortanca yaş 40 (19-60) yıl] alındı. Aortik elastik özellikler ve sol ventrikül diyastolik fonksiyonları ekokardiyografi ile değerlendirildi. Bulgular: Aortun sistolik ve diyastolik çapı her iki grupta da benzer olmasına rağmen aortik elastik özelliklerde Behçet grubunda an- lamlı olarak bozulma olduğu görüldü [Aortik Gerilim (%): 10,65 (5,56-16,90) - 7,21 (1,72-13,6) p=0,001; Aortik Esneklik (cm2xdyn- 1x10-6): 4,14 (1,71-9,22) - 2,89 (0,69-5,23) p=0,002; Aort Sertlik İdeksi: 2,75 (2,24-3,55) - 3,21 (2,51-4,60) p=0,001]. Behçet grubun- da diyastolik parametrelerde kontrol grubuna göre olumsuz yönde farklılık saptandı. Ancak belirgin bir diyastolik disfonksiyon saptanmadı. Hastalık süresi ile aortik elastik özellikler arasında ilişki saptanmadı. Sonuç: Behçet hastalarında aortik elastik özelliklerde bozulma olması hastalığın subklinik kardiyak tutulumunun bir göstergesi olabilir. Ekokardiyografi ile aortik elastik özelliklerin değerlendirilmesinin en önemli avantajı kolay uygulanabilir, tekrarlanabilir ve ucuz bir yöntem olması nedeni ile tüm hastalarda takip amaçlı kullanılabilir olmasıdır., Aim: Although cardiac involvement in Behçet's disease is rare, it is an important factor due to its significant effects to the prognosis of the patient and its role in increasing the risk of mortality. In this study, we aimed to assess the aortic elastic properties, an important predictor of cardiovascular disease and left ventricular diastolic functions in patients with Behcet's disease without any other disease. Material and Method: In our study, 25 patients with a diagnosis of Behçet's disease [14 F, median age 40 (20-64) years] were included. A control group of 25 healthy volunteers with similar demographic characteristics [12 F, median age 40 (19-60) years] were admitted. Aortic elastic properties and left ventricular diastolic functions were assessed by echocardiography. Results: Despite the systolic and diastolic diameter of the aorta being similar in both groups, there was a significant deterioration of the aortic elastic properties in Behçet's group. [Aortic strain (%):10.65 (5.56-16.90) - 7.21 (1.72-13.6) p=0.001; Aortic distensibility (cm2xdyn-1x10-6): 4.14 (1.71-9.22) - 2.89 (0.69-5.23) p=0.002; Aortic stiffness index: 2.75 (2.24-3.55) - 3.21 (2.51-4,60) p=0.001]. Compared to the control group, an abnormal variation was observed in the diastolic parameters in Behçet’s group. However, there is no significant diastolic dysfunction. There was no association between the duration of disease and aortic elastic properties. Conclusions: The impairment of aortic elastic properties in patient with Behçet's disease may be an indicator of subclinical cardiac involvement. The most important advantage of the evaluation of aortic elastic properties by echocardiography is the fact that it is an easily reproducible and inexpensive method that can be used for follow-up in all patients.

Coronary artery fistulae are characterized by communications between a coronary artery and a cardiac chamber or another vascular structure. They are usually congenital, but acquired forms may occur. Most patients are usually asymptomatic. However, some studies have emphasized that the incidence of symptoms and complications increases with age, particularly after the age of 20 (Liberthson et al. 1979, Hong et al. 2004). We aimed to present a very rare form of fistula originating from the first diagonal artery and connecting into the left ventricle.

Bacillus cereus is a motile and spore-forming Gram posi­tive rod and is a microorganism species which cause in­fections such as food poisoning, endocarditis, bacteremia sepsis. In our case; 57 years old, acute myeloid leukemia diag­nosed female patient who had bone narrow transplantation and getting chemotherapy was interned with prediagnosis of neutropenic fever and pneumonia in internal medicine clinics which she admitted with high fever. Since B.cereus growth in peripheral vein blood cultures, catheter blood cultures was performed simultaneously. B.cereus growth repeated in the cultures so catheter caused bacteremia was considered as the diagnosis and antibiotherapy was expanded. After treatment, no growth was observed ex­cept catheter blood cultures, general condition of the pa­tient improved and her fever disappeared. Pulsed-field gel electrophoresis was performed in order to investigate the clonal relatedness between B.cereus isolates which were grown in three cultures of separate times and isolates were observed as the same clone. Blood cultures yielded B.cereus are usually considered the possibility of contamination so some clinical laborato­ries do not perform identification at species level. But the species which is isolated may be the cause of infection, so this approach may be time-consuming for the patient and cause a delay in necessary treatment. In such cases; repeat blood culture should be requested and a decision should be done whether to perform species identification according to the patient\'s immune system condition and clinical presentation., Bacillus cereus, hareketli, endospor oluşturabilen Gram pozitif fakültatif aerop basil olup, besin zehirlenmesi, en­dokardit, bakteriyemi ve sepsis gibi enfeksiyonlara neden olmaktadır. Olgumuzda, 57 yaşında akut myeloid lösemi tanılı, kemik iliği nakli olmuş, kemoterapi almakta olan bayan hasta yüksek ateş şikayetiyle başvurduğu dahiliye kliniğinde nötropenik ateş ve pnömoni öntanılarıyla yatırıldı. Peri­ferik kan kültürlerinde Bacillus cereus üremesi üzerine kateterle eş zamanlı kan kültürleri yapıldı. Kültürlerde B.cereus üremesi tekrar eden hastada kateter kaynaklı bakteriyemi tanısı konularak antibiyotik tedavisi genişle­tildi. Tedavi sonrasında kateter kan kültürleri dışında üre­mesi görülmeyen hastanın genel durumunun düzeldiği ve ateşinin düştüğü gözlendi. Üç ayrı zamanda alınan kültürlerde üreyen B.cereus izo­latları arasında klonal ilişkinin araştırılması amacıyla Pul­sed-field jel elektroforezi yapıldı ve izolatların aynı klon olduğu görüldü. Kan kültürlerinde Bacillus türlerinin üremesi genellikle kontaminasyon olasılığını düşündürmekte olduğundan bazı klinik laboratuvarlar tür düzeyinde identifikasyon yapmamaktadır. Fakat izole edilen tür enfeksiyon etkeni olabileceğinden, bu yaklaşım hasta için zaman kaybına ve gerekli tedavinin gecikmesine neden olabilir. Bu du­rumlarda kan kültürü tekrarının istenmesi, hastanın im­mün sistem durumu ve klinik tablosu göz önünde bulun­durularak tür tayininin yapılıp yapılmamasına karar ver­mek gerekmektedir.
