Background: Headache is one of the leading neurological disorders both globally and nationally, responsible for significant morbidity and disability. Migraine headache disorder is a common headache disorder affecting at least 11% of world’s population. Case Report: We present a case of a patient who presented with migraine headaches associated with hemifacial spasm (only during acute migraine attacks) along with upper cervical pain. She was offered right-sided C2-C3 and C3-C4 intraarticular facet joint injections with steroid and local anesthetic under fluoroscopy. Significant relief in headaches along with a complete resolution of hemifacial spasms was noted. Conclusion: This outcome raises the possibility of underlying pathophysiological processes that could have been interrupted by cervical facet joint steroid injection to stop the facial spasms. To the best of our knowledge, this is the first case report of migraine headaches associated with hemifacial spasm that responded to cervical intraarticular facet joint injection. Context: Secondary acute myeloid leukemia (sAML) may arise from a previous clonal disorder of hematopoiesis, usually from myelodysplastic syndrome, chronic myeloproliferative neoplasm, or as a complication in patients receiving chemotherapy or radiotherapy for a primary malignancy, including hematological and non-hematological/solid malignancies (therapy-related AML [t-AML]). Overall, frequencies of sAML and tAML were 19.8% and 6.6%, respectively. It is associated with specific clinical features with a uniformly poor prognosis. Objective: To study patients of t-AML with respect to clinical presentation, laboratory parameters (including flow cytometry immunophenotyping), cytogenetic and molecular studies, and clinical outcome. Design: This study was a retrospective analysis of patients of t-AML from January 2011 to December 2019. Patients: The patients underwent a detailed hematological work up: peripheral blood smear, bone marrow examination along with flow cytometric immunophenotyping, and karyotyping and molecular studies (NPM1, FLT3ITD/TKD, BCR/ABL, PML/RARA, INV16, AML-ETO). Outcomes of the patients were also reviewed. Results: A total of 20 cases of therapy-related AML were diagnosed in patients treated for various malignancies of the breast (9 cases), prostate (3 cases), and pancreas (2 cases), as well as lymphomas (NHL: 4 cases and HL: 2 cases). There were 7 males and 13 females (M:F: 1:1.85) with a median age of 63 years (range 35–82 years). Median latency period to t-AML was 3.5 years (range 0.5–11.0 years). Median haemoglobin was 72.5 g/L (61–151 g/L), WBC count 3.7× 109/L (1.02–169.1×109/L), platelet count 40 × 109/L (12–257 ×109/L, peripheral blood and bone marrow blast percentage 48% (5-98%) and 76% (20–98%), respectively. AML with monocytic differentiation was the most common morphological and flow cytometric subtype (50% cases). Complex karyotype was seen in 50% cases. Molecular studies for NPM, FLT3ITD/TKD, PML/RARA, AML-ETO, and INV16 showed 8 cases with NPM1 positivity and 4 case with FLT3ITD positivity. Only 4 patients (20%) achieved complete remission but relapsed within 1 year of duration. Ten patients succumbed to their disease, the majority within 1 year of t-AML diagnosis. Conclusion: The study highlights the development of therapy-related AML following chemotherapy and/or radiotherapy as an important therapy-related complication. The latency period and clinical manifestations may differ depending on the type of cytotoxic drug, cumulative dose, and dose intensity. A close hematological follow-up should always be done for an accurate and timely diagnosis.

EBV-associated PTLD is increasingly recognized as an important cause of morbidity and mortality in both solid organ and hematopoietic stem cell transplant recipients. Mortality rates due to PTLD and virus-induced HLH are reported to be quite high. We report a case of EBV-associated PTLD and HLH in a child after liver transplantation who was successfully managed due to timely intervention. This case highlights that measurement of EBV load by quantitative polymerase chain reaction assays is an important aid in the surveillance and diagnosis of PTLD and early detection of EBV-induced PTLD, and aggressive treatment with rituximab is a key to survival in patients who have undergone liver transplantation.

Hepatosplenic T-cell lymphoma is a rare haematopoietic malignancy that comprises less than 1% of Non-Hodgkin lymphomas. We are reporting a case of a 26-year-old female, who presented with pallor, weight loss, jaundice, pancytopenia and hepatosplenomegaly. The bone marrow examination showed infiltration by lymphoid cells. These cells on flow cytometric evaluation showed the phenotype of hepatosplenic T cell lymphoma. The cells were positive for CD3, CD8, CD56 and TCR γδ and negative for CD5, CD4, CD8, CD16, CD57, TCRαβ along with B cell markers. This case is reported for being a rare clinical entity and its presence in an immunocompetent female making it rarer.

Early T-cell precursor acute lymphoblastic leukemia (ETP-ALL) is characterized by the specific immunophenotype of CD1a-, CD8-, and CD5weak/- with stem cell or myeloid marker expression [1]. This leukemia is associated with increased genomic instability, a high remission failure frequency, and hematological relapse. ETP-ALL patients also represent a distinct molecular T-ALL subgroup with a low frequency of NOTCH1 mutations and a high frequency of FLT3 mutations. ETP-ALL with FLT3 mutation has also been described as a new ETP-ALL subgroup with unique immunophenotypic features and high levels of CD2, myeloid antigen CD13, and CD117 expression, indicating a stem cell phenotype [2]. Mixed-phenotypic acute leukemia (MPAL), T/Myeloid subtype is rarely suspected from morphology, except in a small subset of cases, which have a distinct dual-blast population with both lymphoid and myeloid features. Therefore, final diagnoses of MPAL are based on flow cytometric immunophenotyping. Common cytogenetic abnormalities associated with MPAL include t(9;22) and MLL rearrangements, which may be restricted to B-Myeloid cases, whereas T-myeloid cases exhibit generally non-recurring chromosomal abnormalities [3]. Herein, we report a case of MPAL (T/Myeloid) with trisomy 4 and t(6;14)(q27:q22) in a 25-year-old man, the diagnosis of which was directed by the morphological presence of Auer rods and myeloperoxidase (MPO) staining, despite exhibiting a flow cytometric ETP-ALL phenotype.

The anatomy and embryology of occipital bone are complex and may present with a varied ossification pattern and masquerade as fractures. There are only a few articles in the literature and none on CT appearance of occiput ossification and sutures. Awareness of the normal and variant anatomy, appearance and position along with age of fusion of sutures will be helpful in confidently differentiating variant suture from fracture. This will be particularly important in children in the field of non-accidental injury with its attendant medicolegal complications. We describe the normal anatomy, embryology, normal and variant sutures of occipital bone as published in the literature so far and present a pictorial review of 15 cases of variant sutures. With CT as the primary modality in assessing for head injury now, it is important to know the CT features of these sutures, their appearance, position and variants and be able to utilize 3D-CT to be able to give a more definitive diagnosis.

Back pain is an unusual complaint in children as compared to the adult population but is more likely to be pathological. We present a longitudinal study of an 11-year-old patient who presented with a vertebral hemangioma which remained stable for a period of about 20 months followed by aggressive growth resulting in spinal cord compression. This is only the fourth reported case of aggressive vertebral hemangioma in children; however, we document for the first time the evolution of this lesion demonstrating a period of stability followed by aggressive growth of the lesion. We discuss the imaging differential diagnosis which includes asymptomatic vertebral hemangioma and malignant lesions. We also discuss different treatment options available and their complications. In our patient, among the risk factors for an aggressive hemangioma, female gender and involvement of posterior elements were positive. However, the natural history of this lesion is not well known in children. We need to study vertebral hemangioma further especially in children to understand the natural course and help identify lesions which have aggressive potential and prevent spinal cord compression. This will also help to manage the lesion earlier with less aggressive therapies such as embolization and ethanol injection rather than surgery. Key words: back pain, pediatrics, vertebral hemangioma

SummaryIntroduction Peripheral blood and bone marrow smear examination is an important basic tool for the diagnosis of different haematological conditions including haematological malignancies. We created a newer modification of the conventional Leishman and Giemsa stains as Leishman and Giemsa (L&G) stain and compared the efficacy and reliability of this stain with conventional stains. The study was performed to evaluate the staining efficacy, feasibility, time and cost of L&G stain over the conventional Leishman and Giemsa stains. Methods A pilot study was carried out in the Department of Haematology of our hospital from October 2013 to December 2013. Hundred selected cases, each with peripheral blood and bone marrow smears were taken, and three sets of the smears were prepared from each sample – one for L&G stain and other two – one each for conventional Leishman and Giemsa stains. This staining is further incorporated in our routine standard operating protocols for staining of all the peripheral blood smears in automated stainer, Sysmex SP10. Result The average grading score from each staining methods from all the three experts was compiled. Back pain and presentation with spinal canal hemorrhage in hemophilia is not common; however, these are significant clinical issues and may lead to significant neurological issues and morbidity. We present an interesting case of severe back pain in a young patient with moderate hemophilia A. Imaging confirmed subarachnoid hemorrhage in the spinal canal without intracranial hemorrhage. To the best of our knowledge this is the first described case report of subarachnoid hemorrhage in hemophilia A in the English literature. We also describe the anatomy and imaging features of hemorrhage in the different spinal canal compartments, including the subarachnoid space. Spinal canal hemorrhage in hemophilia is an emergency and serious condition and must be diagnosed and treated promptly. It is important to be aware of the diagnostic features of the spinal canal hemorrhage and carefully assess the spinal canal in hemophiliacs on cross-sectional studies.

SUMMARY: Opitz syndrome is a rare autosomal recessive disorder of cholesterol metabolism associated with mental retardation and multiple congenital malformations. It is also uncommonly associated with congenital glaucoma. We describe the orbital findings on CT in this rare case of a patient with Opitz syndrome who presented with congenital glaucoma, with a review of the literature. The CT findings of congenital glaucoma, which have not been described before in the literature, are also discussed. It is important for the radiologist to be aware of this rare association. It is also important to be aware of the findings of congenital glaucoma on CT because patients with Opitz syndrome and other syndromes associated with learning difficulties may not present with typical clinical features of glaucoma. A high index of suspicion will lead to a correct diagnosis and earlier intervention.

TO THE EDITOR: Granulocyte colony-stimulating factor (G-CSF)-associated blastocytosis with a lack of an associated left shift poses a challenge for accurate assessment of the underlying disease [1]. In particular, blastocytosis after chemotherapy for acute leukemia may mimic a recurrence of underlying leukemia. Only a few cases of myeloblastosis in AML or ALL or of leukoerythroblastosis in AML after G-CSF therapy have been described anecdotally in the literature [2,3,4]. Herein, we report a case of myeloblastosis in ALL within 2 days of G-CSF administration masquerading as early relapse, causing a diagnostic dilemma. This was compounded by a striking lack of telltale evidence that G-CSF treatment was effective, such as increased granulation and a left shift. A 25-year-old man diagnosed with common acute lymphoblastic leukemia antigen [CALLA] positive, precursor B-cell ALL (Fig. 1A) completed his first phase of induction chemotherapy with methotrexate, cyclophosphamide, and cytarabine. His day 36 bone marrow showed 2.8% blasts with positive minimal residual disease (MRD) (0.046%). To accelerate recovery from persistent neutropenia, he was treated with G-CSF; an adequate response was noted, with a total leukocyte count (TLC) of 2.9×109/L. On day 70, he completed his second phase of induction chemotherapy and was started on G-GSF therapy for severe neutropenia (TLC: 0.76×109/L). On day 73, his peripheral blood smear showed marked leukopenia with a predominance of lymphocytes and 2% blasts without any left shift or increased granulations. This lack of prototypical G-CSF response along with previous MRD positivity and presence of 2% blasts led to a suspicion of relapse, and bone marrow examination was advised. Marrow aspirate showed hypocellular marrow with 14% blasts, and focal areas showed up to 25% blasts (Fig. 1B), along with a lack of a maturing myeloid component, which suggested relapse. Trephine biopsy, in contrast, showed hypocellularity with only a few scattered CD34 positive blasts. This necessitated flow cytometric evaluation, where the blasts showed negativity for B lymphoblastic markers and expressed myeloid markers, including CD13, CD33, and CD34, suggesting that they were regenerating myeloblasts (Fig. 1C-F). No further therapy was administered and the patient was kept on close hematological follow-up. The counts recovered after 4 days, with the typical responses of a left shift and increased granulation. Follow-up marrow examination after complete recovery of blood counts, 10 days after discontinuing G-CSF therapy, revealed cellular marrow elements with 4% blasts (regenerating) and negative MRD (20%) in the bone marrow, with an absence of telltale G-CSF-induced changes, such as presence of immature granulocytes, monocytosis, or prominent granulations on peripheral smear. To conclude, the regenerating blast percentage may increase after G-CSF therapy to >20%, without concomitant increases in other mature myeloid precursors. The lag time before complete response to G-CSF therapy may be as long as 10 days, as seen in our case, posing potential problems in management and a challenge in therapeutic decisions. A careful morphological assessment of the blasts at the time of diagnosis, aided by awareness of these G-CSF-associated aberrant patterns and flow cytometric characterization can help guide management decisions.

TO THE EDITOR: Paraprotein is an abnormal immunoglobulin (Ig) or part of an Ig in the blood or urine that is produced by a clonal population of B cells and plasma cells. Production of a monoclonal Ig paraprotein is associated with various types of B-cell non-Hodgkin's lymphomas (NHLs). Paraproteinemia is associated with about 20% of patients with indolent types of NHL, whereas it appears to be rare in aggressive lymphomas [1]. Immunofixation (IFX) and conventional serum protein electrophoresis (SPEP) are useful tools to detect even low levels of monoclonal Igs. Herein, we report a case of diffuse large B cell lymphoma with a very high level of IgG kappa monoclonal gammopathy, which was rarely reported in the literature [2].
