Abstract: For the High-Luminosity (HL-LHC) phase, the upgrade of the Compact Muon Solenoid (CMS) experiment at CERN will include a novel MIP Timing Detector (MTD). The central part of MTD, the barrel timing layer (BTL), is designed to provide a measurement of the time of arrival of charged particles with a precision of 30 ps at the beginning of HL-LHC, progressively degrading to 60 ps while operating in an extremely harsh radiation environment for over a decade. In this paper we present a comparative analysis of the time resolution of BTL module prototypes made of LYSO:Ce crystal bars read out by silicon photo-multipliers (SiPMs). The timing performance measured in beam test campaigns is presented for prototypes with different construction and operation parameters, such as different SiPM cell sizes (15, 20, 25 and 30 $\rm \mu m$), SiPM manufacturers and crystal bar thicknesses. The evolution of time resolution as a function of the irradiation level has been studied using non-irradiated SiPMs as well as SiPMs exposed up to $2\times 10^{14}~n_{eq}/cm^2$ fluence. The key parameters defining the module time resolution such as SiPM characteristics (gain, photon detection efficiency, radiation induced dark count rate) and crystal properties (light output and dimensions) are discussed. These results have informed the final choice of the MTD barrel sensor configuration and offer a unique starting point for the design of future large-area scintillator-based timing detectors in either low or high radiation environments.

2. Learning policies for resource allocation in business processes

Abstract: Efficient allocation of resources to activities is pivotal in executing business processes but remains challenging. While resource allocation methodologies are well-established in domains like manufacturing, their application within business process management remains limited. Existing methods often do not scale well to large processes with numerous activities or optimize across multiple cases. This paper aims to address this gap by proposing two learning-based methods for resource allocation in business processes. The first method leverages Deep Reinforcement Learning (DRL) to learn near-optimal policies by taking action in the business process. The second method is a score-based value function approximation approach, which learns the weights of a set of curated features to prioritize resource assignments. To evaluate the proposed approaches, we first designed six distinct business processes with archetypal process flows and characteristics. These business processes were then connected to form three realistically sized business processes. We benchmarked our methods against traditional heuristics and existing resource allocation methods. The results show that our methods learn adaptive resource allocation policies that outperform or are competitive with the benchmarks in five out of six individual business processes. The DRL approach outperforms all benchmarks in all three composite business processes and finds a policy that is, on average, 13.1% better than the best-performing benchmark.

Abstract: Cerium-doped Lutetium-Yttrium Oxyorthosilicate (LYSO:Ce)is one of the most widely used Cerium-doped Lutetium based scintillation crystals. Initially developed for medical detectors it rapidly became attractive for High Energy Particle Physics (HEP) applications, especially in the frame of high luminosity particle colliders. In this paper, a comprehensive and systematic study of LYSO:Ce ($[Lu_{(1-x)}Y_x]_2SiO_5$:$Ce$) crystals is presented. It involves for the first time a large number of crystal samples (180) of the same size from a dozen of producers.The study consists of a comparative characterization of LYSO:Ce crystal products available on the market by mechanical, optical and scintillation measurements and aims specifically, to investigate key parameters of timing applications for HEP., Comment: 38 pages, 22 figures

Abstract: Background: Despite the well-known efficacy of anti-COVID-19 vaccines in preventing morbidity and mortality, several vaccinated individuals are diagnosed with SARS-CoV-2 breakthrough infection, which might require hospitalisation. This multicentre, observational, and retrospective study aimed to investigate the clinical characteristics and outcomes of vaccinated vs. non-vaccinated patients, both hospitalised with SARS-CoV-2 infection in 3 major hospitals in Northern Italy. Methods: Data collection was retrospective, and paper and electronic medical records of adult patients with a diagnosed SARS-CoV-2 infection were pseudo-anonymised and analysed. Vaccinated and non-vaccinated individuals were manually paired, using a predetermined matching criterion (similar age, gender, and date of hospitalisation). Demographic, clinical, treatment, and outcome data were compared between groups differing by vaccination status using Pearson's Chi-square and Mann-Whitney tests. Moreover, multiple logistic regression analyses were performed to assess the impact of vaccination status on ICU admission or intra-hospital mortality. Results: Data from 360 patients were collected. Vaccinated patients presented with a higher prevalence of relevant comorbidities, like kidney replacement therapy or haematological malignancy, despite a milder clinical presentation at the first evaluation. Non-vaccinated patients required intensive care more often than their vaccinated counterparts (8.8% vs. 1.7%, p = 0.002). Contrariwise, no difference in intra-hospital mortality was observed between the two groups (19% vs. 20%, p = 0.853). These results were confirmed by multivariable logistic regressions, which showed that vaccination was significantly associated with decreased risk of ICU admission (aOR=0.172, 95%CI: 0.039–0.542, p = 0.007), but not of intra-hospital mortality (aOR=0.996, 95%CI: 0.582–1.703, p = 0.987). Abstract: A large portion of modern olive plantings is dominated by one main cultivar (‘Arbequina’) planted at super high density (SHD, 1,500 tree ha‑1), leading to a dangerous reduction in olive biodiversity. Very few cultivars have growth and productive patterns that suit the SHD system requirements. Partial intensification and mechanization with several cultivars may represent the safest and easiest alternative. The efficient use of limited natural resources, such as water, in SHD systems, requires precise management. Continuous plant-based sensing is a promising approach to managing water, mainly tested on the cultivar ‘Arbequina’. In this study, we evaluated the sensitivity of two promising indicators of plant water status in two Sicilian olive cultivars differ in their level of drought tolerance: 1) the minimum daily output pressure (Ppmin) measured with the leaf patch clamp pressure (LPCP) probe and 2) the maximum daily relative growth rate difference (RGRrange) measured with fruit gauges. Based on the indicators trends, we defined four water status conditions and related them to specific midday stem water potential (Ψstem) thresholds. In addition, the relation between Ppmin and RGRrange highlighted a greater tendency of the more drought tolerant cultivar to maintain leaf turgor under water deficit. This was accompanied by an increase in daily variations of leaf turgor and fruit shrinkage, not observed in the less drought tolerant cultivar. These results suggest tailoring plant-based water management protocols on cultivar-specific physiological responses to water deficit.

Abstract: Background: The coronavirus disease 2019 (COVID-19) presents an urgent threat to global health. Prediction models that accurately estimate mortality risk in hospitalized patients could assist medical staff in treatment and allocating limited resources. Aims: To externally validate two promising previously published risk scores that predict in-hospital mortality among hospitalized COVID-19 patients. Methods: Two prospective cohorts were available; a cohort of 1028 patients admitted to one of nine hospitals in Lombardy, Italy (the Lombardy cohort) and a cohort of 432 patients admitted to a hospital in Leiden, the Netherlands (the Leiden cohort). The endpoint was in-hospital mortality. All patients were adult and tested COVID-19 PCR-positive. Model discrimination and calibration were assessed. Results: The C-statistic of the 4C mortality score was good in the Lombardy cohort (0.85, 95CI: 0.82−0.89) and in the Leiden cohort (0.87, 95CI: 0.80−0.94). Model calibration was acceptable in the Lombardy cohort but poor in the Leiden cohort due to the model systematically overpredicting the mortality risk for all patients. The C-statistic of the CURB-65 score was good in the Lombardy cohort (0.80, 95CI: 0.75−0.85) and in the Leiden cohort (0.82, 95CI: 0.76−0.88). The mortality rate in the CURB-65 development cohort was much lower than the mortality rate in the Lombardy cohort. A similar but less pronounced trend was found for patients in the Leiden cohort. Conclusion: Although performances did not differ greatly, the 4C mortality score showed the best performance. However, because of quickly changing circumstances, model recalibration may be necessary before using the 4C mortality score.

Abstract: Cerium-doped Lutetium-Yttrium Oxyorthosilicate (LYSO:Ce) is one of the most widely used Cerium-doped Lutetium based scintillation crystals. Initially developed for medical detectors it rapidly became attractive for High Energy Particle Physics (HEP) applications, especially in the frame of high luminosity particle colliders. In this paper, a comprehensive and systematic study of LYSO:Ce ([Lu(1-x)Yx ]2SiO5:Ce) crystals is presented. It involves for the first time a large number of crystal samples (180) of the same size from a dozen of producers. The study consists of a comparative characterization of LYSO:Ce crystal products available on the market by mechanical, optical and scintillation measurements and aims specifically, to investigate key parameters of timing applications for HEP.

Abstract: During the COVID-19 2020 outbreak, a large body of data has been provided on general management and outcomes of hospitalized COVID-19 patients. Yet, relatively little is known on characteristics and outcome of patients managed in Internal Medicine Units (IMU). To address this gap, the Italian Society of Internal Medicine has conducted a nationwide cohort multicentre study on death outcome in adult COVID-19 patients admitted and managed in IMU. This study assessed 3044 COVID-19 patients at 41 referral hospitals across Italy from February 3rd to May 8th 2020. Demographics, comorbidities, organ dysfunction, treatment, and outcomes including death were assessed. During the study period, 697 patients (22.9%) were transferred to intensive care units, and 351 died in IMU (death rate 14.9%). At admission, factors independently associated with in-hospital mortality were age (OR 2.46, p = 0.000), productive cough (OR 2.04, p = 0.000), pre-existing chronic heart failure (OR 1.58, p = 0.017) and chronic obstructive pulmonary disease (OR 1.17, p = 0.048), the number of comorbidities (OR 1.34, p = 0.000) and polypharmacy (OR 1.20, p = 0.000). Abstract: Cerium-doped Lutetium-Yttrium Oxyorthosilicate (LYSO:Ce)is one of the most widely used Cerium-doped Lutetium based scintillation crystals. Initially developed for medical detectors it rapidly became attractive for High Energy Particle Physics (HEP) applications, especially in the frame of high luminosity particle colliders. In this paper, a comprehensive and systematic study of LYSO:Ce ($[Lu_{(1-x)}Y_x]_2SiO_5$:$Ce$) crystals is presented. Abstract: Background: Immune-checkpoint inhibitors (ICIs) have increased and improved the treatment options for patients with non-oncogene-addicted advanced stage non-small cell lung cancer (NSCLC). However, the role of ICIs in oncogene-addicted advanced stage NSCLC patients is still debated. In this study, in an attempt to fill in the informational gap on the effect of ICIs on other driver mutations, we set out to provide a molecular landscape of clinically relevant oncogenic drivers in programmed death-ligand 1 (PD-L1) positive NSCLC patients. Methods: We retrospectively reviewed data on 167 advanced stage NSCLC PD-L1 positive patients (≥1%) who were referred to our clinic for molecular evaluation of five driver oncogenes, namely, EGFR, KRAS, BRAF, ALK and ROS1. Results: Interestingly, n = 93 (55.7%) patients showed at least one genomic alteration within the tested genes. Furthermore, analyzing a subset of patients with PD-L1 tumor proportion score (TPS) ≥ 50% and concomitant gene alterations (n = 8), we found that n = 3 (37.5%) of these patients feature clinical benefit with ICIs administration, despite the presence of a concomitant KRAS gene alteration. Conclusions: In this study, we provide a molecular landscape of clinically relevant biomarkers in NSCLC PD-L1 positive patients, along with data evidencing the clinical benefit of ICIs in patient NSCLC PD-L1 positive alterations.

Abstract: Coronavirus disease of 2019 (COVID-19) is associated with severe acute respiratory failure. Early identification of high-risk COVID-19 patients is crucial. We aimed to derive and validate a simple score for the prediction of severe outcomes. A retrospective cohort study of patients hospitalized for COVID-19 was carried out by the Italian Society of Internal Medicine. Epidemiological, clinical, laboratory, and treatment variables were collected at hospital admission at five hospitals. Three algorithm selection models were used to construct a predictive risk score: backward Selection, Least Absolute Shrinkage and Selection Operator (LASSO), and Random Forest. Severe outcome was defined as the composite of need for non-invasive ventilation, need for orotracheal intubation, or death. A total of 610 patients were included in the analysis, 313 had a severe outcome. The subset for the derivation analysis included 335 patients, the subset for the validation analysis 275 patients. The LASSO selection identified 6 variables (age, history of coronary heart disease, CRP, AST, D-dimer, and neutrophil/lymphocyte ratio) and resulted in the best performing score with an area under the curve of 0.79 in the derivation cohort and 0.80 in the validation cohort. Using a cut-off of 7 out of 13 points, sensitivity was 0.93, specificity 0.34, positive predictive value 0.59, and negative predictive value 0.82. The proposed score can identify patients at low risk for severe outcome who can be safely managed in a low-intensity setting after hospital admission for COVID-19.

Abstract: This paper presents the development of a cabin noise testing equipment that will be used to evaluate the interior noise of regional aircraft as well as to aid the development of noise reduction techniques. The innovative noise generation system consists of three loudspeaker arrays positioned around the fuselage circumference to synthesize a pressure field that is similar to the pressure field seen by the fuselage during a flight. The acoustic pressure field generated by the loudspeakers is measured by a number of microphones scattered on the fuselage surface. These microphone signals are then fed back to the controller with the purpose of minimising the error between the target pressure field and the measured one by means of an iterative learning approach. The number and location of the microphones used in the control loop are selected through a pre-test optimisation analysis, which aims to reduce the time and cost of the set-up. A small-scale electroacoustic demonstrator has been built to develop the feedback control approach. A frequency domain multi-input multi-output feedback controller is used to replicate the random pressure field generated by the turbulent boundary layer excitation. The multiharmonics of the propeller induced excitation are then added to the time histories of the broadband noise using a time waveform replication technique. Different arrangements of the driving signal distribution are investigated, and the results are then presented in terms of accuracy of the pressure field reproduction.

Abstract: Objectives: To evaluate the effect on quality of life (QOL)of the addition of cisplatin to single-agent chemotherapy in the treatment of elderly patients with advanced non-small cell lung cancer (NSCLC)enrolled in two parallel phase 3 trials, MILES-3 and MILES-4. Patients and methods: Advanced NSCLC pts, >70 years old, performance status (PS)0–1, were eligible. Patients were randomly assigned to chemotherapy without or with cisplatin. EORTC QLQ C30 and LC13 questionnaires were planned at baseline, end of cycle 1 and end of cycle 2 in both trials and were used for joint QOL analysis. Trial-specific data including questionnaires at non-shared time-points were used for additional analyses. Intention-to-treat strategy was applied. Analyses were adjusted for baseline QOL, stage, performance status, gender, age, size of centre, trial, histotype and non-platinum companion drug. Results: Overall, 458/531 pts (86%)answered baseline questionnaire and missing rates over treatment were slightly higher among patients receiving cisplatin. Mean change in sore mouth after cycle 2 was worse with cisplatin (P = 0.02). The size of differences between arms was in the small-medium range for peripheral neuropathy and alopecia (0.25 and 0.31 after one and 0.28 and 0.36 after two cycles, respectively)and for nausea/vomiting, sore mouth and dysphagia after two cycles (0.26, 0.38 and 0.25, respectively)always in the direction of worsening with cisplatin. Using a 10% change from baseline as clinically relevant threshold to categorize response, there was no significant difference between the arms. Time to deterioration of sore mouth and alopecia, with progression/death as competitive risk, was shorter with cisplatin (HR 1.72 95%CI 1.02–2.89, P = 0.04 and HR 1.84 95%CI 1.09–3.10, P = 0.02, respectively). Conclusion: The addition of cisplatin to single agent chemotherapy worsens sore mouth and alopecia and does not improve any QOL items in elderly patients with advanced NSCLC.

Abstract: In the era of precision medicine, the identification of several predictive biomarkers and the development of innovative therapies have dramatically increased the request of tests to identify specific targets on cytological or histological samples, revolutionizing the management of the tumoral biomaterials. The Food and Drug Administration (FDA) has recently approved a selective neurotrophic tyrosine receptor kinase (NTRK) inhibitor, larotrectinib. Contemporarily, the development of multi-kinase inhibitors with activity in tumors carrying TRK fusions is ongoing. Chromosomal translocations involving the NTRK1, NTRK2, and NTRK3 genes result in constitutive activation and aberrant expression of TRK kinases in numerous cancer types. In this context, the identification of tumors harboring TRK fusions is crucial. Several methods of detection are currently available. We revise the advantages and disadvantages of different techniques used for identifying TRK alterations, including immunohistochemistry, fluorescence in situ hybridization, reverse transcriptase polymerase chain reaction, and next generation sequencing-based approaches. Finally, we propose a diagnostic algorithm based on histology and the relative frequency of TRK fusions in each specific tumor, considering also the economic feasibility in the clinical practice.

Abstract: Purpose: To test the efficacy of adding cisplatin to first-line treatment for elderly patients with advanced non-small-cell lung cancer (NSCLC) within a combined analysis of two parallel phase III trials, MILES-3 and MILES-4. Patients and Methods: Patients with advanced NSCLC who were older than age 70 years with Eastern Cooperative Oncology Group performance status 0 to 1 were randomly assigned to gemcitabine or pemetrexed, without or with cisplatin. In each trial, 382 events were required to detect a hazard ratio (HR) of death of 0.75, with 80% power and two-tailed a of .05. Trials were closed prematurely because of slow accrual, but the joint database allowed us to analyze the efficacy of cisplatin on the basis of intention-to-treat and adjusted by trial, histotype, non-platinum companion drug, stage, performance status, sex, age, and size of the study center. Results: From March 2011 to August 2016, 531 patients (MILES-3, 299; MILES-4, 232) were assigned to gemcitabine or pemetrexed without (n = 268) or with cisplatin (n = 263). Median age was 75 years, 79% were male, and 70% had nonsquamous histology. At a median 2-year follow-up, 384 deaths and 448 progression-free survival events were recorded. Overall survival was not significantly prolonged with cisplatin (HR, 0.86; 95% CI, 0.70 to 1.05; P = .14) and global health status score of quality of life was not improved, whereas progression-free survival (HR, 0.76; 95% CI, 0.63 to 0.92; P = .005) and objective response rate (15.5% v 8.5%; P = .02) were significantly better. Significantly more severe hematologic toxicity, fatigue, and anorexia were found with cisplatin. Conclusion: The addition of cisplatin to single-agent chemotherapy does not significantly prolong overall survival, and it does not improve global health status score of quality of life in elderly patients with advanced NSCLC.

Abstract: Various diseases and conditions can cause bony defects in the maxillofacial region. Since microvascular bone graft transfers were introduced in 1970, new possibilities in reconstructive surgery have arisen. Various methods of treatment have been described for severely atrophic mandibles. These methods are: (1) the placement of four endosteal implants of short length, (2) the placement of a trans-mandibular implant (3) the placement of endosteal implants in combination with vertical distraction osteogenesis or (4) the placement of endosteal implants in combination with an onlay or interpositional bone graft. The patient was operated on to harvest bone for dental implant insertion at the Galeazzi Institute (Milan, Italy) in August 2016. He presented high resorption of edentulous mandible. Bone from the anterior approach was harvested started with a saw, and then a chisel and mallet were used. After 6 months removal of the screws of synthesis (1 fractured is left in place). Insertion of endosseous implants After 3 months loading with mixed prosthesis (bar + overdenture) with locator. Several Authors have reported pain on walking, weakness of the abdominal wall, frank herniation, and disturbances of gait. A few other approaches have been suggested to reduce impairments at the donor site such as taking only the inner cortex, which will reduce the amount of bone available and its implantability. Others have suggested leaving the iliac crest and harvesting the underlying bone. It is concluded that in treatment combining an onlay graft and simultaneous implant placement in the extremely resorbed mandible, considerable graft resorption must be anticipated.
