At present, the world we are in is continuously changing, with us rushing after it. Unconsciously, we are adapting to the standards of constant productivity, success and mass consumerism that characterize modern society at the expense of physical and mental well-being. Thus, little consideration is given to mindfulness and self-care that, instead, restore our relationship with the natural environment. The purpose of this thesis is to draw attention to the value of the present moment through the design of a wooden piece of furniture, which embodies Japanese aesthetic sensibilities and Finnish minimalism. This combination results in the production of a lounge chair that expresses its unassuming nature through sincerity. Following the wabi-sabi aesthetic, which finds beauty in the impermanence of life and the subtlest details, the research consists of a juxtaposition between the philosophical theory and practical design development. The exploration is grounded on a “learning by doing” method, which involves awareness of the natural material and its constraints during the whole experimental design journey. The bodily sensations perceived while crafting the physical product emerge in a conscious understanding of the responsibilities designers bear in taking decisions while designing. These can lead to expected or unexpected outcomes: here stands the Japanese wisdom of accepting the changes and trusting the process. Acknowledging the tools and materials as part of the art makes the designer reflect on the significant relationship that bonds him/her to the craft piece. Thus, he/she will be apt to cherish every moment, not only focusing on the final result. Although the lounge chair does not want to establish its place in an already saturated furniture market, it aims to let the designer and the user realize the importance of life small nuances: the former while crafting the furniture, the latter while lounging on it. Acute appendicitis is commonly encountered in daily practice. The perforation of a retrocecal appendix can result in retroperitoneal abscess a condition that is still recognized as life-threatening. This emphasizes the importance of early diagnosis and prompt management for this uncommon complication. We present a case of unusual presentation of retroperitoneal abscess from perforated appendicitis and a review of the Literature. a 44-year-old woman arrived to our observation for the appearance by about 3 days of abdominal pain on the right quadrants irradiated to the lower limb. at the opening, we drained abscess cavity buffered on the caecum and its mesentery, which deepened in the back-peritoneum, affecting the ipsilateral ileo-psoas muscle and limbing the vertebral plane. appendix stump was perforated. We performed appendectomy with multiple washings of the peritoneal cavity. The patient was discharged at 19th postoperative day. The development of a retroperitoneal abscess, however rare, remains one of the most serious complications of acute appendicitis and is always associated with the perforation of a retrocecal appendix. its insidious onset may delay diagnosis and treatment making this clinical entity a challenge for the surgeon who will have to distinguish between primary and secondary psoas abscess when some combination symptoms such as fever, flank pain and limitation of hip movement arise. acute perforated appendicitis can occasionally manifest as complicated retroperitoneal abscess. it has to be taken into account in the differential diagnosis of a primary psoas abscess. The surgeon’s experience is a key point in therapeutic strategy.

We propose two optimal representations for the elements of trace zero subgroups of twisted Edwards curves. For both representations, we provide efficient compression and decompression algorithms. The efficiency of the algorithm is compared with the efficiency of similar algorithms on elliptic curves in Weierstrass form.

The pathogenesis of diverticular disease and acute diverticulitis is still unclear and many different hypotheses have been formulated. Seemingly, there are several related factors such as chronic inflammation, gut microbiome, obesity and the immunogenic properties of fat tissue and diet. Inflammation plays a pivotal role in diverticular disease and acute diverticulitis. The aim of the present review is to investigate the role of inflammation in diverticular disease as well as in mild and complicated acute diverticulitis with a focus on current research and treatment perspectives

La stenosi anale è un restringimento fibroso del canale anale. Si sviluppa, nella maggior parte dei casi, in seguito a procedure chirurgiche proctologiche con ampia escissione anodermica o in presenza di infiammazione anale cronica in pazienti con malattia di Crohn. Per quanto rara, questa condizione è molto debilitante per i pazienti. I sintomi includono stitichezza, dolore e defecazione con anguinamento e una riduzione del calibro delle feci. La diagnosi è essenzialmente clinica. La prevenzione della stenosi post-chirurgica si basa su una scrupolosa tecnica chirurgica e su un follow-up ampio e attentamente pianificato. Il trattamento si basa su un approccio inizialmente conservativo con la regolarizzazione del transito delle feci attraverso l’idratazione, fibre alimentari e lassativi formanti massa. Il ruolo della dilatazione meccanica nel trattamento della stenosi anale è ancora oggetto di discussione. Per i casi gravi che non rispondono al trattamento conservativo si deve ricorrere alla chirurgia. L’approccio chirurgico è adattato alla gravità e all’estensione fino ad arrivare all’anoplastica con flap mucoso., Anal stenosis is a fibrous narrowing of the anal channel. It develops, in most cases, in the aftermath of proctologic surgical procedures with extensive anoderm excision or in the presence of chronic anal inflammation in patients with Crohn Disease. However rare, this condition is thoroughly debilitating for the patients. Symptoms include constipation, pain and bleeding with defecation and a reduction of the caliber of stools. Diagnosis is essentially clinical. Prevention of postsurgical stenosis is based on a scrupulous surgical technique and on an extensive and carefully- planned follow up. Treatment is based on an initially conservative approach with regularization of stool transit through hydration, dietary fibers and bulk-forming laxatives. The role of mechanical dilatation in the treatment of AS is still debated. For severe cases and cases that are not responsive to conservative treatment we must resort to surgery. Surgical approach is tailored on the severity, position and extension. The most severe cases require anoplasty procedures with mucosal or anal flaps. INTRODUZIONE: Gli elevati costi di ospedalizzazione hanno incoraggiato una maggiore attenzione verso la riduzione della degenza ospedaliera. Al giorno d’oggi, molte procedure chirurgiche possono essere eseguite in regime di day-surgery. Una degenza ospedaliera breve comporta molti vantaggi per i pazienti: minori disagi, minori rischi di infezione ospedaliera e un veloce ritorno al lavoro. In proctologia, le procedure in regime di day-suregry non sono ancora molto diffuse per paura delle possibili complicazioni. In questo lavoro riassumiamo la nostra esperienza di 15 anni, sottolineando la possibilità di eseguire anche procedure complesse in anestesia locale-regionale e in un contesto di day-surgery. MATERIALI E METODI: I pazienti devono essere classificati come ASA I o II. RISULTATI: Tra il 2005 e il 2015 la nostra unità operativa ha eseguito un totale di 2688 procedure proctologiche in day surgery. 1062 procedure sono state completate in anestesia locale esclusivamente e tutti i pazienti sono stati dimessi tra due e tre ore dopo la procedura senza complicazioni significative. Negli altri 1626 pazienti il blocco posteriore perineale è stato eseguito in 932 casi (57,3%), ha fornito un controllo ottimale del dolore nel 51,6% dei casi (301 pazienti) mentre nel 5,7% dei casi c’è stata la necessità di somministrazione endovenosa di Fentanil. DISCUSSIONE E CONCLUSIONE: La day surgery è oggi una realtà concreta, resa possibile da un’attenta selezione dei pazienti, un’accurata tecnica chirurgica, un attento monitoraggio del paziente nel periodo postoperatorio e un monitoraggio continuo dell’efficacia dei farmaci antidolorifici. Negli ultimi decenni il nostro team chirurgico ha sviluppato tecniche modificate di anestesia loco-regionale che ci consentono di eseguire procedure anche complesse e di dimettere il paziente entro 24 ore., INTRODUCTION: The actual high hospitalization costs have encouraged a growing attention towards reducing hospital stay. Nowadays, many simple surgical procedures are carried out in a one-day surgery regimen . A shorter hospital stay brings many advantages for the patients: lesser inconvenience, a lower risk of hospital infection and an earlier return to work. In proctology, day surgery polices are still uncommon because surgeons fear possible complications. In this paper we sum up our 15 years experience, stressing the possibility to perform even complex procedures in local-regional anesthesia and in a day-surgery context. MATERIALS AND METHODS: In our experience, to be candidate to one-day surgery proctological procdures, patients should be classed as ASA I or II. RESULTS: Between 2005 and 2015 our operative unit executed a total of 2688 proctological procedures in a one-day surgery. 1062 procedures have been completed under local anesthesia exclusively and all patients have been discharged between two and three hours after the procedure without significant complications. In the other 1626 patients perineal posterior block was performed in 932 cases (57.3%) and provided an optimal pain control in 51,6% of cases (301 patients) while in 5,7% of cases there was the need for an intravenous administration of Fentanil. DISCUSSION AND CONCLUSION: Day-surgery is nowadays a concrete reality, made possible by an attentive selection of patients, an accurate surgical technique, an attentive patient monitoring in the postoperative period and a continuous monitoring of the effectiveness of pain medications. Over the last decades our surgical team has developed modified techniques of loco-regional anesthesia that allow us to perform even complex procedures and discharge the patient within 24 hours. 3,5-Diaryl-4,5-dihydroisoxazoles were synthesized and evaluated as monoamine oxidase (MAO) enzyme inhibitors and iron chelators. All compounds exhibited selective inhibitory activity towards the B isoform of MAO in the nanomolar concentration range. The best performing compound was preliminarily evaluated for its ability to bind iron II and III cations, indicating that neither iron II nor iron III is coordinated. The best compounds racemic mixtures were separated and single enantiomers inhibitory activity evaluated. Furthermore, none of the synthesised compounds exhibited activity towards MAO A. Overall, these data support our hypothesis that 3,5-diaryl-4,5-dihydroisoxazoles are promising scaffolds for the design of neuroprotective agents.
