Auditory brainstem implants (ABIs) are neuroprosthetic devices that stimulate the cochlear nuclei without any connection between the inner ear and brainstem [1,2]. Composite scalp and cranium defects, which require microsurgical reconstruction, result from tumor resection, radiation, trauma, severe burn injuries, and rarely vasculitic disorders. In the current study, the authors aim to compare the outcomes of the fasciocutaneous flaps and musculocutaneous free flaps used for the reconstruction of extensive composite scalp and cranium defects. From 2010 to 2017, 21 patients who underwent composite scalp and cranium defect reconstruction with a free flap were retrospectively identified. Eighteen patients had squamous cell carcinoma, 2 patients had meningioma, and 1 patient had Ewing sarcoma. Thirteen musculocutaneous free flaps including latissimus dorsi and vertical rectus abdominis flaps and 9 free fasciocutaneous flaps including radial forearm and anterolateral thigh flaps were used. Only 1 flap loss was encountered. No neurologic impairment in postoperative period was reported. The mean length of stay in the hospital, the duration of surgery, and total volume of blood transfusion for the fasciocutaneous flap group were significantly shorter than those for musculocutaneous flap group. No flap atrophy was reported in fasciocutaneous flap group. Reconstruction of the composite scalp and cranium defects with fasciocutaneous free flaps allows shorter hospitalization, less blood transfusion and less flap atrophy than those of musculocutaneous flaps. To this respect, their usage should be prioritized in such challenging patients.

Epilepsi bir hastalık ya da sendrom olmaktan çok beyin fonksiyonlarını etkileyen semptomlar kompleksidir. Gelişmiş ülkelerde oldukça sık rastlanan bir tablo olup, 100.000'de 20-70 arasında görülmektedir. Epilepsinin en sık görülen formu olan mesial temporal lob epilepsisi (mTLE) genellikle hipokampal sklerozla birliktedir. Bu hastalardan alınan örneklerde hipokampal hücre yoğunluğunun hipokampal odaklı olmayan epilepsi hastalarına göre daha az olması hipokampal skleroz ve temporal lob epilepsi arasında bir ilişki olduğunu destekler bulgulardır. Glutamatla oluşan yaygın sinaptik uyarılma sonucu iyonotropik ve metabotropik glutamat reseptörleri ile etkileşim TLE patofizyolojisinde önemli etken olarak kabul edilmektedir. Bu çalışmanın amacı oluşturulan temporal lob epilepsi modelinde grup I metabotropik glutamat reseptör ekspresyonu üzerine bir antiepileptik ajan olan lakozamid'in etkilerini incelemektir. Böylece TLE etyopatogenezinin anlaşılmasına ve tedavi edilmesine katkı sağlanabileceği düşünülmektedir. Bu çalışmada denekler 4 gruba ayrılmışlardır. Bunlar; Grup 1 Sham grubu (n=8): Sadece burr hole açılan, intrahipokampal enjeksiyon yapılmayan grup; Grup 2 Kontrol grubu (n=8): Burr hole açılarak, intrahipokampal serum fizyolojik (SF) enjeksiyonu yapılan ancak intrahipokampal kainik asit ve lakozamid uygulanmayan grup; Grup 3 Kainik asit (KA) grubu (n=8): Burr hole açılarak, intrahipokampal KA enjeksiyonu yapılan, ancak lakozamid tedavisi yerine 2 hafta boyunca intraperitoneal SF uygulanan grup; Grup 4 Lakozamid (LCS) grubu (n=8): Burr hole açılarak, intrahipokampal KA enjeksiyonu yapılan ve 2 hafta boyunca 50 mg/kg dozunda intraperitoneal LCS tedavisi uygulanan grup. Bu deneklere anestezi uygulandıktan sonra Rat Brain Atlas'tan faydalanılarak dorsal hipokampus hedeflenmiş ve sterotaksik çerçeve kullanılarak hamilton iğnesi yardımı ile hipokampal enjeksiyon yapılmıştır. Daha sonra denekler dekapite edilerek histopatolojik incelemeler yapılmıştır. Histopatolojik olarak bakıldığında kontrol grup olarak kullanılan 2. Grup deneklerde hipokampusta skleroz görülmezken, grup 3 ve grup 4 deneklerde sklerozun oluştuğu gözlenmiştir. Enjeksiyonun yapıldığı taraf hipokampuslarda mGluR1 ve mGluR5 miktarları yüksek olarak bulundu. Lakozamid enjekte edilen grupta grup 1 mGluR ekspresyonu üzerine etki görülmedi. İntrahipokampal olarak enjekte edilen kainik asit sonrasında TLE modeli oluşturuldu. Yapılan immünohistokimya ve Western Blot çalışmaları sonucunda grup 1 mGluR ekspresyonunda anlamlı artış gözlenmedi. Yine yeni bir antiepileptik ajan olan Lakozamid'in grup 1 mGluR ekspresyonu üzerine herhangi bir etkisi görülmedi. Rather than being a disease or a syndrome, epilepsy is conglomerate of symptoms that effect brain functions. It is a frequent condition with a prevalance of 20-70 per 100.000 population in developed countries. Hippocampal sclerosis accompanies the most common form of epilepsy – the mesial temporal epilepsy (mTLE). Histologic examinations of hippocampal specimens in patients with temporal lobe epilepsy shows a decreased cellular density compared to that of extrahippocampal epilepsy patients; this finding suggests a relation between hippocampal sclerosis and temporal lobe epilepsy. It is accepted that interaction with ionotropic and metabotropic gluamate receptors due to widespread synaptic situmalation with glutamate, is an important factor in temporal lobe epilepsy. Our aim is to investigate the effects of lacozamide – an antiepileptic agent – on group I metabotropic glutamate receptor expression in a temporal lobe epilepsy model. Thus this study is thought to contribute the understanding of TLE etiopathogenesis and treatment. Subjects were divided into 4 groups. These are: Group 1 Sham (n=8): Only burr hole was performed without intrahippocampal injection; Group 2 Control (n=8): Burr hole and intrahippacampal saline injection without kainic acid and LCS; Group 3 Kainic acid (KA) group (n=8): Burr hole, intrahippocampal kainic acid injection and 2 weeks of intraperitoneal saline treatment without LCS; Group 4 Lakozamid (LCS) group (n=8): Burr hole, intrahippocampal KA injection and 50mg/kg intraperitoneal LCS treatment for 2 weeks. After anesthesizing animals, they were fixated in stereotactic frame. With the aid of coordinates shown in Rat Brain Atlas, injection was performed at dorsal hippocampus using a Hamilton needle. Soon animals were decapitated and histopathologic examination were performed. There was no hippocampal sclerosis in group 2 (control); however sclerosis was visualized in group 3 and 4. Measurements shows increased expressions of mGluR1 and mGluR5 on the side of injection. There was no effect of lacozamide injection on group 1 mGluR expression. We have performed a rat model of TLE with intrahippocampal kainic acid injection. Immunohistochemical and Western Blot studies shows that there is no significant increase in group 1 mGluR expression.
