No. of bitstreams: 0 Previous issue date: 2020-12-01 Bill and Melinda Gates Foundation Fundação de Amparo à Pesquisa do Estado do Rio de Janeiro (FAPERJ) Pooled data analysis in the field of maternal and child nutrition rarely incorporates data from low- and middle-income countries and existing studies lack a description of the methods used to harmonize the data and to assess heterogeneity. We describe the creation of the Brazilian Maternal and Child Nutrition Consortium dataset, from multiple pooled longitudinal studies, having gestational weight gain (GWG) as an example. Investigators of the eligible studies published from 1990 to 2018 were invited to participate. We conducted consistency analysis, identified outliers, and assessed heterogeneity for GWG. Outliers identification considered the longitudinal nature of the data. Heterogeneity was performed adjusting multilevel models. We identified 68 studies and invited 59 for this initiative. Data from 29 studies were received, 21 were retained for analysis, resulting in a final sample of 17,344 women with 72,616 weight measurements. Fewer than 1% of all weight measurements were flagged as outliers. Women with pre-pregnancy obesity had lower values for GWG throughout pregnancy. GWG, birth length and weight were similar across the studies and remarkably similar to a Brazilian nationwide study. Pooled data analyses can increase the potential of addressing important questions regarding maternal and child health, especially in countries where research investment is limited. This cross-sectional study sought to assess the effect of demographic and socioeconomic factors, chronic diseases, and health counseling on dietary patterns of the elderly. Food consumption was assessed by means of a reduced eating frequency questionnaire distributed to 1,426 elderly residents in Pelotas, State of Rio Grande do Sul, Brazil, and dietary patterns were obtained using main component analysis. Two dietary patterns were identified: healthy diet (wholegrain food, fruits, vegetables, and milk) and Western diet (sweet snacks, fried foods, frozen foods, processed meats, and fast food). Females, non-smokers, and alcohol-free individuals tended more toward the healthy diet, whereas males, smokers, alcohol-dependent, and overweight individuals tended more toward the Western diet. Higher income and schooling were associated with a higher tendency toward both diets, and chronic diseases were not associated with either diet. Receiving counseling for adopting healthy habits was associated with a higher tendency toward a healthy diet and a lower tendency toward the Western diet. There is a need to increase effective guidance on healthy habits by health services to promote healthier eating practices, with special attention to elderly males, smokers, alcohol-dependent and overweight individuals.

The objective of this study is to evaluate the effect of milk consumption, dietary calcium intake and nutrient patterns (bone-friendly and unfriendly patterns) from late adolescence to early adulthood, on bone at 22 years of age. Cross-sectional analysis was performed with 3,109 participants from 1993 Pelotas (Brazil) birth cohort in the follow-ups of 18 and 22 years of age. Bone mineral density (BMD) of the lumbar spine, right femur and whole body were assessed at 22 years using a dual-energy X-ray absorptiometry (DXA). The exposure variables (dietary calcium, milk and nutrient patterns) were created by combining the consumption frequencies between the two follow-ups (always low, moderate, high, increase or decrease). Multiple linear regressions were performed, stratified by sex. In the right femur site, men classified into the “always high” (mean = 1.148g/cm²; 95%CI: 1.116; 1.181) and “increased” categories of milk consumption (mean = 1.154g/cm²; 95%CI: 1.135; 1.174) presented a slightly low BMD comparing with low (mean = 1.190g/cm²; 95%CI: 1.165; 1.215) and moderate (mean = 1.191g/cm²; 95%CI: 1.171; 1.210) categories. In addition, men always classified in the highest tertile of the “bone-unfriendly” pattern presented the lowest mean of whole body BMD (mean = 1.25g/cm²; 95%CI: 1.243; 1.266). No associations were observed between the categories of dietary calcium intake and “bone-friendly” pattern and each of the three BMD outcomes. These results point to the fact that diets composed of inhibiting foods/nutrients can contribute negatively to bone health. O estudo teve como objetivo avaliar o consumo de leite, a ingestão dietética de cálcio e os padrões de nutrientes (favoráveis e desfavoráveis à saúde óssea) do final da adolescência até o início da idade adulta sobre a massa óssea aos 22 anos de idade. Foram realizadas análises transversais em 3.109 participantes da coorte de nascimentos de 1993 de Pelotas, Rio Grande do Sul, Brasil, nos acompanhamentos realizados aos 18 e 22 anos de idade. Aos 22 anos, foi avaliada a densidade mineral óssea (DMO) da coluna lombar, do fêmur direito e do corpo inteiro, utilizando absorciometria de raios-x de dupla energia (DXA). As variáveis de exposição (cálcio dietético, leite e padrões de nutrientes) foram criadas pela combinação das frequências de consumo entre os dois acompanhamentos (sempre baixo, moderado, sempre alto, aumentado ou diminuído). Foram realizadas análises de regressão linear múltipla, estratificadas por sexo. No fêmur direito, homens classificados nas categorias de consumo de leite “sempre alto” (média = 1,148g/cm²; IC95%: 1,116; 1,181) e “aumentado” (média = 1,154g/cm²; IC95%: 1,135; 1,174) apresentaram DMO ligeiramente baixa, comparado com as categorias de consumo “sempre baixo” (média = 1,190 g/cm²; IC95%: 1,165; 1,215) e “moderado” (média = 1,191g/cm²; IC95%: 1,171; 1,210). Além disso, os homens sempre classificados no tercil mais alto do padrão “favorável à saúde óssea” apresentaram a média mais baixa de DMO de corpo inteiro (média = 1,254g/cm²; IC95%: 1,243; 1,266). Não foram observadas associações entre as categorias de ingestão de cálcio dietético e do padrão “favorável à saúde óssea” e cada um dos três desfechos de DMO. Os resultados corroboram o fato de que dietas constituídas de alimentos e nutrientes inibidores podem afetar negativamente a saúde óssea. El objetivo de este estudio es evaluar el efecto del consumo de leche, ingesta dietética de calcio y patrones nutricionales (beneficiosos y no beneficiosos para los huesos), desde la adolescencia tardía hasta la etapa adulta temprana, en huesos con 22 años de edad. Se realizó un análisis transversal con 3.109 participantes procedentes de la cohorte de nacimiento de 1993 en Pelotas, Rio Grande do Sul, Brasil, con seguimientos a los 18 y 22 años de edad. Se evaluó la densidad mineral ósea de la columna lumbar, fémur derecho y todo el cuerpo a los 22 años, utilizando densitometría ósea por absorción de rayos X (DXA). Las variables de exposición (calcio dietético, leche y patrones nutricionales) se crearon mediante la combinación de las frecuencias de consumo entre los dos seguimientos (siempre bajo, moderado, alto, incremento o disminución). Se realizaron regresiones múltiples lineales, estratificadas por sexo. En la zona del fémur derecho, los hombres fueron clasificados en “siempre alto” (media = 1,148g/cm²; IC95%: 1,116; 1,181) e “incrementado” en las categorías de consumo de leche (media = 1,154g/cm²; IC95%: 1,135; 1,174) presentó una ligeramente baja densidad mineral ósea (DMO), comparándolas con las categorías bajas (media = 1,190g/cm²; IC95%: 1,165; 1,215) y moderadas (media = 1,191g/cm²; IC95%: 1,171; 1,210). Asimismo, los hombres siempre fueron clasificados en el tercil más alto del patrón “no beneficioso para los huesos”, puesto que presentaron la media más baja de todo el cuerpo de DMO (media = 1,254g/cm²; IC95%: 1,243; 1,266). No se observaron asociaciones entre las categorías de ingesta de calcio dietético y los patrones “beneficiosos para los huesos” en cada uno de los tres resultados de DMO. Estos resultados indican el hecho de que las dietas que inhiben alimentos/nutrientes pueden contribuir negativamente en la salud de los huesos.

Neste sentido, é proposto que a massa óssea em idade avançada tenha fatores determinantes no desenvolvimento inicial da vida, podendo resultar em um risco aumentado de fratura e osteoporose futura. Nesta tese, trabalhamos com dados da Coorte de 1993 de Pelotas e, foi investigado o efeito da alimentação, através do consumo de cálcio dietético, leite e de padrões de nutrientes, criados a partir de nutrientes que afetam a absorção de cálcio, nomeados de osso amigo (caracterizado pelo consumo de oxalato, proteína, cálcio e sódio) e osso não amigo (caracterizado pelo consumo de cafeína e com carga negativa de vitamina D) (Artigo 1) e do índice de massa corporal (IMC), índice de massa gorda (IMG), índice de massa magra (IMM) e obesidade (Artigo 2) da adolescência ao início da idade adulta sobre a Densidade Mineral Óssea (DMO) do corpo inteiro, coluna lombar e fêmur direito aos 22 anos de idade. Também foi avaliado o efeito do tamanho do corpo ao nascer e do crescimento em estatura e peso em diferentes períodos da infância e da adolescência sobre a DMO do corpo inteiro no início da idade adulta (Artigo 3). Em todos artigos a DMO foi obtida através da absorciometria de raios-x de dupla energia (DXA) e as análises foram estratificadas por sexo. O primeiro consta de uma análise longitudinal com os acompanhamentos realizados aos 18 e 22 anos de idade e obteve uma amostra de 3.109 indivíduos. Em relação ao consumo de leite entre os acompanhamentos, observamos efeito negativo para o sexo masculino, nas categorias de aumento e diminuição do consumo com DMO de corpo inteiro e DMO do fêmur. Para a ingestão de cálcio, observou-se associação positiva para o sexo feminino, na categoria aumento do consumo e DMO de corpo inteiro. Nas análises de padrões de nutrientes, homens com moderada ou diminuição da adesão ao padrão não amigo apresentaram aumento na DMO de corpo inteiro e as mulheres que também tiveram um consumo diminuído apresentaram aumento na DMO da coluna lombar. O segundo artigo longitudinal, também foi realizado com os acompanhamentos dos 18 e 22 anos de idade, com uma amostra final de 2.968 participantes. Observou-se associação positiva dos três índices sobre os desfechos ósseos, porém com maior magnitude para o IMC, seguido pelo IMM, sugerindo-se que a maior parte do efeito positivo da massa corporal sobre o osso seja decorrente da quantidade de massa magra. O último artigo, sobre crescimento condicional e DMO do corpo inteiro, contou com dados dos acompanhamentos realizados ao nascimento, 1, 4, 11, 15 e 18, com desfecho ósseo medido aos 22 anos, totalizando 756 indivíduos. Os efeitos do ganho de peso e comprimento/altura foram examinados utilizando peso relativo condicional (CWh) e comprimento/altura condicional (CH). Para os homens, maior ganho de altura aos 4, 11 e 18 anos foi associado com maior DMO do corpo inteiro após análise ajustada, sendo o resultado com maior magnitude dos 11 anos. Para as mulheres, a DMO foi associada ao ganho de altura em todas as avaliações até os 15 anos, com maior medida de efeito aos 4 anos. Em relação ao peso corporal, para os homens, maior peso ao nascer e ganho aos 4, 15 e 18 anos repercutiu em maior DMO, com maiores coeficientes aos 18 anos. Para as mulheres, com exceção ao peso ao nascer, todas as variáveis de ganho de peso estiveram associadas à maior DMO, com maiores coeficientes observados para os 4 anos. Throughout life, the quantity and quality of bone mineral mass are determined by genetic and environmental factors and also depend on previous experiences, such as nutritional status, food consumption, physical activity and hormone levels. In this sense, it is proposed that bone mass in advanced age has determinants in the initial development of life which may result in an increased risk of fracture and future osteoporosis. In this thesis, we worked with data from the 1993 Pelotas Cohort and investigated the effect of diet, through the consumption of dietary calcium, milk and nutrient patterns, created from nutrients that affect calcium absorption, named bone friendly (characterized by the consumption of oxalate, protein, calcium and sodium) and unfriendly pattern (characterized by consumption of caffeine and negatively charged vitamin D) (Article 1) and body mass index (BMI), fat mass index (FMI), lean mass index (LMI) and obesity (Article 2) from adolescence to early adulthood on bone mineral density (BMD) of the whole body, lumbar spine and right femur at 22 years of age. It was also evaluated the effect of body size at birth and growth in height and weight at different periods of childhood and adolescence on full body BMD in early adulthood (Article 3). In all articles BMD was obtained through dual-energy X-ray absorptiometry (DXA) and analyzes were stratified by sex. The first one consists of a longitudinal analysis with the follow-ups performed at 18 and 22 years of age and it obtained a sample of 3,109 individuals. Regarding the consumption of milk between the follow-ups, we observed a negative effect for males, in the categories of increase and decrease of consumption with BMD of whole body and BMD of the femur. For the calcium intake, a positive association was observed for the female, in the category of increased consumption and BMD of the whole body. In the analysis of nutrient patterns, men with moderate or decreased adherence to the unfriendly pattern had an increase in the BMD of the whole body and women who also had a decreased consumption had an increase in lumbar spine BMD. The second longitudinal article was also performed with the follow-ups at 18 and 22 years of age, with a final sample of 2,968 participants. A positive association of the three indices on bone outcomes was observed, but with a higher magnitude for BMI, followed by LMI, suggesting that most of the positive effect of body mass on bone is due to the amount of lean mass. The last article, about conditional growth and BMD of the whole body, had data from follow-ups performed at birth, 1, 4, 11, 15 and 18, with a bone outcome measured at 22 years of age, totaling 756 individuals. The effects of weight gain and length/height were examined using conditional relative weight (CWh) and conditional length/height (CH). For men, greater height gain at 4, 11 and 18 years was associated with greater BMD of the whole body after adjusted analysis, being the result with greater magnitude the one at 11 years. For women, BMD was associated with height gain in all assessments up to 15 years, with the greatest measure of effect at 4 years. Regarding body weight, for males, greater birth weight and gain at 4, 15 and 18 years had a higher BMD, with higher coefficients at 18 years. For women, except for birth weight, all variables of weight gain were associated with the highest BMD, with higher coefficients observed for the 4 years.

The objective of this study was to investigate intergenerational breastfeeding practices according to parental sex and age at delivery in the 1993 Pelotas (Brazil) birth cohort study. This is a prospective birth cohort study, and at the 22‐year follow‐up, a substudy with all children of the cohort members who had become parents was conducted (93Cohort‐II). First generation breastfeeding data were collected at 3 months and 4‐year‐old follow‐ups. In the 93Cohort‐II, parents answered a questionnaire about their children's breastfeeding practices. Adjusted Tobit and Poisson regression models with robust variance were applied to estimate the association between predominant parental breastfeeding duration and exclusive breastfeeding duration of the children at 3 and 6 months. Out of 3,810 cohort participants, 955 (25%) had delivered at least one live‐born infant, and 1,222 children were assessed. Fifty‐four percent of parents were ≤19 years old. Direct effects of predominant parental breastfeeding duration on exclusive breastfeeding duration of their children were only observed when data were stratified by parental age: children born to parents aged ≥20 years old and who were predominantly breastfed for at least 3 months presented higher exclusive breastfeeding duration and higher prevalence of being exclusively breastfed for at least 3 months. When analyses were stratified by mothers and fathers, the result remained significant only among mothers. Longer predominant breastfeeding duration in the first generation was associated with longer exclusive breastfeeding duration in the second generation, but only among older mothers. Education and social support surrounding breastfeeding should be intensified among fathers and younger parents to create a positive environment supportive of breastfeeding. [ABSTRACT FROM AUTHOR]

O abuso de drogas em geral caracteriza-se como fenômeno disseminado em todo o mundo, com consequências graves, representando uma ameaça significativa para a saúde e para a estrutura social e econômica das famílias, comunidades e nações. O objetivo principal do estudo foi avaliar a prevalência de consumo de álcool, tabaco e drogas ilícitas, bem como seus fatores associados nos ingressantes na Universidade Federal de Pelotas.Em particular, visou determinar o consumo simultâneo de álcool e drogas ilícitas e o consumo simultâneo de álcool, cigarro e drogas ilícitas, identificando assim os indivíduos com maior risco à saúde. Seus resultados poderão ser utilizados para desenhar futuras intervenções no combate ao tabagismo, ao uso de álcool e drogas ilícitas entre jovens, sobretudo nas populações universitárias.Os instrumentos propostos para responder aos objetivos foram o questionário AUDIT, para avaliar consumo de álcool, 3 perguntas para avaliar tabagismo, 10 perguntas para avaliar o consumo de cinco tipos de drogas ilícitas na vida e nos últimos 30 dias. Encontramos que 92,00% referiram já ter consumido álcool na vida;11,02% eram fumantes atuais, e 43,90% referiram uso de pelo menos uma droga na vida, e 23,30%, de pelo menos uma droga nos últimos 30 dias. O consumo simultâneo de álcool, cigarro e drogas foi de 5,21%, sendo os fatores de risco associados a este consumo classe econômica mais elevada, idade mais jovem quando da experimentação de drogas e área de concentração do curso exatas e agrárias. Quanto ao consumo simultâneo de álcool e drogas, a prevalência foi 6 de 13,61%, e os fatores de risco associados foram sexo masculino, classe econômica A, presença de depressão, idade mais jovem quando da experimentação de álcool, área de concentração de curso linguística, letras e artes e o fato de morar sozinho ou com amigos. The present study aimed the increase of knowledge about the consumption of licit and illicit drugs in the college students and the additive behavior. Drug abuse in general is characterized as a widespread phenomenon worldwide with serious consequences, posing a significant threat to the health and social and economic structure of families, communities and nations. The main aim of study was to evaluate the prevalence of alcohol, tobacco and illicit drug use, as well as its associated factors among freshmen students from the Federal University of Pelotas. In particular it aimed to determine the simultaneous consumption of alcohol and illicit drugs and the simultaneous consumption of alcohol, cigarettes and illicit drugs, thus identifying individuals with greater health risk. His results will be used to draw future interventions against smoking, alcohol and illicit drugs use among young people, especially in college students. The instruments proposed to respond to the objectives were the AUDIT questionnaire, to evaluate alcohol consumption, 3 questions to evaluate smoking, 10 questions to evaluate the consumption of five kind of illicit drugs in lifetime and in the last 30 days. We found that the lifetime use of alcohol was 92.00%, current smoker prevalence was 11.02%, and the lifetime use of at least one illicit drug was 43.91% and at least one illicit drug use in the last 30 days was 23.30%. The simultaneous consumption of alcohol, tobacco, and illicit drugs was 5.21% and the associated risk factors were higher economic class, younger drug experimentation age and exact and agrarian sciences. Regarding the simultaneous consumption of alcohol and illicit drugs, the prevalence was 13.61% and associated risk factors were being male, from higher economic class, depressed, younger alcohol experimentation age, courses from linguistics, letters and arts area and living alone or with friends.

Trata-se de um estudo de coorte prospectivo com 161 gestantes (2-40 anos) acompanhadas com 5-13 (linha de base), 20-26 e 30-36 semanas em um centro municipal de saúde no Rio de Janeiro, Brasil. Análise de regressão linear simples e múltipla foram realizadas para testar a associação entre a ocorrência de violência entre parceiros íntimos e o escore de adesão a cada um dos padrões alimentares. As análises foram ajustadas para idade, renda per capta, paridade, história de aborto espontâneo, índice de massa corporal pré-gestacional, tabagismo atual e consumo de álcool atual. Nas regressões brutas e ajustadas, a exposição à VPI global e física foram associadas a maior adesão ao padrão processado. Gestantes que sofreram violência global apresentaram, em média, um acréscimo de 0,469 (IC 95%: 0,051; 0,885) no escore de adesão ao padrão alimentar processado. Aquelas que sofreram violência física ao longo da gestação apresentaram um aumento médio de 1,032 (IC 95%: 0,442; 1,620) no escore de adesão ao mesmo padrão. This study aimed evaluate the association of violence between intimate partners and its relation with feeding patterns during pregnancy in a cohort of pregnant women treated in a health care center in the city of Rio de Janeiro. It is a cohort prospective study with 161 pregnant women (2 – 40 years old) followed within 5 – 13 (base line), 20 26 and 30-36 weeks in a health care center in Rio de Janeiro, Brazil. Analyses of simple and multiple linear regressions were realized to test the association between violence occurrence among intimate partners and the adhesion score of each feeding patterns. The analyses were adjusted by age, income, parity, spontaneous abortion history, body mass index pre- pregnancy, and current smoking and alcohol habits. In the bruit and adjusted regressions, the expositions to global and physical VPI were associated to greater adhesion to the processed pattern. Pregnant women suffered global violence presented, on average, an increase of 0,469 (IC 95%: 0,051; 0,885) in the adhesion score from on feeding pattern processed. Those who suffered physical violence during pregnancy presented an average increase of 1,032 (IC 95%: 0,442; 1,620) in the adhesion score to the same pattern.

- Abstract
This cross-sectional study sought to assess the effect of demographic and socioeconomic factors, chronic diseases, and health counseling on dietary patterns of the elderly. Food consumption was assessed by means of a reduced eating frequency questionnaire distributed to 1,426 elderly residents in Pelotas, State of Rio Grande do Sul, Brazil, and dietary patterns were obtained using main component analysis. Two dietary patterns were identified: healthy diet (wholegrain food, fruits, vegetables, and milk) and Western diet (sweet snacks, fried foods, frozen foods, processed meats, and fast food). Females, non-smokers, and alcohol-free individuals tended more toward the healthy diet, whereas males, smokers, alcohol-dependent, and overweight individuals tended more toward the Western diet. Higher income and schooling were associated with a higher tendency toward both diets, and chronic diseases were not associated with either diet. Receiving counseling for adopting healthy habits was associated with a higher tendency toward a healthy diet and a lower tendency toward the Western diet. There is a need to increase effective guidance on healthy habits by health services to promote healthier eating practices, with special attention to elderly males, smokers, alcohol-dependent and overweight individuals.

- Abstract
Objective To identify vegetarianism prevalence and associated factors among students at a public university in Southern Brazil. Methods This was a cross-sectional study carried out in 2017-2018 with first-year university students. A questionnaire was administered to assess diet type (omnivorous or vegetarian). Logistic regression was used to estimate odds ratios (OR) and 95% confidence intervals (95%CI). Results Of 2,706 eligible individuals, 1,841 formed part of the sample. The predominant type of diet was omnivorous (93.6%), followed by lacto-ovo vegetarian (5.4%), strict vegetarian (0.7%), ovo vegetarian (0.2%) and lacto vegetarian (0.1%). Females (OR=0.58 - 95%CI 0.38;0.89), students reporting harmful use of alcohol (OR=2.02 - 95%CI 1.34;3.04) and students enrolled in Linguistics, Modern Languages and Arts courses - compared to those enrolled in Applied Social Sciences and Humanities courses (OR=2.59 - 95%CI 1.42;4.71) - were more likely to be vegetarians. Conclusion Vegetarian diet was associated with the female sex, alcohol use, and taking linguistics, modern language and arts courses.

Objective: to describe the prevalence of noncommunicable diseases (NCDs), long term (current) drug therapy, blood pressure levels and capillary glycemia among a nursing team at a charitable hospital in Pelotas/RS., Methods: This was a cross-sectional study. The questions were adapted from the Vigitel system questionnaire (Telephone Surveillance of Chronic Disease Risk and Protection Factors). All participants had their blood pressure and blood glucose measured., Results: Of the 272 staff assessed, 29.4% reported having NCDs, 48.9% were on long term (current) drug therapy and 73.9% reported a family history of NCDs. Among those interviewed, 20.6% reported using antihypertensive drugs, of these 27.7% had high blood pressure levels, and 2.6% reported using antihypoglycemic agents, 42.9% of whom had high blood glucose., Conclusion: Among those professionals who stated they had high blood pressure and diabetes, those who were on medication had higher blood pressure and glucose levels than those who were not on medication.

Objective: to estimate the prevalence of disability related to basic and instrumental activities of daily living and its association with socioeconomic, demographic, behavioral and health characteristics in the elderly., Methods: population-based cross-sectional study in Pelotas, Brazil, in 2014; Katz and Lawton scales were used to assess the outcomes using Poisson regression., Results: the study included 1.451 elderly individuals; the prevalence of disability for basic and instrumental activities was 36.1% and 34.0%, respectively, and 18.1% in both; higher prevalence of functional disability were observed individuals ≥80 years (PR=3.01; 95%CI 2.17;4.18), not working (PR=2.02; 95%CI 1.13;3.60) and those with multiple morbidities (PR=3.28; 95%CI 1.38;7.79); and lower in individuals with ≥12 years of schooling (PR=0.40; 95%CI 0.24;0.66), and that were physically active (PR=0.42; 95%CI 0.21;0.82)., Conclusion: functional disability was associated to individuals older than 80, with less schooling years and affected by multiple morbidities.
