Abstract

Background Maternal morbidity occurs several times more frequently than mortality, yet data on morbidity burden and its effect on maternal, foetal, and newborn outcomes are limited in low- and middle-income countries. We aimed to generate prospective, reliable population-based data on the burden of major direct maternal morbidities in the antenatal, intrapartum, and postnatal periods and its association with maternal, foetal, and neonatal death in South Asia and sub-Saharan Africa. Methods and findings This is a prospective cohort study, conducted in 9 research sites in 8 countries of South Asia and sub-Saharan Africa. We conducted population-based surveillance of women of reproductive age (15 to 49 years) to identify pregnancies. Pregnant women who gave consent were include in the study and followed up to birth and 42 days postpartum from 2012 to 2015. We used standard operating procedures, data collection tools, and training to harmonise study implementation across sites. Three home visits during pregnancy and 2 home visits after birth were conducted to collect maternal morbidity information and maternal, foetal, and newborn outcomes. We measured blood pressure and proteinuria to define hypertensive disorders of pregnancy and woman's self-report to identify obstetric haemorrhage, pregnancy-related infection, and prolonged or obstructed labour. Enrolled women whose pregnancy lasted at least 28 weeks or those who died during pregnancy were included in the analysis. We used meta-analysis to combine site-specific estimates of burden, and regression analysis combining all data from all sites to examine associations between the maternal morbidities and adverse outcomes. Among approximately 735,000 women of reproductive age in the study population, and 133,238 pregnancies during the study period, only 1.6% refused consent. Of these, 114,927 pregnancies had morbidity data collected at least once in both antenatal and in postnatal period, and 114,050 of them were included in the analysis. Overall, 32.7% of included pregnancies had at least one major direct maternal morbidity; South Asia had almost double the burden compared to sub-Saharan Africa (43.9%, 95% CI 27.8% to 60.0% in South Asia; 23.7%, 95% CI 19.8% to 27.6% in sub-Saharan Africa). Antepartum haemorrhage was reported in 2.2% (95% CI 1.5% to 2.9%) pregnancies and severe postpartum in 1.7% (95% CI 1.2% to 2.2%) pregnancies. Preeclampsia or eclampsia was reported in 1.4% (95% CI 0.9% to 2.0%) pregnancies, and gestational hypertension alone was reported in 7.4% (95% CI 4.6% to 10.1%) pregnancies. Prolonged or obstructed labour was reported in about 11.1% (95% CI 5.4% to 16.8%) pregnancies. Clinical features of late third trimester antepartum infection were present in 9.1% (95% CI 5.6% to 12.6%) pregnancies and those of postpartum infection in 8.6% (95% CI 4.4% to 12.8%) pregnancies. There were 187 pregnancy-related deaths per 100,000 births, 27 stillbirths per 1,000 births, and 28 neonatal deaths per 1,000 live births with variation by country and region. Direct maternal morbidities were associated with each of these outcomes. Conclusions Our findings imply that health programmes in sub-Saharan Africa and South Asia must intensify their efforts to identify and treat maternal morbidities, which affected about one-third of all pregnancies and to prevent associated maternal and neonatal deaths and stillbirths. Abstract

Introduction In Africa, historically, fever in children younger than 5 years has been considered to be due to malaria and has been treated immediately with antimalarial drugs without laboratory confirmation [...], Objective To evaluate current practices and standards of evaluation and treatment of childhood febrile illness in Southern Province, Zambia. Methods From November to December 2013, we conducted a cross-sectional survey of facilities and health workers and we observed the health workers' interactions with febrile children and their caregivers. The facility survey recorded level of staffing, health services provided by the facility, availability and adequacy of medical equipment, availability of basic drugs and supplies and availability of treatment charts and guidelines. The health worker survey assessed respondents' training, length of service, access to national guidelines and job aids for managing illnesses, and their practice and knowledge on management of neonatal and child illnesses. We also conducted exit interviews with caregivers to collect information on demographic characteristics, chief complaints, counselling and drug dispensing practices. Findings This study included 24 health facilities, 53 health workers and 161 children presenting with fever. Facilities were insufficiently staffed, stocked and equipped to adequately manage childhood fever. Children most commonly presented with upper respiratory tract infections (46%; 69), diarrhoea (31%; 27) and malaria (10%; 16). Health workers insufficiently evaluated children for danger signs, and less than half (47%; 9/19) of children with pneumonia received appropriate antibiotic treatment. Only 57% (92/161) were tested for malaria using either rapid diagnostic tests or microscopy. Conclusion Various health system challenges resulted in a substantial proportion of children receiving insufficient management and treatment of febrile illness. Interventions are needed including strengthening the availability of commodities and improving diagnosis and treatment of febrile illness. Objectif Evaluer les pratiques et les normes actuelles (revaluation et de traitement des maladies febriles infantiles dans la Province meridionale de la Zambie. Methodes De novembre a decembre2013, nous avons mene une enquete transversale aupres d'etablissements et de professionnels de sante et nous avons observe les interactions entre les professionnels de sante, les enfants fievreux et leurs proches. L'enquete realisee dans les etablissements s'est interessee a la dotation en personnel, aux services sanitaires dispenses par l'etablissement, a la disponibilite et a la convenance des equipements medicaux, a la disponibilite de medicaments et de fournitures elementaires et a la presence de directives et de feuilles d'observation. L'enquete realisee aupres des professionnels de sante a analyse la formation des repondants, leur nombre d'annees de service, leur acces aux directives nationales et aux aide-memoires pour la prise en charge des maladies, ainsi que leur pratique et leurs connaissances en matiere de prise en charge des maladies infantiles et neonatales. Nous avons egalement interroge les proches a leur sortie afin de recueillir des informations sur les caracteristiques demographiques, les principales plaintes, les conseils et les pratiques d'administration des medicaments. Resultats Cette etude a examine 24 etablissements de sante, 53 professionnels de sante et 161 enfants atteints de fievre. Les etablissements manquaient de personnel, de produits en stock et d'equipements pour prendre en charge correctement les cas de fievre infantile. Les enfants etaient le plus souvent atteints d'infections des voies respiratoires superieures (46%; 69), de diarrhee (31 %; 27) et de paludisme (10%; 16). Les professionnels de sante n'evaluaient pas suffisamment les signes de danger chez les enfants, et moins de la moitie (47%; 9/19) des enfants atteints de pneumonie ont recu un traitement antibiotique adapte. Seuls 57% (92/161) ont fait l'objet d'un depistage du paludisme, au moyen de tests de diagnostic rapide ou par microscopie. Conclusion A cause des differents problemes du systeme sanitaire, une part importante d'enfants ne beneficie pas d'une prise en charge et d'un traitement suffisants des maladies febriles. Des interventions sont necessaires, notamment pour augmenter la disponibilite des produits et ameliorer le diagnostic et le traitement des maladies febriles. Objetivo Evaluar las practicas y los estandares de evaluation y tratamiento actuales de las enfermedades febriles en los ninos en la Provincia del Sur, Zambia. Metodos De noviembre a diciembre de 2013, se realizo una encuesta transversal de centros y trabajadores sanitarios y se observaron las interacciones de los trabajadores sanitarios con ninos con fiebre y sus cuidadores. La encuesta de los centros registro el nivel de personal y servicios de salud ofreddos en el centra, la disponibilidad e idoneidad de los equipos medicos, la disponibilidad de medicamentos y suministros basicosy la disponibilidad decronogramasy directrices de tratamiento. La encuesta a los trabajadores sanitarios evaluo la formation de los encuestados, la duration del servicio, el acceso a directrices nadonales y ayudas laborales para la gestion de enfermedades y su practica y conocimiento acerca de la gestion de las enfermedades neonatales e infantiles. Tambien se realizaron entrevistas de salida con cuidadores para recopilar information acerca de las caracteristicas demograficas, los motivos de consulta, asesoramiento y practicas de distribution de lidad de la atencion en la Zambia rural medicamentos. Resultados Este estudio abarco 24 centros sanitarios, 53 trabajadores sanitarios y 161 ninos con fiebre. Los centros no contaban con personal, suministros y equipos suficientes para gestionar de forma adecuada la fiebre en ninos. Las afecciones mas comunes presentadas por los ninos eran infecciones del tracto respiratorio superior (46%; 69), diarrea (31 %; 27) y malaria (10%; 16). Los trabajadores sanitarios evaluaron a los ninos de forma insuficiente en busca de signos de peligro, y menos de la mitad (47%; 9/19) de los ninos con neumonia recibieron el tratamiento antibiotico adecuado. Solo el 57% (92/161) fue sometido a pruebas de malaria utilizando pruebas de diagnostico rapido o microscopicas. Conclusion Varios desafios del sistema sanitario llevan a un gran porcentaje de ninos que reciben una gestion y tratamiento insuficientes de las enfermedades febriles. Es necesario intervenir para fortalecer la disponibilidad de productos basicos y mejorar el diagnostico y el tratamiento de las enfermedades febriles.

Abstract

Priority setting represents an even bigger challenge during public health emergencies than routine times. This is because such emergencies compete with routine programmes for the available health resources, strain health systems and shift health-care attention and resources towards containing the spread of the epidemic and treating those that fall seriously ill. This paper is part of a larger global study, the aim of which is to evaluate the degree to which national COVID-19 preparedness and response plans incorporated priority setting concepts. It provides important insights into what and how priority decisions were made in the context of a pandemic. Specifically, with a focus on a sample of 18 African countries pandemic plans, the paper aims to: (1) explore the degree to which the documented priority setting processes adhere to established quality indicators of effective priority setting and (2) examine if there is a relationship between the number of quality indicators present in the pandemic plans and the countrys economic context, health system and prior experiences with disease outbreaks. All the reviewed plans contained some aspects of expected priority setting processes but none of the national plans addressed all quality parameters. Most of the parameters were mentioned by less than 10 of the 18 country plans reviewed, and several plans identified one or two aspects of fair priority setting processes. Very few plans identified equity as a criterion for priority setting. Since the parameters are relevant to the quality of priority setting that is implemented during public health emergencies and most of the countries have pre-existing pandemic plans; it would be advisable that, for the future (if not already happening), countries consider priority setting as a critical part of their routine health emergency and disease outbreak plans. Abstract

Background: Injection drug users and female sex workers are two of the populations most at risk for becoming infected with HIV in countries with concentrated epidemics. Many of the adults who fall into these categories are also parents, but little is known about the vulnerabilities faced by their children, their children's sources of resilience, or programmes providing services to these often fragile families. This review synthesizes evidence from disparate sources describing the vulnerabilities and resilience of the children of female sex workers and drug users, and documents some models of care that have been put in place to assist them. Review: A large literature assessing the vulnerability and resilience of children of drug users and alcoholics in developed countries was found. Research on the situation of the children of sex workers is extremely limited. Children of drug users and sex workers can face unique risks, stigma and discrimination, but both child vulnerability and resilience are associated in the drug use literature with the physical and mental health of parents and family context. Family‐centred interventions have been implemented in low‐ and middle‐income contexts, but they tend to be small, piecemeal and struggling to meet demand; they are poorly documented, and most have not been formally evaluated. We present preliminary descriptive data from an organization working with pregnant and new mothers who are drug users in Ukraine and from an organization providing services to sex workers and their families in Zambia. Conclusions: Because parents' drug use or sex work is often illegal and hidden, identifying their children can be difficult and may increase children's vulnerability and marginalization. Abstract

Background: Maternity waiting homes (MWHs), defined as residential lodging near health facilities, are an intervention to improve access to maternal care recommended by the World Health Organization. Little is known about utilization of MWHs by HIV-positive women. This paper describes: 1) maternal awareness and utilization of MWHs in rural Zambia among HIV-positive women, and 2) health outcomes for HIV-positive women and their infants with regards to utilization of MWHs. Methods: Data were collected from recently delivered women (delivered after 35 weeks in the previous 12 months) living >9.5 km from 40 health facilities in rural Zambia. For our analysis, primary outcomes were compared between self-identified HIV-positive and HIV-negative women in the sample. Primary outcomes include: 1) awareness of MWHs and 2) utilization of MWHs. We summarized simple descriptive statistics, stratified by maternal self-reported HIV status. We conducted bivariate analyses using chi-square tests, t-tests and Wilcoxon rank sum test. Abstract

Background: Utilizing maternity waiting homes (MWHs) is a strategy to improve access to skilled obstetric care in rural Zambia. However, out-of-pocket (OOP) expenses remain a barrier for many women. We assessed deliveryrelated expenditure for women who used MWHs and those who did not who delivered at a rural health facility. Methods: During the endline of an impact evaluation for an MWH intervention, household surveys (n = 826) were conducted with women who delivered a baby in the previous 13 months at a rural health facility and lived >10 km from a health facility in seven districts of rural Zambia. We captured the amount women reported spending on delivery. We compared OOP spending between women who used MWHs and those who did not. Amounts were converted from Zambian kwacha (ZMW) to US dollar (USD). Results: After controlling for confounders, there was no significant difference in delivery-related expenditure between women who used MWHs (US$40.01) and those who did not (US$36.66) (P=.06). Both groups reported baby clothes as the largest expenditure. MWH users reported spending slightly more on accommodation compared to those did not use MWHs, but this difference represents only a fraction of total costs associated with delivery. Conclusion: Findings suggest that for women coming from far away, utilizing MWHs while awaiting delivery is not costlier overall than for women who deliver at a health facility but do not utilize a MWH.
