Background. In December 2019, a Coronavirus 2019 epidemic (COVID-19) was reported, caused by a severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which occurred in the city of Wuhan, Hubei province, China. Perceived risk of contracting diseases has led many Governments and Healthcare Organizations to implement a variety of control and protection measures for the population, in particular for health professionals who have made contact with positive Covid-19 patients. In this publication, we have carried out a review of the information available, in order to share the prevention and protection measures for health and safety at work, which a University Hospital of Pavia, in Northern Italy, has remodulated, according to the changed scenario in which professionals finds themselves carrying out their profession in the post lockdown, in account to the specificity of processes and methods of work organizing, which overall, they serve to characterize risks, in order to be able to prevent them in the best possible way for patients, visitors and healthcare professionals.Introduzione. Nel dicembre 2019, è stata segnalata un’epidemia di Coronavirus 2019 (COVID-19), causata da una grave sindrome respiratoria acuta coronavirus 2 (SARS-CoV-2), avvenuta nella città di Wuhan, nella provincia di Hubei, in Cina. La percezione del rischio di contrarre malattie, ha portato molti Governi e Organizzazioni Sanitarie ad attuare una varietà di misure di controllo e protezione per la popolazione, in particolare per gli operatori sanitari, che hanno avuto contatti con pazienti Covid-19 positivi. In questa pubblicazione, abbiamo effettuato una revisione delle informazioni disponibili, al fine di condividere le misure di prevenzione e protezione per la salute e la sicurezza sul lavoro, che un Ospedale Universitario di Pavia, nel nord Italia ha rimodulato, secondo il mutato scenario in cui gli operatori sanitari si ritrovano a svolgere la loro attività nel post lockdown, in considerazione della specificità dei processi e dei metodi di organizzazione del lavoro, che nel complesso servono a caratterizzare i rischi, al fine di poterli prevenire nel miglior modo possibile per pazienti, visitatori e operatori sanitari.

Although there has been increasing interest in assessing patient satisfaction with health care during the past year, relatively few studies investigated hemodialyzed patient satisfaction.Cross-sectional study of the relationship between burnout in nephrologists and nurses and patient satisfaction with their care.Nephrologists (n = 68), nurses (n = 334), and hemodialyzed patients (n = 695) from 10 dialysis centers across northern Italy.Staff burnout was assessed using the Maslach Burnout Inventory. Three staff burnout subscales were developed by using factor analysis: Emotional Exhaustion, Depersonalization, and Personal Accomplishment.Patient satisfaction was assessed by means of a multichoice questionnaire that incorporated 4 patient satisfaction dimensions: (1) completeness of medical information, (2) emotional relationship with health care staff, (3) performance of dialysis center staff, and (4) organizational aspects of health care service. Each item was scored on a 4-point scale ranging from 1 (very dissatisfied) to 4 (very satisfied). Spearman rank-order correlation was used to examine relationships between patient satisfaction and each of the 3 staff burnout subscales for each dialysis center.Overall, burnout scores were lower than the Italian normative sample, with no significant differences between physicians and nurses. However, considering each dialysis center separately, in 2 centers, nurses registered higher emotional exhaustion levels compared with physicians, with statistically significant differences (P = 0.004 and P = 0.003, respectively). Analysis of patient overall satisfaction assessments showed general appreciation for the "courtesy" and "kindness" of staff, but evidence of problems regarding organizational aspects and structural factors. However, the most critical dimension was for "information." There was a significant positive correlation between staff personal accomplishment and client satisfaction (P0.01) and a significant negative correlation between staff emotional exhaustion and patient satisfaction (P0.01). No significant correlation was found between staff depersonalization and patient satisfaction level.Factor analysis should be considered exploratory and confirmed in future use of the questionnaire.Our results suggest that high levels of burnout in physicians and nurses are associated with poor patient satisfaction in dialysis units. Identifying and preventing staff burnout may increase patient satisfaction with health care.
