2. A Historical Perspective of Medical Education

Abstract

Even though there are significant developments in recent years in medical education, physicians are still needed reform and innovation in order to prepare the information society. The spots in the forefront of medical education in recent years; holistic approach in all processes, including health education, evidence-based medicine and professionalism. Medical education; undergraduate, postgraduate and continuing medical education covering the period varies from country to country for the duration of this period. Increasing day pursuing the importance of medical education and student-centered education and is spread across the country as well as in blended learning model that includes educational centered education. Medical faculty, students and teaching staff should be informed about innovations and developments should offer opportunities for that matter, should support administrative and economic necessarily.

8. Classification of hemiplegia through gait analysis and machine learning methods.

Abstract

Objective: Gait analysis is a method that is used for understanding normal walking and determining the stage of the disease as it affects walking. It is important to objectively determine the stage of the disease in order to decide interventions and treatment strategies. This study aims to determine the Brunnstrom Stage of the hemiplegic patients with an analysis of gait data. Patients and Methods: In the first part of the study, the gait signal data were taken from 28 post-stroke hemiplegic patients and 7 healthy individuals with three-axis accelerometers. In the second part, new gait data were collected from 15 healthy individuals through an accelerometer on the anteroposterior axis. First the accelerometer signals were decomposed to Daubechies 5 (Db5) level six wavelets using MATLAB software. Subsequently, these attributes were classified through several classifier and machine learning algorithms on WEKA and MATLAB software packages to predict the stages of hemiplegia. Results: The highest accuracy rate in the prediction of hemiplegia stage was achieved with the LogitBoost algorithm on WEKA with 91% for 35 samples, and 90% for 50 samples. This performance was followed by the RUSBoosted Trees algorithm on the MATLAB software with an accuracy of 86.1% correct prediction. Conclusion: The Brunnstrom Stage of hemiplegia can be predicted with machine learning algorithms with a good accuracy, helping physicians to classify hemiplegic patients into correct stages, monitor and manage their rehabilitation. Abstract

Children of seasonal agricultural workers, are a group of migrants in Turkey where agricultural production is intense, having a significant part of the year in unhealthy habitats in agricultural areas and having access to services [1,2]. In this group, many temporary or permanent health problems may arise due to the conditions of accommodation and malnutrition [3]. Our aim is to assess the growth and development of children of seasonal agricultural workers. The study is a cross-sectional field study in the agricultural regions of Eskişehir city and districts starting from 1 July 2017. In our study, children of 119 seasonal agricultural workers under the age of 18 were included. In the study, 11 different agricultural regions were visited and the height, weight, body mass index and head circumference of the children were measured and the percentile values were calculated. In the study group ; children of seasonal agricultural workers were 53 (44.5%) male and 66 (55.5%) female. Their ages ranged from 0 to 16 years with a mean of 7.37 ± 3.58 years. 29.4% of the children of seasonal agricultural workers had both height and weight measurements of 5P or less. Growth and development retardation is one of the important diseases among the children of seasonal agricultural workers.

18. Intestinal microbiota composition of patients with Behçet's disease: differences between eye, mucocutaneous, and vascular involvement. The Rheuma-Biota study

Abstract

[Objectives] Changes in microbiota composition affect the aetiology and patho-genesis of chronic diseases, including Behçet's disease (BD). However, no studies have analysed the potential gut microbiota changes among different clinical forms of BD. This study evaluated the intestinal microbiota composition of patients with BD and healthy controls and also compared differences between patients with BD with respect to eye, mucocutaneous, and vascular involvement., [Methods] In this prospective cohort study, 27 patients diagnosed with BD according to the International Study Group criteria and 10 age- and sex-matched healthy controls were included. Detailed intestinal microbiota analysis was performed., [Results] There were no differences between the BD group and the control group in terms of alpha and beta microbial diversity and abundance indices (p>0.05). Actinomyces, Libanicoccus, Collinsella, Eggerthella, Enetrohabdus, Catenibacterium, and Enterobacter were significantly higher in the BD group than in the control group. In addition, Bacteroides, Cricetibacter, Alistipes, Lachnospira, Dielma, Akkermansia, Sutterella, Anaerofilum, Ruminococcease-UCG007, Acetanaerobacterium, and Copropaacter were lower in the BD group than in the control group. When we compared three different system involvement (eye, mucocutaneous, and vascular), the linear discriminant analysis effective size revealed a difference for the following genera: Lachnospiraceae NK4A136 in the uveitis group; Dialister, Intestinomonas, and Marvinbryantia in the mucocutaneous group; and Gemella in the vascular group., [Conclusions] The composition of intestinal microbiota was significantly different in patients with BD compared with healthy adults. Ours is the first study to show differences in microbiota composition in isolated mucocutaneous, eye, and vascular involvement. These findings should be evaluated in a larger series.

Abstract

Objective: Breast cancer is the second most common cause of cancer-related mortality in women after lung cancer in the World. Early diagnosis and treatment is life-saving. Our aim with this study is to evaluate the knowledge, attitudes and behaviors of breast cancer awareness of health care professionals and patients who apply to our Family Medicine Policlinic in Eskisehir.Methods: A total of 424 female patients were included in the study. "Evaluation of the Knowledge, Attitudes and Behaviors of Breast Health of Patients and Healthcare Personnels Attending to the Family Physician Policlinic Form" which was prepared by the researchers was applied with a face-to-face interview method.Results: A total of 424 female patients were included in the study, 178 of them were health professionals. The average age was 40.2 ± 11.1 years. 54.1% (n= 153) of 283 patients who performed BSE were health care personnels and health care personnels’ performances of BSE were found to be statistically higher than non-health care personnels. 47.9% (n= 203) of participants in study are over 40 years of age and 66.5% (n= 135) of them are screened by mammogram, 33.5% (n= 68 ) of them did not have a mammogram. There was no significant difference about screened by mammography between health care personnels and non-health care personnels.Conclusion: While attending preventive health services, they should have positive attitudes towards developing health for health care personnel, which is also an example of cancer screenings., Amaç: Meme kanseri, akciğer kanserinden sonrakadınlarda kansere bağlı mortalitenin en yaygın ikinci nedenidir. Erken tanı vetedavi hayat kurtarıcıdır. Bu çalışmanın amacı, Eskişehir'de Aile Hekimliği Polikliniğimizebaşvuran hasta ve sağlık çalışanlarının meme kanseri hakkındaki bilgi, tutum vedavranışlarını değerlendirmektir.Gereç ve Yöntem: Çalışmaya toplam 424 kadın hastaalındı. Araştırmacılar tarafından hazırlanan “Aile Hekimliği PolikliniğineBaşvuran Hasta Ve Sağlık Çalışanlarının Meme Sağlığı Hakkında Bilgi, Tutum VeDavranışlarının Değerlendirilmesi” anket formu yüz yüze görüşme yöntemiyleuygulanmıştır.Bulgular: Çalışmamıza 424 kadın hasta dâhiledilmiştir. Bu hastaların 178 i sağlık çalışanı olan katılımcılardı. Katılımcılarınyaş ortalamaları 40.2±11.1 yıldır. BSE uygulayan 283 hastanın% 54.1'i (n= 153)sağlık personeli ve sağlık personelinin BSE performansının sağlık çalışanlarınagöre istatistiksel olarak daha yüksek olduğu bulunmuştur. Çalışmayakatılanların% 47.9'u (n= 203) 40 yaşın üzerindedir ve bunların% 66.5'i (n= 135)mamografiyle tarandığını belirtmiştir ,% 33.5'i (n= 68) mamografi taramasınasahip değildi. 36. Comparison of the C14 urea breath test and histopathology in the diagnosis of Helicobacter pylori in the elderly

Abstract

To compare the sensitivity and specificity of C[14] urea breath test with histopathological findings in biopsies obtained through endoscopy.In this cross-sectional study, conducted at the Medical Faculty Ankara University, Ankara, Turkey between January and October 2009, 100 patients aged 65 and above who fulfilled the criteria for inclusion were enrolled. Oesophago-gastroduodenoscopy was carried out on the patients after eight hours of fasting, using a Fujinon endoscope, by the same expert. C14 urea breath tests were carried out on all patients with a capsule containing 37 kBq(1micro Ci) C14 urea/citric acid. Evaluation of all data was carried out using SPSS 11.5. Categorical variables were compared through Pearson Chi squared or Fisher's exact test. P0.005 was taken as statistically significant.The average age of the patients was 71 +/- 5 years; 43 (43%) were male; and 57 (57%) were female. Helicobacter pylori positivity was found to be 35 (35%) in histopathological evaluation, and 36 (36%) in the C14 urea breath test. Of the 35 patients, 32 (91.4%) were found to be positive by the C14 urea breath test also. Of the 65 patients who were found to be negative on histopathology, 61 (93.8%) were also found to be Hp negative by the C14 urea breath test, and 4 (6.2%) were evaluated as false positive. A statistically significant correlation (p0.001) was found between histopathological and C14 urea breath test results. Compared with histopathology, the sensitivity of the urea breath test was 91.4%, and its specificity was 93.8%. A statistically significant (p0.001) relationship was found between histopathological results and the results of the C14 urea breath test.The sensitivity and specificity of the C[14] urea breath test in the group of elderly patients selected was high. The C14 urea breath test can be considered in the elderly group for diagnosis of Helicobacter pylori infection.

38. Determination of Organics, Phenolics, Sugars and Vitamin C Contents of some Cherry Cultivars (Prunus avium)

Abstract

Organic acid, phenolic compound, sugar and vitamin C (L-ascorbic acid) contents of the standard Turkish cherry cultivar "0900 Ziraat" as well as the local cherry cultivars of "Beyrudi", "kisa sap" and "uzun sap" fruits were determined. Concerning organic acid contents in the examined cultivars, malic acid content was identified to be higher (13,914 g/kg in "kisa sap" local cultivar) than other organic acids. Gallic acid content was identified to be higher than the contents of other phenolic compounds and the highest amount was detected in "0900 Ziraat" cultivar (95,512 mg/100 g). Among the sugar types of fructose, glucose and sucrose, glucose was identified to be the dominant sugar in the examined cultivars and the highest amount was detected in local cultivar (215,907 g/kg). In terms of vitamin C content, the highest amount was detected in "kisa sap" local cultivar (11.448 mg/100 g). In addition, it was found statistically important differences among organic acid contents, phenolic contents, sugar and vitamin C in cherry cultivars. (c) 2012 Friends Science Publishers

In critical patients, under conditions that cause an increase in metabolic state and level of stress, there occurs considerable difference in glucose levels between capillary and venous measurements. This study aimed to investigate the comparability of capillary and venous glucose values, according to the perfusion index level obtained with the Masimo Radical-7® device, in critical patients aged 18 years and over.Method: We conducted this prospective and observational study in the emergency department of the Eskisehir Osmangazi University hospital between November 3, 2008 and February 2, 2009.Results: The blood glucose of 300 critical patients was checked by finger stick in the emergency unit. The participants with normal vital signs had perfusion index between 0 and 5; the results obtained by the two methods were more consistent for perfusion index values of 6 and over. The results were most consistent in aged participants with normal vital sign findings and low perfusion index and in young patients with high perfusion index. In the cases where at least one of the vital signs was abnormal, the glucose values obtained by the two methods were more consistent when the perfusion index was 6 or over. In this group, independently from the perfusion index value, the consistency was higher in younger patients compared with aged patients.Conclusion: In the emergency department, perfusion index value measured by Masimo Radical-7 and capillary blood glucose levels can serve in blood sugar management in critically ill pa
