1. Caregiver burden and affecting factors for patients with schizophrenia

Abstract

Caregiver burden is a complex concept that is influenced by many different factors. This study aimed to evaluate caregiving burden in caregivers of schizophrenia patients and to examine the effect of gender and other socio-demographic and clinical characteristics on caregiving burden. This study included 100 (50 female, 50 male) patients, who had been diagnosed with schizophrenia and their caregivers. The Zarit Caregiver Burden Scale (ZCBS), Hamilton Anxiety Rating Scale (HAM-A), Hamilton Depression Rating Scale (HAM-D), and Symptom Checklist-90-Revised (SCL-90-R) were administered to the caregivers. Insight Rating Scale (IRS), Calgary Depression Scale for Schizophrenia (CDSS), Positive and Negative Syndrome Scale (PANSS), Functional Recovery Scale in Schizophrenia (FRSS), Global Assessment Scale (GAS) were administered to the patients. The caregivers of male patients had significantly higher HAM-A, HAM-D, ZCBS total and all subscale, and SCL-90-R total and all subscale scores. The male patients had significantly higher PANSS general psychopathology subscale and CDSS and lower IRS scores. Multiple regression analysis revealed that male patient gender, living in urban areas and HAM-D scores of the caregivers were predictive of ZCBS total scores. This study found that schizophrenia has a significant burden. In addition, male patient and depression level of the caregivers were determined as the predictors of the caregiven burden. 2. Evaluation of retinal nerve fiber layer, macular, and choroidal thickness in schizophrenia: spectral optic coherence tomography findings

Abstract

OBJECTIVES: The study was performed to evaluate the retinal nerve fiber layer (RNFL), macular, and subfoveal choroidal thickness (SFCT) in patients with schizophrenia and healthy controls using spectral domain optical coherence tomography (SD-OCT). METHODS: Fifty-nine schizophrenic patients and 36 age- and sex-matched healthy volunteers were enrolled in this cross-sectional clinical study. The RNFL, macular, and SFCT thickness measurements obtained by SD-OCT were compared between the groups. RESULTS: The mean age of 59 patients included in this study was 34.64 years and the mean age of 36 healthy controls was 32.08 years, mean illness duration was 10.33 years, mean total Positive and Negative Symptom Scale (PANSS) score was 75.18, and mean CGI-S score was 3.88. Macular thickness in the superior inner (350.97 ± 16.51 vs. 341.81 ± 16.35), nasal inner (348.97 ± 17.53 vs. 340.25 ± 17.55), inferior inner (345.5 ± 17.59 vs. 335.47 ± 16.92), temporal inner (333.28 ± 17.96 vs. 321.97 ± 19.96), and temporal outer (289.14 ± 14.10 vs. 281.29 ± 15.31) segments were significantly decreased in schizophrenic patients. However; RNFL thickness and choroidal thickness (CT) measurements between the two groups were similar. No significant correlation was found between illness duration, PANSS score, CGI-S score and RNFL thickness and macular thickness measurements. There was a weak negative correlation between disease duration and CT. CONCLUSIONS: The findings from this study suggest that macular thickness measurements are reduced in schizophrenic patients but do not indicate any significant change in RNFL or choroids. Further studies are needed to determine the potential application of optical coherence tomography as a tool for the diagnosis and monitoring the progression of this disease.

Abstract

Introduction: Compared to the generalpopulation, cardiovascular diseases are more common in schizophrenia patientsand the mortality rate is higher than the general population. One of theexplanations for increased cardiovascular events in patients with schizophreniais the increase in platelet activity. Platelets are essential for progression ofatherosclerotic lesions, plaque destabilization, and thrombus formation. Meanplatelet volume (MPV) is a measure of platelet size and a good marker ofplatelet function and activation, which increases in cardiovascular diseases.MPV is routinely reported during complete blood count analysis. The aim of thepresent study was to evaluate MPV values of patients with schizophrenia. Methods: In this retrospective study,hospital-records of the patients who were consecutively admitted to psychiatryinpatient clinic between January 2015 and January 2017 with the diagnosis ofschizophrenia were reviewed. Healthy subjects with no personal history ofpsychiatric disorder were included as a control group.Results: A total of 100 (59 female, 41 male)schizophrenic patients who had been consecutively admitted to the psychiatryinpatient clinic and 37(20 female, 17 male) healthy controls were included in this retrospective study. Therewere no significant differences between the patient group and control group inthe terms of age (mean age patient group vs control group: 37.72 vs 35.03, p=0.081) and sex(p=0.603). Body mass index (BMI) wasfound to be significantly different between groups, higher in the patient group(p=0.001). The MPV was found to besignificantly higher in patient group compared with the control group (MPV,patient group vs control group: 10.34 fL vs 9.97 fL, p=0.041). Platelet count (PC) was significantly lower in the patient group (PC,patient group vs control group: 234.36 vs 267.38, p=0.008) Therewas no correlation between MPV and BMI (p=0.354, r=0.10), andduration of illness (p=0.530,r=0.06). Conclusions: As a result,increased MPV and decreased PC were found in a group of schizophrenic patientin this study. Since increased MPV and decreased PC are evaluated as riskfactors for cardiovascular diseases in the general population, they can also be considered as apredictor of risk factors for cardiovascular diseases that are more frequently encountered in schizophrenia.

Abstract

Objective Metabolic abnormalities such as diabetes, dyslipidemia, abdominal obesity, metabolic syndrome, and abnormal levels of plasma adipokines have been observed in patients with schizophrenia. This study aimed to investigate the differences and correlations of plasma vaspin levels with metabolic parameters in patients with schizophrenia and to compare with healthy controls. Method We measured plasma levels of vaspin and metabolic parameters of 100 patients with schizophrenia and 95 healthy controls. Patients with schizophrenia were evaluated with the Positive and Negative Syndrome Scale (PANSS) and The Global Assessment of Functioning. Results Mean levels of body mass index, waist circumference, triglyceride, and low-density lipoprotein cholesterol of the patients were statistically higher than those of the healthy controls (p = 0.002, p Conclusions Our study showed a significant relationship and positive correlation between vaspin and PANSS scores in schizophrenia patients with obesity. Vaspin may play an important role in the metabolic processes of patients with schizophrenia.

WOS: 000472523600010, PubMed: 31056625, Objectives: To investigate the relationship between suicide attempts and demographic-clinical variables and to assess the methods used in suicide attempts by comparing schizophrenia patients with and without suicide attempts. Methods: A retrospective study with a total of 223 schizophrenia patients aged 18-65 years that were admitted to the Department of Psychiatry, Selcuk University and the Beyhekim Psychiatric Clinic Konya Training and Research Hospital, Konya, Turkey, between January 2014 and January 2018 The data collection forms created by researchers were completed using hospital medical records. Results: It was determined that 40.8% of schizophrenia patients attempted suicide at least once and that 39.6% of schizophrenia patients who attempted suicide had recurrent suicide attempts. Those with suicide attempts had a significantly longer mean duration of untreated psychosis and a higher total number of hospitalizations compared to those without suicide attempts. In addition, the use of depot antipsychotic drugs was significantly lower in those with suicide attempts. There was a statistically significant difference in the presence of traumatic life events between those with and without suicide attempts. Conclusion: Suicidal behavior is an important problem in schizophrenia. Identifying risk factors and high-risk individuals will guide us in the development of preventive interventions.

Abstract

Medroxyprogesterone acetate (MPA) is a steroidal progestin which is used as a contraceptive, in hormone replacement therapy, dysmenorrhea and amenorrhea. A 19-year-old female with bipolar affective disorder who was started on MPA for the treatment of secondary amenorrhea developed manic episodes while taking MPA pills. She was in remission for three years when MPA was commenced. As manic symptoms emerged following the onset of MPA treatment, risperidone and valproic acid was administered, which was then switched to aripiprazole and lithium treatment because of side effects. The manic episode resolved three weeks after hospital admission. This case report highlights the risk of commencing hormone pills in patients with personal history of affective disorders because of the possibility of MPA-induced manic episode.

Abstract

Objective: Clozapine is a second-generation antipsychotic used for treatment-resistant schizophrenia. Despite its effectiveness, clozapine is largely underused due to serious side effects such as leukopenia or neutropenia. We aimed to review whether to continue, discontinue or rechallenge clozapine treatment after such haematological side effects. Methods: We reviewed and summarized the literature on the use of clozapine, how to deal with its side effects, and suitable options in case of any haematological problems. Then, we described several cases successfully treated with clozapine and lithium after development of neutropenia or leukopenia Results: We present three patients with treatment-resistant schizophrenia. While they had demonstrated poor response to multiple antipsychotic trials, clozapine was started. Clozapine induced neutropenia; or leukopenia developed in some cases that was successfully reversed after lithium onset. Increased serious side effects related with coprescription of lithium and clozapine were not observed. Conclusion: Lithium increases neutrophil and total white blood cell count as a side effect that may be useful in patients who develop neutropenia or leukopenia while being treated with clozapine.

Abstract

The aim of this study was to elucidate the chronic effects of tobacco smoking on the P300, a neurophysiological index of cognitive function. Prospective study participants were recruited from a family medicine polyclinic. We selected 32 right-handed smokers who had smoked more than 15 cigarettes per day, by inhalation, for more than 2 years. The control population consisted of 32 right-handed, age-matched healthy individuals who had never smoked. Event-related potentials (ERPs) were recorded with the auditory "oddball" two-tone discrimination task. The data from the central (Cz) and frontal (Fz) electrodes were analyzed. The P300 and N1 amplitudes at Fz were lower in the study population compared to the control group. The early component of ERP, the measure of mental speed (N1) latency at Fz was prolonged in the study group compared to the controls, possibly because early cognitive processes such as sensory input or initial encoding of sensory information were delayed in this group. For those who smoke, a decreased N1 amplitude might indicate delayed information processing and possibly short-term memory disturbance. Thus, chronic tobacco smoking may produce prefrontal cognitive dysfunction.
