Science - Abstract
Abstract Competition among bacteria for carbohydrates is pivotal for colonization resistance (CR). However, the impact of Western-style diets on CR remains unclear. Here we show how the competition between Klebsiella oxytoca and Klebsiella pneumoniae is modulated by consuming one of three Western-style diets characterized by high-starch, high-sucrose, or high-fat/high-sucrose content. In vivo competition experiments in ampicillin-treated mice reveal that K. oxytoca promotes K. pneumoniae decolonization on all dietary backgrounds. However, mice on the high-fat/high-sucrose diet show reduced pathogen clearance. Microbiome analysis reveals that the combination of Western-style diets and ampicillin treatment synergize in microbiome impairment, particularly noticeable in the presence of high dietary fat content. The diet-independent degradation of ampicillin in the gut lumen by K. oxytoca beta-lactamases facilitates rapid commensal outgrowth, which is required for subsequent pathogen clearance. Our findings provide insights into how diet modulates functional microbiome recovery and K. oxytoca-mediated pathogen elimination from the gut.

Introduction The aim of this study was to implement a model of healthcare for patients with heart failure (HF) and to assess the differences between men and women in the study population. Material and methods Through care and an extensive educational panel (dietary recommendations, physical activity, titration of diuretics, psychological support), emphasis was placed on prevention. All patients had a baseline cardiology consultation with recommendations regarding treatment, self-care, diet, and exercise. Based on regular measurements of weight and blood pressure HF patients were telemonitored by nurses, primary care physicians and referred to cardiologists when necessary. The questionnaire was conducted in all patients at baseline after enrollment to the study and after 3 months of telemonitoring and extensive educational process. Results The study finally included 140 men with an average age of 66 years (SD: 56–71) and 163 women with an average age of 64 years (58–72). The ischemic origin of HF was present in 78% of men and 73% of women (p = 0.40), and 31% of men and 16% of women declared myocardial infarction in the past (p = 0.002). Men more often survived sudden cardiac arrest (7.8% vs. 1.2%; p = 0.004) and had implantable cardiac devices compared to women (16% vs. 3%; p = 0.001). Male patients reported significantly more often alcohol consumption, smoking, and less frequently any physical activity. Women significantly more often than men suffered from cancer, depression, and thromboembolic events and less frequently from obstructive sleep apnea. The mean left ventricular ejection fraction (LVEF) was 43% (SD: 30–58) for men, and 57% (45–63) for women (p = 0.0001), and women suffered mainly from heart failure with preserved ejection fraction. There were no differences between genders regarding the number of hospitalizations in last 12 months. The men, despite lower LVEF, felt short of breath/tired when climbing the stairs up than women (3 [2–4] vs. 2 floors [1–3]; p = 0.001), had higher distance in meters when walking on flat ground (400 [200–400] vs. 300 m [100–400]; p = 0.0001), and less frequently had to get up to go to the toilet at night (p = 0.03). Men also suffered significantly less often from shortness of breath at rest, swelling of the lower limbs and shortness of breath that wakes them up at night. Only 35% of men and 19% of women had HF self-care training, and only 35% of men and 46% of women knew how to increase the dose of diuretic on their own if shortness of breath or swelling increases. Based on the EuroQol-5 dimensions 5-levels questionnaire, significantly more women than men reported moderate and serious problems with moving around, serious problems with performing ordinary activities independently as well as moderate and serious pain problem and a significant level of anxiety. Men more often than women reported serious problems with self-care. Conclusions Women suffer more often from HF symptoms and have worse quality of life assessed in EQ-5D-5L than men despite their higher LVEF.

As can be seen from research, 44% of oncological problems disclose problems in relationships with a partner. About 80% of oncological patients report deterioration of the quality of their sexual life. Although the situation improves, a significant majority of ill persons do not obtain assistance in the scope of sexuality during oncological treatment. Intimacy is an important sphere of life and can support the process of recovery. Intimacy plays special role in the difficult period of the fight against disease. Independently of the applied method of oncological treatment, side effects of therapy can influence one’s sexual life. This means that an increasing number of persons who are healed from cancerous disease are exposed to the long-term undesirable influence of treatment. The process of oncological treatment has an essential impact on the intimate life of those who suffer from oncological diseases. To a great extent, this process and the disease itself contribute to a decrease of the quality of life due to the appearance of symptoms caused by earlier menopause, they negatively influence the feeling desire and sexual performance. Unfortunately, despite the development of oncosexology, sexual problems of oncologically ill persons are not noticed, and sometimes they are played down or overlooked due to everyday problems.

Objectives The aim of the research was to diagnose the key didactic problems encountered in educating refugee children from Ukraine in Polish schools. Material and methods The research material was obtained using a nationwide online diagnostic survey among teachers teaching refugee children from Ukraine (N=1720). The research tool was a didactic difficulty scale consisting of 12 questions in the form of a five-point Likert scale. Results Statistical analysis showed that cultural issues and language barriers strongly influence didactic difficulties. The main factors complicating the transmission of knowledge in Polish schools are the lack of teachers' competencies to work with linguistically and culturally diverse children and the low motivation of Ukrainian students to learn. The greatest didactic problems concern the diagnosis of the child's knowledge level and special educational needs, as well as the motivation to learn Polish. Conclusions Teachers with highest qualifications had fewer problems with transmitting knowledge and assessing students, but the level of professional advancement did not affect their ability to motivate students and build relationships with their families. The most problems were experienced by teachers of grades I-III, which may be due to the adaptive difficulties of the youngest students. Support is needed for all teachers, regardless of workplace, in the areas of communication, intercultural education, and managing cultural differences.

Introduction Inflammation in fetuses and newborns, particularly in the population of preterm infants, is associated with increased mortality and adverse health outcomes. It has been shown that the placental transfer of polyunsaturated fatty acids (PUFAs) is limited in very preterm infants. Studies have demonstrated that low levels of PUFAs correlate with the severity of inflammation in infants' bodies. They are also associated with an increased incidence of diseases, including bronchopulmonary dysplasia and retinopathy of prematurity. One of the beneficial effects of PUFA supplementation is its inhibitory action on inflammatory processes in the body. Aim of the study Supplements containing fatty acids are among the most popular complementary health interventions introduced in children. Considering this potential benefit, we would like to present research studies that describe the relationship between the supplementation of polyunsaturated fatty acids and the levels of inflammatory markers in preterm infants. Materials and Methods A review of randomized clinical trials (RCTs) published in 2020-2024 regarding the relationship between the supplementation of polyunsaturated fatty acids and the levels of inflammatory markers in preterm infants. Three studies meeting specific selection criteria were identified. Results All the studies discussed in our work demonstrate that supplementation with polyunsaturated fatty acids in preterm infants reduces the levels of inflammatory markers. Research consistently shows that PUFA supplementation lowers IL-6 levels, a cytokine considered a risk factor for sepsis in preterm infants. Conclusion Supplementation with polyunsaturated fatty acids holds potential for modulating inflammatory processes in preterm infants, with efficacy likely influenced by factors such as the specific type of fatty acid used, timing and form of administration, and individual variations in response.

Introduction Bone health is a critical component in maintaining physical fitness and overall quality of life. Among the factors influencing bone tissue structure, an appropriate level of physical activity plays a pivotal role. Numerous studies in the literature report the beneficial effects of regular exercise on bone mineral density (BMD). Objective This study aims to review current scientific reports and summarize the state of knowledge regarding the impact of physical activity on bone mineral density. Materials and Methods A review of randomized controlled trials (RCTs) published between 2022 and 2024 was conducted to evaluate the effects of physical activity on bone mineral density. Four studies meeting predefined selection criteria were identified. Results The findings demonstrate that physical activity significantly impacts bone mineral density. Both aerobic and resistance training contribute to increased BMD. Resistance training, particularly when combined with other forms of physical activity or dietary interventions, offers notable benefits, especially in populations with excess body fat. Conclusions Regular physical exercise, particularly resistance training, plays a significant role in improving bone mineral density.

PurposeSystemic sclerosis (SSc) affects blood vessels, internal organs, and skin. In ophthalmology, SSc impacts the choroid. The choroidal vascularity index (CVI) measures the vascular component of the choroid and may serve as a biomarker for the disease staging and prognosis. Studies have reported reduced choroidal thickness and altered CVI in SSc, which supports the theory of vascular damage. This study aimed to examine interocular symmetry in choroidal parameters among SSc patients. It has provided the insight into the disease symmetry and assessed the representativeness of examining one eye.MethodsThis prospective single-center cross-sectional study included 33 patients with SSc and 40 healthy controls. The patients underwent ophthalmological examination (including refraction, visual acuity, IOP, biometry, slit-lamp biomicroscopy, dilated fundus examination, and spectral-domain optical coherence tomography) and rheumatological evaluation. Various parameters of the choroid in the macular and peripapillary regions were analyzed, including choroidal thickness, choroidal volume, and CVI. The interocular asymmetry in the choroidal parameters was quantified using signed and absolute differences. The correlation analysis between the left and right eyes was based on the intraclass correlation coefficient (ICC), Spearman’s correlation coefficient, and paired Wilcoxon test.ResultsThere were no significant differences in the macular and peripapillary choroidal parameters between fellow eyes in both SSc patients and controls (p > 0.05). The parameter that showed the lowest correlation among those examined was CVI—in both groups, as well as in both examined areas. The interocular correlation of choroidal parameters was stronger in the peripapillary area than in the macular area in both groups. In general, the results were confirmed in subgroup analyses stratified according to sex, SSc subtype, Scl70 antibody positivity and previous and/or active digital ulcers.ConclusionThere is interocular symmetry of the choroidal parameters in patients with SSc and controls included in our study. The parameters from one eye are representative of the fellow eye of a given patient. This conclusion may contribute to the design and interpretation of future studies. It also broadens our knowledge of SSc pathophysiology.

Introduction and purpose Over the years piercing has become popular as a form of art and self-expression that may concern various areas of the body. However, it may pose significant risks to health. This literature review aims to provide insight into the matter of oral piercing and to examine recent studies investigating the implications of oral piercing on health. It emphasizes the necessity of promoting education regarding this matter and points out the most significant preventive measures such as regular dental examinations and maintaining proper hygiene that should be taken to minimize the negative consequences of oral piercing on health. The state of knowledge According to the examined studies investigating the implications of oral piercing on health, a wide range of complications have been reported. Gingival recession, damage to dentition and gingivitis should be mentioned among the most frequently discussed. Moreover, studies have demonstrated a correlation between the severity of complications and the location of the piercing, specifying the adverse effects of the piercing within lip and tongue areas in particular. They have also underscored the impact of prolongated wear time on the risk of complications, indicating the elevated risk associated with extended periods of wear time. Conclusions To minimize adverse effects of oral piercing on health, it is crucial to underscore the role of proper education in this field and the importance of taking preventive measures such as performing the piercing procedure by well qualified professionals who endure sterile work conditions and maintaining proper aftercare of the ornament. The role of regular dental examinations taken in order to detect and treat potential complications at early stages is also vital.

Introduction Hypertension is one of the most common chronic diseases, and its prevalence continues to rise. Among the lesser-known factors contributing to the increased incidence of hypertension is vitamin D deficiency. Vitamin D, primarily known for its role in calcium and phosphate metabolism, has been implicated as one of the factors influencing cardiovascular function. Aim of the study The objective of this review is to analyze the current literature regarding the impact of vitamin D supplementation on blood pressure levels. Materials and Methods This review incorporates literature from the PubMed database, which was searched using the keywords "hypertension" and "vitamin D." Randomized controlled trials published between 2020 and 2024 were included. Results The literature review reveals various aspects of the relationship between vitamin D supplementation and blood pressure regulation. Hypertension remains one of the most prevalent chronic diseases, with its frequency consistently rising. Despite the development of new treatment methods, many patients still fail to achieve effective blood pressure control, posing a significant challenge to healthcare. Increasing evidence suggests that vitamin D may play a role in regulating blood pressure, which is particularly relevant given the widespread deficiency of this vitamin in global populations. Vitamin D deficiency is a common issue affecting a substantial portion of society, making the topic of supplementation and its potential effects even more urgent. Conclusion There is evidence to suggest potential benefits of vitamin D supplementation on blood pressure regulation, although the study results are inconclusive and do not yet support definitive clinical recommendations.

Introduction Migraine, a common neurological disorder, significantly reduces quality of life, particularly among women. Despite advancements in treatment, therapeutic options remain limited, and side effects are common. The ketogenic diet (KD), characterized by low carbohydrate and high fat intake, has shown promise in reducing migraine frequency and severity. Aim of the study The aim of this study is to review the literature on the effectiveness of of the ketogenic diet on the frequency of migraine attacks and their severity. Materials This review analyzed two studies by Di Lorenzo et al. (2018) and Caprio et al. (2023), both demonstrating that KD significantly reduced migraine days compared to non-ketogenic diets. Additional benefits included reduced inflammatory markers and improved body weight. Results While findings are promising, limitations such as small sample sizes, focus on overweight individuals, and lack of long-term safety data highlight the need for larger studies. Conclusions KD offers potential as an effective alternative for managing migraines, but further research is required to confirm its benefits and safety.

Summary: Diet significantly influences the human gut microbiota, a key player in health. We analyzed shotgun metagenomic sequencing data from healthy individuals with long-term dietary patterns—vegan, flexitarian, or omnivore—and included detailed dietary surveys and blood biomarkers. Dietary patterns notably affected the bacterial community composition by altering the relative abundances of certain species but had a minimal impact on microbial functional repertoires. However, diet influenced microbial functionality at the strain level, with diet type linked to strain genetic variations. We also found molecular signatures of selective pressure in species enriched by specific diets. Notably, species enriched in omnivores exhibited stronger positive selection, such as multiple iron-regulating genes in the meat-favoring bacterium Odoribacter splanchnicus, an effect that was also validated in independent cohorts. Our findings offer insights into how diet shapes species and genetic diversity in the human gut microbiota.

Medicine - Abstract
Objective Systemic sclerosis Impact of Disease (ScleroID) is the first comprehensive patient-reported outcome measure (PROM) specifically developed for systemic sclerosis (SSc). We investigated the performance of ScleroID in patients with diffuse cutaneous SSc (dcSSc), as a prerequisite for its use in randomised controlled trials (RCTs) testing potentially disease-modifying drugs. Methods All patients with dcSSc from the large, multicentric, ScleroID cohort were included. SSc-Health Assessment Questionnaire (HAQ), EuroQol-5 Dimensions and 36-item Short Form Health Survey (SF-36) were used as comparators. The study includes a longitudinal arm with a reliability visit at 7±3 days and a 12 months follow-up visit. The performance of ScleroID in dcSSc was assessed according to the Outcome Measures in Rheumatology filter. Results In total, 152 dcSSc patients were analysed (29% male, median age 54 years). ScleroID reflected well the disease impact of dcSSc, showing a good construct validity with high Spearman’s correlation coefficients with comparators (SSc-HAQ, 0.79, 95% CI (0.69, 0.86); HAQ-Disability Index, 0.72 95% CI (0.60, 0.80); SF-36 physical score, −0.69 95% CI (−0.77, –0.60)). The internal consistency was strong (Cronbach’s alpha 0.87, split-half reliability coefficient 0.88).In the longitudinal arm, 44 patients had a reliability visit and 113 had a follow-up visit, of whom 19/113 (17%) reported a significant change (11 improved, 8 worsened). ScleroID showed a good consistency and discriminative ability with excellent test–retest reliability (intraclass correlation coefficient 0.89, 95% CI (0.84, 0.92)) and moderate sensitivity to change (standardised response mean −0.63 in the improved subgroup and 0.48 in the worsened subgroup), but superior to the comparators. Conclusion The European Alliance of Associations for Rheumatology (EULAR) ScleroID performs well for patients with dcSSc. This supports its inclusion and regular assessment as PROM in RCTs.

This paper presents the theoretical axioms of chorems based on cartographic semiotics. Employing geometric and functional elements and the cartographic state-of-the-art, a regional, national chorem has been created. This chorem portrays the barriers to Polish regional development. It depicts the main development centres, industrial areas and zones of constraint created by the country’s external borders, borders of the European Union (EU) and major rivers. The paper also defines chorematic affordance as the dynamic and processual feedback between the domain expert and the geographical or socio-economic processes. The elaborate chorematic diagram contributes not only to cartographers and geographers but also to public administration dealing with sustainable development, as it is intricately tied to regional development, emphasising the crucial importance of effective land management, equitable land distribution and sustainable development.

The sous vide cooking method offers advantage in preparing meat dishes in advance. This study aimed to evaluate the effect of refrigerated storage on the quality attributes of pork loin sous vide cooked under selected, optimised temperature and time parameters. Pork loin was cooked at varying temperatures (57–63 °C) and times (3.5–5.5 h), followed by refrigerated storage for up to 7 days. Analytical methods, including TBARS index for lipid oxidation, instrumental colour measurement, texture analysis, and volatile compound profiling, were used to assess changes in meat quality over time. The results indicated that heat treatment and storage time significantly influenced lipid oxidation, colour, and texture. The highest TBARS values were observed at higher cooking temperatures (61 °C and 63 °C), reflecting increased lipid oxidation. Colour changes were also temperature- and time-dependent, with a decrease in redness (a*) and an increase in lightness (L*) and yellowness (b*). Sensory evaluation revealed that juiciness, aroma intensity, tenderness, and flavour acceptability were strongly correlated with overall acceptability, while physical characteristics like colour and texture had a lesser impact. The study highlights the impact of sous vide cooking parameters on the quality and sensory attributes of pork loin, suggesting that optimised heat treatment can help preserve desirable meat characteristics during refrigerated storage.

Protein-enriched yogurts have become increasingly popular among consumers seeking to boost their daily protein intake. The incorporation of milk proteins and protein preparations in yogurt production not only enhances nutritional value but also improves texture, viscosity, and overall sensory properties—key factors that influence consumer acceptance. The main objective of this study was to evaluate the influence of casein and whey protein preparations on the physicochemical properties, viability of lactic acid bacteria, and sensory attributes of yogurts. Yogurts were enriched with 2% (w/w) protein preparations, including micellar casein preparation (CN85), whey protein isolate (WPI), whey protein concentrate (WPC60), and protein preparations obtained from skim milk by membrane filtration: micellar casein concentrate (CN75) and serum protein concentrate (SPC). The yogurts were produced using the thermostatic method, and their chemical composition, rheological properties, syneresis, firmness, lactic acid bacteria population, and sensory attributes were evaluated. The effects of high-protein preparations derived from skim milk through laboratory-scale membrane filtration processes (SPC, CN75) were compared with those of commercially available protein preparations (SMP, CN85, WPI, and WPC). Obtained results demonstrated that the membrane filtration-derived preparations (SPC and CN75) exhibited advantageous physicochemical properties and supported robust viability of yogurt and probiotic bacteria. However, their sensory quality was marginally inferior compared to the commercial preparations (SMP, CN85, WPI, and WPC). These findings indicate the potential applicability of membrane filtration-derived protein preparations in yogurt production while underscoring the necessity for further investigation to enhance and optimize their sensory characteristics.

The present study was undertaken to investigate the impact of curdlan and buttermilk addition on the physicochemical and sensory attributes of processed cheeses (PCs), thereby elucidating their potential utility in culinary applications. Comprehensive analyses were conducted to assess the chemical composition, textural and rheological properties, microstructural features, and sensory characteristics of PCs. The findings indicate that the addition of curdlan notably decreased both the hardness and stickiness of cheeses but also significantly reduced their meltability. Conversely, an increase in buttermilk content was correlated with enhanced hardness of the cheese matrix. In addition, buttermilk provided a creamier taste, thereby increasing the product’s appeal for consumers. The colorimetric analysis revealed that buttermilk-induced cheese darkening via the Maillard reaction, whereas curdlan addition resulted in a modest increase in yellowness. Buttermilk-containing cheeses received high scores for smoothness and flavor in sensory evaluations. A marked decrease in the functional properties of PCs, such as meltability, was observed when buttermilk addition exceeded 2.5%. Overall, the results of this study suggest that the controlled addition of curdlan and buttermilk positively affects the texture, color, and flavor of PCs, and it provides valuable information for the cheese industry.

This study highlights the critical role of maintaining high power quality in autonomous grids, particularly in complex systems with significant non-linear loads, such as shipboard power systems. Power quality issues such as voltage harmonic distortion and imbalance can lead to serious malfunctions and even accidents. This paper proposes an advanced predictive control algorithm with feedback from the supply current developed for a shunt active power filter. The presented control algorithm is characterized by high effectiveness in compensating supply current harmonics, imbalance, and reactive power, thereby improving power quality. This high performance in executing compensation tasks has been confirmed through experimental tests conducted in an autonomous grid powered by a diesel generator. The research results also include the system’s adaptability under varying load conditions, the impact of delays, and the converter’s non-linearities on the operation of the active power filter.

Lipoprotein(a) [Lp(a)] is made up of a low-density lipoprotein (LDL) particle and a specific apolipoprotein(a). The blood concentration of Lp(a) is approximately 90% genetically determined, and the main genetic factor determining Lp(a) levels is the size of the apo(a) isoform, which is determined by the number of KIV2 domain repeats. The size of the apo(a) isoform is inversely proportional to the blood concentration of Lp(a). Lp(a) is a strong and independent cardiovascular risk factor. Elevated Lp(a) levels ≥ 50 mg/dl (≥ 125 nmol/l) are estimated to occur in more than 1.5 billion people worldwide. However, determination of Lp(a) levels is performed far too rarely, including Poland, where, in fact, it is only since the 2021 guidelines of the Polish Lipid Association (PoLA) and five other scientific societies that Lp(a) measurements have begun to be performed. Determination of Lp(a) concentrations is not easy due to, among other things, the different sizes of the apo(a) isoforms; however, the currently available certified tests make it possible to distinguish between people with low and high cardiovascular risk with a high degree of precision. In 2022, the first guidelines for the management of patients with elevated lipoprotein(a) levels were published by the European Atherosclerosis Society (EAS) and the American Heart Association (AHA). The first Polish guidelines are the result of the work of experts from the two scientific societies and their aim is to provide clear, practical recommendations for the determination and management of elevated Lp(a) levels.

Background: Malignant gliomas represent a heterogenous group of brain cancers that are characterized by infiltrative growth that lacks a clearly identifiable tumor border. The lack of the possibility of radical surgical resection and targeted therapy results in a poor prognosis. Although Temozolomide (TMZ) is still the leading chemotherapeutic agent in glioma treatment, its efficacy is limited due to the development of tumor resistance. Therefore, there an urgent need to improve the diagnosis and treatment of these tumors. Finding and developing biomarkers that are specific to glioma could be useful for both identifying therapy targets and monitoring treatment as well as for constructing a personalized therapy. However, there are still no reliable markers that would change the quality of glioma treatment. Methods: In this study, differences in the expression of 84 cancer-related proteins in three glioma cell lines were analyzed using the dot-blot method: commercial T98G cells and two patient-derived cell lines. The influence of TMZ on changes in protein expression, cell morphology, and migration was also investigated (Proteome Profiler Human XL Oncology Array, LeviCell System, Microscopic imaging). The lines that were analyzed were characterized by a remarkably different plasticity of protein expression and the proteomic alterations that were induced by TMZ. Results: A dot-blot analysis revealed ten proteins that were common to all of the lines and five (Cathepsin b, FGF, Survivin, AXL, Osteopontin) that were modulated by the TMZ. As a result of the exposure of TMZ, the proteins that are involved in chemoresistance and invasion (TIE-2, Thrombospondin) were detected in both the HROG02 and T98G cell lines. In the control culture (not exposed to TMZ) of HROG17 cells, the proteins that are involved in metabolism were strongly suppressed. Conclusions: The presented data sheds new light on the modulatory effect of Temozolomide on the expression of a protein panel: Cathepsin b, fibroblast growth factor (FGF), Survivin, AXL, and Osteopontin that may suggest their potential as therapeutic targets or biomarkers to monitoring therapy effects. However, further high-throughput analysis and detection of the proteins in the body fluids are necessary.
